New technique proposes to improvesafety in difficult heart surgery

NEERAJ SHARMAMOHALI, MAY 12

Dr Harinder Singh Bedi , Director ofCardiovascular Sciences at the Ivy Hospital ,Mohali has devised a new method to enlarge theaorta to overcome difficulty in Aortic RootEnlargement (ARE), which is a technicallydifficult surgery. He has already tried this newtechnique in a pig’s heart and it worked verywell. Dr Bedi has also presented this technique toa distinguished peer group at the 14th CME ofthe prestigious Association of North Zone CardioThoracic & Vascular Surgeons (ANZCTVS) .The method was discussed in great detail andapplauded by all. The chief guest in CME andPresident of the Indian Association of CTVsurgeons also gave his valuable input on this new‘Bedi technique’ . Dr Bedi said that Aortic RootEnlargement (ARE) is a technically difficultsurgery required in a few patients who need alarger size aortic valve replacement than ispossible with conventional methods . If anadequate sized valve is not put in – the patientwill suffer from an iatrogenic aortic stenosis and

can have a poor prognosis. There are generally 3surgical methods of ARE and all 3 may not bepossible in some specific subset of patients,”Heart surgeons occasionally need to operate ona case who has already had a previous surgerywhich precludes the use of these techniques.Surgeons have been looking for a method toovercome this problem, Dr Bedi informed. DrBedi has proposed an incision in the commissurebetween the left and the right coronary artery andto take it down to the annulus . Then this gap isclosed with a pericardial or Dacron patcheffectively enlarging the annulus by 1 or 2 sizeswhich is adequate.

YUGMARG, CHANDIGARH, FRIDAY, MAY 13, 2022

EDITORIAL

Thinning IndiaIndia is getting fatter. According to the National

Family Health Survey 2019-20, the percentage ofobese women increased to 24% from 20.6% in

2015-16, while the percentage for obese men rose to22.9% from 18.9% (in 2005-06, only 12.6% womenand 9.3% men were obese). According to a study pub-lished in the Indian Journal of Community Medicinein 2020, more than 135 million Indians are obese. Fur-thermore, the National Centre for Biotechnology In-formation estimates that by 2030, India would ac-count for 27.8% of global obesity, which would ac-count for 5% of the world population. The rise in obe-sity among Indians is alarming since it is the leadingcause of hypertension, diabetes, cardiovascular dis-eases, certain types of cancers and arthritis.

The Centre is planning to impose a tax on foodsthat are high in sugar, fat and salt. Niti Aayog is cur-rently reviewing the evidence in order to determinethe appropriate rate of tax to be levied on these foods. Will a tax on obesity-causing foods deter people fromconsuming them? One can try and answer this ques-tion based on the international experience with such atax — the ‘sin tax’.

Public Health England notes in a report that inMexico, where 10% tax on sweetened soft drinks wasintroduced in 2014, the total volume of taxed bever-ages purchased over the year was 6% lower than whatwould have happened without the tax. However, evi-dence from other countries is not as compelling. InFinland, additional taxes were levied on confec-tionery, ice cream and soft drinks in 2011. While theprice of soft drinks increased by around 6% annuallybetween 2011 and 2013, consumption declined by anaverage of slightly more than 1%. In Philadelphia, animposition of a tax of 1.5 cents per ounce on sweet-ened beverages sold within city limits in 2017 causedsales to fall marginally in low income neighbour-hoods.

International evidence on the effect of a tax onobesity-causing food is mixed. It is unclear whethersuch a tax would address India’s obesity problem. In-deed, the success of such a tax will depend on theprice elasticity of obesity-causing foods (the respon-siveness of demand for such foods to changes in theirprices), which, in turn, depends on what proportion ofincome people spend on such goods and whether suchgoods are perceived as addictive. If people spend asmall proportion of their income on such foods and/orthese foods are addictive, the price elasticity is likelyto be low rendering the proposed tax ineffective.

Plastics in atmospherepollute oceans

Anew study investigates the atmos-phere as a related cause of plasticpollution in the waters of our planet.

The findings of the research were publishedin the journal 'Nature Reviews Earth & En-vironment' by an international team of re-searchers including experts from the AlfredWegener Institute, the Institute for Ad-vanced Sustainability Studies in Potsdam, and the GEOMAR Helmholtz Centre forOcean Research.

According to estimates, by 2040, thelevel of plastic pollution could reach 80million metric tons per year. Plastic parti-cles have now been detected in virtually allspheres of the environment, e.g. in waterbodies, the soil and the air. Via ocean cur-rents and rivers, the tiny plastic particlescan even reach the Arctic, Antarctic orocean depths.

A new overview study has now shownthat wind, too, can transport these particlesgreat distances - and much faster than wa-ter can: in the atmosphere, they can travelfrom their point of origin to the most re-mote corners of the planet in a matter ofdays.

Today, between 0.013 and 25 millionmetric tons of micro and nano plastic peryear are transported up to thousands ofkilometres by ocean air, snow, sea sprayand fog, crossing countries, continents andoceans in the process. This estimate was ar-rived at by an international team of 33 re-searchers, including experts from the Al-fred Wegener Institute, Helmholtz Centrefor Polar and Marine Research (AWI), andthe Institute for Advanced SustainabilityStudies in Potsdam (IASS) and the GEO-MAR Helmholtz Centre for Ocean Re-search in Kiel.

"Air is a much more dynamic mediumthan water," said co-author Dr MelanieBergmann from the AWI. "As a result, mi-cro and nano plastic can much more quick-ly penetrate those regions of our planet thatare most remote and still largely un-touched." Once there, the particles could

affect the surface climate and the health oflocal ecosystems. For example, when thesedarker particles are deposited on snow andice, they affect the ice-albedo feedback, re-ducing their ability to reflect sunlight andpromoting melting.

Similarly, darker patches of seawaterabsorb more solar energy, further warmingthe ocean. And in the atmosphere, mi-croplastic particles can serve as condensa-tion nuclei for water vapour, producing ef-fects on cloud formation and, in the longterm, the climate.

How do plastic particles get into the at-mosphere?

First of all, through human activities.Particles produced by tyres and brakes inroad traffic, or by the exhaust gases fromindustrial processes, rise into the atmos-phere, where they are transported by winds. However, according to the overview study, there is also evidence suggesting that a sub-stantial number of these particles are trans-ported by the marine environment.

Initial analyses indicate that microplas-tic from the coastal zone also finds its wayinto the ocean through eroded beach sand. The combination of sea spray, wind and

waves forms air bubbles in the water con-taining microplastic. When the bubblesburst, the particles find their way into theatmosphere. As such, transport to remoteand even polar regions could be due to thecombination of atmospheric and marinetransport.

Consequently, it is important to under-stand interactions between the atmosphereand ocean, so as to determine which parti-cle sizes are transported, and in whichquantities. The atmosphere predominantlytransports small microplastic particles, which makes it a much faster transportroute that can lead to substantial deposits ina broad range of ecosystems.

As Melanie Bergmann explained: "Weneed to integrate micro and nano plastic inour measurements of air pollution, ideallyon an international scale as part of globalnetworks." For this purpose, in a first step, first authors Deonie Allen and Bergmannbegan collecting samples of microplastic inthe air, seawater and ice during a Polarsternexpedition to the Arctic last year.

Joining forces to grasp the microplasticcycle Understanding and characterising themicroplastic cycles between the ocean and

atmosphere will require joint efforts. In thisregard, in the study, the team of researchersled by first authors Deonie Allen and SteveAllen from the University of Strathclyde,Glasgow, outlines a global strategy for cre-ating a seamless, incomparable database onthe flow of micro and nano plastic betweenthe ocean and atmosphere.

"There are so many aspects of the emis-sions, transport and effects of microplasticin the atmosphere that we still don't fullyunderstand," said co-author Prof Tim But-ler from the IASS. "This publication re-veals the gaps in our knowledge - and pres-ents a roadmap for the future."

Two dedicated working groups fromthe Joint Group of Experts on the Scientif-ic Aspects of Marine Environmental Pro-tection (GESAMP) prepared the study. Ac-cording to study co-author and GESAMPmember Prof Sylvia Sander from GEO-MAR: "The study makes it clear that acomprehensive grasp of the ocean, and ofthe effects of human influences on it, canonly be achieved by networking re-searchers and their data. The great chal-lenges of our time are on a global scale. Ac-cordingly, we have to pursue answers topressing questions with expertise that is ascomprehensive and international as possi-ble. That can only be done by working to-gether."

GESAMP is a conglomerate of elevenorganisations belonging to the United Na-tions. Its goal is to arrive at a multidiscipli-nary, science-based understanding of themarine environment. To date, the networkhas already collaborated with more than500 experts from countries around theglobe on a range of questions. Micro andnano plastic in the air is also relevant forhuman health. In a recently released Britishstudy, microplastic was detected in thelungs of 11 of 13 living human beings."This is yet another reason why we need tointegrate plastic into monitoring pro-grammes for air quality," Bergmannstressed.

DNA provides unique insights ofmoa and climate change

According to a study atthe University of Ota-go, ancient Moa DNA

has provided insights into howspecies respond to climatechange.

The findings of the re-search were published in thejournal 'Biology Letters'.

By analysing the ancientDNA of the extinct easternmoa, researchers from the De-partment of Zoology foundthat giant birds altered theirdistribution as the climatewarmed and cooled.

Lead author Dr Alex Ver-ry said the species was spreadacross the eastern and south-ern South Island during thewarmer Holocene period butwas restricted to the southernSouth Island during the heightof the last Ice Age about25,000 years ago.

This is in comparison tothe heavy-footed moa, whichretreated to both southern andnorthern regions of the SouthIsland, while the upland moainhabited four different areas.

"The eastern moa's re-sponse had consequences forits population size and geneticdiversity - the last Ice Agelead to a pronounced geneticbottleneck which meant it

ended up with lower geneticdiversity than other moa liv-ing in the same areas," Dr Ver-ry said.

The study is the first timehigh throughput DNA se-quencing, which simultane-ously sequences millions ofpieces of DNA, has been usedto investigate moa at the pop-ulation level.

The findings highlighthow past climate change im-pacted species in differentways and that a 'one size fitsall' model is not practical.

"It makes us wonder whatis going to happen to speciesas they attempt to adapt to cli-

mate change today and intothe future? Will they also at-tempt to move to new areas inorder to survive? For somespecies this will not be possi-ble, some species will run outof space, such as alpinespecies which will have tomove upward but can only goso far until there is no moreup," he said.

Co-author Dr Nic Raw-lence, Director of Otago'sPalaeogenetics Laboratory, said the research is a rare ex-ample of the impacts of pastclimate change on extinctmegafauna from NewZealand.

Taliban suppress protests by womenagainst mandatory hijab in Kabul

Taliban militants dispersed ahandful of women whostaged a demonstration in

Kabul against the decision to makehijab mandatory and also detainedjournalists.

In the protest in Kabul, the pro-testers chanted 'The burqa is notour hijab' and 'Afghan women inthe Taliban's grip - the Afghan peo-ple in need a mouthful of bread,"three days after the Taliban tookthe decision to make "the hijabmandatory," The Khaama Press re-ported.

They took to the streets withthe slogan "Bread, Work, and Free-dom," claiming that their rallies

were a continuation of Afghanwomen's protests.

The protest began at the Ansarisquare of Shahr-e-Naw and contin-ued just outside of the Ministry of

Interior, where the Taliban sur-rounded them and detained jour-nalists, according to The KhaamaPress citing Zhulia Parsi, one of theprotesting girls.

"The Taliban snatched the girls'smartphones and took them away,"she claimed. According to Zhulia,Taliban forces reportedly rippeddown the banners and dispersed theprotesters. She further said thatthey wanted to take the women in-side the ministry and force them toconfess. Earlier, regarding themandatory hijab, the US stated thatif the Taliban do not halt their ac-tions, it will increase pressure onthe regime. At a press briefing onMonday, State DepartmentSpokesperson Ned Price said thepressure will continue until the Tal-iban rescind some of their recentdecisions restricting women's and

girls' rights, reported The KhaamaPress. "We have explicitly raisedthis concern with the Taliban, andwe have some instruments at ourdisposal that we are prepared toutilize if we believe the Taliban'srecent crackdown on women willnot be reversed or repealed," saidPrice. On Saturday, the Talibanleader issued a new directive re-quiring all Afghan women to wearthe Islamic hijab. Taliban's state-ment outlined the steps taken bythe Ministry for the Propagation ofVirtue and the Prevention of Viceofficials to oversee the implemen-tation of the mandatory hijab, re-ported Khaama Press.

The first step in this process isto find unveiled women's homesand advise and warn the women'sparents. In the second stage, thewoman's father or guardian is sum-moned to the concerned depart-ment, and a case is filed against thewoman's father or parents, and theperson's trial begins, reportedKhaama Press. During their previ-ous rule, from 1996 to 2001, theTaliban denied women the right towork and education, and thegroup's stance over the past ninemonths, which has dominatedAfghanistan, suggests that thegroup is once again pressing forstricter controls.

6 in 1 vaccination - a big boon for babiesand the parents: says Dr. Rajiv Gupta

mritsar: Children are exposed to an innumerable number of germs, some of Awhich can cause serious diseases. A child's

immune system is still developing, and it cannot fight against all deadly diseases. According to the World Health Organization, immunization is one of the most successful public health interventions to prevent deaths from infections such as diphtheria, pertussis (whooping cough), and tetanus in all age groups. The 6 in 1 combination vaccination protects children from 6 serious illnesses: diphtheria, pertussis, tetanus, haemophilus influenza type B, hepatitis B, and poliomyelitis. Speaking about this, Dr. Rajiv Gupta, MD Pediatrics, Dr Rajiv Gupta's Children Hospital Amritsar, said, "Through the combination vaccination the child gets protected against six diseases in a single injection, with minimal to no side effects. Therefore, it is a boon for children and parents alike. By reducing the discomfort for children, combination vaccination also eases the parent's stress. Pediatricians play an important role in providing information about different vaccination options to parents and help them make an informed choice for their children." According to the immunization schedule of the Indian Academy of Pediatrics, children would need to take the DTP-IPV-Hib-HepB vaccines at the age of 6, 10 and 14 weeks. 6-in-1 vaccination provides protection for these 6 diseases.

NEWSBuilders & Developers Welfare Association of Kharar

and Zirakpur meets Harpal Singh CheemaSHASHI PAL JAINKHARAR, MAY 12

Builders & Developers WelfareAssociation of Kharar andZirakpur had a meeting withPunjab Finance Minister HarpalSingh Cheema and discussedvarious problems being faced byReal Estate industry in Khararand Zirakpur. The difficultiesbeing faced by of Builders &Developers of Kharar werebrought to the notice of HarpalSingh Cheema by Dr.AmritBansal President of the Builders& Developers WelfareAssociation of Kharar. He wastold that Builders and developersin the state are facing a lot ofproblems which is resulting inmigration of future projects to theneighboring states of Himachaland Haryana. It was expected thatnew government in Punjab, willgive listen to the problems beingfaced by the realtors of the stateand pay full attention to solvethese and help us to boost realestate in the state and so thatrevenue could be generated onhigher lever. He was told thatdelay in getting approvals was amajor challenge to this industryas they were getting completeapprovals in more than one-yeartime, which becomes a burden ondeveloper and project costincreases many times, resulting

high price of homes to end usersand major losses of revenue tothe government. They said thatthey have calculated that abuilder developing a project inPunjab is generating approx. Rs.14,00,00,000 (Fourteen crorerupees) per acre and as onaverage a project of 10 acres’land is source of one hundredforty-three crores revenuegenerated to state government.

They said that ease ofapproval can be helpful inbringing huge revenue to the stategovernment. They requested forsingle window system or self-certification scheme for ensuringall the approvals with in 30 days.

They told the minister thatthere were outstanding dues ofbuilders against EDC/IDC andEWS charges. Builders/developers couldn’t depositthese charges due to bad

condition of real estate market inlast 10 years. Now these chargeshave become a big burden due toheavy interest implemented onthe amount. They requested tointroduce one-time settlementscheme by offering interestwaiver and govt. will be able toget huge amount within sixmonths. They said that there is aprovision that a builder candeposit charges against EWSflats & he will be permitted to usethat FAR in general categoryflats. These charges areapplicable Rs. 1500/- Sq. feet for25 meters size flat but in localgovt. these charges are Rs. 2000/-per sq. feet for 30 Meters flat. Bythese difference of charges, builders working in municipalareas are suffering and are notable to pay these charges. Due tothis disparity, charges inGMADA Area Mohali is Rs.

1500/- Sq. feet and in the samecity Rs. 2000/- per sq. Feet areapplicable with in municipalarea. How it is justified that twodifferent charges are applicable inone city? Moreover, thesecharges are applicable Rs. 2000/-per sq. feet in Kharar/Zirakpur,where sale price of flat is 60% incomparison to Mohali. Thesecharges should be uniform andfor same size of the flat (25meters) and request to apply Rs1500 per sq. feet for 25 meters inZirakpur/ Kharar Area.

They said that EDC/IDCcharges collected fromdevelopers/builders are not beingspent in same area. EDC/IDCshould be spent for up gradationof surroundings of relatedproject. There are many areas ofPunjab, which are falling/ addedunder municipal areas, but fromlong time Zoning is notmentioned in the master plansand builders/ developers are notable to develop projects in theseareas. They said that taking anelectrical connection has becomea night mare. There is disparityand pick & choose method isbeing adopted by departmentwhile applying rules in each case.Delay of electrical connection toa project is not only affectingdevelopers/ builders, but alsoaffecting revenue generation ofthe state.

While she goes on with her introduction, she is interrupted by Farah who cracks a joke about Ananya winning a National award for 'Khaali Peeli'(also the name of her movie opposite actor Ishaan Khatter).

Ananya gets all excited and bursts into joy. Farah Khan then proceeds to channel Chunky Panday's role from the Housefull series and says, "I'm a joking". The post got huge responses from fans, friends and family. The main highlight of the post came when Ananya's father actor Chunky Panday took to the comment section of Ananya's post and wrote, "Farah you should get the Award for Overacting in this Video", Farah reacted and replied, "@chunkypanday apni beti ko sambhal pehle".
