Week Ending September 23, 1983

DOE Remedial Action Plan for Canonsburg, Pennsylvania Site

On September 15, 1983, NRC concurred in DOE's Remedial Action Plan for theCanonsburg, Pennsylvania site subject to DOE's provision of additionalinformation and NRC agreement on certain aspects of the plan before work atthe site is initiated. This is the first Reredial Action Plan completed byDOE for one of the uranium mill processing sites designated in Title I of theUranium Mill Tailings Radiation Control Act of 1978.

Informal Assessment of Decentralized Safeguards Functions at Regions I and II

Licensing responsibilities No. 50.54 and 70.32 were delegated by NMSS toRegions I and II in February 1983. In July and August 1983, NRR and NMSSstaffs developed a Licensing Function Assessment Plan and notified the regionsof scheduled visits. Several NRR and NMSS staff jointly visited Region I onSeptember 20, 1983 and Region II on September 21, 1983 to review theirprogress in implementing assigned functions, to identify areas of concern tothe regions, and to assure the adequacy of headquarters guidance.

EPA Publishes Advance Notice on Low-Level Waste

The WM staff is preparing comments in response to the August 31, 1983, EPApublished Advance Notice of Proposed Rulemaking on the agency's intent todevelop standards and/or guides on low-level radioactive waste disposal. Thecomment period ends November 29, 1983.

Vogtle Electric Generating Plant, Units 1 and 2

Application for an Operating License was docketed September 16, 1983 by GeorgiaPower Company for the Vogtle Electric Generating Plant, Units 1 and 2.

The Vogtle units are Westinghouse 4-loop PWRs located near Waynesboro, Georg|a.Bechtel is the architect / engineer. Unit 1 is approximately 58% complete. TF3,

applicant's projected fuel load date is September 1986.

LaSalle County Station, Unit 1

On September 14, 1983, with LaSalle Unit 1 in cold shutdown, between 5,000 and10,000 gallons of water were inadvertently drained from the reactor vessel.The fuel remained covered at all times. The event occurred when a reactoroperator opened the low pressure coolant injector valve in accordance with theapproved surveillance procedure. As called for by the procedure, both therecirculation valve to the suppression pool and the containment spray valvewere open. The static level of the water in the reactor vessel caused water todrain from the reactor vessel past a failed check valve to the suppression pooland out the lower level containment spray nozzle. The drainage occurred forapproximately three minutes before it was terminated by a combination ofoperator action and automatic containment isolation due to low vessel waterlevel. The licensee is evaluating the failed check valve to determine thecause of the failure.

.Monticello loss of Both Off-Gas Recombiners Causes Monticello to Reduce Power

Around 2:30 p.m. on September 16, 1983, Monticello reduced power from 100% toabout 25% because both off-gas recombiners spuriously tripped. The plant hadbeen experiencing problems with a flow control valve in the "B" train when the"A" train spuriously tripped.

Since both recombiners were unavailable, the plant reduced power to maintainvacuum in the condensor as long as possible. If one train had not beenrestored within a few hours, the plant would have either scranmed on loss ofvacuum or been scrammed manually. The flow control valve was fixed and the "B"train declared operable. Power operation has since resumed.

On September 27, 1983, the NRC staff, including Mr. Denton, Director of NRR,will meet with GPU Nuclear representatives at 7:00 p.m. in the MSCO (MiddletownBorough Hall), 60 Emaus Street, Middletown, PA 17057, to discuss GPU Nuclear'srequest for NRC approval to conduct load tests of the TMI-2 reactor buildingpolar crane.

On September 28, 1983, the Three Mile Island Unit 2 Advisory Panel will meetfrom 7:00 p.m. to 10:00 p.m. in the Holiday Inn, 23 South Second Street,Harrisburg, Pennsylvania. The major topic for the meeting will be polar craneissues.

Pilgrim Nuclear Station - Failure of Relay for Recirculation Pump Trip onLow-Low Water Level

On September 13, 1983, a short time after smelling snoke in the control room,operators at Pilgrim Nuclear Station removed power from one of the two relaysthat trips both recirculation pumps on low-low water level. Because both ofthese relays must be deenergized to accomplish recirculation pump trip, no pumptrip occurred. The insulation on the overheated relay coil was found'to becompletely destroyed by the time it was removed from service. The relay isGeneral Electric Type liFA which is the same type that failed September 6,1983at Duane Arnold.

The licensee replaced the relay with a GE-recommended improved design (Centurytype), and at this time plans no further corrective action.

San Onofre Unit 3

On September 16, 1983, a license amendment authorizing full power operation wasissued for San Onofre Unit 3. The amendment included a condition requiringreporting of Auxiliary Feedwater System failures and their cause until thefirst refueling outage, as required by the Commission. As of September 20,1983, San Onofre Unit 3_ as at 10% power, preparing to go to the 20% powerwplateau for additional tests.

NRC TMI PROGRAM 0FFICE WEEKLY STATUS REPORT

September 18 - 24, 1983

REACTOR BUILDING ACTIVITIES:

Five reactor building entries were completed during the week of September 18,1983, involving pre-head lift activities, routine maintenance and housekeeping.Tasks of special interest included: evaluation of the refueling canal / reactorvessel seal plate suitability, reactor building sump sampling using the reactorbuilding sump pump, and a video inspection of the 282 ft. elevation.

Proce'd'ures and hardware are being prepared to retrieve the 35 ft. of tygontubing which was inadvertently drcpped into the reactor vessel onSeptember 12, 1983, during reactor coolant system (RCS) sampling. Threereactor building entries have been scheduled for the week ofSeptember 25, 1983.

POLAR CRANE STATUS:.

The TMI Program Office is continuing the review of all licensee documentsrelated to the reactor building polar crane. As previously reported in theWeekly Status Report dated September 19, 1983, the Office of Investigation (01)has issued two reports, one on the technical allegations relating specifically

to the polar crane and a second discussing other allegations. The Office ofInternal Affairs (0IA) has also issued a report discussing NRC/GPU interfaces.~

The Chairman of the Commission, Nunzio Palladino, has tasked the TMI ProgramOffice with responding to the 01 report findings in October.

SPENT FUEL P0OL '"A" REFURBISHMENT /SDS Modifications:

The lifting lug on the second charcoal filter has been repaired and all sixfilters and driers have been moved from the north end of the pool and loweredto the truck bay floor. Following proper sealing they will be shipped as LSAwaste. Removal of concrete shield blocks continued.

On Sep' ember 19, 1983 the submerged demineralizer system (SDS) modifications,t

(as part of "A" fuel pool refurbishment), were completed, including leaktesting of the new pipes. These modifications included:

Reroute of the piping from the reactor building sump pump to the--

reactor coolant system (RCS) manifold, bypassing the tank farm

Installation of a new submersible pump in place of the steam eductor--

in the lower tanks standpipe, to allow complete emptying of thesej tanks -

'

Reroute of the piping from the off-gas liquid separator tank bottom--

to the RCS manifold, allowing direct pumping of the collected liquidsto the miscellaneous waste holdup tank or other plant tankage

Disconnection of the charcoal filters and driers once used to treat--

effluent from the tank farm vents before the SDS vent and drainSEP 2 31983 subsystem was operational.

The final goal of the refurbishment work is to restore the "A" fuel pool to itspreaccident condition. The 'A' pool will be used to stora the fuel canisterscentaining reactor core debris removed from the reactor vessel. The presentplanning schedule for major tasks is as follows:

1. Tank decontamination: the tanks will first be flushed with water andhydrolased; SDS operability is a prerequisite for these operations asthe generated waste water will have to be processed. Decontaminationoperations for the four upper tanks will extend through earlyNovember, and for the two lower tanks through mid-November. If waterdecontamination appears insufficient, it will be followed by chemical.,

decontamination which could extend the work through mid-December.

2. Removal of the radiological shields: the shields consist of aconcrete block wall around the pool (presently being removed) and 162 ft. thick (20 ton) concrete shield slabs located above the poul.Removal of the slabs will require construction and testing of riggingequipment (scheduled to be completed by late September), and weldingand testing of the lifting lugs on the northernmost slabs (throughmid-October). The actual removal of the 16 shield slabs will beginin October and is scheduled to be completed in early January,1984.

3. Removal of the tanks: tank removal work includes cutting the inletand outlet pipes, sealing off openings, lifting the tanks, loweringthem into the truck bay and shipping them offsite. The actualremoval operation will begin in late 1983 and be completed by March1984.

'

4. Removal of the structural steel: i.e. the steel beams used tosupport the tanks and the concrete slabs. That work is scheduled toextend from late December 1983 to mid-March 1984.

5. Modifications to the fuel transfer mechanism: modification work isscheduled to start in mid-March 1984 and be completed by the end of1984.

.

6. General trash removal and pool cleanup, followed by pool linerinspection, eventual repair, and test: this work is scheduled toextend from August 1984 to February 1985. The pool will then befilled with water and should be ready to receive fuel canisters.

AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES:

Installation of specialized decontamination equipment in the 328 ft. elevationdecontamination facility addition occurred this week. The new equipment -

includes: a tool decontamination unit, with ancillary equipment attachmentsfor adaptation to perform remote decontamination or hose cleaning; anelectro-polisher unit; a vibratory finisher and an ultra-sonic unit. Construc-tion on the physical structure is completed with only plumbing and electricalwork associated with ' equipment installation being performed. Most of thesupporting procedures governing operation of the facility and its equipmenthave completed the review process and are in place. The new facility isscheduled to commence operations in October.

WASTE MANAGEMENT ACTIVITIES:

1. SDS Liner Shipments. Plans are being teat for the eventual shipment anddisposal of the pre and final SDS partic.bte filters (sand and "Cuno"filters) used in the early SDS processing tasks.

2. EPICOR Demineralizer Shipments. EPICOR dcmineralizer F-42, F-47, F-41K-7 and H-8 have been dewatered and are being prepared for shipment.

PENNSYLVANIA PUBLIC UTILITIES COMMISSION (PaPUC) ACTION ON GPU FUNDING REQUEST:

On Sept' ember 20, 1983, in a unanimous preliminary vote, the PaPUC rejectedMetropolitan Eaison's (50% owner of TMI) and Pennsylvania Electric's (25% ownerof TMI) request to allow the use of an additional $16 million a year of ratepayer funds for the cleanup of TMI Unit 2. Presently, $22 million a year ofrate payer funds from these two Fennsylvania utilities and $12 million per yearfrom Jersey Central Power and Light (25% owner of TMI) for a total of $34 -million a year are allowed for cleanup activities at TMI Unit 2. Thecommission voted not to increase rate payer funds to the level outlined in the -

Thornburgh plan because the nuclear industry contribution has failed tomaterialize.

In early 1983 GPU had projected 576 million for 1983 and $100 million in 1984.The 1984 projection anticipated 550 million from rate payers and $10-15 millionfrom the indus.try. Unless additional financial sources are forthcoming,,

present cleanup activities may have to be further reduced.

RICKOVER TO ASSESS GPU NUCLEAR MANAGEMENT:,

.GPU. has reached an agreement with Admiral Hyman G. Rickover to conduct a two-month detailed management assessment of GPU Nuclear Corporation.

Admiral Rickover will have complete and open access to all GPU Nuclear people,facilities and information in conducting the assessment. His report of

findings and conclusions will be made public at the appropriate time..

.

In lieu of compensation, GPU will make a contribution to charities designatedby Admiral Rickover.

1. On September 27, 1983, the NRC staff including Mr. H. Denton, Director ofOffice Nuclear Reactor Regulation, will meet with GPU Nuclear representa *tives at 11:00 AM in the MCSO (Middletown Borough Hall), 60 Emaus Street,Middletown, PA 17057, to discuss GPU Nuclear's request for NRC approvalto conduct load tests of the TMI-2 reactor building polar crane. Themeeting is open to the public for observation. Questions from the publicwill De accommodated by the NRC at the TMI-2 Advisory Panel meeting thefollowing evening (see below).

IAEA Safeguards Advisory Group Meeting

NMSS staff represented the USG at the Advisory Group Meeting on Nuclear Facility-

Design Assisting the Implementation of IAEA Safeguards on September 5-9, 1983.' Advisors from nine countries attended the meeting. Results of meetings heldi during the previous three years were included in a draft document discussed.

Guidelines will be issued in the IAEA Safeguards Information series for use byfacility designers, as well as organizations interested in the development ofnuclear power, and individuals in the field of international safeguards. The

.Guidelines resulting from this Advisory Group consist of nine chapters address-

' ing general guidelines, as well as guidelines specifically for LWRs, reprocess-ing plants, on-load reactors, and away-from-reactor (AFR) wet storage. Addi-!

; tional chapters addressing LEU and MOX fuel fabrication plants, as well as AFR! dry storage, are planned by the IAEA Scientific Secretary.

DOE Remedial. Action Plan for Canonsburg, Pennsylvania Site

. On September 15, 1983, NRC concurred in DOE's Remedial Action Plan for theCanonsburg, Pennsylvania site subject to DOE's provision of additional

: information and NRC agreement on certain aspects of the plan before work at'

the site is initiated. This is the first Remedial Action Plan completed byDOE for one of the uranium mill processing sites designated in Title I of the

j Uranium Mill Tailings Radiation Control Act of 1978.

Informal Assessment of Decentralized Safeguards Functions at Regions I and II|

50.54 and 70.32 licensing responsibilities were delegated to Regions I andII in February 1983. In July and August 1983 NRR and NMSS staffs developeda Licensing Function Assessment Plan and notified the regions of scheduledvisits. Several NRR and NMSS staff jointly visited Region I on September 20,1983 and Region II on September 21, 1983 to review their progress in imple-menting assigned functions, to identify areas of concern to the regions, andto assure the adequacy of headquarters guidance,

Consulting to IAEA Safeguards Departrent

On September 12-16, 1983 NMSS staff consulted with the Safeguards EvaluationDivision staff on developments in tha IAEA quality assurance program.Emphasis was placed on issues related to program procedures drafted by theNMSS staff for the IAEA in July 1982, and on subsequent implementation ofelements of this program by IAEA. Assistance was primarily provided forquality assurance and control for inspection activities related to use ofIAEA seals by IAEA inspectors.

Meeting with Utility Nuclear Waste Management Group

WM staff met with the Utility Nuclear Waste Management Group to discuss theWaste Form and Waste Classification requirements in 10 CFR Part 61 in addi-tion to the technical positions which provide guidance for meeting the 10CFR Part 61 requirenents. WM staff also discussed their efforts in coordi-nating with NRC inspectors so that the 10 CFR Part 61 requirements would be '

interpreted on a consistent and reasonable basis. Costs to implement therule were presented and appear to be within the estimates made by the staff.

EPA Publishes Advance Notice on Low-Level Waste

The WM staff is preparing comments in response to. the August 31, 1983, EPApublished Advance Notice. of Proposed. Rulemaking on the agency's intent to -

| The EGCB, DEPER is evaluating the potential generic implication of| recently reported failures of HFA relays at Duane Arnold and Pilgrim.

As part of this effort, Region III has surveyed plants in their'

region to see what plants have replaced the HFA relays with centuryrelays in accordance with GE's recommendation. A memo telefaxed tothe other Regional Offices this week requested that they also surveythe plants in their respective regions. The Region III memo is anattachment for the telefax. Findings from the survey are requestedto be forwarded to EGCB for evaluation. Various options to resolvethe problem will be discussed with the regions and the most appro-priate action will be determined.

An informal meeting was held between B&W, EPRI, and NRC to discuss

sign. pretest analysis and the MIST downcomer/ Reactor Vessel Vent Valves de-OTISB&W agreed to provide a document which would present facility scoping

analysis with respect to each test, as defined in the OTIS testmatrix. The analysis will be extrapolated from previous pretestcalculations performed for GERDA. In addition, B&W will identifyadditional code calculations that may be necessary to adequatelyscope each test.1

B&W has internally agreed to a design of the MIST downcomer andRVVV. The downcomer is a pipe with an upper annular section dividedinto four quadrants by vanes. Four RVVV's are proposed to controlfluid interaction between the reactor vessel upper plenum and thefour quadrants in the downcomer. This design is intended to allowsimulation of loop asymetries driven by RVVV operation. Thedisadvantage in this disiin is code modeling uncertainties becauseof complex piping geometry and inadequateinstrumentation to trackfluid phase change and flow. A characterization study (separateeffects testing) may be nceded to adequately understand RVVV operation(chattering imbalances) during selected transients.

Appendix K Revision

RES is performing best estimate large-break LOCA calculations usingthe TRAC code as part of an effort supporting a proposed modificationof 10 CFR Part 50 Appendix K. RES has also requested that thereactor vendors perform calculations using evaluation models modifiedto include several changes to Appendix K. The objective of thiswork is to quantify the conservatism in the existing evaluationmodels and the change in conservatism which would result if theserule changes were made.

A meeting was held on September 13, 1983 with RES, NRR, LANL, andINEL staff to specify the best estimate TRAC calculations to baperformed. Representatives from reactor vendors were also invitedto provide conraents and to discuss coordination of the effort. Asa result of this meeting, RES has decided to perform a best estimateTRAC calculation for reactors from each of the four vendors. Thiscalculation, while best estimate, would be for a plant operating attechnical specification limits (e.g., maximum peak power, loss ofoffsite power, some ECCS systems out). A second calculation isalso planned for at least one (possibly more if time permits) plantwhich would be for the plant initially operating at the mostprob 6ble conditions with most probable power and ECCS behavior.These calculations are scheduled to be completed late CY 1983. The MS-2 steam generator has been disassembled in preparation forthe up coming transient tests. Westinghouse is conducting metallographyon selected tubes at their own cost. Since the MB2 has a welldocumented history of operation, the Materials Engineering Branchwas briefed.

The scaling studies have now been completed showing that with theexception of the dead space effect, there is no problem in scalabilityof the tube data to full-size steam generator units.i

The dead space in the MB-2 can be eliminated, but this would requireadditional costs cur:ently not within the program scope. Westinghouseis proceeding with preparing a better cost estimate of eliminatingthe dead space.

BNL Plant Analyzer

The following milestones were achieved in this program:

(1) BOP modeling and coding have been completed, checked out, andare being coupled to the main HIPA (Highly Interactive PlantAnalyzer) program.

(2) Controls modeling and coding have been completed; checking and

( debugging continues.

(3) The input / output hardyare and logic have been completed andare being interfaced with AD-10. Twenty-two(22) channelswill be initially available to vary such inputs as conto 11erfailures, valve positions, pump speeds, and set points.

(4) Colorgraphics capability has been implemented using an IBMpersonal computer and a color monitor to display online labeledcolored graphs of the major plant parameters. The system willbe expanded later to include high resolution mimics of theplant. |

University of California |

Professor V. E. Schrock, University of California, Berkeley, California,presented the results of the research program entitled " CriticalDischarge Through Cracks," to members of NRC. The objectives ofthe program are to provide test data and a validated model forcalculating critical mass flow rate through cracks.

Tests were carried out using slit geometry with the ratio of lenoth to dia-meter varyin,g from 80-400. The results showed that when the ratio of length

to diameter is known for single phase flow, the critical discharge can becomputed using the homogeneous, thermal equilibrium model.

TMI-2 Core Examination

R. B. Foulds participated for NRC in the TMI-2 examination program TechnicalEvaluation Group (TEG) September 15-16, 1983 planning session for TMI-2Core Examinations. The group is formed of members of the GEND four partyagreement on TMI-2 examinations plus technical representatives from industryand National Laboratories. The group's objective is to assure that theCore Enmination Plan and its implementation are technically adequatefor obtaining data useful to the NRC, other government agencies, utilities,and the nuclear industry to enhance the safety of commercial reactors.NRC initiatives in the meeting were accepted and backed up by industryand laboratory representatives. Significant accomplishments in the meet-ing include agreement on the first releasable edition of the core examin-ation plan document, EG&G-TMI-6169, to be published in October and agree-ment on development of a letter to be sent promptly to GPU Nuclear officiallyrequesting their attention to several current data gathering needs. Chiefamong these current needs is sampling for characterization of the coredebris bed.

Task Analysis Research Product

The task analysis of nuclear power plant control room crews has beencompleted and the published report is now available in NUREG/CR-3371Volumes 1 and 2. The research was conducted at eight nuclear power plantsites using utility control room crews in training simulators or actualcontrol rooms. Data were obtained from 45 operating sequences coveringnormal, abnormal, and emergency operating conditions. The data were compiledand edited with appropriate quality control / assurance and entered into acomputerized data base which is available for analysis.

Research Report on Low-Energy Photon Dose Deposition in Tissue. Slaband Spherical Phantoms

NUREG/CR-3425, Low-Energy Photon Dose Deposition in Tissue Slab and SphericalPhantoms, was completed by the University of Illinois and published inSeptember 1983. This report provides conversion factors (rem / roentgen)for the computation of tissue dose (rem) from air dose (roentgens) as measuredby personal dosimeters of survey instruments. It confirms the validity ofsimilar conversion factors listed in Table 1, Section 20.4, of the proposeddraft version of 10 CFR Part 20.

Proposed Revision to Respirator Certification Regulations

The proposed revision of the National Institute of Occupational Safety andHealth /Mine Safety and Health Administration (NIOSH/MSHA) joint respiratorcertification regulations is being sent to the affected regulatory agenciesfor review--OSHA, MSHA, and NRC. The gist of the revised draft is goingto be making the performance tests required for certification more meaningfuland indicative of actual equipment performance in use. To facilitateachievement of this goal, NIOSH proposes relying on the respirator manufacturersto conduct tests to show compliance with performance requirements in Part 11of 30 CFR. To assure compliance with performance tests, NIOSH would retainthe right to witness manufacturers' testing, request further testing, andperform audits of certified equipment.

On Wednesday Messrs. Terpstra and Dekkers of the Netherlands Energy ResearchFoundation (ECN) met with representatives. of the RES Reactor Risk Branch todiscuss " phased mission" analysis, a Dutch approach to fault tree analysis.

On Thursday a nine-member seismic qualification delegation from the FederalRepublic of Germany met with representatives of the NRR Division of Engineering,the RES Division of Health, Siting and Waste Management, and the ACRS staff todiscuss the aseismic design of nuclear power. plants.

On Thursday Mr. Jan Mattson of the Swedish Nuclear Power. Inspectorate met withrepresentatives of the NRR Core Performance Branch to discuss current fuellicensing practices, and the RES Analytical Models Branch to discuss RELAP 5and TRAC.

Low-Level Waste - California

Staff members Of the Office of State Programs and the Division of Waste Manage-nent, HMSS, met Wednesday, September 21, with Alan Pasternak, Executive Director,California Radioactive Materials Management Forum (a group of California generatorsof low-level waste). He reviewed the provisions of California Senate Bill 342,recently passed by the California Legislature which is awaiting signature by theGovernor. The legislation provides for a California low-level waste site andrequires the Governor to negotiate and enter into an agreement or compact withother States. The Health Department must develop regulations within six months.Prospective operator / licensees must submit qualification statements within threemonths (following the promulgation of regulation) and the department must thenselect a license designee within 45 days. If no qualification statements aresubmitted, the Resources Agency.is directed to establish a site.

Pennsylvania Public Utility Commission Rejects Funding Request for TMI-2 Cleanup

As reported in a preliminary notification dated September 21, 1983, the PaPUCvoted unani'mously on September 20 to reject (on a preliminary basis) MetropolitanEdison's (50% owner of TMI) and Pennsylvania Electric's (25% owner of TMI) requestto allow the use of an additional $16 million per year of ratepayer funds forTHI-2 cleanup. Presently $22 million per year of ratepayer funds from these twoPennsylvania utilities and $12 million per year from Jersey Central Power andLight (25% owner of TMI) for a total of $34 million per year are allowed forTMI-2 cleanup. The Commission voted not to increase ratepayer funds to the leveloutlined in the Thornburgh plan because the nuclear industry contribution hasfailed to materialize.

AE00/BNL Meeting on Diversified Data-Gathering System

On September 19, 1983, AE0D and Brookhaven National Laboratory (BNL) staffmet at BNL to further discuss the scope and schedule for the current phaseof the diversified data-gathering system development project. Work for thisphase began August 15, 1983 and will conclude in May 1984 with documentationof experience in using multivariate statistical techniques to analyze reactoroperational data of various types.

Trial Use of Interactive PRA Model for Event Assessment

On September 15-16, 1983, AE00 staff received initial training in the use ofthe NSAC-developed SIEVE Code for quantification of operational event impor-tance. The SIEVE Code will be installed at INEL and AE00 engineers will useit in conjunction with normal event screening for a period of three months.The objective of this trial program is to assess the contribution of the SIEVEtool to understanding event significance and to assess the potential of suchcodes to allow operational events to be classified as to significance.

Damage to Vacuum Breaker Valves as a Result of Relief Valve Lifting, AE00/E322

An engineering evaluation was completed on events at three BWR plants in whicha total of 12 vacuum breaker valves were found damaged after actuation of safetyrelief valves (SRVs). The damage involved a potential loss of function anddifferent valve sizes and manufacturers. Since the valves were partially open,steam could pass to the drywell through the opening following additional SRVactuations or leaking of an SRV. This would result in a steam environment inthe drywell. In addition, a partial loss of vacuum breaker function may resultin additional hydrodynamic loads on the piping and structure in the torus. AE00suggested that NRR review the adequacy of current requirements for vacuumbreaker valve design and qualification, effect of leaking steam to the drywellon environmental qualification of equipaent, and whether loss of partial vacuumbreaker function could introduce hydrodynamic loads to the piping and structurein the torus that have not been evaluated.
