NOTICE TO PROPOSERS:
A. This Addendum shall be considered part of the contract documents for the above-mentioned project as though it
had been issued at the same time and incorporated integrally therewith. Where provisions of the following supplementary data differ from those of the original contract documents, this Addendum shall govern and take
precedence.
B. Proposers are hereby notified that they shall make any necessary adjustments in their estimate on account of this Addendum. It will be construed that each Proposer’s proposal is submitted with full knowledge of all modifications
and supplemental data specified therein. Acknowledge receipt of this addendum in the space provided on the proposal form. Failure to do so may subject Proposer to disqualification.

3. Project Information
● ALE Upgrades at Colby Glass Elementary School
● List RFCSP# 2021-003
● Proposals will be received by the NISD Purchasing Dept. in person at 607 Richland Hills Dr. #700, San
Antonio, TX 78245-2149 or electronically through the District’s ESM solution on or before February
23, 2021 at 10:30am. Please refer to NISD Purchasing website for details and instructions on the use
of the District’s ESM Solution.
● Brief Project scope description. Complete HVAC upgrade of the existing H building and 1,225 SF
addition to Building ‘H’ at Colby Glass Elementary. Addition & renovation consists of enlarging existing
ALE, ECSE, and Pre-K classrooms, their respective bathrooms and closets, new Teaching Kitchen, and
new Mechanical and Electrical rooms.
● Successful Offeror will be notified on or before March 24, 2021.
● Project must be complete and ready for occupancy no later than December 28, 2021.
● The project budget is listed as $2,700,000.00
● Plans are available through the consultant or certain plan houses. Coordinate with consultant. Please
sign the sign in sheet which will be distributed as an addendum. If attending virtually please identify
yourself, the company you’re representing, and your email in the chat. If you are joining us through
phone conference, please email Jeff Rodriguez at [email protected] to ensure your
presence is documented in the sign in sheet.
● Request for clarifications or interpretations shall be submitted in writing no later than five days prior
to the last date for receipt of proposals. All request shall be submitted to the consultant and
questions will be responded to where appropriate in the form of an addendum to all plan holders and
interested parties.
4. Proposal Documents
● Proposal shall be submitted with the proper proposal security as defined in the Front End
Specification and shall include all alternates, project name and number and original copies with
original signatures and seals. Proposal security needs to be submitted with the proposal package
meeting the requirements listed on NISD Purchasing website.

● Contractors are encouraged to refer to Proposal Checklist listed in the Table of Contents, which
identifies forms to be submitted and the number of copies for consideration.
● Proposal Form
o Offeror will list bid pricing both numerically and written for base bid and alternate(s). Base bid
sum will include allowance(s) as listed in Item #1 of the Proposal Form. (Contingency
$100,000.00, Structural Steel Allowance $5,000.00, TAB $24,000.00, Structural Concrete Rebar
$3,000.00, Unit Price Allowance $20,000.00)
o Offeror will list the names of the Project Manager, Project Superintendent. A single person
cannot be listed as a Project Manager and Project Superintendent. If two names are provided for
Project Manager and/or Project Superintendent, both resumes will be reviewed and the points
will be issued based on the individual with the least amount of experience. Please note resumes
for the Project Manager and Project Superintendent will need to be submitted under Exhibit B-
Project Management Ability. Any other resumes provided may be reviewed, but will not be
considered for experience as a Project Manager and/or Project Superintendent.
5. Proposal Selection Criteria
● NISD ranks offerors by best value and not by lowest bid. Therefore, it is important to submit a
complete proposal package as identified in the Front End Specifications.
● Proposals are evaluated on five divisions
o Relevant Experience
o Project Management Ability
o Past Performance
o Subcontractor and Supplier Support Capability
o Price Proposal
● Per the Evaluation Matrix provided in the Front End Specifications each division will be evaluated by
but not limited to a committee consisting of NISD Administrators, NISD Project Manager and
Consultant(s).
o Relevant Experience (Max 10 Pts)
o Experience as a Contractor (Max 3 Pts) – Number of years as a contractor under the
submitted Contracting name and as listed in the A-305 - Contractor Qualification Statement.
In order to receive max points, the contractor will need to reflect 9 or more years in business
on the Contractor Qualification Statement A-305 under the submitted contracting name.
o Representative Projects (Max 7 Pts) – List of projects of similar dollar value (in relation to the
Budget) and scope, which are substantially complete. List shall include Project Name,
Architect/Engineer, Cost of Project, date of completion, scope of project and Contact Person.
Projects are not required to be K-12 or education related facilities. In order to receive max
points contractor will need to provide 7 or more projects and will not be penalized for
submitting additional projects.
o Project Management Ability (Max 15 Pts)
o Project Manager Experience (Max 6 pts) – Offeror shall provide a resume(s) for the
individual(s) listed on the Proposal Form for review under Exhibit B-Project Management
Ability. Resume(s) shall provide experience of the individual(s) as a Project Manager, with the
current or past employers. Total years of construction experience will not suffice. Years as an
owner of a contracting company can supplement experience.
o Project Superintendent Experience (Max 6 pts) – Offeror shall provide a resume(s) for the
individual(s) listed on the Proposal Form for review under Exhibit B-Project Management
Ability. Resume(s) shall provide experience of the individual(s) as a Project Superintendent,

with the current or past employers. Total years of construction experience will not suffice.
Years as an owner of a contracting company can supplement experience.
o Project Support (Max 3 Pt) – Offeror shall provide written evidence of sufficient resources
necessary to manage, staff and successfully complete the project. This written description
shall include the year the company was founded, number of offices, number of employees
and additional resources to ensure the completion of the project by the listed deadlines.
Offeror is encouraged to provide an organizational chart.
o Past Performance (Max 15 Pts) – Contractors with no prior experience with any NISD will be
given points based on the references/information provided by clients/End users listed in the
Exhibit A – Relevant Experience portion of the proposal. All contractors with experience with
NISD will be evaluated based on their recent substantially complete projects.
o Timely Completion (Max 3 pts)
o Cooperation with Owner(s) Owner’s Representative, and Consultants (Max 3 pts)
o Coordination of Trades (Max 3 pts)
o Quality of Workmanship (Max 2 pts)
o Warranty Responsiveness (Max 2 pts)
o Punchlist Length & Completion (Max 1pts)
o Safety Record (1pt)
o Subcontractor and Supplier Support Capability (Max 25 Pts) – Offeror will be evaluated based
on subcontractors and suppliers listed under Schedule 2 in the proposal package. Please note all
Items of Work need to have the name of subcontractor/supplier. If multiple names are
provided for a single item of work, contractor runs the risk of submitting a non-compliant bid.

PART 1 – GENERAL
1.01 DESCRIPTION: Furnish and place TENSAR Geogrid (TX-5) in accordance with the lines and grades shown on the plans or as directed by the Engineer.
PART 2 - MATERIALS
2.01 MATERIALS: Provide TENSAR Geogrid (TX-5) in conformance with the Items and requirements stated herein. No material substitutions will be allowed for geogrid on this project unless the contactor can prove the unavailability of the preferred product. In cases that substitutions are allowed by the owner and engineer, the material must conform to the following:
2.01.1 Geogrid Reinforcement. Texas Department of Transportation Materials Specification DMS 6240, “Geogrid for Base/Embankment Reinforcement,” of the type as shown on the plans. The sampling, testing, and rejection criteria of that specification shall govern.
2.01.2 Unapproved Materials. Material substitutions for geogrids not conforming to the physical requirements of TxDOT DMS 6240 must be submitted with an alternative design proposal to the Engineer for consideration. Alternate design proposals must be accompanied by the test data from an approved laboratory showing all design and index properties in accordance with the test properties shown in TxDOT DMS 6240. If approved, the Engineer will provide written authorization. Allow a minimum of 14 days for the approval process.
PART 3 – CONSTRUCTION
3.01 GENERAL:
A. Subgrade soil shall be prepared in accordance with City of San Antonio Specification Item 104, “Street Excavation and Item 107, “Embankment”. The upper 6 inches of subgrade shall be compacted to 95% of maximum dry density as determined by the standard proctor test (ASTM D698). The moisture content of subgrade soils should be maintained between -1 to +3 percentage points of the optimum moisture content.
B. Geogrid reinforcement shall be rolled out parallel to the road direction at the proper elevation and alignment as shown on the construction drawings.
C. Geogrid sections shall be overlapped a minimum of 1 foot in both directions. Placement of geogrid around corners will require cutting of geogrid product and diagonal overlapping. Unless otherwise directed by the Engineer, plastic ties shall be used at overlaps. The transverse spacing

of the ties shall be 4 to 5 feet and the longitudinal tie spacing shall be 10 to 20 feet, unless otherwise approved by the Engineer.
D. The geogrid shall be pinned at the beginning of the backfilling section, but shall be left free to stretch or relieve tension throughout the remainder of the work area.
E. Contractor shall take steps to ensure that geogrid sections do not separate at overlaps during construction.
F. Base material shall be placed and compacted in accordance with the City of San Antonio Specification Item 200, “Flexible Base”. This material shall be back dumped from trucks riding on top of the reinforced base material and bladed on to the grid area.
G. Tracked construction equipment shall not operate directly upon the geogrid. A minimum base thickness of 6 inches is required prior to operation of trucked vehicles over the geogrid.
H. Rubber tired equipment may pass over the geogrid at slow speeds, less than 5 miles per hour if the subgrade material is capable of supporting the loads without excessive rutting or causing damage to the grid. Equipment operators shall avoid sudden braking or sharp turning.
I. If ruts are created in the base material due to construction traffic, they shall be filled with additional base material rather than blading adjacent material into the rut.
J. Sections of geogrid, which are damaged by construction activity, shall be repaired or replaced at the Contractor’s expense. All repaired sections shall contain a minimum 3 foot overlap in all directions.
PART 4 – MEASUREMENT
4.01 MEASUREMENT: Accepted work performed as prescribed by this item will be measured by the square yard of base reinforcement complete in place in accordance with the plans with no allowance made for width of overlaps required.
PART 6 – PAYMENT
5.01 PAYMENT: The work performed as prescribed by this item will be paid for at the contract unit price bid per square yard, measured as prescribed above, for “Base Reinforcement” which price shall be full compensation for furnishing all labor, materials, equipment and other items necessary and incidental to completion of work.

PART 1 – GENERAL
1.01 DESCRIPTION: Construct a base course for surfacing, pavement, or other base courses composed of crushed stone, and constructed as herein specified in one or more courses in conformance with the typical sections shown on the plans and to the lines and grades as established by the Engineer.
PART 2 - MATERIALS
2.01 MATERIALS: Furnish uncontaminated materials of uniform quality that meet the requirements of the plans and specifications. Notify the Engineer of the proposed material sources and of changes to material sources. The Engineer may sample and test project materials at any time before compaction throughout the duration of the project to assure specification compliance.
Base materials should meet the requirements set forth in the Texas Department of Transportation (TxDOT) 2014 Standard Specifications for Construction of Highways, Streets and Bridges; Item 247, Type A, Grade 1-2.
PART 3 - EQUIPMENT
3.01 EQUIPMENT: Provide machinery, tools, and equipment necessary for proper execution of the work. Provide rollers in accordance with Item 210, “Rolling.” Provide proof rollers in accordance with TxDOT Item 216, “Proof Rolling,” when required.
PART 4 – CONSTRUCTION
4.01 GENERAL: Construct each layer uniformly, free of loose or segregated areas, and with the required density and moisture content. Provide a smooth surface that conforms to the typical sections, lines, and grades shown on the plans or as directed. Stockpile base material temporarily at an approved location before delivery to the roadway. Build stockpiles in layers no greater than 2 feet thick. Stockpiles must have a total height between 10 and 16 feet unless otherwise shown on the plans. After construction and acceptance of the stockpile, loading from the stockpile for delivery is allowed. Load by making successive vertical cuts through the entire depth of the stockpile. Do not add or remove material from temporary stockpiles that require sampling and testing before delivery unless otherwise approved. Charges for additional sampling and testing required as a result of adding or removing material will be deducted from the Contractor’s estimates. Haul approved flexible base in clean trucks. Deliver the required quantity to each 100-foot station or designated stockpile site as shown on the plans. Prepare stockpile sites as directed. When delivery is to the 100-foot station, manipulate in accordance with the applicable Items.
4.02 Preparation of Subgrade or Existing Base. Remove or scarify existing asphalt concrete pavement in accordance with Item 104, “Street Excavation,” when shown on the plans or as directed. Shape the subgrade or existing base to conform to the typical sections shown on the plans or as directed. When new base is required to be mixed with existing base, deliver, place, and spread the new flexible base in the required amount per station. Manipulate and thoroughly mix the

new base with existing material to provide a uniform mixture to the specified depth before shaping. When shown on the plans or directed, proof roll the roadbed in accordance with TxDOT Item 216, “Proof Rolling,” before pulverizing or scarifying. Correct soft spots as directed. The upper 6 inches of subgrade shall be compacted to 95% of maximum dry density as determined by the standard proctor test (ASTM D698). The moisture content of subgrade soils should be maintained between -1 to +3 percentage points of the optimum moisture content.
A. Placing. Spread and shape flexible base into a uniform layer with an approved spreader the same day as delivered unless otherwise approved. Construct layers to the thickness shown on the plans. Maximum lift thickness shall be 8 inches of loose material. Maintain the shape of the course. Control dust by sprinkling, as directed. Correct or replace segregated areas as directed, at no additional expense to the City. Place successive base courses and finish courses using the same construction methods required for the first course.
B. Compaction. Compact in courses not to exceed 8 inches compacted depth using density control unless otherwise shown on the plans. Multiple lifts are permitted when shown on the plans or approved. Bring each layer to the moisture content directed. When necessary, sprinkle the material in accordance with TxDOT Item 204, “Sprinkling.” Begin rolling longitudinally at the sides and proceed towards the center, overlapping on successive trips by at least ½ the width of the roller unit. On superelevated curves, begin rolling at the low side and progress toward the high side. Offset alternate trips of the roller. Operate rollers at a speed between 2 and 6 mph as directed. Rework, recompact, and refinish material that fails to meet or that loses required moisture, density, stability, or finish before the next course is placed or the project is accepted. Continue work until specification requirements are met. Perform the work at no additional expense to the City.
a. Ordinary Compaction. Roll with approved compaction equipment as directed. Correct irregularities, depressions, and weak spots immediately by scarifying the areas affected, adding or removing approved material as required, reshaping, and recompacting.
b. Density Control. Compact to a minimum density of 95% of the maximum dry density determined by the modified Proctor test (ASTM D1557). The moisture content of the base materials show be maintained within 2 percentage points of the optimum moisture content. The Engineer will determine roadway density of completed sections in accordance with TxDOT’s standard laboratory test procedure Tex-115-E. The Engineer may accept the section if no more than 1 of the 5 most recent density tests is below the specified density and the failing test is no more than 3 pounds per cubic foot below the specified density.
C. Finishing. After completing compaction, clip, skin, or tight-blade the surface with a maintainer or subgrade trimmer to a depth of approximately ¼ inch. Remove loosened material and dispose of it at an approved location. Seal the clipped surface immediately by rolling with a pneumatic tire roller until a smooth surface is attained. Add small increments of water as needed during rolling. Shape and maintain the course and surface in conformity with the typical sections, lines, and grades as shown on the plans or as directed. In areas where surfacing is to be placed, correct grade deviations greater than ¼ inch in 16 feet measured longitudinally or greater than ¼ inch over the entire width of the cross-section. Correct by loosening, adding, or removing material. Reshape and recompact in accordance with Section 200.4.C, “Compaction.”
D. Curing. Cure the finished section until the moisture content is at least 3 percentage points below and above optimum or as directed before applying the next successive course or prime coat.

5.01 MEASUREMENT: Flexible base will be measured by the square yard method per thickness shown in the proposal. Measurement by the square yard is a plans quantity measurement. The quantity to be paid for is the quantity shown in the proposal unless modified by the Engineer. Additional measurements or calculations will be made if adjustments of quantities are required. Measurement is further defined for payment by the square yard of surface area in the completed and accepted final position. The surface area of the base course is based on the width of flexible base as shown on the plans.
PART 6 – PAYMENT
6.01 PAYMENT: The work performed, and materials furnished in accordance with this Item and measured as provided under “Measurement” will be paid for at the unit price bid for the types of work shown below. No additional payment will be made for thickness or width exceeding that shown on the typical section or provided on the plans for square yard measurement.
Sprinkling and rolling will not be paid for directly but will be subsidiary to this Item unless otherwise shown on the plans.
Where subgrade is constructed under this Contract (Subgrade Treatment), correction of soft spots in the subgrade will be at the Contractor’s expense. Where subgrade is not constructed under this project, correction of soft spots in the subgrade will be paid in accordance with pertinent Items.
Payment will be made for the type and grade specified. For square yard measurement, a depth will be specified. This price is full compensation for furnishing materials, temporary stockpiling, assistance provided in stockpile sampling and operations to level stockpiles for measurement, loading, hauling, delivery of materials, spreading, blading, mixing, shaping, placing, compacting, reworking, finishing, correcting locations where thickness is deficient, curing, furnishing scales and labor for weighing and measuring, and equipment, labor, tools, and incidentals.

A. This item shall govern for concrete sidewalks and driveways, composed of Portland Cement concrete, constructed as herein specified on an approved subgrade, in conformity to the lines, grades and details shown on the plans or as established by the OWNER.
1.02 SUBMITTALS
A. The Contractor shall submit the following in accordance with the provisions of Section 01 33 23, “Shop Drawings, Product Data and Samples.”
B. A list identifying the types and sources of materials proposed for this work.
1. Laboratory test reports demonstrating compliance with these specifications.
2. Material certificates signed by the material producer and the Contractor certifying that each material item complies with or exceeds specified requirements.
PART 2 PRODUCTS
2.01 CONCRETE
A. All concrete for sidewalks, curb and gutter, and driveways shall conform to Item 300, “Concrete” of the City of San Antonio Standard Specifications for Construction. Concrete shall be Class A unless otherwise specified or indicated on the Drawings and Details.

PART 3 CONSTRUCTION
3.01 SIDEWALKS, DRIVEWAYS, CURB, AND SIDEWALKS WITH FOOTINGS
A. The subgrade shall be excavated and shaped to the lines, grades and cross section shown on the plans, or as directed by the Owner, and shall be thoroughly compacted. A cushion, 2-inch minimum thickness, of crusher screenings, gravel, crushed rock or flex base material shall be spread, wetted thoroughly, tamped and leveled. The cushion shall be moist at the time the concrete is placed. If the subgrade is undercut, or the natural ground is below "top of subgrade" then necessary backfill shall be made with approved material and compacted with a mechanical tamper. Hand tamping will not be permitted.
B. Where the subgrade is rock or gravel, 70% of which is rock, the 2-inch cushion need not be used. The Owner will determine if the subgrade meets the above requirement. Forms shall be of metal or well-seasoned wood of a section satisfactory to the OWNER; clean, straight, free from warp, and of a depth equal to the thickness of the finished work. All forms shall be securely staked to line and grade maintained in a true position during the depositing of concrete. Before concrete is placed, forms shall be thoroughly oiled with a light form oil.
C. Expansion joint material, 1/2-inch-thick, shall be provided at intervals not to exceed 50 feet and where the new construction abuts the existing curbs or driveways if the Inspector deems it necessary. The expansion joint material shall be placed vertically and shall extend the full depth and width of the concrete.
D. Round smooth dowel bars ½ inch in diameter and 14-inches in length shall be spaced 12-inches apart at each expansion joint. Seven inches of each dowel shall be thoroughly coated with hot oil asphalt or greased, so
that it will not bond to the concrete. Approved types of slip joints may be used in lieu of coating ends of dowels.

E. Concrete driveway shall be reinforced as shown on the plans. Reinforcement for driveway No. 4 (1/2") reinforcing steel placed not more than 12 inches on centers both directions. All reinforcement shall be placed equidistant from the top and bottom of the concrete. Care shall be exercised to keep all steel in its proper position during the depositing of concrete. Splices in the No. 4 bars shall have a minimum lap of 30 bar diameters. Concrete driveway pavement shall have a minimum of 6” flexible base material as per Item 32 1116.16 “Flexible Base”.
F. Concrete shall be placed in the forms and spaded, tamped and thoroughly compacted until mortar entirely covers the surface and has a monolithic finish. The top surface shall be floated and troweled to a uniform smooth surface, then finished with a camel hairbrush or wood float to a gritty texture. The outer edges and joints shall be rounded with approved tools to the radii shown on the plans.
G. Immediately after finishing, the surface shall be protected by a membrane curing compound, or by wetted cotton or burlap mats. Either method shall be subject to approval by the Inspector.
H. All necessary excavation for the sidewalk section, will be considered incidental work pertaining to this item, and will not be paid for directly. The adjacent excavation and grading of the slopes shall he done in a manner acceptable to the OWNER.
PART 4 MEASUREMENT AND PAYMENT

MECH-ELEC-PLBG ITEMS FOR ADDENDUM NO. 2 NOTICE TO PROPOSERS:
A. This Addendum shall be considered part of the contract documents for the above-mentioned project as though it had been issued at the same time and incorporated integrally therewith. Where provisions of the following supplementary data differ from those of the original contract documents, this Addendum shall govern and take precedence.
B. Proposers are hereby notified that they shall make any necessary adjustments in their estimate on account of this Addendum. It will be construed that each Proposer’s proposal is submitted with full knowledge of all modifications and supplemental data specified therein. Acknowledge receipt of this addendum in the space provided on the proposal form. Failure to do so may subject Proposer to disqualification.
REFERENCE IS MADE TO THE DRAWINGS AND THE PROJECT MANUAL AS NOTED: PROJECT MANUAL: AD No 2, MEP. Item 1: To the Project Manual, Section 23 0700 (HVAC INSULATION).
1. Part 3 Execution: ADD Par. 3.1.E. Prime ALL indicated Mechanical Room chilled water piping, fittings, unions, flanges, fittings, nuts, bolts/threads, etc. with rust-colored steel pipe primer (Rustoleum, or approved equal).
