To avail Group Medical Insurance coverage in 2022, please review and renew/enroll self, dependents (Spouse & children) and / or dependent parents from January 1, 2022 till January 31, 2022. HDFC Ergo General Insurance Company Limited will be your Insurer while Medi Assist Insurance TPA Pvt. Ltd. will service as the Third-party administrator (TPA) for the Group Medical Insurance Coverage (GMC) policy of IBM India in the calendar year 2022. Both HDFC Ergo General Insurance and Medi Assist will facilitate administration of the policy for IBM India employees.

Please note that all IBM India employees (Regular and Fixed Time Hires) who opts for additional top-up, and parents’ coverage, in 2021 policy, will continue to have the same cover in 2022 policy, owing to the two-year lock-in of 2021 & 2022 for both top-up & parental selections.

Basis the above declaration, please find below the actions available under 2022 enrolment window for employees having their date of joining before January 1, 2022 on the Medi Buddy portal.

Base plan

By default, all active IBM India employees will be enrolled for base coverage 3 Lakhs family floater effective Jan 1 under GMC 2022.

Employees can enroll their dependents (Spouse and children) afresh / verify existing dependents details / remove dependents under GMC 2022 by Jan 31,2022 OR If If do not wish to continue under GMC 2022, can OPT OUT of MediBuddy portal by Jan 31, 2022

Note: Owing to 2 year lock-in there is no option for modifying the parental or top up policy coverage.

Employees joining IBM on or after January 1, 2022, have the following actions available under enrollment window of 2022 or within 30 days of their joining date.

• Choose to cover their spouse and/or children under the employee policy for the base cover 3 lacs (with or without additional top-up).

• Choose to cover their parents under the parent’s policy 2022 (individual/floater) • Option to avail top-up coverage in 2022 • Choose to opt-out of the employee policy in 2022.

Employees who are on LOA (GI, HCAM, US Onsite etc)

• By default, all employees are covered for base cover (3Lakhs) under GMC 2022 effective Jan 1,2022 and premium co-share need to be paid (deducted in March’22 salary) unless and until employee performs OPT-OUT action in medi-buddy portal before Jan 31,2022.

• Employees can add their dependents (Spouse & children) / opt for top-up cover / enrol parents however coverage will be limited to Indian grounds.

• Employee who has availed top-up coverage in 2021, cannot de-select / opt out of 2022 policy.

• Employees who enrol their parents in 2021, cannot remove parents under 2022 policy.

• Once the employee opts out of the policy, they & their immediate family are not eligible for any benefits.

• Employee who opts-out from ESC policy, will still be eligible to enrol their dependent parents into the parental policy under 2022 policy by Jan 31,2022.

• Once an employee has opted out of the 2021 policy, they may re-join the policy in 2022 for base cover INR 300,000 only but will not be eligible for TOP UP or Parental enrolment in 2022.

• The employee can log in to Medi Assist website (https://portal.medibuddy.in) or logon to the MediBuddy mobile app to opt out for 2022 policy year by Jan 31,2022.

• In case employee opts out, employee would only be eligible for hospitalization benefits on account of accident during the course of employment and/or treatment of occupational diseases, as required under applicable law for a sum insured of INR 200,000.

Employee, Spouse and Children Policy (Both Regular and Fixed Term Hires)

• All IBM India regular (full time and part time) and Fixed term hire employees may avail medical insurance coverage

• The coverage under this policy is INR 3,00,000 family floater for the nuclear family (nuclear family defined as employee, spouse and up to 4 dependent children)

• Employees will have the opportunity to renew/review their enrolments to the Group medical Insurance Policy from January 1 till January 31, 2022

• Employees who wish to edit member details, may log in to the Medi Assist’s MediBuddy portal(https://portal.medibuddy.in) using their User ID and password to subscribe. Alternatively, they can also log in to MediBuddy mobile app (for iOS and Android) to enroll. Please scroll down to learn how to complete online enrolment.

• The employees who wish to opt out of the ESC policy 2022 (both regular and Fixed Term Hires) are required to opt-out of the policy by logging in to MediBuddy and marking the opt-out option. The employees who do not submit the opt-out declaration within the renewal period will be covered under the policy by default, starting January 1,2022 and premium co-share for the same will be deducted from their salary.

• The Opt out option will be disabled for employees in 2022 with top-up coverage selected in 2021 owing to the 2-year lock-in on the voluntary top-up and voluntary Parental coverage for 2021 & 2022.

Additional coverage under family floater (Regular and FTH’s with tenure > 12 months)

• An employee joining on or after 2nd January 2022 availing the insurance policy has the option of buying additional coverage for his or her nuclear family inexcess of INR 3,00,000 up to a maximum of INR 7,00,000 (total maximum of INR 10,00,000).

• Available top-up options are INR 1 lac, INR 2 lacs, INR 3 lacs, INR 4 lacs, INR 5 lacs, 6 lacs and 7 lacs • The incremental premium including the goods and services tax incurred due to additional

coverage will be deductedfrom the employee’s salary. Please refer to Medi Assist portal (https://portal.medibuddy.in) for premium rate chart.

• Employees who have joined on / between December 2nd 2021 to December 31, 2021 will have both 2021 and 2022 enrolment windows open simultaneously while 2021 enrolment window will remain open till 30 days from their Date of joining, 2022 renewal window will be open till Jan 31,2022.

• New hire employees joining IBM on or post January 1, 2022 can opt for 2022 additional top- up coverage directly in MediAssist website (https://portal.medibuddy.in) or alternatively log in to Medibuddy mobile app.

• Annual premium for additional coverage under ESC policy (inclusive of applicable GST (currently @ 18%)) is as follows:

Please note, selection of additional coverage/ top-up also enables enhancement in the OPD sub-limit. The OPD sub-limit specified above is inclusive of base OPD coverage benefit of INR 10,000 and enhanced OPD amount (to a maximum of INR 25000) as applicable due to selection of the respective additional top-up coverage.

Parent(s) Policy (Both Regular and FTH’s with tenure > 12 months)

• Employees who has opted for parental coverage in 2021 policy will continue to have the same members, credentials or coverage under 2022 parents’ policy. Owing to the two year lock-in, there is no option for employees to increase/decrease parents cover, add/remove parents, change opted plan (floater/individual) or instalment selection afresh in 2022 policy.

• Under the 2022 Parent's policy, the employees can have their parents covered as under:

• Employees enrolling both parents may choose to cover them under a common floater coverage or under separate individual coverage as required

• Employees enrolling only one parent can make declaration under-individual policy

• An employee availing the policy can cover his/her dependent parent(s) (under individual coverage)/ parents (underfloater coverage) for sum insured options INR 1 lac, INR 2 lacs, INR 3 lacs and INR 5 lacs

• The premium for covering parents including the goods and services tax will be deducted from the employee's salary. Please refer Medi Assist portal (https://portal.medibuddy.in) for premium rate chart.

• Employees who have joined on / between December 2, 2021 to December 31, 2021 will have both 2021 and 2022 enrolment windows open simultaneously and can choose to enroll parents under 2021 policy for coverage upto Dec 31,2021.

• The employees have an option of paying the premium under the parents’ policy as a lump sum amount, or as 2 equal instalments deducted in the months of February and March 2022 (basis the selections made in 2021 policy)

• If an employee does not opt/avail the parents’ premium instalment option in the policy year 2021, premium for parents’ policy for 2022 will be deducted by default as a lump sum from his/her salary in the month of March 2022 under 2022 policy

• The premium amount for the parents’ coverage under parents’ individual coverage and floater coverage is fixed as per the enrollment opted in 2021 policy

• Coverage will be extended to parent(s) above 90 years age if the parent(s) have been covered under the policy on a continuous basis; All such requests should be sent to [email protected]/[email protected] along with the parents’ name, gender, date of birth, city of residence, the city’s pin code and the desired sum insured coverage, individual/ floater plan , premium instalment option if required. The enrolment will be done on a case-to-case basis, and parent details will be incorporated on the portal from the backend.

• The premium rates under the parent policy remains constant in the 2022 policy even if the age band of parent(s) changes. The total premium payable may change with the impact of applicable taxes.

• Annual premium under the parents’ policy (inclusive of applicable GST (currently @ 18%), on premium is as follows:

Key Changes in GMC Policy 2021 & 2022

• 12 Advance procedures covered in the policy.

• 78 Non-Medical items included in the policy.

• Enhanced day care list.

• Age-related macular degeneration treatment – Covered o ESC (up to Sum Insured) o Parental policy (INR 50,000 sublimit)

• Surrogacy coverage up to the maternity sub limit.

• Infertility treatment covered twice in the employment tenure.

Please note:

No member can be covered twice in the policy even if he/she is a dependent of more than one employee. If two or more siblings working with IBM India are found to have enrolled their parents for more than once under the policy, it will be considered as BCG violation and strict action will be taken.

Co-share and Premium applicable for 2022 Health Plan (Regular and FTH employees)

Co-share on Premium

• The premium for Insurance coverage for self and immediate dependents (spouse and children) is shared between the employee and IBM. (20: 80)

• If an employee avails insurance coverage, there would be a deduction of INR 1847.40 (1,565.59+ GST 281.70) per annum from a regular employee’s salary and a deduction of INR 1072.19 (INR 908.63+ GST 163.55) per annum from a fixed term hire employee’s salary in the month of March 2022. Premium for additional coverage (top-up) will be borne by the employee, and will be deducted from the employee’s salary in the month of March 2022

• Employee contribution towards premium for parents’ coverage will be deducted from the employee's salary in the month of March 2022 as a lump sum, or in the months of February 2022 and March 2022 as equal instalments basis the selection made on the parents’ premium instalment option in 2022.(Refer to parents policy section for complete details).

• There would be an additional goods and services tax levied on the insurance premium. • This amount will be eligible for deduction from taxable income within the defined

limits under Section 80 D of the Income Tax Act. • This premium deduction does not require declaration in the investment module of You and IBM tool a

s the deduction will happen automatically from employee’s salary • The Parent Policy also attracts an additional Third-Party Administrator (TPA) charge

of INR 80 per parent peryear and an additional goods and services tax on the same which will also be deducted from the employee’s salary.

Co-pay on Claim

• Co-pay on claim will be 10% for self and 20%for dependents (spouse & children).

• Co-pay on claim will continue to be 20% for dependent parents.

To Unsubscribe 2022 policy (OPTOUT)

• If you wish to unsubscribe and therefore not avail the benefits for yourself, spouse and children (if applicable), please login to Medi Assist portal (https://portal.medibuddy.in) before January 31, 2022. You will be required to tick mark on the OPTOUT declaration

• If you do so before the stipulated time, no premium will be deducted from your salary in March 2022. However, you will still be provided coverage for hospitalization expenses during instances of personal injury caused by accidents or occupational disease. This is as per IBM employment policy which will apply only to the employee and not for dependents.

• Once an employee opts out of the group hospitalization policy he/she will NOT be eligible to re-subscribe during thecourse of the same calendar year. There will be no exception to his policy hence, please be sure before making any decision.

• Employee who wish to opt-out from ESC policy (Employee Spouse & Children) they will still be eligible to continue to cover their parents under the parental policy (if they have enrolled in the policy in 2021)

• Employee joining post 1st January 2022, and who wish to opt-out from ESC policy (Employee Spouse & Children) they will still be eligible to enrol their dependent parents into the parental policy within the enrolment window period time.

How to complete Online Enrolment procedure?

Here are the steps given below for enrolling through the MediBuddy portal

Step 1: Click on the link https://portal.medibuddy.in

Step 2: Enter your User ID and Password

Please use your employeeID@IBM as the username. An initial default password has been set up for you using acombination of your date of birth and your employee id. For example, if your employee id is 123456, yourusername would be 123456@IBM and if your date of birth is 30-November-2014, your initial password would be30112014123456. (ddmmyyyy followed by empid)

Step 3: It’s mandatory to change your password, before you can access any details.

Note: The password is set to default at the beginning of each policy year and can be changed later by the employee as per their convenience.

i. It is mandatory to review and update Self and dependent details

1. Go to “Your Health Policy” box on the home page 2. Click on “Enrolment” 3. To add or edit any details please click the “Add” button or “Edit” button (as required and applicable)

4. To add dependents, click on the “Add+” button on the top right corner of the section

5. Click “Submit” at the bottom of the page

• How to login on the mobile app:

Enter your User ID and Password

2. Please use your MEDIBUDDY portal password. In case you have not logged in to the portal even once and your first interaction is with MediBuddy mobile app, use a combination of your date of birth and your employee id as the password. For example, if your date of birth is 30-November 2014, your initial password would be 30112014123456. (ddmmyyyy and empid continuously)

3. Post entering your username and password for login, you will be required to input an OTP on the application. The OTP is shared to your official mail ID.

Note: The password is set to default at the beginning of each policy year & can be changed later by the employee as per their convenience.

Enrolling through the app:

After you have successfully logged in:

i. Land on the home screen. Choose ‘Enrolment’ ii. Fill in the details as required (mobile number, location, date of marriage (if applicable))

iii. Tick for Mobile Declaration. *This is mandatory* iv. Opt out declaration- If you do not want to enroll your nuclear family tick the check box v. If not opted out, you can fill in/review details of your spouse and at max 4 children, and

choose/review top up cover as you scroll down on the page vi. Input parents’ details under the Parental Coverage section, and select the type of coverage and

the coverage amount for each of them (if covered individually), else choose the common floater sum insured for both

vii. Check the details of your enrolment. If all details are correct, click on the ‘Submit’ button, and tap ‘Ok’ in the pop-up box.

viii. The page will reload after successful submission of details, and the same will be confirmed by a pop-up

Enrolment Tutorial

Introduction

Welcome to the MediBuddy App tutorial. Now you can access the features of MediBuddy portal on your MediBuddy App as well. Here is a step-by-step guide on how to use your MediBuddy App.

1. Download the MediBuddy app on your IOS or Android smart phone. Sign in with your username and password by clicking on “ I have a Corporate Account”

Login ID: Employee ID@IBM

Password: Date of Birth in DD/MM/YYYY format followed by your employee ID

For example, if your employee ID is 123456, your username would be 123456@IBM and if your date of birth is 30-November-2014, your initial password would be 30112014123456.

An OTP is shared to your official email ID. Please submit the same to login to the application on your phone.

Note: Please change your password after the first login.

2. Click on “enrolment” from the home page

3. On the home page, select ‘Enrolment’ to view your enrolment details. Add/review your dependents for 2022 GMC policy. Go to the field that you would like to add/modify by scrolling the page up and down.

Note: You can add dependents only during enrolment window period (Jan 1 to Jan 31st,

2022) or within 30 days of joining in case you are a new hire, i.e., joining IBM India on or

post Jan 2, 2022

4. View personal information and make necessary changes to your profile:

• Please review & update your personal details such as mobile number, location, etc.

• It is mandatory to provide a valid mobile number and accept the mobile declaration

• Update your “Work Location”

• If you wish to enrol your spouse, ensure Marital Status is set to “Married” and “Marriage Date” is updated

• Please update your Bank Details by clicking on “Update Bank Details”

5. Check “OPT OUT Declaration” only if you intend to opt out of IBM Group Hospitalization

Scheme. Please note that your nuclear family (spouse and children) is not covered if you opt out.

Dependent Enrolment:

• Spouse details can be added by clicking on the “Add” link next to the “Beneficiary Details” label, if a spouse is not already added

• If a spouse is already added, you can edit the details by clicking Edit or Delete button

• You can enrol up to four dependent children (till they attain the age of 24 years) into the plan

• Please do read and accept the “Child Declaration” if you enrol any child

Note: You can add differently abled children with no age cap along with a valid

disability certificate shared in jpeg, jpg, png or PDF format with a size limit of 5 MB.

6. Parents enrolment

• Add details of your parent(s)

• Please note that the premium for parents is fully paid by the employee. Premium details

for the selected cover(s) are shown instantly upon selection of sum insured and date of

birth

• You need to mandatorily provide the location and pin code for each of the parents covered • Employees enrolling both parents can avail common floater coverage for both parents, or an

individual coverage for each parent. The premium applicable under floater coverage will depend on the age band of the elder parent and the sum insured selected, while the premium applicable under the individual coverage will depend on each parent's age band and the respective sum insured selected for each.

Note: You can add dependents only during enrolment window period (Jan 1 to Jan 31st,

2022) or within 30 days of joining in case you are a new hire, i.e., joining IBM India on or

post Jan 2, 2022.Also the floater/individual option will reflect once the employee will add

his second parent.

7. Instalment Option

By checking on this box, you will be opting to make the payment towards parent’s premium in two equal instalments to be deducted after window period closure. This option is voluntary, and only available during the annual enrolment window.

If you do not tick the checkbox, by default it implies that you intend to pay the premium amount for the parents cover as a lump sum which will be deducted from your salary in the month of March 2022 for the current year policy. Similarly, annual deduction of premium will be made in the month of March 2022 for the 2022 policy.

8. Top up cover allows you to enhance the sum insured available to you and your nuclear family (spouse and children). Premium payable for the top up cover selected is displayed instantly upon selection of the required sum insured.

9. You need to click the “Submit” button to confirm your changes. Please note that you can edit the enrolment details and save them multiple times during the window period. No edits will be allowed post the 30 days window period. Mid-term addition of spouse and child is allowed all through the policy year, subject to policy terms and conditions.

10. Click the Download eCard button from ‘Your health policy’ tile. Your eCard is downloaded

in the pdf format. Please note: For employees having both 2021 & 2022 enrolment windows (dual window option) open basis their date of joining will have to take top-up and parental coverage in 2021 enrollment page over the portal only.

Introduction to MediBuddy Portal

Welcome to the MediBuddy portal tutorial. Now you can access all policy features on your MediBuddy portal - in a faster and more efficient way. Here is a step-by-step guide on how to use your MediBuddy portal.

1. Go to https://portal.medibuddy.in from your web browser. Sign in with your username and

password.

Login ID: Employee ID@IBM

Password: Date of Birth in DD/MM/YYYY format followed by your employee ID

For example, if your employee ID is 123456, your username would be 123456@IBM and if your date of birth is 30-November-2014, your initial password would be 30112014123456.

Note: Please change your password after the first login.

2. On successful login, you will see the following page from which, you can perform the required action.

3. Click the enrolment button on ‘Your health policy’ tile. The following page appears.

Existing users can review details of Self, Spouse & Child/Children only. Dependent details

have to be added/reviewed for 2022 policy.

4. You can view the employee information and make necessary changes to your profile. You can

add/review your beneficiaries inclusive of your spouse, parents and up to 4 dependent

children.

• Please review & update your personal details such as mobile number, email address. It is

mandatory to provide a valid mobile number and accept the mobile declaration

• If you wish to enrol your spouse, ensure Marital Status is set to “Married” and “Marriage Date” is updated

• Update your “Work Location”

• You can update your “Bank Details” by clicking on the link titled “Click here to update”

placed next to the “Bank Details Status” label. Please note that click on this link opens a pop-up window where you can update your bank details. Please close the pop-up after updating your bank details.

5. Check “OPT OUT Declaration” only if you intend to opt out of IBM Group Hospitalization

6. Dependent Enrolment • Spouse details can be added by clicking on the “Add” link next to the “Beneficiary

Detail” label, if a spouse is not already added • If a spouse is already added, you can edit the details by clicking on “Edit/Remove”

link next to the “Beneficiary Detail” label

• You can enrol up to four dependent children (till they attain the age of 24 years) into the plan

• Please do read and accept the “Child Declaration” if you enrol any child

7. Parents Enrolment • Add/review details of your parent(s)

• Please note that the premium for parents is fully paid by the employee. Premium details

for the selected cover(s) are shown instantly upon selection of sum insured and date of

birth • You need to mandatorily provide the location and pin code for each of the parents covered • Employees enrolling both parents can avail common floater coverage for both parents, or an

individual coverage for each parent. The premium applicable under floater coverage will depend on the age band of the elder parent and the sum insured selected, while the premium applicable under the individual coverage will depend on each parent's age band and the respective sum insured selected for each.

Note: You can add dependents only during enrolment window period (Jan 1 to Jan 31st,

2022) or within 30 days of joining in case you are a new hire, i.e., joining IBM India on or

post Jan 2, 2022. Also the floater/individual option will reflect once the employee will add

his second parent.

8. Instalment Option

By checking on this box, you will be opting to make the payment towards parent’s premium in two equal instalments in the months of Feb 2022 and March 2022. This option is voluntary.

If you do not tick the checkbox, by default it implies that you intend to pay the premium amount for the parents cover as a lump sum which will be deducted from your salary in the month of March 2022 for the current year policy. Similarly, annual deduction of premium will be made in the month of March 2022 for the 2022 policy

* Please note, the premium will be charged basis sum insured selected and the completed

age of parent(s) as on January 1, 2022 for employees with date of joining as or before

January 1,

2022. For employees joining on or after January 2, 2022, the parents’ age completed as on the date of joining will be considered for premium calculation.

9. Choose Top up Cover

Top up cover allows you to enhance the sum insured available to you and your nuclear family

(spouse and children). Premium payable for the top up cover selected is displayed instantly upon selection of the required sum insured.

10. Save

Check all declaration boxes and click the “Submit” button to confirm your changes. Please note that you can edit the enrolment details and save them multiple times during the window period. No edits will be allowed post the 30 days window period. Mid-term

addition of spouse and child is allowed all through the policy year, subject to policy terms and conditions.

11. View Policy

Click the View policy button from ‘Your health policy’ tile. You can view all the details pertaining to your policy cover.

12. Download eCard

Click the Download eCard button from ‘Your health policy’ tile. Your eCard is downloaded in the pdf format.
