Socios En Salud is a medical organization, a subsidiary of the global Socios En Salud organization and affiliated with Harvard Medical School. Since 1996, it has worked in Peru with the mission of giving a preferential health care option to its poorest people. Uniting diverse efforts, Socios En Salud seeks to bring the best of science and modern medicine to the most vulnerable and disadvantaged communities. Socios En Salud is one of the most reputable non-governmental organizations (NGOs) in Peru, having been recognized in 2020 as one of the Peruvian NGOs that had the most resources and impact in the country.

REPORT OF CLINICAL ASSISTANCE AND HUMANITARIAN SUPPORT

With the appearance of COVID-19, Socios En Salud accelerated its activities and quickly responded with tests, contact tracing, clinical assistance and various resources for the communities most affected by the Coronavirus. Soon after, Socios En Salud adapted its clinical programs to mitigate the impact of the pandemic and ensure the safety of patients, health personnel and the various communities it supports. The effort included innovation and technology, as well as strengthening the oxygen ecosystem at different levels of public health care. With the support of various allies and public and private cooperators, this intervention has been expanded to the national level.

Socios En Salud continues its work and has not stopped its activities; Therefore, it presents this report in order to publicize the COVID-19 response in Peru during the first year of the pandemic (March 2020-March 2021).

If we want to stop the spread of COVID-19, we need to take advantage of diagnostic technologies and community mobilization to implement a systematic and widespread detection and surveillance system for COVID-19 in Peru. This objective includes offering rapid diagnostic and molecular diagnostic tests to the community and health personnel and providing them with appropriate personal protective equipment. As in any response to an epidemic, the level of control is based upon quickly finding new cases and isolating them.

We were the first Peruvian organization to provide Peru’s Ministry of Health, in April 2020, the initial batch of 30,000 rapid tests which helped to implement the first rapid response teams in the country that went to affected communities to detect the COVID-19 virus.

Since then, we have donated 68,200 rapid tests and 7,610 SARS-Cov-2 tests using the molecular PCR tests were performed free of charge in our research laboratory, which has the authorization of the National Institute of Health of Peru.

In addition, we have tracking teams, made up of doctors, nurses and nursing technicians, distributed in North Lima and East Lima. Their function is to track COVID-19 contacts through rapid and molecular testing.

Early Detection at COVID-19 Clinics

To prevent crowds in hospitals and to curb Coronavirus infections, the Ministry of Health, through the Directorate of Integrated Health Networks (DIHN) in North Lima, set up spaces for citizens to access testing results and rule out infection by the virus.

The actions deployed seek for the first level of care (health centers and clinics) to have the capacity to identify, diagnose and treat a case of COVID-19.

Rapid Response Teams (RRT) carry out epidemiological surveillance of suspected cases of COVID-19. These primary care teams are made up of health professionals who travel to the districts

of East Lima and North Lima to actively search for COVID-19.

"Covid Maskaq," which means "the one who looks for COVID," is a modern mobile laboratory properly equipped to carry out molecular diagnostic tests to detect SARS-Cov-2.

"We have watched the construction of this mobile laboratory since its first prototypes, and we are happy that it is now a reality and can contribute to the population," says Dr. Leonid Lecca, general director of Socios En Salud, who together with USAID have donated this modern mobile laboratory to the National Institute of Health - NIH of the Ministry of Health of Peru.

Dr. Claudio Ramírez, director of the Directorate of Integrated Health Networks of North Lima (DIHN North Lima), assures all that Socios En Salud is a strategic ally because they seek to improve people’s quality of life,

which implies, fundamentally, addressing worry about the most relevant diseases such as tuberculosis and now, COVID-19.

The Socios En Salud Laboratory, located north of Lima, has the authorization of the National Institute of Health of Peru and a highly trained team for the diagnosis of SARS-Cov-2 using real-time PCR molecular tests.

Hospital improvement and support to public health institutions

Since March 2020, the battle began in health facilities and hospitals across the country for the management of symptomatic and serious cases of COVID-19. Faced with this situation, Socios En Salud, in coordination with various public health institutions, decided to invest in the protection of health professionals on the front line through respiratory and personal protection equipment, medical emergency beds, furniture and temporary mortuaries throughout Metropolitan Lima, Lima’s Provinces and more than 10 of the country’s interior regions.

Temporary Oxygenation Attention Center "Carabayllo"

In alliance with the United States Agency for International Development (USAID) and

Socios En Salud, in coordination with the Directorate of Integrated Health Networks

of Lima Norte, of the Ministry of Health, the donation of 45 portable oxygen devices was made and 45 beds are to be assigned to the

Temporary Oxygenation Attention Center (CATO) in Carabayllo. This center takes in patients who have a mild clinical diagnosis

of COVID-19 and with an oxygen saturation no less than 90%. They will be referred to

health centers in Carabayllo, Puente Piedra and Comas, as well as the Hospital de Apoyo de Carabayllo and the National

Sergio E. Hospital. Bernales.

In addition, 6 oxygen concentrators were donated to the Temporary Oxygenation

Center of Ancón, all of this to strengthen the primary care (health centers and posts)

of the DIRIS Lima Norte.

Faced with the growing need for oxygen to care for patients with respiratory failure, our commitment has been to strengthen the response capacity of hospitals and health centers, through the implementation of medical gas networks and Mainfol oxygen generator cylinder panels. To date, it is estimated that more than 6,746 patients have benefited from respiratory support.

Likewise, an alternative made available by technology is oxygen concentrators which extracts oxygen from the air to deliver it in a purer form and they will be delivered to health facilities, hospitals and temporary oxygen care centers.

To reinforce the operation of the COVID points and coordinate with the teams of the national TB and HIV programs of the Ministry of Health, doctors and nurses were hired to provide care. 2Country TB Proyect: Closing gaps in the national response to tuberculosis with an emphasis on multidrug resistance, vulnerable populations (pediatric and PPL) and community health systems July 2019 - June 2022

On March 12, 2021, the Global Fund issued a Guidance Note stating that indicates that up to 10% of the project budget can be used in the response to the COVID-19 emergency. The Ministry of Health accepts the aid through the official letters 112-2020-DVMSP-MINSA (1st part, March 25) and 185-2020-DVMSP-MINSA (2nd part, May 8).

Global Fund Support to the COVID-19 Response

Strengthening of national labs

In order to strengthen the laboratories with equipment according to the initial diagnosis of needs, biochemical and hematological analyzers, arterial gas analysis equipment and supplies necessary for the management of patients with COVID-19 have been delivered. These kits will also be useful for people affected with other health problems in the jurisdiction of the COVID point or other nearby service stations.

In addition, through the National Institute of Health (NIH), laboratory personnel were trained to diagnose COVID-19 with the LAMP method. Case detection is a bottleneck that has made it difficult to control the pandemic. The LAMP method is an alternative, faster and more accessible method, with a sensitivity comparable to that obtained in the technique RT-PCR.

Through an agreement signed with UNAIDS and through the Emergency Cash Transfers Project for COVID-19, 635 migrant families and vulnerable populations were benefited with a bonus of 760 Peruvian soles. COVID-19 has demonstrated the importance of using telehealth to provide care, especially as a means of reducing the risk of transmission. This work has transcended the response to the crisis, strengthening the response capacity of our health system; In this sense, it was necessary to optimize the processes related to differentiated remote care for people with COVID-19 and those without COVID-19, with adequate reference flows for pregnant women, children and people with health problems, such as diabetes mellitus, hypertension, tuberculosis, and HIV, among others.

In addition, remote care for the most vulnerable groups affected by the COVID-19 pandemic was complemented by sending messages and video calls.

There is an urgent need to activate virtual and community-based healthcare strategies for these communities. Therefore, using technology, it is necessary to implement teleconsultations and tele-mentoring that link patients with health providers for community education, counseling, medication delivery and visits to patients' homes.

Due to the limitations that the pandemic has generated in the framework of COVID-19, Socios En Salud established the strategy for home delivery of medicines through community health agents; However, it was necessary to reach more patients with chronic diseases, and in January 2021, in coordination with the Directorate of Integrated Health Networks Lima Norte, the service “Medications to your Home” was implemented in the districts of Carabayllo and Puente Piedra. We have many years of experience in mobilizing community health agents or promoters (community volunteers) and we know of their key role in attending to various health problems, so we seek to enhance their capacities and integrate them into the efforts of the health services in the education, search and identification of patients and families affected by COVID-19 and other health problems.
