Life

Benefit Description: This benefit provides financial protection and security for an employee’sBeneficiaries, in the unfortunate event of the employee’s death. The benefit amount will be paid as alump sum and it can be either a fixed amount or calculated as a factor of the employee’s GrossAnnual Salary, subject to an overall agreed maximum sum.

Eligibility: Employees of the insured company are eligible for cover if they are:

• Under 65 years of age.• Actively at Work.

Eligible employees who are not at work due to sick leave or disability at the start date of the insurancecontract will be admissible after completing two months of full time employment.

Extended Life Benefit for Insured Members over 65: Normally, cover ends once the Insured Member reaches the age of 65. However, covermay be extended to the age of 70, if required by the company as part of the insurance contract. Under this extended cover the sum insuredwill be reduced proportionally each year.

For example:

Cover will end once the Insured Member reaches the age of70.

Single Event Limit: In the event of a single accident or naturaldisaster resulting in multiple claims, an overall maximumbenefit limit will apply. In the event that this limit is reached,the maximum benefit paid will be apportioned between theBeneficiaries.

Exclusions: Life benefits will not be paid if death was as aresult of:

• Active participation in war or insurrection.• Suicide within one year of the enrolment date.• Being in a country during war, where the British or

American government has recommended their citizensto leave, regardless of nationality.

• Underground/underwater activity.• Above water/aerial activity, unless otherwise specified.

Age 65 to 66: cover for 80% of the original sum insured.

Age 66 to 67: cover for 60% of the original sum insured.

Accidental Death and Dismemberment (AD&D)

Benefit Description: This benefit provides a lump sum to the employee’s Beneficiaries in the unfortunateevent of an employee’s death due to an Accident, or to the employee in the event of a loss of limb orlimb function as a result of an Accident.

Accidental Death benefits are calculated based on a factor of the Gross Annual Salary and are subjectto an overall agreed maximum benefit limit.

Accidental Dismemberment benefits are calculated based on a factor of the Gross Annual Salary andthe degree of the loss of a limb or limb function. This payment is usually between 50% and 100% of theoverall insured amount depending on the type of loss.

For example:

Please note total Accidental Death and Dismemberment benefits shall not exceed 100% of the insured amount. Accidental Death andDismemberment is available as additional cover to Life.

recommended their citizens to leave, regardless of nationality.• Underground/underwater activity.• Above water/aerial activity, unless otherwise specified. • Being under the influence of drugs or alcohol. • Deliberate exposure to danger – except in an attempt to save human life.• Illegal acts.• Intentional inhalation of gas or intentional ingestion of poisons or legally prohibited drugs.• Flying in an aircraft, including Helicopters, unless the employee is a passenger and the

pilot is legally licensed or he is a military pilot and has filed a scheduled flight plan whenrequired by local regulations.

• Extreme Sports or Professional Sports including but not limited to; mountain sports,equestrian sports, water sports, car or motorcycle sports, combative sports, air sports andvarious other sports such as bungee jumping.

The loss of sight in one eye may result in a benefit payment of 50% of the insured amount.

The loss of sight in both eyes may result in a benefit payment of 100% of the insured amount.

Disability

Benefit Description: This benefit provides income replacement for an employee who is unable to perform their employment duties due to an Accident or illness.

The Disability cover is structured as follows:

The benefit amount is generally either a percentage of the employee's Gross Annual Salary, subject to an overall agreed maximum amount,or a fixed amount per month. The benefit paid will be decreased in line with any Other Income received by the employee.

A range of Waiting Periods are available and these are set out in the insurance contract.

Eligibility: Employees of the insured company are eligible for cover if they are:

• Under 65 years of age.• Actively at Work.

Eligible employees who are not at work due to sick leave or disability at the startdate of the insurance contract will be admissible after completing two months offull time employment.

Exclusions: Disability benefits will not be paid if the disability was as a result of:

• Active participation in war or insurrection.• Terrorist activity.• Attempted suicide or intentional self-harm.• Being in a country during war, where the British or American government has

recommended their citizens to leave, regardless of nationality.• Underground/underwater activity.• Above water/aerial activity, unless otherwise specified. • Being under the influence of drugs or alcohol.• Deliberate exposure to danger – except in an attempt to save human life.• Illegal acts.• Normal pregnancy.• Intentional inhalation of gas or intentional ingestion of poisons or legally

prohibited drugs.• Flying in an aircraft, including Helicopters, unless the employee is a passenger

and the pilot is legally licensed or he is a military pilot and has filed ascheduled flight plan when required by local regulations.

• Extreme sports or professional sports including but not limited to; mountainsports, equestrian sports, water sports, car or motorcycle sports, combativesports, air sports and various other sports such as bungee jumping.

• The employee suffering from Mental or Nervous Disorders, except in the caseof Short Term Disability.

Short Term Disability: cover for an employee unable to perform their Own Occupation.Following a Waiting Period, the benefit is paid monthly in arrears for up to two years.

Long Term Disability: cover for an employee after the Short Term Disability has beenexhausted and they are unable to perform any gainful occupation relevant to theireducation, skills and experience. Benefit is paid monthly in arrears until the age of 65or until the person resumes gainful activity.

Accident is an unexpected event, independent of the will of the Insured Member and which arises from a cause outside the individual’scontrol, resulting in a physical injury to the individual.

Accidental Death is an Insured Member’s death resulting from an Accident, provided the death takes place within 24 months after theoccurrence of the Accident.

Actively at Work: This means that the employee is: • Working on a fulltime basis.• Carrying out all duties of his Own Occupation.• Not working contrary to medical advice.

Beneficiary (Beneficiaries) for an Insured Member for Life and Accidental Death benefits shall be:• His/Her spouse or legally declared civil partner at the time of death, or failing that,• His/Her legally declared child or children in equal shares, or failing that,• His/Her estate.

Alternatively, an Insured Member may nominate the Beneficiaries of the insured benefit by completing and signing a Nomination ofBeneficiaries form.

Gross Annual Salary: is the annual salary amount before any deduction for income tax. It includes contractual bonuses and commissionearned. However, other employee benefits such as cars, living accommodation, overtime or discretionary bonuses are not included.

Insured Member is a person covered under the terms of the insurance contract, designated as eligible for inclusion under the terms of theinsurance contract and the company has identified this person to the Insurer and paid the appropriate premium.

Other Income includes:• Any disability benefits the Insured Member is entitled to receive from government entities or another insurance company or his

employer.• Any income from a professional activity.

Own Occupation is the full time occupation of the Insured Member at the time of death or dismemberment in the case of Life or AccidentalDeath and Dismemberment cover or at the time of sick leave in the case of Disability cover.

Single Event is an accident or natural disaster with a maximum period of time of 72 hours. In the case of a natural disaster, if thedisturbances last more than 72 consecutive hours, several events shall be taken into consideration, each one for a maximum period of timeof 72 hours.

However, • In case of tornado, hurricane, cyclone or tempest, claims shall be related to a Single Event if they are linked to the same atmospheric

disturbance.• In case of earthquake, volcanic eruption or tidal wave, claims shall be related to a Single Event if the epicentre is unique and shared.

Waiting Period is the period of time from the commencement date of the contract during which an Insured Member is not entitled tobenefits.
