Admission Orders

While in the pt’s chart click on the “+” sign of the “Orders” tab.

Search for “admit ICU” order sets and choose the most appropriate – intracranial bleed, asthma, heart failure, etc.

When in doubt, use “MED General Admission to ICU” order set.

Note: if you right click the module order set you can save it to your favorites.

Left click on the order set and it expands to show the individual order components.

Click the check box to select the order. Right-click to modify the order.

“A/O/T” order requires the highlighted fields to be modified.

This is an example of a completed “A/O/T” order.

Once the A/O/T order is modified right-click and select “Ordering Physician.”

Within the Ordering Physician section search for the Attending that is staffing the pt and select “Phone w/ Readback.”

Orders needed by all pts:

• A/O/T• Vital Signs (per protocol)• Activity• Notify Provider VS• Resuscitation Status• Diet

Orders to consider for all pts:

• Maintenance fluids• Electrolyte protocol • Scheduled or PRN pain control• Naloxone • Antiemetics• Bowel regimen• AM labs • PT/OT/Speech

DVT Prophylaxis:

• Mandatory order for all admitted pts

• After submitting your orders the VTE Advisor will open in a separate window

• Guides you through DVT ppx selection

VTE Advisor is split into 2 sections: Patient Risk and Recommendations

Select the most appropriate “Patient Type”

Select the appropriate patient “Risk Factors”

Select the appropriate patient “Renal Function”

Note: almost all pts should be eligible for below knee

SCDs or “Intermittent Pneumatic Compression”

After filling in the required information the VTE

Advisor will make anticoagulation

recommendations.

If DVT ppx will NOT be ordered you must

document a reason.
