Welcome to the 32nd GCSMCH&RC

newsletter.

I hope you are staying safe and healthy

through these unusual times! We've been

busy since our last newsletter! As the cases

of COVID-19 increases and continues to

evolve in our country, GSCMCH&RC is

committed to the health and safety of our

staff and providing timely and appropriate

treatment to the ones in need.

Diseases bring in a lot of curiosity with it and the ambiguity on how to

deal with various circumstances in this scenario can be troubling.

Apart from just treating the patients, we have been broadcasting

various informative and awareness videos with the help of our

consultants to educate the audience by large. Our Live Social Media

Talks with the panel of experts has received an amazing response

from the viewers.

The contribution of our staff as healthcare workers has been

commendable and praiseworthy. The commitment to cure and serve

the patients comes with its own set of disadvantages like physical

exhaustion and mental exertion. To curtail this, on the occasion of

Patient Safety Day, poster competition and What an Idea competition

was organized to keep up the motivation and to percolate a wave of

learning along with fun.

As we progress forward for the NABH assessment, we prepare

ourselves to achieve this milestone together and sturdily. All hands-on

board can help us sail through this swiftly. This will again be an

opportunity to add another feather to our crown.

This COVID scenario has helped us in ways to better ourselves in the

field of digitalization. We have improved our MyNet experience

making it user friendly and even more attractive by adding self-

learning modules.

GCS Hospital's COVID Wards has treated more than 3500 suspected and

positive COVID-19 Patients successfully, which is a fabulous achievement for a

private trust Hospital. Since the start of the pandemic, GCS Hospital has been

designated as a COVID-19 facility. GCS Hospital has been a ray of hope for not

only COVID Patients but also non-COVID Patients.

WORLD PHARMACISTS DAY

Pharmacy team of GCS Hospital celebrated World Pharmacists Day by Rangoli making & Pharmacy Stores decoration. Pharmacy team also distributed Immunity boosters to all hospital staff. The team also created posters on Good Pharmacy practices & Patient safety awareness and exhibited at Entrance Area. The team celebrated the occasion with team lunch and cake cutting.

WORLD HEART DAY

On the Occasion of World Heart Day, an insightful session was given by Dr. Tarun Dave to emphasize about the importance of a healthy heart and a healthy life. He spoke about the various factors leading to deteriorating health which leads to cardiac mishaps. Along with that he threw light on the red flags individuals must pay attention to which may lead to heart issues. He advised the audience to make lifestyle modifications, maintain a healthy diet intake and recreational activities which will lead to fitness both mentally and physically. Various heart diseases were described in layman language to explain the audience about the mechanisms and prevention of the same both individually and clinically.

WORLD PATIENT SAFETY DAY

A safe working environment for health workers is essential in ensuring a safe environment for patients especially in current COVID Time. On the occasion of World Patient Safety Day (17th September), GCS Hospital organized a Poster Competition & “What an idea” Competition for employees on the theme “A priority for patient safety” with an objective to put a major emphasis on the learning factor regarding patient safety across the Hospital while increasing health-workers awareness and engagement.

We had received 22 amazing entries for this competition, which were exhibited in Seminar Hall with all the necessary precautions. The employees shared their innovative ideas through their creative posters on a variety of topics related to reducing patient harm and improving patient safety and care quality, enhancing health care safety, etc. The 5 best entries were rewarded with the award.

Let's talk about girls

GCS Womaniya group from GCS Hospital

organized an insightful session - “Let's talk

about girls” – an online panel discussion to

discuss various issues related to teenage

girls and their physical and mental health.

Dr. Bhanu Desai (Asso Professor –

Pediatrics), Dr. Jaishree Bamaniya (Asso.

Professor- Gynec and Obs), Dr. Jigna

Padhiyar (Asso. Professor – Skin and VD)

joined us as expert panelist for this panel

discussion which was moderated by Neha

Lal (Sr. General Manager – Operations and

HR). The panelists discussed various

issues related to teenage Girls' mental and physical health such as psychological changes, hormonal changes, skin

and hair care, sexual awareness dealing with sexual activities during puberty, depression and suicidal thoughts,

boosting self-confidence, addressing parents' relationship with teenage girls, dealing with negative peer pressure,

bad habits such as smoking, alcohol, drugs etc. The expert panelists also guided on how parents should maintain

positive relations with their teenage daughter and listen to her instead of being judgmental and make her

comfortable. Over 3400 people took benefit of this online seminar which was streamed live on Facebook.

GCS Hospital has recently organized one panel discussion by experts to discuss and guide public on how to take care

of women's and children's health during COVID19. Dr. Asha Shah (Professor - Medicine), Dr. Kanupriya Singh (Asso.

Professor - Gynecology & Obstetrics), Dr. Prutha Desai (Asst. Professor - Psychiatry), Dr. Prarthana Kharod Patel (Asst.

Electrical Inspection was held by Office of the Assistant Electrical Inspector

(Government of Gujarat) on 7th August'20. The inspectors inspected our electrical

installations and services in detail, where they had appreciated our maintenance

practices with documentation; and also marked our practices as benchmarks to guide

other hospitals.

hospital for employees for spreading

awareness & providing education on

fire and electrical safety, various

emergency codes etc.

The staff was given hands-on training

o n h o w t o o p e r a t e t h e fi r e

extinguishers and various fire safety

equipments. A proper demonstration

regarding how to deal with fire was

imparted to the staff by giving

practical training on fire fighting. The

motto behind having such training is to

train the employees to utilize their

maximum services at the time of

World Heart Day Camp

In the time of COVID-19, taking care of a heart is more important than ever before. Cardiovascular disease (CVD) has been the major cause of deaths across the world. People with CVD and its risk factors, including diabetes, hypertension and diabetes, are more vulnerable to severe forms of coronavirus. In the time of COVID-19, CVD patients are faced with a double-edged threat. Not only are they more at risk of COVID, but they may also be afraid to seek ongoing care for their hearts. Keeping in view of this, we had organized a Free Cardiology OPD Camp from 21st September'20 to 30th September'20 at GCS Hospital, on the occasion of World Heart Day (29th September). More than 120 Patients took benefit of this camp.

Dr. Prarthana Kharod Patel successfully completed the infant and young child feeding counseling specialist training program by the Breastfeeding Promotion Network of India (A national body affiliated with WHO and IBFAN working for Promotion, protection, and support of breastfeeding) in 2016.

She has been working in the field of infant and young child feeding counseling since then. This year she has been recognized by the BPNI as an authorized breastfeeding and young child feeding counseling specialist, when they launched their mobile application Stanpan Suraksha on August 7, 2020, for reaching out to mothers through telecommunication in the current era.

The First YUVA ISACON National Conference of Anaesthesiology was held on 5-6 September 2020. It was attended by 11,500 participants. GCS Anaesthesia residents Dr. Avni Thacker, Dr. Dhara Kakadiya, Dr. Sanjay Baldaniya participated in a paper presentation.

COVID-19 has been an unprecedented

situation for all of us. More so it has been

difficult for the Surgeons to operate upon

COVID positive patients as well as to decide

upon elective surgeries. Post lockdown

with rising COVID patients we had to start

and perform elective surgeries with utmost

care. We have performed various supra-

major and major surgeries in addition to

routine minor surgeries. Supermajor

surgeries like Whipple's Surgery, Total

Proctocolectomy with an ileal pouch,

Commando with flap reconstruction,

Superficial Parotidectomy, Low anterior

resection, Retro-peritoneal sarcoma,

Pancreatic necrosectomy, etc. Not to

mention we have performed numerous

Exploratory Laparotomies Resection, and

Neuro Surgery and Reconstructive Surgery

75 year old male patient presented with a very long-standing ulcer involving more than half of the head to OPD. The lesion was eroding into the skull bones and also the brain tissues were exposed. He was admitted and a diagnostic Biopsy of the lesion was done which suggested Basal Cell Carcinoma. After thorough medical management of the patient Surgery for the excision and reconstruction was planned.

During the Surgery Involved and eroded Skull bone with safe margins all around the ulcerative lesion was removed with preservation of the dural covering of the Brain by Dr. Kushal Shah, Consultant - Neurosurgeon. Tumor tissue present over the Dura was also removed gently. Then Dr. Pramod Menon (Consultant - Plastic Surgeon) esthetically raised a large scalp flap to cover the defect. The recovery from surgery and anesthesia was uneventful. Histopathology reports suggested free margins from all sides. After 3 weeks of follow-up, the patient is doing well.

48 years old Female Patient was asymptomatic before 13 months. The patient had trauma which resulted in sudden pain and difficulty in movement of the left leg which was diagnosed as Left Femur Subtrochanteric Fracture. The patient was operated for the same at LG Hospital in which DCS plating was done.

8-9 months later patient again presented with sudden pain in the left hip at the operated site which was diagnosed as a broken implant in situ, Patient was operated for the same in which longer DCS was used, and Bone Graft was taken from Left Anterior Superior Iliac Spine and left fibula. Surggeon was Dr. J. G. Patel (HOD, Unit -1, M.S. Ortho).

7 months later, the patient represented at GCS hospital with pain in the same left-side operated hip and difficulty in movement which was diagnosed again as broken implant in-situ due to non-union. For this patient was operated on 29 August 2020 for the same in which

Ÿ Cortical bone graft was taken from Right Fibula Cancellous bone graft was taken from Right Posterior Superior Iliac Spine

Ÿ Fracture is fixed with 11-hole DFL plate & anteriorly 7 holes 3.5mm locking compression plate kept & fixed with screws

At present, all incision sites are healthy, all suture was removed. No complaints from the patient side, the patient is able to seat pain-free.

Laparoscopic spleen preserving distal pancreatectomy for Neuroendocrine tumour tail pancreas (Insulinoma). A 40 yr old lady suffering from repeated hypoglycaemia with confusion altered behaviour, stupor (neuroglycopenic symptoms)( blood sugar : 28 mg/dl), relieved by Glucose administration(Whipple's triad) was found to have Neuroendocrine tumour(Insulinoma) in tail of pancreas. Her fasting Insulin levels were 8.85 microU/ml(normal range: 2.6-24.9), fasting C-peptide was 4.22ng/ml(normal range: 1.1-4.4ng/ml). Triphasic CT abdomen showed a 1.8 cm arterial enhancing lesion in tail of pancreas.

Laparoscopic spleen preserving distal pancreatectomy was done by Dr. Rashesh Solanki (Consultant - Surgical Gastroenterologist).

Postoperative course was uneventful and she was ready for discharge on POD 6. Histopathology revealed a grade 1 Neuroendocrine tumour. IGC is awaited.

Ethics committee meeting was held on 14th September'20 through video-conference. The meeting was chaired by Hon'ble Justice Shri Bankim Mehta (Retd. Justice, Gujarat High Court). First dose of DNA plasmid vaccine of SARS CoV 2 to volunteers was started on 17th August'20. Approval was granted to two Clinical Trials including COVID Vaccine Phase-2, 19 Research Projects Related COVID & 23 Research Projects.

Department of Gynaecological Oncology at Gujarat Cancer and Research Institute got an increase of seats in M.Ch super-speciality Gynaecological oncology course from 1 (One) to 4 (Four) from the academic year 2020-21.

The GCRI has purchased a new mobile blood collection van to collect blood from voluntary blood donors from all over Gujarat having latest technology with the facility to collect four blood donors at a time and also with provision to maintain counselling confidentiality of blood donors . It was purchased under CSR activity, donation received from Gujarat State Petronet Ltd.

Training

We all are getting accustomed to ''New Normal'' in this COVID Pandemic Era! Social distancing, Teleconsultations, online meetings, and Webinars have become part of our professional life. Department of Palliative Medicine, Gujarat Cancer & Research Institute, being nodal institution for ENDs Training, conducted this training on 3-4-5 Sept’20. The 20 hours of training, theory part was taught Online and practical session was at GCRI followed by exam. There were 15 participants which included pain physicians, anaesthesiologist , Gynaecologist, Intensivist & cardiologist.

Palliative Medicine: Training on medical use of Essential Narcotic Drugs

Blood Bank has started a new module for online blood demand to make the service as paperless & for better record maintenance and for more effective and safe blood transfusion services. Training for the same was provided to clinicians (Resident Doctors and Assistant Professors) on 30-31 July and 4-5 August’20 and to nursing staff on 13-14 Aug’20.

Indian Association of Palliative Care (IAPC) was registered in Ahmedabad in March 1994 by Dr. M. T. Bhatia our teacher & Pioneer in Pain and Palliative Care. Again, it was a historical event when we called a zoom meeting of all IAPC members of Gujarat on 28th July 2020 under leadership of Dr. Priti Sanghavi (President IAPC Gujarat Chapter) and senior members to form an IAPC Gujarat Chapter.

Training

Rigorous Infection control & health safety training was imparted to not only to the HCWs working with COVID/NON-COVID patients but also to the NON-PATIENT related staff of the hospitals. The training focused on Hand Hygiene, Respiratory Hygiene, Environmental cleaning, and General Health by Emphasizing on the intake of immune boosters for resisting against acquiring Corona Virus infection by Dr. Parijat Goswami, Professor & Head of Microbiology and Infection control In-charge, GCRI.

Dr. Parijat, gave training specifically to Administrative Staff, Biomedical Staff, Medical record Department Staff, The technician from Radiology, Nuclear Medicine, Radiotherapy, Accounts dept, Human Resources Department, all Security personally, all Canteen Staff members (cooks and servers) Clerks, data operators, Stores / Lenin Department, Interns from GCRI & outside hospitals, Resident Doctors, Nurses, etc. During a period of fifteen days, around 648 NON-PATIENTs related staff of GCRI were trained and all of them were very happy to get training on respiratory breathing exercises with demonstration & on diet rich of immune boosters. During the last five months, the total Staff who were given infection control including donning & doffing training were around 3080 Staff members.

Department of Palliative Medicine has celebrated Teacher’s Day by honouring our beloved teacher Dr. Geeta Joshi. The department actively participated in “Grey to Green GCRI” Plantation. The department has a l s o s h o w n e n t h u s i a s m i n c e l e b ra t i o n o f Independence Day without forgetting their duty and guidelines during current pandemic.

Grey to Green GCRI

A tree plantation drive done on 6-7th August 2020 around new building GCRI under "Grey to Green GCRI" and more than 94 samplings is planted. Enthusiasm shown by all department's staffs and security persons. This drive was supported by Ahmedabad Municipal Corporation, Director Dr. Shashank J Pandya initiated step taken by Dr. Kinna Shah, Assistant Professor, Anaesthesia and Dr. Rajan Garg, Paediatric Surgeon GCRI for this green GCRI event.

COMMUNITY ONCOLOGY CENTRE

Health Check up Services

As we all know, due to COVID-19 pandemic, lockdown was enforced since 22 March & health check up services were suspended at Community Oncology Centre for the time being. We resumed our services with all necessary precautions from 15th July’20.

New arrangements are made as follows:

Ÿ Temperature is measured of all visitors at Entry Gate of Centre.

Ÿ We have created 'Triage Area', where detailed history of all Health check up visitors is taken. SpO2, pulse, temperature & respiratory rate are measured and they are provided triple layer mask.

Ÿ X-ray Chest is done for further screening of corona infection.

Ÿ We strictly follow the norms of 'Social Distancing'.

Ÿ 'Sanitization facility' with Foot operating system is available outside all the rooms.

Ÿ Patient's cot & all medical instruments are sanitized with Sodium Hypochloride solution in between examination of two visitors.

With all these precautions, we have done health check up of 175 people till date. Out of which 133 Female & 45 Males were screened. Approx 50 Female visited centre only for Mammography. Total 183 mammography are done out of which 9 malignant mass lesions are detected. Thus, Community Oncology Centre is serving the society to fight against cancer during this pandemic period.
