TECHNIQUES DE VACCINATION ANTIVARIOLIQUE1

Le vaccin doit être utilisé avant la date-limite indiquée; il doit avoir été stocké et manipulé conformément aux instructions du fabricant.

Le point de choix pour l’inoculation est la face externe du bras, au niveau de l’insertion du deltoïde ou légèrement en arrière de la ligne médiane.

Il ne faut pas employer d’agents chimiques pour préparer la peau avant la vaccination mais la nettoyer à l’eau et au savon, s’il y a lieu, et veiller à ce qu’elle soit sèche lors de la vaccination. L’aiguille ou le vaccinostyle utilisé doit être stérile. Le vaccinateur s’assurera que l’instrument est refroidi au moment de son utilisation.

La technique des pressions multiples consiste à placer une goutte­lette de vaccin sur la peau et à exercer, à l ’aide d’une aiguille aiguisée tenue tangentiellement à la peau, une série de pressions sur une zone cutanée aussi petite que possible (n’excédant pas 6 mm de diamètre). Le vaccinateur exercera, en se servant du biseau de l’aiguille et non de sa pointe, une trentaine de pressions en cinq ou six secondes, d’un mouvement alternatif perpendiculaire à la peau; il appuiera suffisamment pour faire apparaître une trace de sang au point d’inoculation.

La technique des piqûres multiples se pratique au moyen d’une aiguille bifurquée. Après immersion dans une ampoule de vaccin reconstitué, l’extrémité de l’aiguille garde entre ses dents une gouttelette suffisante pour la vaccination. Le vaccinateur tiendra l’aiguille perpendiculairement à la peau en posant le poignet sur le bras du sujet à vacciner. Il fera rapidement, d’un mouvement alternatif perpendiculaire à la peau, sur une zone d’environ 5 mm de diamètre, une quinzaine de piqûres suffisamment appuyées pour faire apparaître une trace de sang au point d’inoculation.

La technique de scarification consiste à pratiquer, au moyen d’un instrument approprié, à travers une goutte de vaccin une seule égratignure linéaire n ’excédant pas 6 mm de longueur. La scari­fication doit être suffisamment profonde pour faire apparaître en 30 secondes au plus, une trace de sang au point d ’inoculation. Le vaccinateur fera pénétrer le vaccin dans l’égratignure en se servant du bord de l’instrument.

SMALLPOX VACCINATION: TECHNIQUES1

The vaccine should be used before the stated expiry date; it should have been stored and handled according to the manufac­turer’s instructions.

The preferred site for vaccination is the outer aspect of the upper arm over the insertion of the deltoid muscle, or slightly behind the midline.

Chemical agents should not be used for the preparation of the skin for vaccination. If necessary, the skin can be cleansed with soap and water. Care should be taken that the skin is dry when the vaccination is made. The needle or vaccinostyle used to per­form the vaccination should be sterile. The vaccinator should ensure that the instrument has cooled before being used.

In the multiple-pressure technique, a small drop of vaccine is placed on the skin and a series of pressures is made within the smallest possible skin area (not more than (4 inch or 6 mm in dia­meter) with the side of a sharp needle held tangentially to the skin. The pressures are made with the side of the needle, not the point; 30 strokes are completed in 5 to 6 seconds, with an up-and- down motion perpendicular to the skin, using sufficient pressures to induce a trace of blood to appear at the vaccination site.

In the multi-puncture technique, a bifurcated needle is used which is inserted into the ampoule of reconstituted vaccine. On with­drawal, a droplet of vaccine sufficient for vaccination is contained within the fork of the needle. The needle is held at a 90° angle (perpendicular) to the skin with the wrist of the vaccinator resting against the arm. Fifteen up and down (perpendicular) strokes of the needle are rapidly made in an area of about 5 mm. The strokes should be vigorous enough so that a trace of blood appears at the vaccination site.

In the scratch method, a single linear scratch not more than 14 inch or 6 mm long is made through the vaccine with a suitable instrument. The scratch should be deep enough to cause a trace of blood to appear at the vaccination site within 30 seconds. The vaccine is rubbed into the scratch with the side of the needle.

MALADIES VÉNÉRIENNES

C anada . — Le nombre des cas de maladies vénériennes déclarés jusqu’à présent au Canada pour 1968 a été moins élevé qu’en 1967 (tableau 1 ). Cependant, on estime que, même si cette diminution est confirmée, elle pourrait être due à des variations de complétude des déclarations plutôt qu’à une diminution réelle de l’incidence. L’analyse de spécimens envoyés au laboratoire pour confirmation du diagnostic de blennorragie tend à confirmer ce point de vue car il semble que l’incidence de cette maladie augmente, particulière­ment parmi les jeunes gens.

VENEREAL DISEASE

C anada . — The preliminary number of cases of venereal disease reported in Canada in 1968 was lower than that for 1967 (Table I). It is considered however that even if maintained this decrease may be due to variation in reporting practices rather than a true decrease in incidence. The analysis of specimens sent for laboratory diagnosis of gonorrhea tends to support this suggestion, as it would appear that this disease may be increasing in incidence, particularly among young people.

POLIOMYÉLITE

Argentine. — Au Ier mars 1969, 93 cas de poliomyélite avaient été signalés en Argentine, contre 16 et 9 respectivement pour les neuf premières semaines de 1968 et de 1967. Les provinces de Buenos Aires et de Mendoza viennent en tête de la morbidité avec 26 cas et 25 cas respectivement. On possède des renseignements sur 53 cas. La distribution par âge se présente de la manière sui­vante: moins d’un an, 19; un an, 21; deux ans, 5; trois ans, 3; quatre ans, 3; 5-9 ans, 2. Trente-deux malades étaient du sexe masculin et 21 du sexe féminin. Quarante et un n ’avaient pas été vaccinés et neuf n ’avaient reçu qu’une seule dose de vaccin; on ne possède aucun renseignement sür l ’état vaccinal des trois autres. L’examen virologique de trois cas a montré que l’agent pathogène était le virus du type I.

POLIOMYELITIS

Argentina. — Until 1 March 1969, 93 cases of poliomyelitis have been reported in Argentina as compared with 16 and 9 respectively in the first nine weeks of 1968 and 1967. The pro­vinces reporting the largest numbers of cases are Buenos Aires (26) and Mendoza (25). Information is available regarding 53 of the cases. The distribution by age is as follows: under 1 year, 19; 1 year, 21 ; 2 years, 5 ; 3 years, 3 ; 4 years, 3 ; 5-9 years, 2. There were 32 males and 21 females. Forty-one of the patients had received no vaccine and nine had received only one dose; informa­tion is not available on the vaccination history of the other three. Virological studies of specimens from three cases have demonstrated that the etiological agent was poliovirus type I.

Canada. — Bien que le nombre des cas de rage animale signalés au Canada ne représente qu’une fraction des cas sévissant parmi les animaux sauvages, il donne néanmoins une idée de l’ampleur du problème. Au cours de la période de 45 ans qui s’est terminée en 1952, moins de 1 000 cas avaient été confirmés chez des animaux, mais la maladie marque une recrudescence spectaculaire depuis une vingtaine d’années. De 1925 à 1967, 19 décès ont été enregistrés dans la population humaine.

Du 1er avril 1968 au 28 février 1969, il a été signalé 1 933 cas de rage animale confirmés au laboratoire, dont 695 chez des animaux domestiques et 1 238 chez des animaux sauvages. Ces cas se répar­tissent ainsi:

RABIES

C anada . — Although the number of cases of animal rabies reported in Canada represents only a fraction of the cases occurring in wild life, it is considered that they provide at least a general indication of the extent of the problem. In the 45-year period to 1952, fewer than 1 000 cases were confirmed in animals, but a dramatic increase has occurred in the past two decades. Between 1925 and 1967 there have been 19 human deaths recorded.

During the period 1 April 1968 to 28 February 1969 there were 1 933 laboratory-confirmed cases of animal rabies reported in Canada of which 695 occurred in domestic animals and 1 238 in wild life. The individual numbers were:
