INTERNATIONAL RESEARCH ACTIVITIES OF THE
NATIONAL INSTITUTES OF HEALTH, USPUS*
A. GENERAL BACKGROUND
Legislation
The Public Health Service has engaged in international research and
training activities under basic authority provided the Surgeon General by
P.L. 86-610.
The International Health Research Act of 1960 was enacted in July of
1960. This legislation provided for the first time an explicit expression
of the will of Congress in respect to the support of health research over-
seas, although it added little in the way of new authority. Two major
purposes of the legislation were cited:
1. To advance the status of the health sciences in the United States
and thereby the health of the American people through cooperative
endeavors with the other countries in health research, and research
training; and
2. To advance the international status of the health sciences through
cooperative enterprises in health research, research planning,
and research training.
Significantly, the responsibility for the achievement of these two
objectives was separated. The responsibility for achieving objective #1 --
advancing the status of the health sciences in the United States through

overseas research activities was given to the Surgeon General and the
Secretary of Health Education and Welfare (HEW). The responsibility for
objective #2 - advancing the international status of the health sciences -
was given to the President. The law indicates that he may delegate this
authority to the Secretary of HEW who may then in turn delegate it to
the PHS.
This clear distinction between the role of the Secretary and the
Surgeon General and that of the President under this Act in terms of the
purposes to which support of health research overseas is to be directed
is most significant. It clearly reinforces the concept of National
Institutes of Health (NIH) objectives set forth below. The intent of
Congress to distinguish between action in support of national objectives
and action aimed at broader international objectives is clearly evident.
Phio0i~ephy
NIR is part of a larger community of agencies and organizations in-
volved in international health activities. We may support or conduct re-
search by delegation of authority from these agencies. We may participate
in the actual support of research and research training based on the sta-
tutory provisions of our own legislation. In either circumstance, our
role is primarily related to our scientific mission.
Although research support programs may influence or affect the conduct
of foreign policy, the NIH international research program is not designed
for this purpose. Thus, a clear distinction must be drawn between tech-
nical assistance (foreign aid) and support of research abroad for domestic
or national purposes.
The assesament of foreign research support by NIH must consider the
aggregate effects on science ecology within a given country, as well as the

ecientific merit of all individual research proposals. The size, scope,
and duration of research commitments may innocently disturb seriously the
plans and aspirations of a foreign domestic research program. Therefore,
action to support research abroad should be taken only after careful con-
sideration of all circumstances involved.
The rapid growth of NIH support of research abroad through research
grants from a level of $100,000 in 1955 to $8.4 million in 1961, provides
sufficient justification for concern that in certain instances, NIB foreign
grants may affect significantly the overall development of health programs
abroad. The NIH contribution to research abroad should be provided in such
a way as to insure that it will be additive rather than a replacement for
existing local support. Since the nature, scope, and magnitude of the
U. S. effort in international health science have been incompletely
assessed and evaluated, it is particularly important that NIH define its
responsibilities within its specific statutory authorizations. Efforts to
broaden our mission at this time beyond this authority can lead to con-
fusion and possibly to interagency problems. Thus, NIH recognizes that
the primary responsibility for certain other international scientific and
health activities, such as the development of medical education, rests
elsewhere. However, only through a delegation of Presidential authority
as contained in the International Health Research Act of 1960 can NIH
fulfill its domestic mission and be more responsive to the needs of other
national and international health agencies.

Research which is relevant to a specific NIH program may be supported by
contract or by research agreement.
The Office of International Research is responsible for establishing
mechanisms adequate to assure that the general policies and specific cri-
teria governing the NIH international programs are applied effectively
and economically.
This mechanism should be adequate to assure that all research appli-
cations are reviewed, preferably on the scene, by a person or group having
the full confidence of NIH. Reviewera may include Study Section or Council
membera, U. S. scientists with personal knowledge of applicants and their
institutions, foreign scientists in whom NIH has confidence, or NIH staff
membera.
Preliminary review should take into account the ability of the scien-
tist, the worth of his proposal, the environment within which he works, and,
when appropriate, the total content of the nations's effort in the medical
aciences.
Cxriteria :fol F~eian Awards
The National Institutes of Health will accept for scientific evaluat-
ion and review research proposals which meet both of the following criteria:
1. The research proposed is outstanding, or original in concept,
or would utilize the talents, skilla, materials, or other resources
not likely to be available in the United States; and
2. The proposal is of such a nature that the results are likely
to advance significantly the status of the health sciences in
both the country of the applicant and the United States.
Mechanisms
Whenever possible, the existing mechanisms of receipt of applications
and the scientific and administrative review of research proposals will be
the responsibility of the Institutes and Divisions involved. However,
those international program areas which go beyond the responsibilities and
interests of a single Institute and which relate to NIH as a whole become
the responsibility of the Office of the Director, NIH. The existence of
activities and problems of this nature (in large measure resulting from the
need to inform and clear international research developments with many other
elements of PHS, Office of the Secretary, HEW, Department of State, etc.)
has led to the establishment of a coordinating office of international
research. In order to perform these functions as well as to facilitate
program analysis and budgetary planning, the following functions are per-
formed by the Office of International Research (OIR), Office of the Director,
NIH:
This office is responsible for coordinating all international activities
of the National Institutes of Health. It advises the Director and the
Office of the Surgeon General on matters relating to the international
aspects of medical research and research training, and asaists the
Institutes in development of international programs. It is specific-
ally responsible for administration of the NIH P.L. 480 program and
coordinating Institute proposals for research under this program in
foreign countries. The office constitutes the central point within
NIH for relationships with the World Health Organization, the Pan
American Health Organization, and other international research and
scientific organizations. The office is comprised of an Office of
the Chief, a Foreign Grants and Awards Section, a Special Foreign

Currency and Program Services Section, and a Program Analysis Section,
The office is also responsible for establishing and administering NIH
overseas offices.
This office will provide a mechanism whereby the National Institutes
of Health can work with international organizations, foreign institutions,
and scientists on a planned, predictable long-range basis.
The OIR, in collaboration with the Institutes, will be responsible for
an organized systematic assessment of medical research potential and
research resources in foreign countries. This will facilitate the
implementation of direct (intramural) conduct of research overseas
by NIH as well as providing information for Study Sections and Advisory
Councils in their review of foreign grant applications.
B. ACTIVITIES IN FISCAL YEAR 1961
The fiscal year 1961 review of the status of the international activit-
ies of the National Institutes of Health indicates substantial progress and
growth have taken place. Two new programs of particular significance have
been launched in addition to the further development of programs continued
from the previoue year. These two programs are the International Centers
for Medical Research and Training and the Special Foreign Currency Program.
Altogether obligation for NIH activities outside the United States (exclu-
sive of the Foreign Currency Program) totaled $12.2 million in fiscal year
1961 in contrast-to a total of $7.1 million obligated for similar purposes
in fiscal year 1960.
In the departmental appropriation (F.Y. 1961), a sum of $5 million was
designated for the development of international activities under the

Surgeon General's authorities provided by Public Law 86-610.
Approximately $2.4 million of this amount has been allocated for the
development of a series of International Centers for Medical Research and
Training in accordance with the direction given in the Senate discussion
of this particular amount. The remainder of this amount ($2.6 million)
was utilized by the several Institutes and the Division of General Medical
Sciences for the support of carefully planned and purposely developed
research projects involving foreign investigators and institutions and
directed toward research and research training objectives of key signifi-
cance to these programs,. A summary of the status of these activities follows.
1. Regular Research Grante and Contracts
Research Grants and Contracts for the support of foreign investigator
working on problems relevant to the general and categorical programs of
the NIH amounted to $5,008,016 for a total of 303 such awards in fiscal
year 1960. Comparable figures for foreign research grants and contracts
in fiscal year 1961 reveal 506 awards made totaling approximately $8.4
million (see Tables I and II).
At current levels of support, the NIH program constitutes one of the
largest U.S. federal agency programs of support for research in foreign
countries. It is expected that the volume of research support for foreign
investigators will continue to increase during fiscal year 1962.
The basic rationale underlying this growing expenditure for the support
of medical research in foreign countries is that there is specific reason
to believe that the work supported will significantly contribute to the
achievement of the program objectives of the National Institutes of Health.
Therse program objectives, in the first place, relate to progress in under-
standing and controlling major categorical diseases of national significance

and, in the second place, to strengthening and advancing the medical and
health sciences generally. The objective of the National Institutes of
Health in providing grant support for research carried out by foreign
investigators and institutions is to secure the benefits to its program
objectives of the superior scientific capability or exceptional research
opportunities, which may be presented by such individuals or facilities
and their scientific endeavors. Reflecting the above rationale the cri-
teria followed by the National Institutes of Health in making awards to
foreign investigators and institutions, beyond the usual scientific
evaluation, are as follows:
(a) The research activities proposed must be significantly relevant
to the program objectives of the NIH, and
(b) The research material is geographically so located that it can
best be studied outside the United States, or
(c) The research facilities are of such nature or in such location
as not to be duplicated in the United States, and
(d) The training, competence or stature of the applicant is such as
to provide a high degree of assurance of productive results
from the work supported.
2. Training Support
During fiscal year 1960, a total of $2.1 million was expended by NIH
for research training activities on an international basis. Comparable
expenditures in fiscal year 1961 were $2.7 million. Plans for 1962 based
on present budgetary determinations contemplate only a modest increase in
these activities.
The NIH International Fellowships Program is a major element of these
overseas training activities. This program initiated in 1958, provides for

supporting the study of outstanding young medical scientists from 34
countries for periods up to two years in U.S. universities and research
institutions. During fiscal year 1961, 90 such awards were made for a
total amount of $700,000.
Support of U.S. scientists studying abroad under the sponsorship of
outstanding research scientists is a program of dominate importance in the
array of NIH international activities. In the past 14 years about 650
U.S. scientists have had the benefit of such training and research experience
in the laboratories and universities of some 40 countries. During fiscal
year 1960, 191 U.S. fellows were supported by NIH training in 28 foreign
countries at a cost of $1.2 million. In fiscal year 1961, this program
supported a total of 217 U.S. fellows at a level of $1.5 million.
Other NIH overseas training activities included a small number (34)
of trainees studying abroad in clinical areas in the amount of $270,000
and an estimated $250,000 for 7 training grants to foreign institutions.
3. Cooperation with WHO and PAHO
During the past year, NIH has worked out agreements with the World
Health Organization and the Pan American Health Organization covering the
manner in which NIH will cooperate with these organizations in the conduct
of research activities sponsored by them. Basically, these agreements
provide for preliminary planning of international research programs on a
collaborative basis and set forth the terms and conditions under which NIH
will support the special research programs of these agencies and the re-
search projects of participating investigators and institutions. Two large
research grants for special research projects have been made to the WHO
thus far in fiscal year 1961:
(a) A grant for the worldwide study of insecticide resistance and

for control of insect disease vectors - in the amount of $204,000
for the first year, with support promised for four years.
(b) A grant for the study of bilharziasis and research into mollus-
cicides, chemotherapeutics, and diagnostic methods related there-
to, in the amount of $78,941 for the first year, with four
additional years of support promised.
A third grant application providing for the development of global studies
relating to the epidemiology of virus diseases has been approved. Applicat-
ions for support of other comparable large-scale international research
programs are expected from WHO. In its March meeting, the National Advisory
Health Council recommended the approval of a grant of 8120,750 to the Pan
American Health Organization through the National Institutes of Health for
the purpose of planning, developing, and organizing an international health
and medical research program for the Americas under the direction of the
Pan American Health Organization. This award was paid with approval by
the President. Twelve awards to PAHO and INCAP, totaling $619,000 were
made in Fiscal Year 1961. (See Tables III and IV)
4. International Centers for Medical Research and Training
This program envisages the development of a series of international
centers for medical research and training based on U.S. universities with
an overseas extension in the form of a research laboratory and training
facility.
These centerá will provide field experience and research opportunities
for U.S. postgraduate physicians and scientists and encourage physicians
and other health personnel of collaborating non-U.S. institutions to par-
ticipate in the development of health research and training resources in
their national environment.

Each of these centers will consist of (1) a professional, schoolbased
organizational entity which would provide a stable, continuing framework
in which programs of medical research, research planning and research train-
ing may be conducted in this country, an4 which is associated with (2) a
research and training establishment oveiseas where these activities may be
pursued under environmental, ethnic, an4 medical conditiona which do not
prevail in the United States.
Applications of five American uni4ersities to sponsor International
Centers for Medical Research and Traniing had been approved for payment.
These American universities and their cooperating institutions overseas
are as follows:
University of California Institute for Medical ResearchKuala Lumpur, Malaya
Tulane University - Universidad del Valle, Calit Colombia
John Hopkins University -- All-India Institute of Hygiene, CalcuttaSchool of Tropical Medicine, Calcutta,India
Louisiana State University- University of Costa Rica, San Jose,Costa Rica
University of Maryland - Medical College at Lahore, East Pakistan
Each of the American universities has requested research and training
funds ranging from $100,000 to $500,000 per year for each year of a five-
year period. An ad hoc Committee on International Centers for Medical
Research and Training reviewed and approved these grant requests, and the
National Advisory Allergy and Infectious Diseases Council concurred, re-
commending to the Surgeon General that these grants be paid.
Of the $5 million appropriated for fiscal year 1961 to support the
International Centers for Medical Research and Training, $2,269,971 has
been transferred from the several categorical appropriations concerned to

the National Institute of Allergy and Infectious Diseases which serves as
the central point for administering this program. The remainder of the
$5 million was used by the Institutes and DGNS for special foreign research
programs in the regular research grants area.
5. Special Foreign Currency Proram
The use abroad of United States-owned foreign currencies (Public Law 480)
permits the extension overseas of NIH intramural research programas. Such
programs, of mutual concern to the NIH and to foreign institutions and
investigators, are carried out through development of collaborative research
agreements with scientists and government agencies of other countries.
(a) Objectives
Research under this program is directed primarily toward solution of
medical problems of particular importance to the U.S. and other countries.
The availability of qualified investigators or the existence of a
unique research situation govern the choice of the host countries to be
designated for the projects.
Provision is included in most projects for training of younger scieintists
and supporting technical personnel, to augment local acientific man power
and contribute to the scientific development of the country involved.
(b) Type of Projects
Medical research may require further study or confirmation of U.S.
findings in other countries where similar diseases are prevalent. The pro-
jecta are aimed at solving characteristic health problems of both countries.
They, for the most part, include problems of infectious diseases, nutri-
tional problema and those involving large population groups where recent
developmentsin epidemiology can permit the delineation of differences in

population characteristics, suggesting new clues to the etiology of certain
diseases. Some of the projects contemplated, of global concern, are in
the field of virus diseases, Some countries, because of the differences
in attack rates of the same diseases, offer unusual opportunities for com-
parative studies in different geographical regions.
Studies will also be continued on those diseases which still persist
in certain areas, offering a threat to those entering them, or which can
spread to other areas. The search for new biological and botanical sources
of pharmaceutical products useful in the fight against disease also will be
expanded.
(c) Policy
Selection of the research to be undertaken receives the same careful
review and scientific appraisal as is currently given all research projects
in the intramural research programs of the individual Institutes. Tech-
nical advice and scientific opinions are also obtained from competent
scientists and scientific bodies within the host countries.
No project will be supported contrary to the wishes of the foreign
country's official body responsible for the over-all planning and co-ordi-
nation of research. Such groups will continue to be used to the maximum
extent possible, to assist in the planning and organization of projects
within each country, Close co-ordination with the research programs of the
World Realth Organization will be maintained.
The funds are intended to support only additive research and will not
be used to substitute for the support which each country would normally
give to its research efforts. Likewise, these funds will not be used to
support projects which would normally be elegible for support from dollar
appropriations.

Public Law 480 funds for medical research were appropriated in 1961
($3.7 million) and 1962 ($5.1 million). A modest request for additional
funds has been submitted for 1963 ($610,000). Tables V and VI reflect these
appropriations and the 1963 request by country.
Funds provided in the Fical Year 1962 appropriation to support this
program are not yet available for obligation and expenditure, as the formal
apportionment request has not yet been returned to us through budget channels.
The negotiation of individual agreements supported from Fiscal Year
1961 funds has proceeded rather slowly. Scientific and administrative NIU
personnel have traveled to the countries where research is to be conducted.
Preliminary scientific agreements have been reached, but the administrative
follow-up to obtain official signatures has been slow. It is believed that
it would be appropriate at this point to allow ourselves a period to evaluate
the effects of this program on the scientific communities of the host count-
ries before additional funda are requested.
Because of the difficulties involved in the administration of this
program, the NIH now is considering a new approach to the utilization of
funds to be provided for future year i.e., that collaborative research
programs be negotiated in general terms with the appropriate scientific
bodies of the governments in host-countries. These national bodies willI
then develop specific program proposals which they will submit to the NIH.
The proposals will then be reviewed. If acceptable, formal documents will
be prepared and official signatures obtained in order to release the funds.
The NIH Advisory Committee on International Research has indorsed this pro-
posal.
(d) Summary by Country
BRAZIL - Authority to obligate funds in the Brazilian appropriation was not

received until August 1961. Following this, NIH representatives conductedpreliminary negotiations in Brazil in October 1961. A scientific negotiat-ion team is leaving for Brazil December 2 and it is expected that threePublic Law 480 research agreements will be concluded shortly. The proposedagreements include studies of schistosmiasis, use of venoms in cancerchemotherapy, and possible use of botanicals native to Brazil in the treat-ment of heart disease. Brazilian cruzeiros have decreased in value fromthe time of allocation to the 11resent to the point where our purchasingpower from the 1961 allocation is about 50% of the value at the time: of -allocation.
Excess foreign currencies were not available for request by NIH for1962 or 1963; however, we will seek additional funds in this country in thefuture.
BURMA - Preliminary contacts have been made with acientists in Burma; however,the government negotiations have not yet been begun. This should be accom-plished within the next 90 days. Projects to be developed with scientistsin Burma include research in fields of nutrition and malaria.
EGYPT - Three research agreements, concerned with various aspects of nutrit-ion and anemia, were negotiated last June by NIH representatives. Thesewere transmitted to Egypt in August for signature.
One has been signed and the funds have been obligated. The other twoare in the process of being signed. We have requested follow-up on the twounsigned agreements from the Embassy and NAMRU-3 and expect information ontheir status in the near future.
Projects proposed for support for the current fiscal year include re-search in schistosomiasis, bladder cancer, intestinal helminths and oph-thalmologic studies.
INDIA - Four research agreementa totaling $1,226,000 were negotiated by anNIH team in August 1961. They have been signed by the collaborating Indianscientists and are now in the process of getting final clearance by theCouncil on Scientific and Industrial Research and the Ministry of Finance.We were informed early in November that Professor M.S. Thacker, Director ofCSIR, was ready to sign the firat agreement concerning nutrition; the secondagreement, which is dependent upon the first, is ready to be signed as soonas the first agreement is effected. Two other agreements in the cancerfield developed with Indian scientists are as yet unsigned.
All projects negotiated in India so far are with scientista in institut-ions coming under the direction of the CSUR. However, NIH has proposed pro-jects with institutions controlled by the Ministry of Health and NIH re-presentatives have held several preliminary discussions with the Ministerof Health. So far the Health Ministry has not been willing to endorse thescientist-to-scientist negotiations.
Funds appropriated for Fiscal Year 1962 are designated to support pro-jects of material interest in periodontal disease, tropical medicine, virusdiseases, neurological disorders, cholera and malaria.

INDONESIA - Negotiations have been started in Indonesia for two projects;studies in mycology and nutrition. The GOI has not as yet indicated theirwillingness to allow their ecientists to enter into agreement with NIH, butinformation received recently indicates we may get official acceptance soon.We have not requested additional funds in Indonesia beyond those providedin 1961.
ISRAEL - Negotiations have been completed and work has begun on two researchprojects in Israel. One is an Epidemiologis study of Rypertension andIschemic Heart D3$ease ($100,000 equivalent) and the other is on methods ofmeasurement of O0w ($70,000) equivalent). Travel support for Americanscientists to attend a genetics research symposium in Jerusalem was alsoprovided from these funds.
Studies in various neurologic and ophthalmologic disorders, atherosclerosis,toxoplasmosis and psychological investigation of infant behavior are pro-posed for support from 1962 funds.
PAKISTAN - In May and June 1961, an NIE team negotiated a nutrition surveyagreement in Pakistan. This agreement is now in force and the initial pay-ment has been made. A second agreement for cholera studies is dependantupon final agreement on the organization and status of the Pakistan-SEATOCholera Research Laboratory. This agreement is now in the final stages ofrenegotiation. Additional support is requested in 1962 for similar programs.
POLAND - A negotiating team has just returned to NIH from Poland bringingwith them a number of negotiated projects. These are currently being preparedfor review by the Office of the Director, NIH. Polish scientists have in-dicated a desire to collaborate in studies of hypertension, brain-behaviorresearch and encephalitides with the funds that will be made available in theFiscal Year 1962 appropriation.
YUGOSLAVIA - Scientist-to-scientist negotiatione have been completed on alarge epidemiological study of cardiovascular disease in Yugoslavia. Thisproposal has not as yet been approved by the government officials of Yugo-slavia.
For support from Fiscal Year 1902 funds, projects have been proposedfor study of pellagra and anemia, arthropod-borne virus diseases and neuro-logical disorders in infancy.
6. NIH - SEATO Cholera Research Program
The NIH is collaborating with the Southeast Asia Treaty Organization
(SEATO) in the developmeñt of a cholera research program. Although cholera
is fortunately no longer a disease threat in our country, it is a major
one in other areas of the world. In May 1959, the SEATO Council initiated
a program looking to the eradication of this disease through cooperative

research by scientists of all the SEATO nations. The Council allocated
$400,000 from the Asian Economic Development Fund for this purpose. The
National Institutes of Xealth was asked to administer the program. In the
comparatively short time since initiation of this forward-looking program,
a laboratory has been established in Thailand and has functioned for almost
a year. Now, another more permanent laboratory has been created in Dacca,
East Pakistan, which was formally open early in December 1961. Some 50
leaders in cholura research and control from SEATO and other nations
gathered in Dacca for a conference on cholera. Epidemiologic, laboratory,
and clinical studies will begin this year.
C. THE IMPACT OF NIH INTERNATIONAL PROGRAMS ABROAD
Although a systematic and thorough appraisal of all our foreign opera-
tions has only been recently initiated, certain preliminary conclusions
seem warranted. We have considerable evidence that the research being
supported is generally of high quality. For this reason, we have con-
cluded that the individual projects are accomplishing effectively the
objectives of the NIH. These foreign activities allow us to bring to bear
the talents and skills of highly competent foreign scientists on the health
problems of the United States. Thus, NIH is now taking advantage of a
vital resource which otherwise would not be available.
On the other hand, we recognize that because of the rapid growth of the
program, its aggregate effects may in certain countries influence the
development and structure of medical science. Usually these secondary
effects are advantageous in that they strengthen the research potential

and productivity of the recipient country. However, we have recently had
experiences which show us that under certain conditions our actiona may
innocently interfere with the orderly development of the programs of foreign
institutions or even foreign countries. Thus, we perceive the need to assess
continuously both the primary and secondary effects of our actions. It is
recognized that while NIH programs are not designed primarily for the ad-
vancement of our foreign policy, they must be sensitive to the needs for
maintaining satisfactory relationships with foreign scientists, foreign
institutions, and foreign governments.
Our attention has only recently been focused on the secondary effects
of our overseas programa because until very recently our foreign activities
in the aggregate did not constitute a significant part of the total research
support structure of any country. Because of the increasing feasibility
and desirability of international cooperation in medical research, our
foreign activities have been increasing rapidly. They still represent only
a small part of our total effort, for example, only about 4 percent of our
grants are awarded in foreign countries. Nevertheless, in fiscal year 1961,
506 grants were made to foreign investigators and the total support amounted
to 8.2 million dollars. Even though we have been very selective in our
awards during fiscal year 1962, it is expected that about 12 million dollars
will be awarded to foreign investigators. These scientists are doing
important work which we believe will advance the status of the health
sciences in the United States as well as in the country of the investigator.
In several countries (Canada, the United Kingdom, Israel, Sweden, and Japan)
our support now ranges between one and two million dollars yearly.
One of the important functions of the new Office of International
Research and ita overseas representatives will be to provide a continuous

appraisal of both the primary and the secondary effects of our activities.
Attempts will be made to assure that the NIH contribution to research
overseas is provided in such a way as to insure that it will add to or
stimulate local support rather than replace the research investment of the
foreign country. The NIH overseas activities have been .undertaken only in
instances where such projects were in the interest of our domestic object-
ives. Nevertheless, our willingness to cooperate with and support other
countries in a mutual effort to inprove the health of their people and our
people furnishes an excellent opportunity for collaborative research. These
programs will increase our knowledge only if they are scientifically pro-
ductive and only if they are supported and respected by the leaders in
foreign medical science. For this reason, we are establishing liaison with
appropriate representativesin those foreign countries where our activities
might affect significantly the status of medical science in that country.
In consultation with foreign scientific groups, we are studying the status
and goals of the health-science programs in these countries so that we may
be sure our own projects supplement or complement their programs. In meet-
ings during the past year with the leaders in medical science in Sweden,
the United Kingdom, France, Israel, and Canada, we have learned that our
programs have resulted in significantly mutual advantages. These discuss-
ions identified way in which the future programs of NIH could be coordi-
nated more effectively with the total research efforts of these foreign
countries.
We have recently assigned a full-time representative to the World
Health Organization in order that we may have available to us the vast
amount of information in that organization in constructing our own inter-
national programs. We are using a variety of other mechanism for studying

the relationship between our own foreign programs and those of other
American and foreign organizations. For example, we are cooperating with
the American Institute of Biological Sciences in their attempts to compile
information concerning research resources abroad. Close relationships-- -
have been established with the Office of the Science Adviser of the
Department of State.
On December 15, official representation was established in Paris,
France through the assignment of two NIH employees who have been accommo-
dated and space provided by the Embassy.
Continuous on-the-scene appraisal of our programs will permit us to
choose selectively those activities best calculated to advance our mission
in the most effective and economical manner. In addition, administrative
advice and assistance will be provided to grantees and to the various
Institutes and the National Institutes of Health. The science-administra-
tors appointed to these positions will also have the responsibility of
negotiating research agreements in connection with our Special Foreign
Currency Program in those countries most easily accessible to their base
of operations.
Since the primary objective of the NIH international program is to
advance the status of the health sciences in the United States, our grants
are most frequently made in countries where adequate research resources
are available and where original and productive ideas arise. This means
that NIH relationships have been most extensive with the countries which
are more advanced in the medical sciences, such as, Canada, Japan, Australia,
Israel, and the Western European countries. The fact that we do not
frequently support scientists in newly-emerging countries also recognizes
that their limited scientific manpower must not be diverted from the high-

priority problems of that country by our support of projects of interest to
us. However, NIH does have programs in emerging countries which are
supported with U.S.-owned foreign currencies. We have been careful to
develop these programs in such a wayas to assure that (1) the projects
supported were of interest to both American and the cooperating country
and (2) the project involved collaboration between foreign investigators
and U.S. scientists. In this way, we assured that the project would not
only achieve its primary objective but, in addition, the collaboration
would tend to elevate the level of science in the cooperating country.
The International Centers for Medical Research and Training which are
being established through grants to American universities have been care-
fully studied in respect to their impact on the medical environment in
which they are located. Suffice it to say that they will strengthen the
medical research potential in the foreign countries in which they are
located. However, we perceive the need for continuous appraisals to make
certain that the participation of foreign scientists in the work of these
centers does not significantly reduce the number of medical scientists who
are available for other important work of the country, such as teaclhing
and medical services. For this reason, we plan to evaluate not only the
research accomplishments of these centers but also their effects on the
medical environment in which they operate.
Thus, it is becoming increasingly apparent that the development of
highly effective international programs will require a careful continuing
appraisal of the relationship between our activities and those sponsored by
and planned by the foreign scientific community. Our experience to date
suggests that by making such an effort we can be sure that our international
research activities accomplish not only our primary goals but also that they
strengthen the medical research potential of the cooperating countries.
