(a) A registered candidate, who is unable to write by himself/herself due to physicaldeficiency/deformity (either 4oo/o or above) shall be allowed to take the help of the writer/scribe onapplication on or before 15.12.2016 in Prescribed Proforma (Annexure-A) as enclosed herewith toappear the examination by fuil fiiling the foilowing conditions. (b) The physical deformity/deficiency must be determined and certified by a registered medicalpractitioner not below the rank of ADMO.(c) The helper writer/scribe must be a regular and continuing first year student of different streams nothaving any of the optional subjects offered by the candidate from a recognized college. Thesubject of the helper writer/scribe should be mentioned on the application forhr as per theresolution of the Examination Committee.(d) The centre superintendent is authorized to allow the helper writer/scriber after verifying his/hercollege ldentity Card.

,The candidate who were booked under Malpractice for Annual H.s. Examination, 2016,whoseresult was published is requested to collect his/her Mark sheet (only pc) from the 6HSE HeadOffice. Accordingly the candidate can fill up the form.All the fresh and Ex-correspondence students are required to fiil up their forms at respectiv e zonaloffices located at Berhampur/Baripada/sambalpur and central office, Bhubaneswar, where theywere admitted/Re-admitted previously. lndividuals desirouS for fiiling up of the forms are requiredto deposit examination fees in shape of Money Receipt (e-Receipt) obtained from state Bank oflndia. along with application form (Form No. 3G) and other iequireo documents.The students admitted in the Aoademic session 2011-12,2012-13,2013-14,2014-15 & 2015-16have to follow the syllabus meant for Annual H. Procejure for depositing Exam. Fees -2017 (Correspondence Course)

usine State Bank Collect facilitvVisit online banking website of SBI by typing https:/lwww.onlinesbi.cclrn in the address bar of the internet

explorer. Once the home page appears click on'State Bank Collect'tab.

Accept the disclaimer clause and clicl< on "Proceed".

Select the State of Corporate/lnstitution as'Odisha'.

Select type of Corporate/lnstitution as 'Educational lnstitutions' and click on 'Go'.

Now select Educational lnstitution Name as'Council of Higher Secondary Education, Odisha'and click on

'Su bm it'.

The student may select the desired payment category from the drop down viz. Exam. Fees-2017

(Correspondence Course)-Student, when the fee is directlv deposited bv the student himself.

Likewise the College/lnstitution may select the desired payment category from the drop down viz. Exam.

important to reprint the e-Receipt subsequent to any successful payment. Hence, the above details may

be provided by the depositor, who is responsible for reprintinB the e-Receipt subsequently as and whenreq uired.

Once required data is filled in click on 'Submit'.

The system will ask you to verify the details entered and confirm the transaction in the next page. Verify

the details and click on 'Confirm'.

The system will request you to select the desired payment mode from various options. The Colleges can

pay the fees using Net Banking facility availed from any of their Banker or visit any SBI Branch to deposit

Cash or-Cheque drawn on any SBI Branch. Please note that the bank charges for various payment modes

are also displayed which is to be borne by the remitter. Carefullv examine the choraes displaved since

the charaes for different pavment mode ore different Select the desired payment mode.

lf Net Banking is selected as payment mode, the system will guide the remitter through the paymentprocess. Once the payment is completed successfully one e-Receipt is generated containing a Reference

Number in PDF format. Take a print out. You may also save this optionally for future reference. The e-

Receipt can also be regenerated/reprinted afterwards from'Payment History'link of 'State Bank

Co I I ect'.

lf 'SBl Branch' is selected as payment mode a Pre Acknowledgement Payment (PAP) form is

generated which contains the relevant details for payment at Branch. The payer takes the printout of the PAP Form and visits any SBI branch for payment through cosh or cheque drawn on SBI

Branch. After the payment is successfully processed by the branch, the branch will give an

acknowledgment of payment on the PAP Form.

Now any time the payer is to return to 'State Bank Collect' link in the home page ofwww.onlinesbi.com for printing the e-receipt from the 'Payment History' link and submit thee-Receipt in the Council along with the necessary accounts statement and forms.
