AbstractBACKGROUND AND PURPOSE:Research priorities, defined by multiple stakeholders, can proximally facilitate the coordination of research projects and national and international cooperation and distally further improve the quality of physiotherapy practice. The aim of this study was therefore to establish physiotherapy research priorities in Switzerland considering multiple stakeholders' opinions.METHODS:A mixed methods design was chosen. For a qualitative identification of physiotherapy research topics, 18 focus group discussions and 23 semi-structured interviews/written commentaries were conducted. For the quantitative analysis, 420 participants prioritized research topics using a two-round Delphi questionnaire survey. The following stakeholder groups were surveyed in the German-speaking, French-speaking and Italian-speaking regions of Switzerland: physiotherapy researchers, practitioners and educators, representatives of patient organizations, public health organizations, health insurers, physicians, nurses, occupational therapists and other health professionals, as well as physical educators.RESULTS:The top five overall physiotherapy research priorities identified were as follows: physiotherapy treatment, physiotherapy assessment and diagnosis, prevention, physiotherapist-patient interaction and physiotherapy professional education at the bachelor level. With regard to diagnostic groups, the highest priorities were placed on musculoskeletal disorders, neurology, orthopaedics, geriatrics and ergonomics/occupational health. Consensus was moderate to high, and only few differences between stakeholder groups were revealed.DISCUSSION:Research directly related to physiotherapy treatment is of highest priority. It should focus on diagnostic groups related to chronicity in anticipation of demographic changes. Multidisciplinary networks for research and practice, alongside sound coordination of research projects, should increase the impact of physiotherapy research. An accurate dissemination of research priorities, defined and supported by multiple stakeholder groups, might strengthen their impact on research and practice.

AbstractHuman T-lymphotropic virus 1 (HTLV-1) infection may be associated with damage to the spinal cord - HTLV-associated myelopathy/tropical spastic paraparesis - and other neurological symptoms that compromise everyday life activities. There is no cure for this disease, but recent evidence suggests that physiotherapy may help individuals with the infection, although, as far as we are aware, no systematic review has approached this topic. Therefore, the objective of this review is to address the core problems associated with HTLV-1 infection that can be detected and treated by physiotherapy, present the results of clinical trials, and discuss perspectives on the development of knowledge in this area. Major problems for individuals with HTLV-1 are pain, sensory-motor dysfunction, and urinary symptoms. All of these have high impact on quality of life, and recent clinical trials involving exercises, electrotherapeutic modalities, and massage have shown promising effects. Although not influencing the basic pathologic disturbances, a physiotherapeutic approach seems to be useful to detect specific problems related to body structures, activity, and participation related to movement in HTLV-1 infection, as well as to treat these conditions.KEYWORDS:HAM/TSP; HTLV-1; pain; physical therapy modalities; quality of life; sensory-motor dysfunction; urinary symptoms

Four themes, namely pre-rehabilitation, rehabilitation and post-rehabilitation stages and the documentation process, were identified. Each theme had different sub-themes. The feedback from the heads of the rehabilitation teams and physiotherapists helped the researcher to gather in-depth details of these themes and sub-themes. A valid, reliable and acceptable treatment recording tool for use by physiotherapists with people with ABI in an inpatient rehabilitation setting was developed using the information gathered during the previous phases, including a literature review. The results show that the newly developed documentation tool has the ability to record comprehensive details of treatment sessions using a very simple coding process in a very quick way. The treatment recording tool offers a sufficiently structured method to collect information about treatment sessions, including treatment tasks, treatment positions, interventions provided, adjuncts used and the treatment duration of each treatment task. Treatment packages (combinations of physiotherapy interventions) were also investigated using a geometric coding process. The results showed that the treatment recording tool records more comprehensive and organised details about physiotherapy treatment sessions compared to SOAP notes completed by the same physiotherapists.

The new treatment recording tool has been designed to improve patient care by facilitating accurate and appropriate communication between physiotherapists, and between physiotherapists and other specialists.

Developing the documentation method in inpatient settings will help other professionals to better understand physiotherapypractice and the role that physiotherapists play in the multidisciplinary rehabilitation service.

Quantitative testing of physiotherapy ultrasound beam patterns within a clinical environment using a thermochromic tileUltrasonics, Volume 58, April 2015, pp :6-10Žauhar G. 

The implementation of the non-standardized method developed at the National Physical Laboratory (UK) supporting the quality assurance of therapeutic ultrasonic beam parameters within a clinical environment is presented. The method consists of exposing an acoustic absorber tile, part of which contains a thermochromic pigment, to the ultrasonic beam, thereby forming an image of the two-dimensional intensity profile of the transducer. Nine different physiotherapy ultrasound treatment heads currently used clinically were tested using this method. Thermochromic images were postprocessed in order to estimate the Effective Radiating Area (ERA) for treatment heads operating within the frequency range from 1 MHz to 3.2 MHz, and nominal applied intensities in the range of 1–3 W/cm2. Experimental results and comparisons with manufacturer specified values of ERA are presented. Differences in the experimentally derived results and the manufacturer values are typically well within 25%. The root-mean squared difference calculated over the nine treatment heads is 15.1%, with the thermochromic material tended to underestimate the ERA.

Because of an increased risk of coronary atherosclerosis and associated morbidity and mortality in patients with RA, efforts to modify risk factors such as cigarette smoking, hyperlipidemia, hypertension, and sedentary lifestyle should accompany treatment directed at RA. Control of the inflammatory process also may contribute to cardiovascular protection, while there is some evidence that statin therapy improves inflammation in patients with RA. These issues are discussed separately.

Effects of short-term continuous positive airway pressure on myocardial sympathetic nerve function and energetics in patients with heart failure and obstructive sleep apnea: a randomized studyHall AB, et al.

CONCLUSIONS: In patients with heart failure with reduced ejection fraction and OSA, short-term CPAP increased hydroxyephedrine retention, indicating improved myocardial sympathetic nerve function, but overall did not affect energetics. In those with more severe OSA, CPAP may improve cardiac efficiency. Further outcome-based investigation of the consequences of CPAP is warranted.

Human Movement and RehabilitationInvestigating mechanisms and conditions affecting the control of human movement that inform new and effective methods for diagnosis and rehabilitationNormal control of movement can go awry in neurological and musculoskeletal conditions. This includes Parkinson’s disease, stroke and traumatic brain injury, as well as low back pain, whiplash and arthritis. To understand the aetiology of such conditions to develop effective treatments, it is first necessary to understand the healthy brain and body.

Exercise, Medicine and HealthInvestigating the mechanisms by which exercise aids health, and prevents and treats diseaseWithin this theme a wide range of research is undertaken to assess the effects of exercise on bodily systems in a diverse range of populations across the lifespan. Our work is important because to understand how exercise can maintain health, and prevent and treat disease we first need to understand the mechanisms by which exercise affects the cells, tissues and systems of the human body. This work is also required to better understand how exercise interventions might be optimized (e.g., intensity, duration, mode) or even individualized, such that they are as effective as possible in eliciting the desired adaptive response. The body’s response to an acute bout of exercise also provides a valuable assessment of physiological functioning, and our ongoing studies are using such tests to assess the benefits of interventions (e.g., exercise, pharmacological or nutritional) or to reveal abnormalities in physiological regulation that may limit physical capacity.
