I. EXECUTIVE SUMMARY

Inspired by the advancement of the modern technology

and the problems regarding enrollment transactions, the

proponents have come up with a computerized

registration system exclusively for school with many

enrollees. Enrollees are attached because of the

proposed system makes the transactions faster and

easier. It will benefit the school and also the

incoming student of Fort Bonifacio High School.

Computerized enrollment system makes work easier and

faster like enrollment transactions are possible with

the emergence of computer technologies.

The study focuses on helping the school in the system

that will help both the enrollment personnel-in-charge

and the students to easily process the enrollment at a

lesser time. It provides efficient and consistent

information services.

The proponents proposed a computerized

registration form to help the school to have an

accurate and reliable data for the students. It can

lead to organizational changes that range from

improvement of day to day operation.

II. INTRODUCTION

Nowadays, technology plays a vital role in

business industry. This makes man work easy and fast.

It also lessens error of work and reduces cost from

paper works up to computerized working system. Many

manual transactions can be computerized by using

software applications or computer systems to make

work easier, efficient and accurate.

Enrollment process is one of the major

transactions performed by schools which involved the

different transactions such as entering and verifying

data of student to register on a particular school,

payment of fees assigning of sections or sectioning,

and scheduling of subjects. Different interrelated

processes build up enrollment procedures called

Enrollment System (ES). ES are used particularly in

recording and retrieving student’s information.

Tracking student’s information is also one feature of

ES, in which the school can trace the standing of a

student.

Fort Bonifacio High School is an institution

located at JP. Rizal Ext., West Rembo, Makati City.

It originated from the Armed Forces of the

Philippines School for Enlisted Men (AFPSEM). It was

established informally in Fort Stotsenbergh (Basa Air

Base) in 1946. It was originally conceived as a

school for enlisted men who aimed to extend the

benefit of secondary education to AFP personnel who

were recruited to fight during the huk campaign.

It was established as a public school in

September 11, 1958 as a result of a M.O.A. between

the Department of Education and the Philippines Army.

It was upgraded by the virtue of the approval of the

DECS into a tertiary educational system and was named

Fort Bonifacio College in 1967.

In 1968, Major Oswaldo P. Villanueva assumed the

presidency. In the span of years, FABC has traveled

far since then. Mrs. Lydia F. Pineda, Principal III,

assumed leadership. The morale and welfare of the

faculty, staff and students were uplifted. High

school department was nationalized in 1988. The

Philippine army ceded the Fort Bonifacio College to

the city government of Makati through a memorandum of

agreement in January 10, 1990. Mr.Samuel C. Bulan was

designated as OIC principal in 2001. FBHS has been

reaping honors and awards academic and sportswise.

In April 2003, Mr. Dominico C. Idanan was

designated as the new principal of the school and

renewed quality of education and desirable values

through a high standard of competence, commitment and

dedication. He introduced a goal-oriented Fort

Bonifacio with a battle cry: Forward Fort Bonifacio

High School and Hit the Star. On February 26, 2004,

www.fbhs-makati.edu.ph, the official website of FBHS

was launched through the initiative of the School's

ICT Coordinator, Mrs. Lilybeth D. Sagmaquen.

Mr. Samuel C. Bulan was again designated FBHS

officer in charge in September 2004. A man of action,

he instituted significant program and projects.

Through his Project Rebound, the school performance

level in the NAT and DAT was raised from the cellar

to third and fourth places respectively.

On October 9, 2006, Mr. Wilmore C. Moredo

assumed his office as the new principal of Fort

Bonifacio High School. With his leadership he

introduced Project WCM which is a search for Worthy

Classroom Management, a school intervention program

where discipline is the foremost objective. "Learning

can be effectively transferred if we infuse

discipline to the students". Mr. Wilmore C. Moredo

emphasized discipline in his advocacy to academic

excellence and commitment.

For many years, Fort Bonifacio High School is

one of performing schools in the City of Makati. The

City Government under Mayor Jejomar Erwin S. Binay

has provided assistance – adequate means and

opportunities by which each teachers and students can

be developed their activities, their interests and

their needs.

On April 6, 2012, Mr. Roberto V. Anir, Principal

IV was chosen as the new School Head, a self-made

man, honest, and dedicated leader. Fully aware of his

basic roles and responsibilities and school

improvement, he institutionalizes SBM system through

shared leadership and applies continuing professional

development program. Moreover, Mr. Anir focuses more

on Skills Enhancement of Internal Stakeholders. In

his professional meeting, he shared his long term

goals inspired by the Three Seeds to Plant – Seed of

Long Term Dream, Seed of Long Term Team and Seed of

Long Term Learning.

The Principal IV of FBHS is Mr. Roberto V. Anir

who started his term last 2012. The present

Administrative Officer IV is Ms. Lina M. Carungcong,

under her is the Registrar II, Ms. Apolinaria Clarita

B. Briones. There are also seven (7) Subject Head

Teachers and two (2) Officers-in-charge.

The current number of enrolled students is 7,716

students: Grade 7 – 1,988 students, Grade 8 – 2,129

students, Grade 9 – 1,942 students and 4th year –

1,657 students. There are eight (8) in the FBHS:

English, Filipino, Mathematics, Science, Social

Studies, MAPEH, Values Education and TLE. There are

also 274 employees currently working in the school.

III. REVIEW OF LITERATURE

This chapter includes the ideas, finished thesis,

conclusions, methodologies and others. Those that were

included in this chapter helps in familiarizing

information that are relevant and similar to the present

study.

Ina study conducted by Berano, Aravelo, Abundo&Laput

(2014).Metropolitan Academy of Manila- School uses the

manual way of computing grades and adding information or

records of both students and professors. The process

entails a lot of effort and by computing and storing of

data manually the process is prone to error especially for

the professors who handle two or more classes. By using

the manual way of computing and storing data, being prone

to error is possible and just by a single mistake,

repeating the process may be required.

Southern California Community College Long Beach City

College has gone live with a Cognos-based enrolment

management system that was designed by Irvine, CA-based

professional services firm e2e Analytics. Prior to

installing the new system, LBCC, which has two main

campuses and multiple satellites that support nine schools

and 34 departments, handled its enrolment via manual

spread sheets. With no reporting, continuity, or standard

practices, and with lengthy error corrections, the

school’s enrolment management budgeting and planning took

up to three months to compile, consolidate, and implement

(Koft 2007).

IV. METHODOLOGY

The researchers of this study have looked, searched

and find for a company or business organization which can

be used for this study that still, as we are now in a

modern technology world, uses manual recording of their

client’s filled up registration forms for the operation of

a certain company.

Then the researchers have found out that Fort

Bonifacio High School, which located in Makati City, still

uses manual recording of their students registration form.

For this reason, the researchers have come up and decided

to propose a computerized registration for Fort Bonifacio

High School. The first step is to ask for permission to

the authorized personnel of the said school to conduct an

interview about the school, the flow of their

current/incoming student registration and ask for the

school’s copy of registration form, and a little bit of

their background and history. The researchers started

doing the paper works and study about making a

computerized registration form using a computer program

called JCreator. The researchers designed and encoded the

registration form using Java language. After the codes had

properly ran, the researchers started the papers to be

compiled or also called documentation of the system.

VI. ANALYSIS

Upon the completion of this study regarding

Computerized Registration Form, the problem that arises from

the Local registration Form is that Manual Recording and

Registration is not applicable anymore in this time of

modern age, because it may cause loss or damage of data, in

case of fire or flood, compared to the Foreign Registration

Form which uses Computerized Registration Form from a

professional IT service provider, but also they are

upgrading their system according to the needs of the School

and the Students.

After the Analysis,the researcher proposed the

system about Computerized Registration form which has the

same purpose of Metropolitan Academy of Manila and Long

Beach City College. The purpose of both developers is to

save time and reduces the amount of paperwork involved in a

transaction. The Computerized Registration form allow for

greater efficiency in performing specific tasks both more

accurately and more rapidly than doing the same task

using.And not only Fort Bonifacio High School that has a

manual registration form, but also the Dagupan National High

School. Both of that school has the same problems and

difficulties in keeping the records of the students. But the

existing system of Dagupan National High school has many

problems, difficulties and concerns because of untimely and

inefficient report generation. The security of each student

records of Dagupan National High School was at high risks

and may cause loss of the accurate data of students.

The manual registration form is hassle for the school staff

and also the students because it takes long hours waiting to

fill up the forms. Keeping accurate dataor records is more

efficient if it is computerized system because the files

save automatically in the system. So the proponents proposed

a computerized system that will make their work easier and

faster. But Southern California Community College Long Beach

City College already had their computerized enrollment

system. They took three months in implementing their system.

The proposed computerized system in Fort Bonifacio High

School is more effective and efficient because the

proponents assured the system they will make is better than

the other system.The computerized system in Fort Bonifacio

High School may result in increase in the incoming students

and have a good flow of enrollment process.

VII. CONCLUSIONS AND RECOMMENDATIONS

This study is for Fort Bonifacio High School in which

the researchers have the objective of improving and

developing the quality of service and changing the slow

progress of the said school due to their manual

process .This study focuses on making computerized

registration form of the school. The use of computer and

information technology to the school enabled both the

students and enrollment personnel-in-charge to easily

process the enrollment at a lesser time.

The researchers of this study would like to recommend

the system to the Fort Bonifacio High School to create an

efficient, fast and reliable computerized registration

form. By using the proposed system, there would be less

error of work and reduced processing time. It will benefit

not only the student but the administration as a whole. We

also highly recommend this system to the computer

programmers to have ideas and enhanced the interface by

adding features. Lastly, the proponents would like to

recommend this study to the future researchers especially

for the incoming BSAT students who will take the PDTAPPS -

Fundamentals in Programming, Database Theory and

Applications to help them reach their goals to make this

program.
