Southern Trust Laboratory Services The Histopathology and Cytopathology laboratories of the Department of Cellular Pathology are located at Craigavon Area Hospital and are part of the main Laboratory complex along with Haematology, Blood Transfusion, Clinical Chemistry and Microbiology with Serology laboratories. There is a satellite laboratory at Daisy Hill Hospital which provides Haematology, Blood Transfusion and Clinical Biochemistry. The Pathology and Laboratory Service (PLS) is managed within the Cancer and Clinical Services Division of the Acute Medical Services Directorate of the Southern Trust. The Associate Medical Director for Cancer and Clinical Services, is Dr Shahid Tariq. The Assistant Director for Cancer and Clinical Services is Mr Barry Conway. The Laboratory Service is led by the Clinical Director, currently Dr Clare McGalie (Consultant Cellular Pathologist), in partnership with the Head of Pathology Services, currently Mr Geoff Kennedy. The Clinical Director is responsible for medical and clinical scientist manpower and for the clinical and strategic direction of the service. This post is appointed by the Director of Acute Services and the Medical Director and is reviewed every three years. The Clinical Director is supported by the Lead

Consultants in each laboratory discipline. The Head of Pathology Service is also responsible for strategic direction and for scientific and administrative staff and for operational issues. They also administer the Laboratory Service budget. The Management arrangements are in accordance with the recommendations of the Strategic Review of Pathology Services, 1995 Paragraphs 422-426. The Departments of Clinical Biochemistry, Haematology, Cellular Pathology and Microbiology hold full UKAS accreditation against ISO 15189. The blood bank is licensed by the medicines and health regulatory authority (MHRA). In keeping with accreditation requirements, there is a Quality Manager, currently Miss Pamela Allen. The Quality Manager is responsible for overseeing the Quality Management System of the PLS. There is a Health and Safety Committee serving the whole laboratory service with nominated officers within each discipline. The named pathologist with responsibility for Cervical Cytology services within the Southern Trust as part of the NI Cervical Screening Programme is Dr Michael McKenna. The named pathologist with responsibility for the pathology service within the Southern Trust which is

part of the NHS Breast Screening Programme is Dr Aaron Ervine. The named pathologist with responsibility for the pathology service within the Southern Trust which is part of the NHS Bowel Cancer Screening Programme is Dr Rajeev Shah. These positions are reviewed annually as part of consultant appraisal.

Post-graduate trainee staff

The Department has been recognised by the Royal College of Pathologists and the Regional Postgraduate Dean, Northern Ireland Medical and Dental Training and Education as being suitable for specialist training. There are Specialist Registrars, on rotation from the Belfast Health and Social Care Trust Histopathology Service. Biomedical scientists The Histopathology and Cytopathology laboratories function as a single department with bi-disciplinary Biomedical Scientists grades 1 and 2 and Medical Laboratory Assistants, uni-disciplinary BMS 3’s for Histopathology and Cytopathology and a Principal BMS 4 who leads the BMS and MLA staff.

Surgical biopsies 19,568 Gynaecological cytology 27,900 Non-gynaecological cytology 2822 Histopathology services A wide range of surgical specimens, diagnostic biopsies and therapeutic resections, are processed and reported. This includes specimens from patients managed by the NHS Breast Screening Programme and the NHS Bowel Cancer Screening Programme. At present, the consultants are general pathologists several of whom have areas of special interest and expertise. The successful appointee will practice as a general cellular pathologist however they will be encouraged to develop sub-specialist expertise in areas to be determined on discussion with colleagues. There is a limit as to the number of Consultant who can report Breast Pathology Cases. Ultimately sub-specialist reporting practice may be fully developed. Dr Swarbrick, Specialty Doctor in Histopathology carries out specimen dissection; the Consultants are expected to supervise and direct her as necessary. The following types of biopsies are reported elsewhere:

Biopsies for suspected malignant lymphoma are processed and reported in the Dept. of Tissue Pathology, Belfast Health and Social Care Trust.

Muscle and nerve biopsies are processed and reported by the Department of Neuropathology, Royal Group of Hospitals, Belfast.

Renal biopsies from the sub-regional Renal Unit at Daisy Hill Hospital are processed and reported by the specialist renal histopathologists, at the Royal Hospitals, Belfast.

Medical liver biopsies are processed and reported by the specialist GI/ hepatobiliary histopathologists, at the Royal Hospitals, Belfast.

The Regional Paediatric Pathology service is provided by Alder Hey Hospital, Liverpool.

Immunocytochemistry is carried out on a daily basis, as required, and the range of antibodies in use is constantly updated. A frozen section service is provided. There is access to a Molecular Pathology Service at the Northern Ireland Molecular Pathology Laboratory, Belfast and at Antrim Area Hospital and to an Electron microscopy service at the Department of Pathology, The Royal Hospitals, Belfast.

Gynaecological cytology services Cervical samples generated by the NHS Cervical Screening Programme are processed using the ThinPrep LBC technique. Dr Michael McKenna is the named pathologist with responsibility for the Cervical Screening Programme, including failsafe and quality assurance, and for the issue of negative or unsuitable cervical smear reports by biomedical scientists. Mrs Caroline Murphy, BMS, is the hospital based NHS Cervical Screening Programme Coordinator for the Southern Trust. HPV testing of LBC samples is carried out in both Altnagelvin Hospital and Antrim Area Hospital Cellular Pathology Departments for triage and test of cure cases. The delivery of the NI Cervical Screening Programme is undergoing regional review in anticipation of the introduction of HPV testing as the primary screening modality and it is likely that gynaecological cytology services will be delivered from fewer than the current four laboratories. The appointee will not be required to participate in Cervical Cytology, unless they so desire. Non- gynaecological cytopathology services The reporting of non-gynaecological cytopathology specimens is carried out only by those pathologists with training and expertise in this area. It would be desirable, however it is not essential for the appointee to have up to date training and be currently competent to report these specimens. Breast cytology specimens, generated by the NHS Breast Screening Programme and from symptomatic breast clinics, are reported. A pathologist is required to be available within the laboratory to provide a rapid reporting service to these clinics which are held in a dedicated Breast Unit, in the Macmillan Building, close to the Laboratories. Breast cytology specimens are procured by clinical or radiological staff.

Fine needle aspirates from other topographical sites are also reported. These are usually carried out by clinicians or, more often, by radiologists using imaging localisation. Endoscopic Ultrasound guided FNAs are carried out in the Radiology Department, Craigavon Area Hospital. A BMS attends these procedures to ensure that an adequate sample is obtained. Post-mortem examinations There are no post mortem examinations carried out by the SHSCT pathologists in the Mortuaries of the SHSCT. All adult hospital or consented post-mortem examinations and medico legal post mortem examinations are carried out by the pathologists of the Histopathology Department of the Belfast HSC Trust. The remains of the deceased are transferred to the Mortuary of the Royal Hospitals, Belfast. All

forensic coronial post mortem examinations are carried out by the State Pathologist or his deputy, in the mortuary of the Department of Forensic Medicine, Belfast. All foetal, perinatal and paediatric consented post mortem examinations are carried out in Alder Hey Hospital, Liverpool. The SHSCT maintains a licence with the Human Tissue Authority for the removal and storage of relevant material from the deceased but not for the making of a post mortem examination. Dr Gareth McClean is the Designated Individual for this licence. Multidisciplinary team meetings The pathologists participate in multidisciplinary team meetings for discussion of the diagnosis, management and treatment of patients with cancer. The meetings take place in Craigavon Area Hospital and some include videoconferencing to clinical colleagues in Daisy Hill Hospital and oncology colleagues in the Cancer Centre, Belfast. Attendance at and organisation of these meetings is shared by the consultant pathologists with a designated lead pathologist as a core team member for each cancer specific site, depending on whether they participate in the relevant NHS screening programmes and on their special interests. On average, the lead pathologist for each MDT is allocated 1 PA in their job plan for preparation attendance and decision management of MDT meetings.

The appointee will be expected to develop an area of special interest complementary to his colleagues, to contribute to the appropriate Multidisciplinary Team and to deputise for Pathologist colleagues at MDT meetings. There are facilities for demonstrating macroscopic and microscopic pathological material, with integrated data projection systems, to audiences in the Medical Education Centre in the main Hospital building. EQUIPMENT Both histopathology and cytopathology laboratories are well equipped, including the following: Leitz, Olympus and Nikon microscopes, including multi-headed microscopes for in-house teaching. Digital cameras for macroscopic and microscopic pathology records. Digital pathology reporting will be rolled out in the Southern trust from June 2021

DEPARTMENTAL FACILITIES FOR THE APPOINTEE An individual office and an individual consultant-grade microscope with teaching head and camera are available within the department for each consultant. Each consultant also has a personal computer with access to, LIMS, e-mail and the internet. Secretarial support for the appointee will be provided from within the department. The laboratory information system currently in use is LabCentre and the Southern Trust Cellular

Pathology Department is part of a regional LIMS. Some of the consultants use a voice recognition system for production of written reports. Others dictate reports which are typed by secretaries, ready for authorisation. The Department has a wide range of up to date reference textbooks. The appointee will have access to the Medical Education Centre within the hospital where there is a well-stocked medical library with books and journals and on-line access to further journals, Up-to-date medical databases such as Cochrane and Medline and the medical library of Queens University Belfast. QUALITY STANDARDS

Laboratory Accreditation The Departments of Clinical Biochemistry, Haematology, Cellular Pathology and Microbiology hold full UKAS accreditation against ISO 15189. External Quality Assurance Schemes Consultants and Cytopathology BMSs who report cervical cytology samples participate in the Cervical Screening Programme Gynaecological Cytopathology EQA Scheme. Some the Consultants participate in the NHS Breast Pathology External Quality Assurance Scheme and all participate in the Scotland and Northern Ireland General Histopathology External Quality Assurance Scheme. Some consultants participate in the National Gynaecological Pathology EQA scheme, the National Slide-based Uropathology EQA scheme and the National Dermatopathology EQA Scheme. Those consultants who report specimens generated by the NHS Bowel Cancer Screening Programme participate in the BCS EQA scheme. The Histopathology laboratory participates in the Ireland Histopathology Technical Quality Assurance Scheme (a member of the UK HTEQA Group). It also participates in the UK NEQAS scheme for Immunocytochemistry, the General, Breast, Lymphoma and Cytology modules. Internal Quality Assurance All the consultants discuss difficult or complex cases with colleagues thereby providing quality assurance and standardisation of reporting. There is a time allocation for this within each consultants job plan.

TEACHING Undergraduate medical students The Specialty Doctor within the Department of Cellular Pathology delivers the teaching to third year undergraduate medical students. This involves providing seven Clinical Pathology Conferences per year to students who are placed in Craigavon Area Hospital for clinical teaching. Input into the assessment of students’ progress is also required. The CPC teaching materials are prepared and administered by Queens University Belfast. The Pathologists also supervise third year undergraduate medical students who choose to spend three weeks in the Department as one of their Student Selected Modules. This involves two students twice a year. Dr Christine Swarbrick is the Undergraduate Educational supervisor. Postgraduate Cellular Pathology trainees The Consultants of the Department contribute to the postgraduate medical training programme. Dr Marie McFarland is the Postgraduate Educational Supervisor.

DUTIES OF THE POST: The post holder will:

Together with consultant pathologist colleagues, be responsible for the provision of a comprehensive histopathology and non-gynaecological cytopathology service to the hospital clinicians and General Practitioners of the Southern Area. This will include being available to offer advice to clinicians on how best to use the Service. The major clinical specialties are: Gastrointestinal, Skin, Breast, Gynaecology, Urology, Respiratory and ENT. The reporting balance is approximately 85% histopathology and 15% non-gynaecological cytology with the latter being shared across the department. There is no requirement to attend one stop clinics or provide rapid assessment of adequacy on cytology as part of this post. However if the applicant has a particular interest in non-gynaecological cytology then opportunities are available. The work is distributed using a modified version of the RCPath workload point’s scheme.

Supervise specimen dissection by the Specialty Doctor, undertake specimen dissection as needed.

Be required to co-operate with their colleagues in the development and improvement/extension of the cellular pathology services and to keep up to date with innovations within the profession.

Be encouraged to develop/enhance a sub-specialist expertise to meet the needs of the department. The area of sub-specialist expertise will be decided on discussion with colleagues and take into account the needs of the service. Assistance in the development of expertise will be provided through granting and funding of study leave. The post holder will be responsible for pathology input into the MDT relevant to their own area of special interest. The job plan will be modified accordingly. Pathologists in the department provide cover for each MDT lead Pathologist when on leave.

Be required to undertake with consultant pathologist colleagues the professional supervision, management and training of trainee medical staff and other professions associated with Cellular Pathology: The hospital is approved for both undergraduate and post graduate teaching. The post holder will be required to participate in undergraduate and postgraduate teaching. Training is available for Specialist Registrars from Belfast Deanery on a 6 month placement (Usually one). Medical students from Queens University Belfast spend three weeks (biannually) within the department as part of one of their Student Selected Modules. The Specialty Doctor delivers the Undergraduate Clinico-Pathological Conferences.

Be required to participate in the Clinical Governance Framework of the Southern Trust. o Audit o Risk Management o Incident reporting and investigation o Attendance at Cellular Pathology Specialty Meetings o Quality Management o Promoting evidence based medicine

Be required to assist in the management and administrative duties of the Department.

Be required to participate in appropriate internal and external quality assurance/ proficiency schemes, relevant to their areas of practice.

Be required to participate in Continuing Professional Development. The Southern Trust will provide time (up to 10 days study leave per year) and financial support for attendance at UK and international meetings and conferences.

Be required to undergo annual appraisal. The Appraisal Scheme is administered in line with national and regional guidelines and as described in “Medical Staff Appraisal Scheme” policy and procedures, available on the Trust Intranet. The RASP online appraisal and revalidation system is in use in the Trust. The appraisal is sent to the Medical Director for review and quality assurance. The Appraisal scheme forms the platform for reviewing information required by the GMC for revalidation.

Provide cover for their pathologist colleagues during their leave of absence (sick / professional leave, annual leave, study leave). Locum cover is not usually provided.

Participate in Category B on-call rota (1%) for clinical advice. There is no provision of an out of hours biopsy reporting service.

Ensure they are contactable by telephone when on-call.

JOB PLAN REVIEW This Job Plan is subject to review at least once a year with the Clinical Director, before being approved by the Director of Acute Services. In the case of a new employee, a review of the Job Plan will take place within 3 months of commencement and annually thereafter. If it is not possible to agree a Job Plan, either initially or at an annual review, there are agreed procedures for facilitation and appeal with the final decision normally being accepted by the Trust Board. The allocation of SPAs included in the Job Plan will be subject to review. Balance between Direct Clinical Care and Other Programmed Activities The Trust expects that all consultants undertake a minimum of 1.5 SPA’s (6 hours) in their job plan every week, however it has been agreed by the Medical Director that all new consultant appointments will be allocated 2SPA’s per week for a period of 12 months to facilitate induction and orientation into the organisation and the role. This position will be reviewed after the consultant completes 12 months in post. The Trust also recognises that there are various activities as identified by all the Associate Medical Directors in each directorate and approved by the Medical Director where additional SPA time will be necessary. Where a newly appointed consultant will be involved in these additional SPA commitments, the precise balance of Programmed Activities in their job plan will be reviewed on appointment and agreed as part of their individual Job Plan review. These Supporting Professional Activities may include participation in training of other staff, medical education, continuing professional development, formal teaching of other staff, audit, job planning, appraisal, research, clinical management and local clinical governance activities are recognised within the Southern Health and Social Care Trust. Programmed Activities for additional HPSS responsibilities and external duties will also be allocated for special responsibilities that have been formally approved and/or appointed by the Trust. GENERAL REQUIREMENTS: The post holder will be required to:

ESSENTIAL CRITERIA – these are criteria all applicants MUST be able to demonstrate either at shortlisting or at interview. The following are essential criteria which will initially be measured at Shortlisting Stage although may also be further explored during the interview stage;

1. Hold Full registration with the General Medical Council (London) with Licence to Practice; 1

2. Hold FRCPath or show evidence of equivalent qualification

3. Applicants that are UK trained, must ALSO be a holder of a Certificate of Completion of Training (CCT) in Histopathology, or be within six months of award of CCT by date of interview. Applicants that are non-UK trained, will be required to show evidence of equivalence to the UK CCT.

4. Experience of Histopathology and current evidence of competency in independent reporting of such cases.

5. Hold a full current driving license valid for use in the UK and have access to a car on appointment.2 The following are essential criteria which will be measured during the interview stage.

1 If successful at interview, applicants will be required to provide proof of their GMC application. Applicants must be

registered, with a licence to practice at the time of appointment. 2 This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving

but who is able to organize suitable alternative arrangements in order to meet the requirements of the post in full.

1. Knowledge of current issues in Cellular Pathology and experience of independent reporting.

2. Understanding of the implication of clinical governance.

3. Have an interest in teaching.

4. Ability to lead and engender high standards of care.

5. Ability to develop strategies to meet changing demands.

6. Willingness to work flexibly as part of a team.

7. Effective Communication skills to meet the needs of the post and interpersonal skills.

8. Ability to work well within a multidisciplinary team.

9. Ability to effectively train and supervise medical undergraduates, postgraduates and biomedical scientists.
