Vision

The fact that you can read our Annual Report is becauseyou are blessed with

sight.

We open two gifts every morning.

Oureyes…

But there are

Pakistanis who are not so lucky and are miredin a life of darknessand despair.

22million

But you can change this by helping us give them the

GIFT ofSIGHT.

Sight is one of the greatest blessings of God, the mostcritical of all senses.Eyes are not only the windows to the soul but also the most important sense that God has gifted us. The absence of any sense reduces the productivity of a person but absence of vision makes them a burden not only on the family but also on the community.

80% of blindness is curable and with a relatively small sum of money they can have their vision restored transforming them into income generating individuals for their family and society.

Diabetesand the eye

LRBT Annual Report 2020 15

One of the complications of long standing and uncontrolled diabetes is diabetic retinopathy, which, if left untreated, can lead to an irreversible loss of vision. This can happen to a third of uncontrolled long standing diabetics.

There are 3 possible interventions for DR: A-VEGF injections (where there is no diabetic macular edema), Pan Retinal Photocoagulation (PRP) where there is edema, Vitrectomy for complex and advanced diabetic retinopathy.

These treatments are very expensive and well beyond the ability of 39% of our population, who live in multi-dimensional poverty, to pay.

11 of our hospitals offer A-VEGF injections,16 PRP & 7 Vitrectomy free of cost for the poor.

Management ofDiabetic Retinopathy (DR)

EFFECTS OF DiabetesGlobally and IN Pakistan

Diabetes is one of the fastest rising public health issues and one of the major causes of blindness in the adult population in their productive age.

Globally, the number of diabetics increased from 285 million in 2010 to 451 million in 2017 and is predicted to reach 693 million by 2045. It is estimated that by 2030, 3.3 million people will go blind from diabetic retinopathy mainly due to the rise in Type II diabetic patients.

According to the 2017 National Diabetic Prevalence Survey the prevalence of Type II diabetes is 17% amongst the total population. A Sightsavers study “Prevention and Control of Diabetic Retinopathy in Pakistan” conducted in 2018 showed that the prevalence of diabetes among adults 20 + years only is 26.3%. All parts of the country are affected, with the highest being in Sindh and lowest in Khyber Pakhtunkhwa. There is good evidence that changing life style: losing weight, increase in physical activity and eating more fruits and vegetables reduces the incidence Type II diabetes.

The major causes of blindness are Cataract and Refractive errors and to a much lesser extent Diabetic Retinopathy (DR), Corneal Diseases(CD) and Glaucoma. LRBT had in its early years focused on the first two at all our hospitals as they accounted for over 90% of blindness and impaired vision. The other 3 were being treated in our Tertiary Hospitals. The issue with DR and Glaucoma is that they are irreversibly blinding diseases. The progress of the disease however can be arrested if diagnosed early and treatment done regularly. In 2006 screening and treatment of Glaucoma began at all our hospitals, With the growth in cases of Diabetic Retinopathy (DR), it has also become a priority.

In this Annual Report we will be focusing on Diabetes. Its effect on vision and our efforts to control this irreversible blinding disease.

NUSRAT is a 30-year-old young mother of 4 children who has been suffering from Type 1 diabetes for many years. Her parents had diabetes as well, so she had a history of the disease. She has seen more tragedy in her early years than many do in their entire lifespan. She had a traumatic childhood and a miserable marriage. Nusrat’s husband was a daily-wage laborer who abandoned his family of four children and wife. Left without any source of income, Nusrat opted to work as a domestic help to support herself and her children. However, recently she felt deterioration in her vision with the left eye being worse affected. She was already overburdened with financial problems: no savings, not enough earnings to cover cost of food, utilities, school fees etc. and the cost of her insulin injections. And now came the challenge of treating her eye problem which was essential if she wanted to retain her job which was the sole source of her income.

Firstly, she tried home remedies and hakim’s tonics but nothing helped so borrowed money from her employer and visited an ophthalmologist who told her the cost of her treatment would be Rs.150,000. She desperately wanted to get the treatment but after hearing the cost she broke down in tears in front of the doctor and shared her financial position. The doctor consoled her and referred her to the local LRBT eye hospital, which offers free of cost eye treatment to poor and needy patients. LRBT doctors examined her and diagnosed not only a cataract but also a problem with her retina. She underwent combined cataract surgery, IOL implantation and micro incision vitrectomy surgery operation.

worked in Muscat Oman as a Garage Incharge for many years. He had a recurring eyesight problem due to diabetes and his retina was swollen which impacted on the effective performance of his job. Shahab couldn’t afford the cost of treatment in Oman so he found a solution by visiting Pakistan periodically so he could to get Avastin injection free of cost at LRBT. However, he could not always adhere to the schedule of injections due to work pressures.

However, tragedy struck the hardworking man, 6 months after the COVID-19 pandemic struck, he was laid off from his job. Following this, it became impossible for him to make both ends meet but was glad that his free treatment continued. Today, he can see better but the situation remains grim with COVID-19, making jobs scarce but Shahab remains positive. He is actively looking for work in Pakistan so he can support his family to the best of his abilities.

shahab firmly believes that his renewed

vigor and healed eyesight will get him

a good job soon.

““

a 24-year-old young man is one of those rare patients who developed diabetes at a very young age of 19. He has aged beyond his years because of the responsibilities that have been thrust upon him from a very early age. Saleem and his family, including his 4 sisters, a brother and aged parents live in a rented house located in a slum of the city. He could barely afford the rental of Rs. 7000 per month and household expenses, let alone pay for any medical costs. This was the reason why he kept postponing his eye treatment. This resulted in his vision deteriorating to such an extent that he could no longer work regularly. He consulted several doctors for treatment, but they failed to improve his sight. The worry constantly haunting Saleem was that without any source of income his family would become destitute.

A silver lining appeared when a cousin of Saleem’s advised him to go to LRBT where he would be receiving state of the art treatment free of cost. Once at LRBT, Saleem was diagnosed with diabetic retinopathy and a Micro-Incision Vitrectomy Surgery (MIVS) was performed the next day by a dedicated team of retinal specialists and paramedics. After his recovery, Saleem was able to get himself a job as an office attendant in an office, serving tea, food, and water, as well as maintaining records and files.

thank you LRBT for giving me a

second chance and restoring my vision,

enabling ME to earn respectably.

44MILLIONTH PATIENTQAMAR JEHAN44

Qamar is the sole bread earner for her family. She supports her aging mother too, who came to live with her when Qamar's father passed away. Qamar would leave her 8 children with her mother while she went to work. She is a lady health worker and has to go door to door in slums to convince residents to opt for the ongoing vaccination drive. She would be left drained but had to look after her three handicapped sons when she would return home. They suffered from intense muscle weakness and their disease had progressed so much that they were unable to even feed themselves.

Despite all these difficulties Qamar remained strong, she married off her two daughters and once her two eldest sons found jobs they got married too and moved out. She and her husband used to run the household paying the rent and utilities. Qamar’s mother was aging and her medical bills were piling up, her youngest son’s health was steadily deteriorating too, she was being pulled in so many directions that she was at a loss of how to cope. Unfortunately, true tragedy struck when her youngest son passed away three years ago. She had no choice but to bounce back but it was her husband who was so grief stricken that he gave up on life altogether and passed away within a month of his son’s death. It is testament to a woman’s strength that Qamar put two loved ones in the grave within the space of a month and yet managed to find the will to go back to work and look after her mother and two disabled sons.

Life just keeps on throwing curveballs for some people and Qamar’s next one came when she started to have issues with her right eye. She was unable to see properly and had to make several visits to the ophthalmologist who initially gave her eye drops and then prescribed glasses. A colleague advised her to visit LRBT Korangi. Before the checkup Qamar’s worst fear was who would look after her two sons. It was during this time that Qamar’s daughter was diagnosed with cancer, Qamar had to look after her 5 grandchildren and work around her daughter’s treatment to take time out to visit LRBT.

Once she came to LRBT Korangi Hospital, she underwent a thorough examination and was diagnosed with retinal detachment due to diabetes. She had a successful micro incision vitrectomy surgery (MIVS) to reattach the retina and after rest is now back to work.

At her follow up visit, Qamar said,

“With life knocking me down every step of the way, coming to LRBT and being treated here was perhaps the only thing that kept me from collapsing under the sheer burden of my responsibilities. When life throws you down you have to hold on to every ray of hope and for me that was LRBT.”

Chairman’sReport

Our achievements were made possible by our generous donors both old and new, whose donations enabled us give the GIFT of SIGHT to 45 million people.

The Fiscal year 2020 has been a challenging one with all our hospitals and clinics being closed by the Government (opthalmology was classified as an elective treatment) for three months from the 4th week of March 2020, due to COVID-19. Emergency services however continued.

We started Teleservices with our ophthalmologists being available to patientsfor consultation. We also distributed food rations to poor needy families from our hospitals during our closure from our own resources and in partnership with Pepsi foundation.

Improvement of facilities at our hospitals continued with another floor added on to the Aisha and Ibrahim Bhaimia OPD Wing at Korangi to house our 6 sub-specialties in more spacious areas. A major upgradation of our Lahore Township Teaching Hospital was also completed providing better seating facilities in the waiting areas and in the wards.

At Shahpur we opened an Optical Shop, constructed two additional Refraction Rooms, three Waiting Areas and renovated the bathroom Block equipping 2 of the bathrooms for disabled patients.

Architectural and structural plans of the new hospital building at Mandra have been approved and construction will commence next year. With the current building of North Karachi Hospital being insufficient to cater to increasing number of patients visiting it and with no space for further expansion we are planning to build a new one. A generous donor has agreed to donate us land and we are looking for a donor who will fund its construction. This takes precedence over the Rahim Yar Khan project which has been deferred until new North Karachi hospital building is completed.

Upgradation of our Ophthalmic Equipment continues and we spent Rs.169 million to acquire state of the art equipment to ensure that despite being free, our patients get the best possible treatment in accordance with the second of our Core Values.

Our achievements were made possible by our generous donors both old and new, whose donations enabled us give the GIFT of SIGHT to 45 million people who would otherwise still be suffering a life of darkness and despair. We would also like express our gratitude to our volunteers, the media who gave us free space and time as well as our staff whose devotion and dedication has made LRBT the largest provider of eye care in Pakistan and indeed in the world.

Review ofOperations

OPD VISITs/SURGERIES

During the year ending June 2020, 2.4 million patients visited our 19 hospitals and 58 Community Eye Health Centers. During the same period 215,709 major and minor surgeries were performed. Cataract, which is the leading cause of curable blindness constituted 84% of major surgeries. Other surgeries included vitreo-retinal procedures, Trabulectomy, Cornea transplants, DCR, Squint etc. Our services were affected by the complete closure of all our facilities by the Government, from 22nd March 2020 to 21st June 2020, due to COVID-19. However, our Emergency departments stayed open throughout the closure. We also operated teleservices where ophthalmologists were available for consultations at all hospitals.

14,923 retinal procedures were performed. This included 12,833 patients who were treated with Avastin for Age Related Macular Degeneration and Diabetic Retinopathy. 24,778 laser procedures were also performed. 211 corneas were transplanted at our Lahore Tertiary, Korangi Tertiary and Quetta Hospitals.

We remain the biggest provider of eye related surgeries for the poor people in the country.

In our aim to ensure the highest quality of surgical care we regularly evaluate ourselves against the WHO/ICEH benchmark for quality cataract surgery. This is expressed in terms of visual outcomes. In 2019/20 we were 7 percentage points higher than the benchmark.

COVID-19 AND IT’S IMPACT

LRBT hospitals were closed from 22nd March 2020 due to the lockdown imposed by the Government of Pakistan who had mandated eye treatment to be elective. Only Emergency and tele-services were kept open. 31,000 patients were seen during the closed down period. On 22nd June 2020, all hospitals were re-opened for the patients after it was ensured that SOPs were fully in place. Arrangements were made at the entrance to conduct triage, followed by disposal of the mask in use, handwashing after which a fresh surgical mask to each patient was required before being permitted into the Hospital building. The number of patients were limited due to social distancing.

The pandemic impacted our services significantly as all our facilities were closed for 3 months. Consequently, patients visit declined from a growth of 4% in the period 1st July 2019 to 22nd March 2020 to a decline of 23% by 30th June 2020 due to closure of all Hospitals and PEC’s. There was a similar effect on surgeries, laser procedures etc.

Our operating expenses during the year were Rs. 1,373 million being 6.7% above last year. Main reason being increase in salaries and wages due to annual increment of employees, off-set by lower expenses due to closure of hospitals for 3 months. Capital Expenditure during the year was 222 million, 21% lower than last year mainly because of reduced level of activity due COVID-19. Our overhead expenses are 4.51% of our revenue expenses which are amongst the lowest in the world.

Expansion ofcapacity

Korangi

With a steady growth in the number of patients requiring to visit our 6 subspecialty clinics there was need to have a separate floor to house them. This project was launched on 1st March 2019 and completed on 15th March 2020. Ms. Khurshid Bhaimia, our longtime and loyal supporter financed this project. The Bhaimia family had earlier sponsored the entire second floor of the main Hospital building and later the construction of a new building where the Outpatients Department is housed.

We are extremely grateful to Ms. Khurshid Bhaimia for her continuous support to LRBT.

Shahpur

Under the current UKAM partnership agreement with Sightsavers, we were required to set up Optical shops in the four project Hospitals to facilitate and enable the poor patients getting quality eye wear at affordable prices from these shops and the facility’s becoming a one window operation without having to travel to other locations just to purchase glasses. As these shops were already operational at the other three Hospitals under an arrangement with another International NGO, it was, therefore, required at the Shahpur Hospital only. In addition, a block of four toilets were also constructed for disabled persons, of which one each was for male and female in the OPD area and one each in the wards. This project began on 1st May 2019 and was completed on 27th December 2019.

For the convenience of the patients, two waiting areas, one each for males and females were also added in the refraction area so that they could comfortably be seated while waiting for their turn.

SOPHISTICATEDEQUIPMENT

The following state of the art equipment was provided to our various Hospitals to enable our doctors to provide quality eye care to the poor people who visit them ensuring that free treatment is supported by the most sophisticated equipment available.

Additionally, the following occasional volunteers also provided valued support: Nizar Din, Rehan Rajput, Sarah Jane Wilford, Heera Rahman, Afshan Mirza, Asad Mirza, Wajeha Qureshi, Amaad Mahmood.

World Sight Day Campaign 2019On World Sight Day the Graham Layton Trust undertook a digital campaign to raise awareness of eye health, supported by social media in luencers who were very effective in reaching new, younger audiences.

Christmas Campaign 2019A postal and digital Christmas campaign provided an opportunity to touch base with donors once again, through which significant further contributions were received.

Global Pandemic 2020Preparations for three Musical Fundraising Evenings in March 2020, with Pakistani Coke Studio star, Komal Rizvi were well underway when escalating reports of the global Coronavirus pandemic forced GLT to postpone the events. This was a devastating blow as we were planning to additionally organize two Galas in London and Manchester for the Autumn of 2020, income from which would have given our revenue a considerable boost. As yet, we are unable to set any future event dates and closely monitor the developing pandemic situation in the UK and abroad.

Ramadan 2020As a result of the pandemic, Ramadan became an even more significant period of fundraising and plans were put in place to utilize opportunities brought about because donors were home-bound and online for longer periods each day. The campaign included nationwide TV advertising, digital marketing including social media, newsletters and offline engagement with donors. Imam Ajmal Masroor was commissioned to produce a series of short videos for GLT on subjects relevant in Ramadan and these were shared by Imam Masroor on his social media platforms and across our own platforms. In response to the pandemic, GLT pledged 20% of all Ramadan income would be used to provide food packs for impacted families. We thank LRBT for supporting this initiative and distributing food packs. The Ramadan campaign generated £203,000.

Young Professionals’ DinnerGLT held a small dinner for young working professionals at Cinnamon Kitchen in London. The event, held in December was a great success, as it provided young professionals an opportunity to mingle with like-minded individuals while helping to raise awareness of GLT.

Other EventsThe running costs for the clinic in Havelian continues to be supported by a generous UK donor for which we are most grateful.

ThanksThe Graham Layton Trust is immensely grateful to all donors for their loyalty and generous support during these uncertain times. We look forward in hope to better times ahead.

Fundraising The fundraising total for the year (01 July 2019 – 30 June 2020) was $303,500. This is the highest amount of donations LRBT USA has raised since the organization was formed and we are extremely grateful to our donors for showing us support, even in the unprecedented times of COVID-19. Ramadan was the strongest month for donations which were received via the website online, text to donate and checks by donors across the United States.

The donor database is growing with many donors making large private donations, including a very generous $100,000 donation in the month of Ramadan.

LRBT USA is sincerely thankful to the individuals who are passionate about the organization and supporting the hospitals and clinics across Pakistan.

Lar Multan HospitalThe generous Zakat from our donors was distributed to LRBT’s Lar Multan hospital in Punjab. The Zakat is covering nearly 10,000 major surgeries in 2020 from cataracts to IOL implants – which is a tiny, artificial lens for the eye. It replaces the eye's natural lens that is removed during the cataract surgery.

The biggest concerns with patients in Multan are cataracts and they are sadly affected with; blurry vision, poor night vision, double vision, headaches and frequent change in optical power.

Throughout COVID, Lar Multan has been fully operational, but they are following strict guidelines from the government, providing masks to all patients, maintaining strict social distancing and ensuring the safety of all doctors and paramedics by wearing the required PPE.

Lar Multan hospital has nine doctors and seven nurses, the hospital is built on 12 kanal (7260 sq yards) and is 28 minutes’ drive from Multan city.

On average the patients that are treated at the hospital are both male and female from the age of 40 – 70. They are working class families and 90% of the patients are from remote areas of extreme poverty and cannot afford treatment or surgeries.

“Stop Diabetic Blindness” CampaignWe continue to campaign to tackle the risks of losing eyesight – also known as diabetic retinopathy. The aim is to raise money to launch screenings for diabetic retinopathy across LRBT hospitals in Pakistan.

Championing the campaign, Noaman Sanni, M.D. says, “People living with diabetes are at particular risk of sight loss – but this complication is avoidable. If you have diabetes and are over 12 you should be having your eyes screened for retinopathy by visiting a LRBT hospital in Pakistan. The screening looks for specific changes in the eye that cause blindness, and it is effective at reducing your risk of sight loss.”

RamadanAhead of the holy month, many plans were set in motion from nationwide TV advertisements, digital marketing including social media, newsletters and offline engagement with donors. LRBT USA continues to work with Benevity which helps raise donations from employees at LinkedIn, Apple Inc. and Microsoft.

LRBT USAGOVERNANCE

Social MediaFacebook has grown significantly in followers in the year and Instagram is strong with an increased volume of engagement and followers. Once a follower is gained, each person is directly messaged to ask for continued support by sharing our posts, joining LRBT USA’s newsletter and also donating.

Newsletters To engage existing and potential donors, over 15 newsletters was distributed in the year. There were many responses and online donations post distribution from existing supporters.

Celebrities In Ramadan, the legendary singer, Salman Ahmad showed support by filming a video to share with our supporters. We shared the video on all our social media channels and distributed via our newsletter service.

TrusteesThe team in the US continues to look for trustees and advisors to help support and develop fundraising initiatives across the United States.

Thank youLRBT USA would like to express their gratitude and sincere thanks to all donors for their generosity to help provide free eye care in Pakistan.

A sincere thank you to; Mr. Adeel Shaikh, Mr. Ahmed Syed, Ms. Ambreen Farooq, Mr. Asim Farid, Mir S. Ali, Mr Muhammad A. Hafeez, Mr. Muhammad Sheikha, Ms. Munira Ahmed, Ms. Nighat Ahmed, Mr. Rahim Ali, Ms. Samina Khan, Ms. Sara Iqbal and many more.
