2020 Great First Responder Awards The Great First Responder Awards program was first held in 2007. The program is the only DFW regional awards program for First Responders. In the first 14 years of the program (2007 2020) a total of 902 awards have been presented. Until 2020, the awards were presented during the first day’s lunch at CareFlite’s annual Emergency Care Update. Due to the COVID-19 Pandemic, the ECU was entirely virtual this year. As a result, awards were presented at each department. During the virtual ECU, each winner was recognized for their contributions. In addition, W. Stephen Love, President and CEO of the DFW Hospital Council, spoke on line about the key role that First Responders have in normal times and how much more they are needed during the Pandemic. He is seen left below during his comments about this program. He is seen below left doing so this year. Each year CareFlite’s President & CEO introduces the presentations. He is seen below right. Welcome to the 14th Annual 2020 Great First Responders’ Award ceremony.

CareFlite and the Dallas Fort Worth Hospital Council are honored to jointly sponsor a regional awards program for the front line of our emergency response system. Those honored today are the best of the best. Chosen by their peers, each represents service above self. From large urban departments to small rural, volunteer fire departments, First Responders answer the call day and night from anyone needing help. They face situations that many of us can’t even imagine. And they do it because they want to help others.

The mission of the Dallas-Fort Worth Hospital Council is to continually improve patient safety and quality of healthcare in our region. We do this by creating innovative solutions through collaboration and coordination of efforts, serving as advocates for day-to-day issues that affect hospitals, while balancing the demands of the region with state and national issues, providing the most accurate, timely and comprehensive information to our members and other constituents; and improving the workforce numbers in our region. DFWHC is a non-profit association for hospitals in the Dallas-Fort Worth area, which has been committed to the continuous improvement of healthcare in the metroplex throughout our 40 year history. DFWHC is comprised of hospitals and healthcare organizations throughout the North Texas region spanning over a 100-mile radius from its headquarters in Irving, Texas.

CareFlite, the oldest joint use air medical program in the United States, was founded in 1979. From that vision came a 501(c)3 non-profit air and ground ambulance service that serves the region and its citizens. Today CareFlite is sponsored by the Baylor, Scott & White Health, JPS Health Network, Methodist Health System, Parkland Health and Hospital System and Texas Health Resources. As a program that itself offers 911/EMS services in Balch Springs, SE Dallas County, Kaufman County, Hill County and Johnson County, CareFlite supports and provides education to First Responders across the region. It is a privilege to join the DFWHC in honoring Great First Responders.

Cleburn Fire Department

Battalion and EMS Chief Cory White (L) and Training Chief Lieutenant Paul Rodriquez (R) are members of the Cleburne Fire Department. When a void in local COVID testing was identified in Cleburne and Johnson County, Lt. Paul Rodriquez, along with EMS Chief Cory White, began to work on an immediate solution. They partnered with the City’s Public Health Authority, Dr. Michelle Beeson, to plan, organize and open a drive through testing clinic. The two men worked tirelessly to ensure that proper procedures were in place to offer quick turn around COVID testing results to residents of Johnson County. The testing Center ran a total of 17 weeks and tested over 2,000 people in need. During that time, Lt. Rodriquez continued to perform the job functions required of a Training Officer as well as the work in the testing center.

Previously Awarded - On August 08, 2019 at 16:54 the Colleyville Fire Department was dispatched for a possible heart attack. CFD arrived on scene to find patient lying supine on the floor, pulseless, with agonal respirations. CPR was initiated immediately and the 12-Lead was applied. Patient’s EKG rhythm revealed Ventricular Fibrillation. Patient was then defibrillated at 360 joules. CPR then resumed as paramedics actively assessed for rhythm changes. During this time the airway was secured via tracheal intubation and an IO was started in the left tibia. After approximately one minute of continued CPR the patient’s rhythm converted to sinus tachycardia. Patient was also found to have a strong carotid pulse at this time. CFD then began post resuscitation (return of spontaneous circulation – ROSC) measures and began transport to Baylor Grapevine. CFD continually reassessed while en route to the hospital and continued working through the return of spontaneous circulation (ROSC) protocol. The patient was stable with a strong pulse when CFD transferred patient care to the Baylor Grapevine ER staff. I am happy to report that this patient had a positive outcome and exited the hospital under their own power on August 16, 2019. I believe the commitment, dedication, and training put forth by the members of the Colleyville Fire Department has played a tremendous role in the lives of the citizens they serve. It has not only affected the patients’ lives but also the lives of their families and friends who still have their loved ones to hold on too.

The firefighters that responded to this emergency and that I would like to nominate are as follows: Firefighter/Paramedic Braden Fargo, Firefighter/Paramedic/FTO Robert Creamier, Firefighter/Paramedic Matt Olson, Firefighter/EMT Martin Mendoza, Firefighter/EMT Bryan Wooten, Engineer Dustin Wynn, Captain Jeremy H. Lewis, and Battalion Chief Tim Folden.

Sergeant Wanda West (Back left) is a 28-year veteran of the Dallas Police Department. She is a native of Chicago Illinois but found her way to Dallas. As a police officer and Senior Corporal, she has served in the patrol division, neighborhood policing unit, and as property crimes detective. Shortly after promoting to the rank of sergeant, her passion for community policing was called upon again. Wanda spent a couple years organizing and hosting the Dallas Junior Police Academy and working with the Dallas Police Volunteers. Wanda is currently assigned to the office of Community Affairs. There she supervises a mixture of officers and non-sworn employees who share the same passion as her for community outreach. Her passion and dedication to the residents of Dallas is shown everyday when she comes to work. It is even on her mind after hours when she is calling her employees about new ideas of clarifying upcoming events. She is a role model for fellow officers and represents the best of the Dallas Police Department

Desoto Fire Rescue

On July 9th 2020 at approximately 2000 the listed crew members responded to the 600 block of East Belt Line in DeSoto to an unconscious person on the track.

Upon arrival of the crew they found the patient in cardiac arrest. With no hesitation this crew performed heroic life saving measures on scene and during transport to a local hospital which ultimately lead to the patient being discharged to a rehab facility. Paramedic’s Lewis, Campbell, and Mansfield has a combined two and a half years of experience (Mansfield 1 month) with DeSoto Fire Rescue. Despite the lack of experience with DeSoto Fire this crew is the future of EMS.

This crew deserves the award due to their fast response and knowledge that they had on this day that leads to a human life being saved. The patient saved will get to see their family and loved ones another day.

DHS - TSA - Federal Air Marshal Service

Carina Bertella (pictured right) volunteered to serve on the DHS-Southwest Border in Del Rio, Texas. During the 90-day deployment Carina treated hundreds of people suffering from many medical issues caused by long travel across difficult terrain. Carina showed great initiative by completing literally thousands of medical assessments and identifying/isolating numerous migratrants and highly infectious diseases. Carina also helped develop a more streamline system for completing medical assessment for the hundreds of newly arrived migrants. Carina with EMT certification played an essential role in the success of the border deployment.

Stephen Tryon (pictured left) volunteered to serve on the DHS-Southwest Boarder initiative in Del Rio, Texas. During his 90 days deployment Stephen treated hundreds of people suffering from medical issues related to long and difficult travel across difficult terrain. As our lead EMT, he completed literally thousands of medical assessments and identified and isolated numerous migratrants and highly infectious diseases. He worked tirelessly 12-hour days without complaint and was an invaluable asset to the deployment

70y of Dixie Dunton- witnessed arrest by family at home. Bystander CPR initiated. M2 arrived on-scene and continued with initial rhythm of Asystole. After approximately 15 minutes of CPR, airway and EPI; patient converted to VF and then subsequently defibrillated twice and ROSC achieved. Patient transported to Hamilton Hospital Helipad for rendezvous with CF6 at Hamilton General Hospital. Patient flown to Providence Waco and subsequently received an AICD and discharged without neuro deficits.

Call #1Essentially, patient was struck by an 18whl tractor-trailer on Hwy 281 five miles south of Hico, Texas. M2 was initial ambulance on-scene followed shortly afterwards by M1 and EMS1 (EMS Supervisor). Major injuries . EMS packaged, IV access, RSI & Airway. CF6 responded, hung blood and flew to Cook's. Total flight time 1hr18m (including scene time I believe).

70yom Jeffrey Becker- Patient had a .410 shotgun in the bed of an ATV; loaded and pointed towards the rear of the ATV. Patient threw a bag of trash into the bed of said ATV while standing at the rear. Shotgun discharged striking the patient nearly dead center in the abdomen at point-blank range. CF was requested but unable due to weather. M2 was initial crew on-scene who packaged and gained IV access. M1 and EMS1 (EMS Supervisor) arrived on-scene shortly afterwards. M2 (with split crews) ground transported patient to BSW Hillcrest Waco. Patient received (2) rounds of TXA, O2, Atropine and IV fluids during the 55m transport following "Permissive Hypotension Protocol" (patient bradycardic due to Lisinopril- resolved with Atropine). Patient was triaged in ED and went directly to surgery for a right colon resection and peritoneal lavage. Patient has since been discharged and returned home with no neurological deficits or sepsis.

Call #4January 13, 2020

The morning was a cold and rather dreary day. Jeffrey Becker, a 70 year old man living outside of the city of Hico in Hamilton County, drove his ATV from his home down a long driveway to the mailbox at the road. After retrieving his mail, Mr. Becker went to rearrange some tools and other items in the bed of his ATV. Unfortunately, the .410 shotgun Jeffrey carries in his ATV for snakes and such on his farm accidentally discharged into the belly of Mr. Becker; at point blank range. Jeffrey drove his ATV back to his home where he told his wife he had been shot. His wife called 911.

Hamilton EMS Medic 21 staffed with two Paramedics; Richard Stout & Dustin Johnson responded from Hico with an 18 minute response. Due to the nature of the call; both the EMS Shift Captain Paramedic Dana Allison, and Medic 11 staffed with FP-C Erik Testerman and EMTB Blake Wooten from Hamilton also responded. En route to the call, Careflite was requested. However, due to the deteriorating weather conditions CareFlite was unable to respond.

Once on-scene Medic 21 found the patient sitting on his front porch with a kitchen towel covering his belly. The patient was conscious and responding appropriately; yet pale and quite diaphoretic rating his pain as "8". Upon examining the wound, Medic 21 found not only the 1" wide burned hole through his shirt but the corresponding center abdominal evisceration underneath. The crew was secured to the stretcher and loaded in the MICU as Medic 11 arrived on-scene along with EMS10, the Shift Captain.

Due to the severity of the injury and the patient's condition, the decision to transport by ground to the closest Level II Trauma was quickly agreed upon and the crew of Medic 11 joined Medic 21 and began emergent transport to BSW Hillcrest in Waco; 57 miles away.

Three Paramedics provided emergency trauma care to Jeffrey Becker during transport provided by one EMTB which included:

- bandaging & dressing the abdominal evisceration wound- IV fluid resuscitation & Permissive Hypotension (achieved & then maintained a MAP of 65)- Administration of both bolus & maintenance infusions of TXA- Fentanyl for pain management- Atropine to defeat the bradycardia caused by Beta Blockers the patient normally took for hypertension

Mr. Becker arrived at BSW Hillcrest in 68 minutes alive, conscious, and appropriately asking how fast he was going to surgery. Due to the pre-hospital large bore IV's, medications, and multiple patient reports; Jeffrey rolled right through the ER, got a unit of blood hung in the hallway, and rolled right into the OR Suite.

He survived his wound and has recovered well at home with no reported deficits after spending several weeks at BSW Hillcrest in Waco. His survival may not have been possible without the quick decisions and excellent collaborative care provided by two separate EMS crews who joined together for what is usually a fatal injury.

Susie Campbell is the Director for Irion County EMS. Susie has gone to bat for our EMS service that has done before such as being paid for a stand-by service. She is a great and very fair boss who will always leave her family to cover staff when there is no one else to cover.

I have the pleasure of knowing Captain Davy Sanchez of Mansfield Fire for many years. Several years ago, Davy attended a class that I taught (Karen Yates) on EMS best practices. We discussed items such as early activation of the Cardiac Cath Lab for STEMI and bringing public access AED’S to the hospital when they have been used.

Davy has embraced both concepts. Every time Davy and his crew run a STEMI call I can guarantee that the Cath Lab gets early activation. The early activation leads to a significant decrease in our door to balloon times, therefore saving heart muscle from damage.

Davy displays true leadership and patient advocacy skills every day that he comes to work. I appreciate his dedication and commitment to great cardiac care.

Mansfield Firefighters Association

Rather than nominating an individual I am nominating a group. Last summer, Miriam Villar, ED staff nurse, was diagnosed with breast cancer. Not only did her co-workers spring into action, but so did the Mansfield Firefighters Association (MFFA). This group quickly reached out to Miriam and her co-workers.

The association offered to drive her to treatment appointments, help with jobs around the house, drive her son to sports, and other needed services. In addition, they changed the entire direction of their yearly breast cancer awareness campaign. They designed a special t-shirt that included Miriam’s initials and a logo symbolizing her. They sell these shirts every fall to raise money for breast cancer research. Typically, the proceeds go to the Komen Foundation or other national organization. In 2019 all proceeds went to Miriam. In the fall of 2019 they presented Miriam with a check for almost $7000.00 from the MFFA. This money went a long way to assist with medical bills.

Firefighters are part of the Emergency Department family. In an instance, they showed just how much they are family. We will forever be grateful for this support of Miriam and her family. They are more than deserving of this award.

I called out to Cheryl to switch with me because, as it turns out, Cheryl is fluent in American Sign Language.

Cheryl quickly moved to the patient’s side as Lindsay and I pulled the stretcher close. In no more than a couple of seconds of smiling at our patient and giving an exaggerated warius-like wave, she was instantly bridging the gap for him to those of us that were trying to help. The near instant look of relief and calm seemed to come over him; he began signing back and forth with Cheryl giving us his complaints of pain, dizziness, medical history, and showing his level of awareness. We were able to determine that his pelvis was also hurting in addition to his right upper leg, and was able answer that he was not having difficulty breathing and that he could feel and move all his extremities.

Here’s where the incredible happened… Cheryl while continuing to sign with our patient, continuously reassessing, began providing patient care as well. She spoke to his boss for him to pass along where he was going, helped perform the detailed physical assessment, identified a decrease in lung sounds in his right lung, assisted in applying a tourniquet when the patient appeared to be bleeding into his thigh, and even write down patient information on the patient care work sheet….all this without failing to keep up conversation with our severely hurt patient.

Through her commitment and care, she recognized that he was beginning to reflect repetitive statements, sign of a possible head injury, but she was able to make him laugh during transport!

I want you to know that that is by far not the first time. There have been a least five times now that her ability has been pivotal in-patient care and outcome. It not only improves patient experience and outcome, but it also reflects so highly of the values that MedStar strives to maintain.

Cheryl Alexander EMTOn August 12th our team was dispatched to a Priority 1 motorcycle versus car incident. Upon arriving on scene, we could cee a mangled sport bike and an adult lying on the ground. We could see that his right femur was broken and likely his pelvis, but the strangest detail stood out, he was visibly in pain but silent. As we approached, the Fire Captain stated that the rider was actually deaf, so they were having difficulty communicating with him.I immediately turned back toward the truck where Cheryl, and our trainee Lindsay, were loading the stretcher with equipment to take to the patient.

Brenna is the Emergency Healthcare Administrator at the RAC. As chair of the EMS committee I work with Brenna on just about a daily basis and she has constantly saved my tail. She works with all the EMS agencies in the area and puts in an untold number of hours while (OFF THE CLOCK). She helps with the county assistance funds, and the local projects grants which means more money and items in the local EMS providers pockets. Brenna, goes above and beyond in her daily job. Brenna is deserving of this special recognition for all her hard work and commitment to EMS in North Texas.

Joe SchmiderJoe is the State EMS Director. You might not consider Joe being on the front lines regarding EMS, but I would argue that he is on the front line every single day. Joe has done an outstanding job during his time at the State and with each and every event he has proven himself to be a true leader at the state level. He understands the boots on the ground and what they need, and he is able to navigate the state and state officials and provide them what they need. He truly is a friend to all in EMS. For all that Joe does for the state and especially for the EMS services in the state, I would nominate Mr. Joe Schmider as a Great First Responder. He truly is deserving of this recognition.
