President’s Address KITANO, Seigo, Japan (2014-2020)

It is my great pleasure to publish A-SPDE 8th Bulletin in 2020 as my first presidential term. Thanks to Prof. William Chao’s historical strong leadership as a past president which I have taken over since 2016 officially, A-SPDE has continuously grown to be such a successful society with many national societies in Asian Pacific regions. With the great contributions and commitments from all societies, I am very much proud of our enthusiasms in terms of educations and development of endoscopy. Our main activities, the continuous training courses of endoscopy, “A-PSDE Indochina Regional Training Program” in Vietnam, Myanmar and Thailand, and also, Indonesia and Cambodia Training Program are very successful and contribute a great opportunity for the education to the future endoscopists in those areas. We have been training the young excellent endoscopist led by many famous experts in these fields more than 1,500 trainees. In this training program, A-PSDE cooperate with AMETS (Asian Medical Education and Training Support) in regards to selecting and dispatching the faculties from Japan as the trainers since 2018. We, A-PSDE would like to continuously cooperate with AMETS for the future activities. I am confident that A-PSDE continues to make a progress in the coming future and grow its activities with our great society members and councilors. Hope to see you all in any A-PSDE activities such as a hands-on training course and in the coming APDW2021, KL in August 2021.

Vice-President’s Address

TAJIRI, Hisao, Japan (2014-2020)

Professor Seigo Kitano, President of APSDE, has been showing us a strong leadership not only in organizing the society but also in training the Asian local trainers. As witnessed by many, APSDE has gained a rapid growth and world reputation since its foundation in 1973. At present, we have members from 23 countries (Afghanistan, Australia, China, Hong Kong, India, Indonesia, Japan, Korea, Macau, Malaysia, Myanmar, New Zealand, Pakistan, Philippines, Singapore, Sri Lanka, Taiwan, Thailand, Vietnam, Bangladesh, Cambodia, Mongolia, and Iran). In particular, APSDE hands-on training courses are the major eye-opening events whenever / wherever organized in Asia. As the Chair of the Education Committee of APSDE, I am very honored and pleased to have played an essential role to support the local organizers to host various hands-on courses in selection of venues and instructors among many candidate cities and experts. APSDE has been conducting training programmes for endoscopists in the Asia-Pacific Region since 2004. The programme was then expanded into the Indochina region, and over the past 5 years, we've seen success in Myanmar, Vietnam, Indonesia and Cambodia. We have also developed training plans in Mongolia and Malaysia which will launch its first activity in 2021. As the demand for APSDE training programme increases, we recognize a need to collaborate with endoscopy experts from the regions to develop contents with quality and training protocols. Therefore, we have established a new organization for the training activities in 2020 by Chair: Prof. Hisao Tajiri and Vice-Chair: Prof. Kazunari Murakami. Objectives and goals of APSDE education committee and workgroups are as follows; inviting the faculties for workshops of educational content so that committee members can understand APSDE activities more, reviewing content of APSDE programme to establish good learning goals for training activities, developing educational content database for quality standardization across ASPDE workshops, and working with like-minded experts from across Asia to develop high-quality training protocols. We would like to further improve APSDE activities with your cooperation. The future of APSDE is promising!!

Condolence Message NIWA, Hirohumi, Japan

(Hon. Presient of A-PSDE)

Professor Hirohumi Niwa, Honorary President of The Asia Pacific Society for Digestive

Endoscopy (A-PSDE) and Honorary President of Japan Gastroenterological Endoscopy

Society, passed away at the age of 87 on Saturday, February 25, 2017. I herewith express

my sympathies and condolences.

Professor Niwa was played a role of President of A-PSDE from December 1992 to March

2000 as he was the 6th President of our society since it was established in 1966. He was

also performed Secretary General of A-PSDE from 1983 to 1992 with serving as a councilor

for long years. In addition, he has been in office of Japan Gastroenterological Endoscopy

during the 48 years before he was appointed as Honorary President, he was appointed as

Chairman of the Finance Committee and a member of the Japanese Journal Editing

Committee, and his excellent leadership helped the Society to run smoothly.

Looking at the history of endoscopes, by Professor Niwa, it was emphasized that the

development and development of endoscopes did not suddenly happen one day, but that

they have gradually developed along with the progress of peripheral science and technology

in light of past achievements and failures. He pointed to the endoscopist in the right direction

for the future of the gastrointestinal endoscopy. Also, he played a leading role in the training

course for endoscopy internationally for collaboration with Asian countries which he was

enthusiastic about the educational activities of many young doctors in developing countries.

I was be able to get an involved as an educational committee chairman as A-PSDE from the

beginning of the training course, and now I managed to develop the group as the president

of A-PSDE with its successful training courses in Asia about 16 times per year.

In 2007 he hosted Asia-Pacific Digestive Week (APDW) as a chairman and it was led to a

successful meeting. Furthermore, he had served as the President of the World

Gastroenterological Endoscopy Society for three and a half years from March 2002 to

September 2005, and establish a soil where Japanese gastroenterologists can play an

active role internationally.

Professor Niwa, thank you very much for all your support. We would like to express our

deepest condolences here as well as to our sincere achievements.

Condolence Message William S.C. Chao, Hong Kong

(Hon. Presient of A-PSDE)

Professor William SC Chao, Honorary President of A-PSDE, passed away on 13 August,

2017.

William SC Chao, MD, graduated from the University of CHIBA, Japan and did his residency

training in England and worked as a consultant at the Prince of Wales Hospital and St. Ann’s

General Hospital before returning to Hong Kong in 1979 to start his practice.

Professor Chao has served as President Elect and President of the World Endoscopy

Organization (WEO), the first Chinese to be elected since WEO/OMED established in 1966

and President of the Asian-Pacific Society for Digestive Endoscopy (APSDE). He is also the

founding President of the Hong Kong Society of Digestive Endoscopy (HKSDE).

A veteran gastrointestinal specialist who excels in endoscopy, Professor Chao is noted for

his remarkable effort and achievement in educating future endoscopists and promoting the

use of the technology, particularly in China and other medically less privileged countries in

Asia. For over four decades, he has been traveling extensively for delivering lectures and

organizing courses in different parts of Asia. To maximize results from limited resources, he

established the “hands-on” training model, under which local doctors with suitable

experience are selected to perform operational procedures under the supervision of expert

endoscopists. In China alone, more than 50 hands-on teaching workshops have been

organized in different cities since 1990, with many thousand endoscopists benefited from

such basic orientation courses on diagnostic and/or therapeutic endoscopy.

In 2000-2014, Professor Chao was elected President of the APSDE and became the first

Chinese who held this prominent position up to now. He continued his interests in developing

therapeutic endoscopy in the region and helped established many recognized endoscopy

teaching centers in China, Australia, Thailand and Vietnam. He is also an honorary professor

of over 35 universities in China and Honorary Consultant of the Chinese Society of Digestive

Endoscopy. He is also awarded Medal of Honour (MH) by Hong Kong Government in 2004.

On behalf of A-PSDE, we wish to express our condolences to his family during this sad time.

We will never forget his achievements and his tremendous contributions made to A-PSDE.

With deepest sadness,

Michael Li

Secretary General (2014-2020)

Chinese Society of Digestive Endoscopy Gastrointestinal endoscopy was first introduced from Japan into China around 1970 used Fiberoptic Endoscopy. Subsequently the Japanese doctors went to China to teach Endoscopy in 1975. From 1973 ERCP was introduced to China by Dr. Chen Min-zhang, who was the Minister of Ministry of Public Health of China. Around 1985 they intended to establish the Endoscopy Society which was not allowed during that time. After over 3 years struggle, with the help of Dr. William S.C. Chao to negotiate with the Authority explaining the importance of having an independent Endoscopy Society, then China Society of Digestive Endoscopy (CSDE) was finally established in 1989 in Beijing. Dr. Chao promised to organize a joint International Meeting between Hong Kong Society of Digestive Endoscopy and Chinese Society of Digestive Endoscopy in every 2 years. Since then during the last 20 years, 10 joint International Meetings have been organized. During the establishment, the President of CSDE was Dr. ZHANG Jin-kun, then Dr. YU Zhong-lin and Dr. ZHANG QI-lian, Dr. LI Zhao-shen and the current President is Dr. Zhan Shu-tian. All of them made great efforts to improve the wide application of gastrointestinal endoscopy in China. With the commitment of these Chinese endoscopists, the application of gastrointestinal endoscopy was widely available in China and much experience had been accumulated in the management of various digestive diseases. With help of Dr. William S.C. Chao and Hong Kong Society of Digestive Endoscopy, the 1st Global Chinese Conference on Gastrointestinal Endoscopy 2005 (GCCGE 2005) was held in Shanghai, China from November 10 to 12, 2005. The GCCGE 2005 had gathered the leading experts in the field of gastrointestinal endoscopy such as Drs. William S.C. Chao, Nib Soehendra, Joseph Leung, Sydney C.S. Chung, Lok Tio, Joseph Sung, Yang. K. Chen, Kenneth Chang, and others. Thousands of Chinese endoscopists benefited from this training model and became leaders in their local centers. The 2nd Global Chinese Conference on Gastrointestinal Endoscopy 2009 (GCCGE 2009) was also held in Shanghai with great success. CSDE and other three Chinese societies (Chinese Society of Surgery, Chinese Society of Hepatology, and Chinese Society of Gastroenterology) successfully took the host of the 2013 Asian Pacific Digestive Week and World Congress of Gastroenterology (APDW/WGO) jointly.

15

Hong Kong Society of Digestive Endoscopy

The Hong Kong Society of Digestive Endoscopy (HKSDE) was founded by Dr. William Chao, Frank

Cheng, Joseph Leung and Sydney Chung in 1982. HKSDE aims to facilitate education, training and

development of endoscopy in Hong Kong and Asia Pacific region. Prior to the formation of HKSDE,

Dr. Frank Cheng was already a veteran Councilor of the Asian-Pacific Society for Digestive

Endoscopy (APSDE), representing the first Hong Kong medical professional sitting on the board of

this regional body.

Over the past 3 decades, technological advances and breakthrough innovations have made

landmark achievements in both diagnostic and therapeutic endoscopy. As a leading loco-regional

society, HKSDE is dedicated to providing quality training programmes in basic and advanced

endoscopy targeting at both knowledge and skills transfer. HKSDE organized its first International

Workshop on Therapeutic Endoscopy with live demonstration in December 1985. Being the first of

its kind in the world, the workshop received overwhelming responses. It subsequently became an

annual event since 1987. Each year, this international event attracts more than 800 delegates from

over 20 countries and the 3-day programme features didactic lectures, live demonstrations of more

than 50 cases of various endoscopy techniques as well as the pathology session.

HKSDE has also been serving as an important key to the development of endoscopy in China. Under

Dr. Chao’s leadership, HKSDE joined hands with the Chinese Society of Digestive Endoscopy

(CSDE) and conducted several Chinese Therapeutic Endoscopy workshops in the 1990s. With Hong

Kong’s reunification to China, there has been increasing demand for collaboration in education and

development of endoscopy between Hong Kong and China, especially in ERCP for treatment of

hepatobiliary diseases. One of the important initiatives from HKSDE was the organization of hands-

on endoscopy and ERCP training courses in the mainland. The hands-on training courses had been

successful with more than 450 endoscopists being trained by experts from Hong Kong and China.

In collaboration with the Endoscopy Centre of Prince of Wales Hospital, The Chinese University of

Hong Kong, an advanced fellowship programme was established to facilitate endoscopy training of

endoscopists from mainland. The programme was supported by a Foundation established by Dr.

Chao. Up to date, more than 50 scholarship recipients of the programme are appointed as Professors

by their respective hospitals/ universities, serving as leaders for education and training of endoscopy

in China.

HKSDE will continue to serve its role in advancing digestive endoscopy and upholding the standard

of endoscopy for Hong Kong, China and Asia.

The Korean Society of Gastrointestinal Endoscopy

The Korean Society of Gastrointestinal Endoscopy (KSGE) was founded on August 14, 1976. Over the last 40 years, it has become one of the largest institutes in Korea with over 8,000 members. The Seminar of KSGE has been regularly held biannually since 1989 and over 3,000 members participate in each seminar. The most critical role of the KSGE is to promote the active academic activities of its members. In order to expand its capacity, the first International Digestive Endoscopy Network (IDEN) was held in 2012, since then IDEN has been held every year. Through IDEN, we had a joint symposium with American Society of Gastrointestinal Endoscopy in 2014 and European Society of Gastrointestinal Endoscopy in 2015. These two joint-symposiums has been held in every IDEN. Total of 843 doctors from 32 countries participated in IDEN 2019. IDEN 2020 will take place from September 24 to 26 in Busan with the theme of ‘One Step Ahead!, Towards the Best Insights on Advanced Endoscopy’, IDEN has been held usually in June, however, it is postponed to September due to the concerns of COVID-19 this year. We are expecting more than 1000 participants from over 30 countries. Also, we have been running the program named as ‘Asia Young Endoscopist Awards Program (AYEA)’ since 2014 to promote technical advancement and collaboration among international societies of GI endoscopy. Every year, around 200 young endoscopists have been participated in this program so far. From this year ‘AYEA’ would be changed to ‘International Young Endoscopist Awards(IYEA)’ in order to expand worldwide. In this year, KSGE is organizing the fourth edition of ‘Korea Digestive Week 2020 (KDDW 2020)’ from November 19 to 21 in Seoul with seven member societies, the Korean Society of Gastroenterology, Korean Association for the Study of the Liver, Korean Society of Neurogastroenterology and Motility, Korean Pancreatobiliary Association, Korean College of Helicobacter and Upper Gastrointestinal Research, Korean Association for the Study of Intestinal Diseases, and Korean Society of Gastrointestinal Cancer. In 2012, the first National Endoscopic Quality Improvement Program was performed by KSGE, and 1,016 hospitals with over 100 beds were evaluated their quality of endoscopic ability. Since then, this program has been held every 3 years by KSGE. In 1981, the first issue of the Journal of Korean Society of Gastrointestinal Endoscopy was published. This journal was published in English since September 2011 issue under the official title of Clinical Endoscopy (CE). The journal was linked to PubMed Central (PMC) on June 28, 2012, and Scopus on November 11, 2013. Currently, 8,300 doctors and scientists are regular members of KSGE. Since 1996, KSGE has issued the Subspecialty Board of Gastrointestinal Endoscopy to doctors who have completed one year of fellowship in the division of gastroenterology at 96 training hospitals, which are qualified by KSGE. The number of the accumulated members of Subspecialty Board is 7,673 in 2020.

New Zealand Society of Gastroenterology

In 1966, several years before the widespread introduction of endoscopy, the New Zealand Society of Gastroenterology (NZSG) was formed. Whilst NZSG was originally formed to promote the study and practice of Gastroenterology in New Zealand, over recent years there has been an increasing requirement for the Society, in partnership with the RACP, to be responsible for advanced training in Gastroenterology. The Society, in conjunction with the Royal Australasian College of Physicians and Royal Australasian College of Surgeons is also responsible for the Conjoint Committee for Endoscopy Training. One of the biggest challenges facing NZSG at present is the introduction of a Colorectal Cancer Screening Program. New Zealand has the highest incidence of colorectal cancer in the world. Each year there are around 1200 deaths from colorectal cancer and 2700 new cases diagnosed. International experience would suggest this death rate could be reduced by as much as 15% with the implementation of a screening programme. However, it is a huge task to introduce a screening programme. Colonoscopy volumes alone will probably increase by 50%. For any screening programme to be effective a very high standard has to be maintained, particularly with respect to diagnostic procedures. The NZSG is aware of this and in conjunction with the Ministry of Health, New Zealand is running “Train the Trainer” workshops to ensure a high standard of training is achieved. From its inception, the NZSG has fostered research and educational and travel grants as well as research support are offered, usually in conjunction with Industry sponsorship.

Further information on the New Zealand Society of Gastroenterology can be found at www.nzsg.org.nz.

The Sri Lanka Society of Gastroenterology History Gastroenterology is a specialty that is widely acclaimed all over the world with large societies been established in all the developed countries, (USA, UK, Germany, Japan to mention a few) and grouped together into regional bodies like Asia Pacific, European, African under one big organization – the world organization of Gastroenterology, Digestive Endoscopy and Colo Proctology – holding large congresses on a periodic basis, attended by thousands of participants. It was about this time in 1965 that Prof. Kuwai Nakayama visited Sri Lanka from Japan and with that visit the sun began to rise and give light to Gastroenterology in this country. Prof. Nakayama was a giant in G I surgery – especially in Oesophageal surgery. He was a great craftsman and, speed was his forte and he could perform three Oesophagectomies during a morning session. At that time there were no Gastroenterologists or Gastroenterology in this country, but the late Dr D F de S Goonewardene was known as the surgeon for the stomach. Prof. Nakayama valued his visit so much, that he decided to donate a Gastroscope to Sri Lanka, but he could not give it to a single individual. So, the first Gastroenterology society was hurriedly formed to receive this donation, and Dr D F de S Goonewardene was elected the first President. The society comprised a cross section of Colombo specialists, which included even a chest physician. Prof. Nakayama donated the first flexible Gastroscope to this country, and it was used exclusively by Dr D F de S Goonewardene on selected patients. The G E Society had very few activities like an occasional lecture by a visiting examiner for the Primary FRCS exam. It was in 1967 that Dr. Nihal Markus proceeded to England for postgraduate studies as a Commonwealth Scholar. In UK at the Royal Infirmary, Edinburgh under Prof. Strong he had a special training in Gastroenterology under the famous Professor Circu. He returned to his motherland, but was not fortunate to have the specialty recognized. Nevertheless he practiced and taught Gastroenterology wherever he worked – Trincomalee, Ratnapura, Negombo & Galle. In 1973, Prof Dayasiri Fernando having trained in Gastrointestinal Surgery, under the guidance of giants in the field like Prof. John Goligher at University department of Surgery (Leeds) returned to Sri Lanka & started practicing GI Surgery. He left for U.K. again in 1976, after being awarded the Smith and Nephew Fellowship for training in Gastrointestinal surgery. He returned to Sri Lanka in 1977 full of enthusiasm with a vision to establish Gastroenterology as a specialty in this country. But he was confronted with a very firm „No‟ from a very hostile bureaucracy – the same barrier Dr. Nihal Markus faced. To them it was – “ what is gastroenterology? No it‟s not important.” Those who had vision like Dr Malinga Fernando and Dr Joe Fernando, two top level administrators at the time, together with Dr Upali Amarasuriya, the then M.S. of Kalubowila Hospital helped them enormously, when Gastroenterology was struggling to be born in this country. At that time the facilities for diagnosis and treatment were minimal. G.I. imaging was very limited. Barium meal and screening, Barium enema, Cholecystogram, I.V. Cholanigiogram (with all its complications) and per op. cholangiograms were the order of the day. There was no U.S. scanning In 1981, again after much persuasion the first Gastroduodenoscope (end viewing) was brought to this country – a big step forward. We could now view with accuracy the lumen of the upper G.I tract, take biopsies and take photographs using the endoscope. It was only a fibro optic endoscope and did not have a separate endoscopy room to house it. So, the endoscopies were done in the corridor of the operating theatre which was a make shift arrangement.

33

The Sri Lanka Society of Gastroenterology

In 1983 the Gastroenterology unit was shifted to the Colombo General Hospital. At Prof Dayasiri

Fernando‟s request the PGIM created an Endoscopy unit in the University Department of Surgery,

where he was appointed to be in charge. The Chairman of the Nawaloka Group of Hospitals agreed to

build a small unit attached to ward 18. The Minister of Health declared this open on the 10th of May

1984. This is the first separate Endocscopy unit of the General Hospital Colombo. Another, slim

endoscope was donated by well wishers from Japan and it was used on children, gingerly moving onto

sclerotherapy of Oesophageal varices of both adults and children. At about the same time, in 1985, Sri

Jayewardenepura hospital started Endoscopy and Dr. N S Jayasinghe and Dr Yoheswaram were sent

on a Japanese government scholarship for training in ERCP and colonoscopy. They returned and Dr.

Jayasinghe started ERCP for the first time in Sri Lanka.

In December 1986, Dr Gamini Buthpitiya was the first to do a colonoscopy in the country at Peradeniya, using his personal colonscope. Teaching of Gastroenterology began at this stage with the PGIM, SLMA and the College of Physicians arranging special lectures. In 1986 Dr. Nihal Markus was called back to his creator and eternal rest. Prof Dayasiri Fernando with the blessings of his teachers and senior colleagues, and with the help of a group of keen and enthusiastic young doctors saw the birth of the new Gastroenterology and Digestive Endoscopy Society of Sri Lanka. He was elected the President and Prof. Janaka de Silva, held the post of the secretary. Dr. P.T. de Silva, Vice President of the Society, delivered the inaugural Nihal Markus oration. Within a very short period of time the Society was accepted and elected to the governing councils of both Asia Pacific Society of Gastroenterology and Asia Pacific society of digestive Endoscopy. We were also elected to the ethics committee and the minimally invasive surgery committee of the world organization of gastroenterology. Money was required to maintain our membership and as our finances were limited, Japanese Society of Gastroenterology paid our subscriptions for many years, as a friendly gesture, to a developing sister society. A few years later, the society was rewarded with the decision of the governing councils to hold a mini congress of Asia Pacific in Sri Lanka. This was done amidst turbulence in the country and the bombing of the Kolonnawa oil tanks. The Chief Guest on this occasion was the Prime Minister, late Sirimavo Bandaranaike and the guest of honor was Minister of Justice Prof G L Peiris. Another important activity that was organized was the Joint Academic Sessions with the College of Surgeons of Sri Lanka. The specialty of Gastroenterology (Surgical) was approved for training by the PGIM in 1990, thanks to the untiring efforts of Prof. Dayasiri Fernando, The year 1990 also witnessed significant advance in the field of GI surgery. With a personal gift of a stapling device, from the American Embassy the anastomosis of bowel in low anterior resection began, using this instrument. The other significant advance was when Dr K L Fernando did the first laparoscopic cholecystectomy in 1992, using his own instruments, at Ragama. This was a giant step forward for G.I. surgery. The Sri Jayawardenapura Hospital purchased a laparoscope and laparoscopic instruments and Dr Yoheswaran started performing laparoscopic cholecystectomy in 1993. The Dept. of surgery, Colombo soon followed at the GH Colombo purchasing a Laproscope with donations from friends and well-wishers. This was then followed by Laparoscopic cholecystectomy in Teaching Hospital Kandy in 1998. At present there are gastroenterology and GI surgery units in all teaching and provincial hospitals with specialists in gastroenterology and GI surgery. All advanced procedures in therapeutic endoscopy and advanced laparoscopic GI surgery is performed regularly in these training centres. In future the Medical & Surgical divisions of the Gastroenterology in this country will advanced further in to sub specialties like Hepatology, colorectal surgery, hepatobiliary Surgery & Upper GI Surgery.

Thai Association for Gastrointestinal Endoscopy (TAGE) TAGE has been continuously growing, while the members including surgeons and physicians from all over the country. The association annually holds 3 academic events in Thailand, namely Live Demonstration, Endoscopy Challenge, and Therapeutic Days. Also, TAGE cooperates with international organization in organizing various events and activities every month. Furthermore, the Executive Board has always been delivering lectures, spreading knowledge, and attending many academic activities. There have been major changes this year, namely the outbreak of COVID-19; TAGE fortunately held an academic seminar in February just before the pandemic and cooperated with China in giving lectures on the disease. Later on, we provided national advice about COVID-19 and also had articles about the virus published in international journals. In Thailand, doctors in each provincial hospital can presently provide international standard endoscopy services; and quite a large number of hospitals competent to perform advanced endoscopic procedures including Endoscopic Retrograde Cholangio Pancreatography (ERCP) and Endoscopic Ultrasound (EUS) are located in each region of the country to provide patients with medical services, knowledge, and trainings constantly. During the outbreak, knowledge and information on endoscopy are shared and exchanged through media and social media. Next year, TAGE is going to intentionally pursue the same policies; yet our methods to spread knowledge are mainly dependent upon the COVID-19 situation. Nonetheless, TAGE determines to achieve our ambition to spread and broaden our knowledge of gastrointestinal endoscopy, as well as to be a national and regional medical hub abidingly.
