1] 24 April 19 .. Topic – Ultrasonography for anaesthesiologists

Speaker –1 ) Dr. Mahesh shettanawar spoke on basics of USG from

Anaesthetist point of view.

2) Dr. Aditi Sadalge - USG guided blocks & her

experience at Ganga hospital during her

fellowship.

Attendance was 38.

Take home message .. USG guided blocks gives good success rate with low dose requirement

and reduces complications .

2] 29 May 19 .. Topic - Newer antiplatelets and anticoagulants and its

Anaesthetic implications .

Speaker – Dr. Vinayak Mali.

This was followed by discussion about PPF guidelines of ISA done by D. Sandeep Kadam .

Attendance was 42.

Take home message – if patient is on antiplatelets or anticoagulants we must follow the

guidelines to avoid medicolegal issues and in emergency situation can use the bridging

therapy and undergo surgery by keeping blood products ready.

3] 26 Jun 19 ..Topic- Balanced crystalloid –Need of hour.

Speaker – Dr. Abhijit Gawali.

Attendance was 40.

Take home message—by using balanced crystalloid in critically ill patients we can reduce the

complication rate including acute kidney injury.

4] 31 Jul 19 .. Topic – 2 topics for discussion

1) Anaesthesia for awake craniotomy

2) Highlights of Adenosine induced transient asystole for

Intracranial aneurysm clipping.

Speaker – Dr .Prakash Bharamgaudar .

Attedence – 45

Take home message – for awake craniotomy pt . selection as well as proper councelling of

patient is important .

And for aneurysm clipping by using adenosine induced arrest we can reduce morbidity and

mortality but at the same time there has to be proper communication and prompt reactions

between surgeon and anaesthesiologist.

Thane. He has performed many difficult intubations by using these .

CICV was highlighted by Dr Bapat, senior anaesthesiologist fro Mumbai.He said in today’s world of

litigation one should not face such a situation. However preparation for needle cricithyrotomy, treal jet

ventilation, surgical cricothyrotomy should be there.

In awake intubation Dr Prasanna Mahajan presented acase where adult male patient of recurrent Ca

Alveolus with hemi mandibulectomy done was intubated over a FOB. Airway problem, topical

anaesthesia and clinical difficulties were discussed.

In a different approach this CME was arranged Dr Vikas Nayar and Dr Manju Sinha were expert . and 6

cases were presented by our own members

1. Laprascopy in obstetric patient

A case scenario of non obstetric surgy like appendisectomy in pregnant paient was discussed. Taking care

of pregnancy was stressed. This topic was presented b y Dr Krishna, Dr Kanchan and Dr Mayuresh.

2. Failed Spinal in emergency LSCS.

This case scenario was discussed by DR Prakash and Dr Madhuri. Favouring to give GA over repeat Spinal

was also discussed, however currently preference is to repeat spinal

3. Obstetric complications and Anaesthesia care

Dr Pallavi and Dr Shruti discussed problems like PPH, Placenta Increta etc where anesthetist has to work

hard to save life of patient .after 3 units of RL or crystalloids giving colloid till blood is available, 3 units of

blood with 3 units of FFP and platelets and so on was emphasized.

4. Cardiac case and Pregnant patient was discussed by Dr Shruti and Pooja . Proper cardiac evaluation

and its need was stressed%.

Problems related to hyperstimulation syndrome was discussed. This was presented by Dr Prasanna

The first academic activity of the tenure 2019-20, the 'Mrs. Vaishali Malegaonkar Memorial Interesting

Case Presentation Competition' was held at MT Paithankar Hall, IMA House on the 19th

April 2019,

Friday. A total of 8 clinical cases were presented. Interesting cases presentation was followed by healthy

discussion and interactive session between the contestants and the judges, along with the audience

which was amazing.

Jitesh Chauhan, Consultant Anaesthesiologist, Dr. Kiran Vyawhare, Consultant Anaesthesiologist & Pain

Physician, Wochardt Hospital, Dr. S K Deshpande, Consultant Cardiac Anaesthesiologist, Dr. Kamlakar

Pawar, Consultant Anaesthesiologist, Dr. Yogesh Zawar, Assistant Professor, Super speciality Hospital &

GMC,Nagpur, Dr. Ravi Mundada, Consultant Anaesthesiologist, Suretech Hospital and Dr. Aditi Bhatnagar,

Resident, Anaesthesiology, NKPSIMS presented the cases.

The Event was judged by Senior Experienced Anaesthesiologists from the city, Dr. Bhau Rajurkar & Dr.

Pratibha Deshmukh. The first prize was won by Dr. Rashmi Shingade for her presentation "Haemoptysis

after failed subclavian vein cannulation" and the second prize went to Dr. S K Deshpande for his case

"Successful management of Flash Re-expansion Pulmonary Edema (RePE) - "Place matters". The MOC

for the day was Dr. Sarita Joglekar. The event concluded with the vote of thanks by Hon. Secretary Dr.

Soma Cham followed by dinner.

Take home message: The competition brought to the fore, cases which are rare and challenging for the

anesthesiologists and the opportunity to showcase, discuss and learn more by sharing.

SUBJECT : DEBATES FOR ANAESTHESIOLOGISTS

The Indian Society of Anaesthesiologists, Nagpur City Branch held a non-academic debate for the

members of the Society, on the 18th

of May, 2019 as ‘Debates for Anaesthesiologists’. Three topics

encompassing the non-clinical aspect of the Anaesthesiologists were fiercely debated upon. Freelance

and Corporate Anaesthesiologists working at prestigious Hospitals and Medical Institutes from Nagpur,

put forth their points of opinion in an intense and intellectually high fought debate.

The debate started with Dr. Umesh Ramtani, Assistant Professor at IGGMC, Nagpur, who spoke in favour

of the motion ‘Given a choice, I would like to be reborn as Anaesthesiologist!’ & Dr. Gunjan Badwaik,

Consultant Anaesthesiologist spoke against the motion. The second topic of debate was ‘Working as a

Corporate Anaesthesiologist is more gratifying than a freelance anaesthesiologist’. Dr. Ravindra

Mundada spoke for the motion and Dr. Sachin Sadavarte spoke against the motion. Dr. Saurabh Barde

spoke for and Dr. Vikram Alsi spoke against the motion on the third topic of debate ‘Conferences are a

waste of time and money’.

The debate was judged by two distinguished senior Anaesthesiologists of the city, Dr. Vivek Bhargava and

Dr. Narendra Tamhane. The President along with the judges congratulated and awarded the winner of

the competition Dr. Gunjan Badwaik and the runner up, Dr. Vikram Alsi.

The master of ceremony was Dr. Swapna Bhure and ended with the vote of thanks delivered by Hon.

Secretary Dr. Soma Cham.

Take home message: Debates help in putting forth opinions democratically, knowing, learning and

comprehending adverse views, so as to expand ones’ horizon of acceptability & thus holds true for the

anaesthesiologists too.

Prakash Khetan, Consultant Nephrologist, Shravan Hospital, Nagpur, Dr. Rahul Saxena, Liver Transplant

and hepato-biliary surgeon, Nagpur, Dr. Sameer Jahagirdar, Plastic Surgeon, OCHRI, Nagpur, Dr. Rajurkar,

Senior Consultant Anaesthesiologist, Nagpur & Jaya Jairam, Co-ordinator, MOHAN foundation, Mumbai

and delivered highly informative lectures both academically and regarding legal aspects of organ

retrieval.

The chairpersons for the various sessions were Dr. Manjrekar, Dr. Anjali Savargaonkar, Dr. Lawange, Dr.

Sheetal Dalal, Dr. Koichade & Dr. Bhople. Dr. Sarita Joglekar & Dr. Kiran Vyawhare played moderators for

the panel discussion.

Dr. Ajay Keoliya accepted to be the Chief guest of the CME & Dr. Manjrekar, Prof & Superintendent was

the Patron of the CME. The CME started with samai prajwalan. Dr. Manisha Shembekar; President, ISA-

NCB welcomed august the gathering, stressing on the importance of organ retrieval being the need of

hour, as there is a huge gap between demand and supply. Dr. VC Shelgaonkar, conceptualized the idea

behind the CME, highlighted on the role of an Anaesthesiologist in the process of deceased Organ

retrieval. Dr. Keoliya congratulated ISA NCB & Department of Anaesthesiology, IGGMC, Nagpur for the

CME on a novel subject and appreciated the efforts taken for the it.

The main attraction was the eye-opening lecture given by Dr. Vibhavari Dani, ex-Dean, GMC & IGGMC,

Nagpur and serving as President, ZTCC (Zonal Transplant Centre Co-ordinator) on the enormous

combined efforts taken by the ZTCC coordinators to just make one organ retrieval and transplantation

successful.

The masters of ceremony were Dr. Amrusha Raipure and Dr. Vikas Choudhari. Dr. Soma Cham, Hon.

Secretary, ISA-NCB proposed Vote of Thanks.

Take home message: Deceased organ donation is around only 0.34 per million which is very low

compared to the rate in the other developed countries. The act has gained momentum and the role of

an Anaesthesiologist in the process of transplantation is unequivocal, but through the CME, the role of

Anaesthesiologist in the process of maintenance of a brain stem dead / deceased patient and during

the retrieval of organs is also of vital importance & has been emphasized on. Moreover, donations of

kidney, liver, heart, lungs, eyes are known & myths and misconceptions involving bone, skin retrieval

were cleared.

Indian society of Anaesthesiologists, Nagpur City branch held a CME on “Common Encounters in

Obstetric Anaesthesia” on 21st

July, 2019 at Hotel Centre Point, Ramdaspeth, Nagpur. Every

anaesthesiologist has to deal with challenging cases related to pregnant patients and at odd times

throughout the day. Dr. Manisha Shembekar, President, in her welcome address stressed the need to

update the knowledge about the recent advances in Obstetric Anaesthesia so as to better the outcome of

an obstetric intervention and having trained anaesthesiologists in the peripheral and rural areas would

drastically reduce maternal mortality and morbidity.

Dr. Ankur Jain, neurologist delivered a talk on postpartum headache and the need to investigate the

patient thoroughly to ascertain the exact cause.

Dr. Chaitanya Shembekar, Obstetrician & Gynaecologist talked about the obstetric concerns in a patient

with anti-coagulants while Dr. Saurabh Barde, Anaesthesiologist, highlighted the anaesthetic

management of such patients.

Dr. Nitin Deshpande, cardiologist, spoke on peripartum cardiomyopathy which strikes as a bolt from the

blue and can be life-threatening & the timely need of diagnosis & intervention.

Dr. Sunanda Gupta, senior Anaesthesiologist from Udaipur, delivered a talk on management of post-

partum haemorrhage, which is leading cause of maternal mortality. She also gave tips as how to

efficiently manage morbidly obese pregnant patients, which has now emerged as a new challenge for the

anaesthesiologist nowadays.

Dr. Anita Shetty, Professor of Anaesthesiology at KEM Hospital, Mumbai updated the gathering on the

current concepts in the management of Pregnancy induced hypertension.

Dr. Kausalaya Chakravarthy from Hyderabad, delivered a talk on how to optimally use uterotonics

without the fear of over infusing during complicated Caesarian sections. She also talked and emphasized

on the necessary aspects that has to be taken care of during the administration of General Anaesthesia to

the parturient, when needed.

Dr. Charuta Gadkari spoke on the efficacy and safety of supraglottic airway in the parturient. Dr.

Geetendra Sharma, obstetrician and well known medicolegal expert from Ahmedabad spoke on how to

tackle the ethical dilemmas faced in the process of managing a pregnant patient, while Dr. Ashish Ganjre,

Anaesthesiologist & Intensivist, stressed on the importance of an Obstetric ICU in the face of changing

scenario in the field of Obstetrics.

The lecture series was followed by Panel Discussion on ‘Our problems, your solutions’. The Moderators

were Dr. Manisha Shembekar & Dr. Pratibha Deshmukh. Dr. Soma Cham, Hon. Secretary proposed the

vote of thanks.

A novel initiative was undertaken, wherein, the same set of questions with multiple choice answers were

asked before and after the lecture. The answers were given through electronic voting pads by the

audience. The effort was an instant hit amongst the audience as it garnered maximum involvement.

The CME highlighted the current trends that have evolved in the management of pregnant patients in an

effort to bring down maternal mortality rate to 100 from 167 by 2020.

On the 18th

of August, 2019, Sunday, ISA NCB organized the annual ‘Dr Malti Phadke Intercollegiate

Anaesthesia Post-graduate Quiz Competition’ at IMA Auditorium, IMA Annexe, Nagpur. Team of three

Anaesthesia Post-graduate students from various Medical Colleges in Vidarbha, GMC, Nagpur, IGGMC,

Nagpur, NKPSIMS, Nagpur, JNMC, Sawangi (Meghe), MGIMS, Wardha and PDDMC, Amravati participated

in the Quiz Competition. The quiz started with the welcome address by President Dr. Manisha

Shembekar. Addressing the gathering, she said that Quizzes are all about knowledge, nerves and

spontaneity & it takes a lot to participate in such activities. She expressed gratitude to Dr. Malti Phadke,

retd. Prof & Head, Dept. of Anaesthesiology, GMC, Nagpur, for instituting the award for the winning team

& runner up team in her name for this interesting and informative annual event and thanked all the

Heads of the Anaesthesiology Department of the various Medical Colleges for encouraging their students

and sending their team for participation.

The Quiz Masters were Dr. Ketki Marodkar & Dr. Dipak Ruparel. The organizing team of the Quiz

Competition included Dr. Ravi Mundada as Scorer, Dr. Ritesh Borkar as Buzzer Referee & Dr. Swapna

Bhure as Timer. Dr. Chandrashekharan Cham, ex-President ISA, NCB worked as the co-ordinator of the

Quiz Competition. The judges for the competition were Dr. Shubhada Deshmukh, Prof. Dept. of

Anaesthesiology, NKPSIMS, Nagpur & Dr. Pratibha Deshmukh, Senior Consultant Anaesthesiologist.

The Quiz was interesting, enjoyable and intellectual with well planned rounds. Questions for the

audience were equally exciting and audience participation was enthusiastic too. The winning team was

the Team from GMC, Nagpur comprising of Dr. Pompy Devraj, Dr. Anuradha Kulkarni & Dr. Bhagyashree

Sarda whereas the runners-up was the team from IGGMC, Nagpur comprising of Dr. Adithya S

Chiranjeevi, Dr. Ashwin R Krishnan & Dr. Mahesh Kaware. Certificate of participation was given to all

participants & trophies handed over to the winning and runner-up teams. Dr. Soma Cham proposed the

Vote of Thanks.

Take Home message: Quizzes are helpful in teaching & learning both the participants and the

audience through an interesting and enjoyable manner with a display of nerves and patience

by the participants. Putting together a quiz is a very strenuous, mind boggling and innovative

work done by the quiz masters.

2019 SAP conducted 3 monthlyacademic meetings and 1 annual conference during this period

April – “Masterclass On Kyphoscoliosis And Anaesthesia”

Dr. Balavenkat from Coimbatore graced the monthly meet organised at Ruby Hall Clinic auditorium on 4th

April The session was designed towards PG students .

Case presentation by DNB resident with Dr. Balavenkat as moderator followed by extensive discussion on

anaesthesia for kyphoscoliosis followed.

The session was rounded up by panel discussion on recent controversies in anaesthesia.

The meeting was attended by 75 delegates

Pune Regional Anaesthesia Course (PRAC) 2018 was conducted in association with Society of

Anesthesiologists Pune

USG Workshop conducted on 5th April at Sancheti Hospital & Cadeveric workshop at Bharati

Vidyapeeth

Conference was conducted on 6th and 7th of April at Hyatt Hotel, Pune

The annual conference with renowned international faculties encompassing recent advances as well as

reviewing the basics carters to novices as well as leaders in regional anaesthesia.

It was an Interactive session of Panel discussions with two anaesthetists and two obstreticians on either

side.

Surgical aspects of importance and anaesthesia aspects were discussed on following topics, Endoscopic

surgeries, medicolegal aspects, Obstretic haemorrhage, Cardiopulmonary disorders followed by an open

forum to discuss other areas of importance.

A total of 70 delgates were in attendance.

JULY - CME on Obesity And Anaesthesia – CME on Obesity and anaesthesia was conducted at Inamdar

Hospital.

The CME was distributed over all areas in obesity starting from Pathophysiology and pre-anaesthesia

evaluation to specific difficult sreas like Airway and obesity and Regional anaesthesia which is a challenge

in the obese. 70 delegates were present.

Surgeons perspective in what are the types of surgeries, what is metabolic syndrome and critical steps

during surgery were also discussed.

Dr. Sandeep Patil from inamdar hospital organised the CME with a host of other members from Pune. Dr.

Neeraj rayate and Dr. Jayshree Todkar were the surgeons who graced the occasiion.

The 4th

annual conference of the Society Of Anaesthesiologists, Pune was organised on 17th

and 18th

August 2019 at The JW Marriott Hotel And Convention Centre on Senapati Bapat Road, Pune with a One

day AIRWAY WORKSHOP was conducted on previous Sunday 11th

August at Deenanath Mangeshkar

hospital.

The theme for this year was “Controversies To Consensus” encompassing all specialities encountered.

Renowned faculties from all over India were invited giving the audience an enriching yet exhaustive

academic experience.

The conference also included a session on Medicolegal aspects with Dr. Adv. Milind Salunkhe giving

valuable insights into various aspects of anaesthesia practice. His lecture followed by Dr. Pratibha Kane

summed up the legal aspects of concents and special case scenarios.

The Dr. A M Deshpande oration was delivered by Professor Dr. Mrs. Jayshree Sood with the topic

“Balancing The IV Fluids”. She was felicitated after the oration.

The inaugural function followed was attended by senior dignatories, past and present ISA presidents and

secretaries from Pune and PCMC with Dr. Mrs. Jayshree Sood invited as the Chief Guest.

This was followed by the Key Note Address delivered by Surg. Comm. Manish Honwad from Port Blair

with the lecture “Critical Care Air Transport: Challenges for the anaesthesiologists in remote locations”

and was well appreciated by all.

ISA Aurangabad conducted five academic activities from April 2019 to

August 2019. 1) Installation of new committee (27 April 2019)

• Dr. Milind Pol – President ISA, Maharashtra state chapter was chief guest for the installation

programme. Dr. Balaji Asegaonkar, secretary & Dr. Sangeeta Deshpande, Treasurer MSCISA also

attended the activity

• Dr. Surekha Shinde, Professor GMC Aurangabad delivered a lecture on Recent use of anticoagulant

medication and its significance in anaesthesia practice

• Thirty members attended the programme.

• Thirty five members attended the activity. Discussions on these topics were beneficial to all.

3) CME for dentists (30th

June 2019)

• IDA in collaboration with ISA, Aurangabad conducted CME on Medical emergencies in dental

practice

• Topics were Medical emergencies in dental clinic, Management of anaphylaxis and L.A. Toxicity,

Preparation to tackle medical emergencies in dental practice, BLS with hands on training and Panel

Discussion on different case scenarios.
