Last year in this column I reviewed the growth of the AAGL’s relationship with our Chinese colleagues.1 I am pleased to report new areas of cooperation are develop-ing. Professor Jinghe Lang, the President of the Chinese

Obstetrical and Gynecological Association (COGA is the counter part of ACOG), visited us at the recent Vancouver meeting. Our past president C.Y. Liu, (Chair of the Chinese Liaison Committee) has reported that Professor Lang will add COGA’s name along with the AAGL to the meetings we recognize in China. There are currently 4 meetings scheduled for 2015, listed below, with a possible 5th in Shanghai.

• April 24-26, 2015 in Chongqing Chair: Professor Zhiqing Liang

• June 12-14, 2015 in Hangzhou Co-chairs: Professor Jun Li (Women’s Hospital) and Professor Songying Zhang (Sir Run Run Shaw Hospital)

• Aug. 22-24, 2015 in Xian Chair: Professor Xiang Xue

• Dec. 11-13, 2015 in Beijing Chair: Professor Zhenyu Zhang

Professor Lang will also work with Dr. Liu to try to make the AAGL’s SurgeryU platform avail-

able to our Chinese members. Currently this is not allowed by the Chinese government. Their consent would allow AAGL members the oppor-tunity to view very skilled Chinese surgeons in live case demonstrations.

Language is, of course, an issue for many Chinese surgeons. With the help of our publisher Elsevier and support from Olympus, we have translated and made available selected articles from JMIG in hard copy and on our website (www.jmig.org). The JMIG App is currently available to them in English and to date, over 200 Chinese gynecologists have downloaded it. Looking ahead, Elsevier will begin promoting AAGL information on their Elsevier China web-site. We are also working on translating basic information on our website for launch in 2015.

Areas of future cooperation are unlimited. China has more than 200,000 Obstetricians and Gynecologists. Our relationship with China and the huge numbers of patients they care for will potentially allow collaborative studies to be done to better understand complex surgical issues and to determine best practices.

The AAGL and the Israeli Society of Gynecologic Endoscopy (ISGE IL) are proud to invite you to participate in the 11th AAGL International Congress on Minimally Invasive Gynecological Surgery to be held in Jerusalem, Israel, June 16-19, 2015.

Under the slogan, “Join us at the Holy LandSCOPE, where ancient history and innovative start-ups intertwine,” the organizing committee is planning a scientific program that will ensure the participation of the most relevant speakers

from all over the world. Participants will be able to learn about the latest local developments in the field of minimally invasive gynecology, as well asinternational developments in research, practice, treatments and medical devices.

The congress will offer gynecologists, residents, students, OR nurses andmedical companies many opportunities for discussion and debate on new medical and scientific information, treatments and innovations in the field of gynecologic surgery. The program will incorporate live surgery, during which attendees will have an online opportunity to address and question the surgeons. The scientific program will include the following topics: Endometriosis, Pelvic Floor Defects, Gynecologic Cancer, Hysterectomy, Ovarian Pathology, Pre- and Post-Operative Care, Robotic Surgery, Education and Simulation, Imaging, Energy, Complications, Hysteroscopy and Uterine Pathology. Particular attention will be focused on future developments andinnovations in endoscopy. Specific programs are planned for young endoscopists, residents and OR nurses.

Inspirational vistas…invigorating cold, crisp air…these are some of my fondest memories of the 43rd AAGL Global Congress in Vancouver, British Columbia. I can’t believe how quickly that

week came and went. This year’s meeting marked a first for the AAGL by moving the global congress to an international venue. Our Canadian hosts were absolutely won-derful and welcomed us, along with the rest of the world, with arms wide open. Records were broken with 61 countries represented and 1,827 attendees registered for the meet-ing, approximately a quarter of whom were at this year’s event for the first time!

Our theme of setting new standards in MIGS through knowledge and innovation rang loudly throughout the week. Controversial topics such as tissue extraction were tackled

head on during our opening ceremonies and new standing-room-only events such as the Stainless Steel Surgeon made their successful debut. Professor Mats Brännström, our Jordan M. Phillips Keynote Speaker, further inspired all of us with his pioneering work in the area

of uterine transplantation.An exciting change to this year’s meeting was

the introduction of 50% new faculty who not only complemented our established leading global clinician-educators, but also instilled fresh programming and perspectives that fueled much of the cutting edge curriculum reflected in the 25 post-graduate courses, 6 surgical tuto-rials, and 5 panel discussions. Especially excit-ing for me was the ability to integrate cadaveric teaching models throughout the week.

We received a record-breaking number of meeting evaluations this year (929), with 93% indicating they were “very satisfied” with the faculty. In a separate open-ended question, almost half of respondents used the following positive adjectives to describe

the content: “fresh,” “practical,” “relevant,” “innovative,” “stimulating,” and “diverse.” We were pleased to find that there was almost unanimous agreement (99.8%) with the state-ment that we “integrated the latest advances in clinical practice, scientific research and technical innovation.” In further testament to the value and relevance of the scientific content, 49% stated that they learned new surgical techniques that they will be able to apply to practice with a greater level of confidence. When specifically asked which area they lacked an understanding of before the Congress, 33% stated tissue extraction and morcellation. Consequently, after the congress, 88% of those same respondents felt confident that they could apply the methods learned to their practice.

Amidst the updated curriculum, the back-bone of the scientific program was, as always, the multitude of oral presentations, surgical videos and virtual posters presented at this year’s meeting, which drew a record-breaking 804 abstracts submitted for consideration. In between the various sessions, our beautiful exhibit hall showcased some of the latest

developments in minimally invasive surgical technology.

A special thank you goes out to all of the faculty, abstract reviewers, my scientific program committee, the incredible AAGL office staff and our phenomenal industry partners, whose steadfast support makes events such as the 43rd AAGL Global Congress possible. 2014 is a year to celebrate for the AAGL. We have met controversy with con-viction and dedication, established collaborative initiatives with associated organizations, while pro-viding excellent educa-

tion and opportunities for training. As the membership grew, our financial standing strengthened, which led to the assessment of our internal structure to ensure continued growth of the organization.

Collaboration – Advancing women’s health, in particular minimally invasive gynecology, requires a global approach. By partnering with similar organizations, AAGL will have a larger impact worldwide. In partnership with the European Society of Gynaecological Endoscopy (ESGE), European Board and College of Obstetrics and Gynaecology (EBCOG), European Academy of Gynaecological Surgery (EAGS), European Network of Trainees in Obstetrics and Gynaecology (ENTOG), and American Congress of Obstetricians and Gynecologists (ACOG), a joint statement was released in April 2014 recommending that every hospital teaching endoscopic surgery make available a dry lab for training. Simulation-based train-ing will provide gynecologists the necessary skills prior to training in the operating room. In addition to this collaboration, we are working closely with ESGE planning future research and studies, including a sarcoma registry, joint educational meetings, and inter-national fellowship opportunities.

Education – For many years, we have acknowledged the lack of adequate training in minimally invasive gynecology in most residency programs. There is a critical need for a unified curriculum to train and creden-tial surgeons in gynecologic endoscopy. The Essentials in Minimally Invasive Gynecology (EMIG) was developed to offer residents, fellows and practicing physicians an avenue for learning and practicing knowledge and skills critical to gynecologic endoscopy.

The program consists of a curriculum of 22 chapters, in addition to cognitive and skills tests, to provide training in the fundamentals of minimally invasive gynecologic surgery. Development and testing of training models is in progress and implementation of the program is scheduled for 2015.

JMIG – Under the leadership of Dr. Tommaso Falcone, several new features have been added to the Journal of Minimally Invasive Gynecology, including acceptance of video articles and an application for use on tablets and smartphones. In 2014, the journal added 500 pages due to the increase in high

quality scientific submissions, and in 2015 the number of issues will be expanded to further meet this need. Additionally, the journal is now published in Chinese, extending the global impact of AAGL.

Morcellation – Obstacles provide oppor-tunity. While morcellation has posed a chal-lenge, the topic has increased awareness of potential problems and the need for further scientific investigation into alternative treat-ment methods. The AAGL Tissue Extraction

Task Force has addressed this issue and it remains a work in progress.

Assessment – One of the greatest accom-plishments this year was the organization’s internal examination. The AAGL Board of Trustees focused inward to ensure the foun-dation of the organization is growing pro-portionately with its expanding membership. Currently, the bylaws are under revision to conform to California Corporations Code for Non-Profit Organizations and for consistency with AAGL’s current policies. The revised bylaws will be sent to the general member-ship for ratification. Additionally, all newly

elected Board members will receive training on the AAGL bylaws, policies and require-ments for 501(c)(3) non-profit organizations, so they may fully comprehend their fiduciary responsibility as stewards of the organization.

It has truly been a pleasure serving the membership as President of the Board of Trustees, and I thank you for the honor.

AAGL SurgeryU broadcasts up to 10 live events per year, featuring some of the best and brightest surgeons in the field of minimally invasive gynecologic surgery. Each event features a live or pre-recorded feed from the surgeon’s OR and an opportunity to ask the surgeon questions by way of a real time chat system. To date, AAGL members have watched over 440,000 combined minutes of live video on SurgeryU since these events launched in 2010.

AAGL also provides the SurgeryU Video Library, which is the most comprehensive online collection of surgical demonstrations available in the field of minimally invasive gynecology. The SurgeryU Video Library currently has more than 1500 surgical videos, which are searchable online at AAGL.org.

This past November in Vancouver, AAGL staged its largest SurgeryU pro-duction to date with “Stainless Steel Surgeon,” a 90-minute educa-tional demonstration between expert surgeons

Dr. Rosanne M. Kho (Vaginal Surgery), Dr. Ted T.M. Lee (Laparoscopic Surgery), and Dr. Michael Pitter (Robotic Assisted Laparoscopy). This innovative and creative session was highly anticipated because the procedure (hysterectomy) and the surgeons were kept secret until it started. The event was presented in the main ballroom of the

43rd AAGL Global Congress on a stage featuring three ORs that were specifically built for this session. Each surgeon was given 30 minutes to demonstrate their sur-gical techniques on a fresh, unembalmed cadaver. An unprecedented live audience of 1,100 attendees was present in the ball-room to witness the event, plus there were an additional 1,403 viewers from all over the world watch-ing live on SurgeryU via the high definition camera that provided both external and internal views of each of the surgical demos.

Although the event ran for just 90 minutes, months of planning by dozens of peo-ple made this event a suc-cess. Here is a sneak peak at what happened behind

the scenes to make Stainless Steel Surgeon possible:

• About 6 months before the event, the three Stainless Steel Surgeons were selected by AAGL Global Congress Scientific Program Chair Dr. Arnold Advincula (their names were kept a secret until just moments before the event started).

• The Scientific Program Committee iden-tified that the three major routes of surgery for hysterectomy should be demonstrated by these expert surgeons.

• Over $4 million of medical equip-ment was flown to Vancouver from all over the world to build the three Stainless Steel Surgeon ORs on stage.

• Building upon the success of the previ-ous 50 live events, the AAGL-SurgeryU team added new high-definition cameras and video equipment to the stage to bring the event to viewers all over the world.

• Over $2 million of audiovisual hardware was put in place in the ballroom to seamlessly produce the video portion of the event.

• The live video stream was transmitted to more than 5,000 streaming servers around the world via our content distribution network to provide the fastest, clearest picture possible to viewers in every corner of the world.

• Most importantly, three women dedi-cated themselves to the advancement of medical education by gifting their bodies after life so that they could be used to teach these valuable surgical techniques to minimally invasive surgeons everywhere.

INDICATION Essure is indicated for women who desire permanent birth control (female sterilization) by bilateral occlusion of the fallopian tubes.

IMPORTANT SAFETY INFORMATIONPrescription OnlyCaution: Federal law restricts this device to sale by or on the order of a physician. Device to be used only by physicians who are knowledgeable hysteroscopists; have read and understood the Instructions for Use and Physician Training manual; and have successfully completed the Essure training program, including preceptoring in placement until competency is established, typically 5 cases.

Who should not use Essure • Essure is contraindicated in patients who are uncertain about ending fertility, can

have only one insert placed (including contralateral proximal tubal occlusion or suspected unicornuate uterus), have previously undergone a tubal ligation, are pregnant or suspect pregnancy, delivered or terminated a pregnancy less than 6 weeks prior to the Essure procedure, have an active or recent upper or lower pelvic infection, or have a known allergy to contrast media.

• Patients undergoing immunosuppressive therapy (e.g. systemic corticosteroids or chemotherapy) are discouraged from undergoing the Essure procedure.

• Uterine or fallopian tube anomalies may make it diffi cult to place Essure inserts.

Pregnancy Considerations • The Essure procedure should be considered irreversible. Patients should not

rely on Essure inserts for contraception until an Essure Confi rmation Test [modifi ed hysterosalpingogram (HSG)] demonstrates bilateral tubal occlusion and satisfactory location of inserts.

• Effectiveness rates for the Essure procedure are based on patients who had bilateral placement. If Essure inserts cannot be placed bilaterally, then the patient should not rely on Essure inserts for contraception.

• Effects, including risks, of Essure inserts on in vitro fertilization (IVF) have not been evaluated.

• Pregnancies (including ectopic pregnancies) have been reported among women with Essure inserts in place. Some of these pregnancies were due to patient non-compliance or incorrect clinician interpretation of the Essure Confi rmation Test (modifi ed HSG).

Procedural Considerations • Perform the Essure procedure during early proliferative phase of the menstrual

cycle. Terminate procedure if distension fl uid defi cit exceeds 1500cc or hysteroscopic time exceeds 20 minutes as it may signal uterine or tubal perforation. Never attempt to advance Essure insert(s) against excessive resistance. If tubal or uterine perforation occurs or is suspected, discontinue procedure and work-up patient for possible complications related to perforation, including hypervolemia. Do not attempt hysteroscopic Essure insert removal once placed unless 18 or more trailing coils are seen inside the uterine cavity due to risk of fractured insert, fallopian tube perforation or other injury.

• DO NOT perform the Essure procedure concomitantly with endometrial ablation. Avoid electrosurgery on uterine cornua and proximal fallopian tubes without visualizing inserts.

Nickel AllergyPatients who are allergic to nickel may have an allergic reaction to this device, especially those with a history of metal allergies. In addition, some patients may develop an allergy to nickel if this device is implanted. Typical allergy symptoms reported for this device include rash, pruritus, and hives.

MRI InformationThe Essure insert was determined to be MR-conditional according to the terminology specifi ed in the American Society for Testing and Materials (ASTM) International, Designation: F2503-05.

Clinical Trial Experience • Safety and effectiveness of Essure is not established in patients under 21 or over

45 years old, nor in patients who delivered or terminated a pregnancy less than 8-12 weeks before procedure. Women undergoing sterilization at a younger age are at greater risk of regretting their decision.

• The most common (≥10%) adverse events resulting from the placement procedure were cramping, pain, and nausea/vomiting. The most common adverse events (≥3%) in the fi rst year of reliance were back pain, abdominal pain, and dyspareunia.

This product does not protect against HIV infection or other sexually transmitted diseases.

Remembering Jaroslav (Jerry) F. Hulka, M.D. (1930 – 2014) Friends and colleagues were saddened to learn of the passing of the AAGL’s 5th President, Jaroslav F. Hulka on November 24, 2014. He served on the first AAGL Board of Trustees and was active in many AAGL programs.

Jerry, as he was known to all, attended undergraduate school at Harvard University and Columbia Medical School where he completed a residency in Obstetrics and Gynecology, and a research fellowship in immunology. He was a member of the fac-ulty of the University of Pittsburg, School of Medicine for six years. He then moved to Chapel Hill to join the faculty in the School of Medicine (Department of Obstetrics and Gynecology), School of Public Health (Department of Maternal and Child Health) and the Carolina Population Center, where

he continued to pursue his clinical and research goals until his retirement in the late 1990s.

Jerry was dedicated to helping patients attain their reproductive goals. He developed the Hulka clip for occluding the Fallopian tubes and his textbooks provided the first artis-tic drawings of laparoscopic procedures. He was a mentor to many young gynecologists. His avocation was playing the French horn which he enjoyed throughout his professional life and into retirement.

He was a man of many interests and he excelled in all. He was a quiet and gracious scholar who was respected by all who knew him and he will be missed.
