Hi readers!

Welcome back to our fifth edition of CRC E-newsletter. How time flies. It’s the end of 2018.

How are you? I hope 2018 has brought you all the success, blessing, good health, happiness and joy.

What have you achieved so far in research field? Have you been involved with any clinical trials or any sorts of

research? If that so, I bet you have familiarised yourself with the term ‘Informed Consent’. In this issue, we would like to

illuminate about the Informed Consent topic, which is one of the very important part before a research is conducted

especially in human being. As you know, informed consent is an essential process whereby it protect subject autonomy

and human rights. Guidelines stated that in obtaining and documenting informed consent, the investigator should comply

with the applicable regulatory requirements and should adhere to Good Clinical Practice (GCP) and to the ethical

principles that have their origin in the Declaration of Helsinki. Let’s have a read in details about the importance of

Inform Consent inside.

In this issue, we also would like to share with all readers about our HKL Research Day 2018 that held on 30th

August 2018. It was a successful event participated by more researchers compared to last year’s event, and attended by

many VIPs and prominent figures in research industry that shared their vast knowledge and experiences. In this

e-newsletter edition, we have included the winners of the research poster competition too. Congratulation to all the

winners!! Let’s take a look on the example of their research posters here. Who knows it can instil some ideas and tips to

the new researchers out there who are about to learn on how to do a research poster.

Apart from that, we would also like to share other trainings which were successfully organized by CRC HKL

throughout 2018 inclusive of the interdepartmental trainings that were mutually agreed between CRC HKL and few other

clinical departments in HKL. Looking forward for more interdepartmental trainings collaboration in the coming years.

As for publication, we have listed our HKL staffs’ Publications for 2018. By looking at the number of

publications that has increased year by year, it shows something that we should be proud of. Congratulations to all

researchers who manage to published their research findings. We believed it was not an easy-peasy process but definitely

worth and satisfying once the paper has successfully published right? I hope the achievements that we shared in this

newsletter edition will motivate more young researchers out there to continue their research work and aim to get the

research findings published.

Why need of informed consent?

Informed consent is crucial as it

promotes respect for subject’s

autonomy and it also protects the

participants’ right in any research

conducted involving human. The

intent is that participants can

participate in research voluntarily

with full information about what it

means for them to take part, and

that they give consent before they

participate the research.

What is informed consent?

Informed consent is a voluntary

agreement to participate in

research and it’s not merely a

form that is signed but it is a

process, in which the subject has

the understanding of the research

and its risks.

What are the components in a valid informed consent?

An Informed consent emphasizes a process where the clinical research

participant must receive and comprehend information appropriately to make an

autonomous decision. An informed consent process can be termed as complete,

valid, and meaningful if all four criteria of

information disclosure, competence, comprehen-

sion, and voluntariness are effectively satisfied. It

is essential to consider that competence or

capacity of an individual to make decision

depends on his/her ability to understand relevant

information, to appreciate the nature of situation

along with its consequence, to reason the given

information, and the ability to communicate

choice.

INFORMED CONSENT

Evolution of informed consent

Informed consent evolution begins with Nuremberg Code (1947) which developed in response to the Nuremberg

Trial of Nazi Doctors who performed unethical experimentation during World War II. This code emphasized that

voluntary consent is required in clinical research studies and researchers should minimise risk and harm, make

sure the risks not significantly outweigh the potential benefit in any clinical research.

Later in 1964, World Medical Association in Helsinki, Finland adopted 12 principles to guide researchers on ethical

considerations related to biomedical research. It emphasizes the distinction between the medical care that

directly benefits the patients and research that may or may not provide direct benefit.

In 1979, The Belmont Report sets forth tree principles underlying the ethical conduct of research which are

Respect to person, Beneficence and Justice. Later in 1982, the WHO and Council for Internationals Organizations

of Medical Science, CIOMS created the International Ethical Guidelines for research involving human subjects in

1982. The goal of the guidelines is to support and help implement the ethical principles of the Helsinki

Declaration.

Guideline for GCP was developed by International Conference on Harmonisation (1996) to provide a unified

standard for the European Union, Japan and United States of America to protect the right and wellbeing of human

subject involved in clinical trials and to facilitate mutual acceptance of clinical data by the regulatory authorities

in these regions. Up to date, this guideline is frequently update and widely used as a guidance in protecting

subject autonomy and safety in biomedical research.

The informed consent document must be written in language that is easily understood by the participant, it must minimize the possibility of coercion or undue

influence, and the subject must be given sufficient time to consider participation before giving the consent.

What are the crucial elements that have

to be included in informed consent ?

Malaysian GCP obliged that the Cultural sensitivity aspect (bovine, porcine)

has to be included in the informed consent documents.


How do we get informed consent from subjects who are not able to give

informed consent?

For participants who are not able to give informed consent (e.g., unconscious, minor

(<18 years old), or those suffering from severe mental illness or disability), it has be

obtained from a legally acceptable representative (LAR). LAR is an individual or a legal

body authorized under applicable law to consent, on behalf of a prospective participant,

to the individuals’ participation in the clinical trial. If the participant or LAR is unable to

read/write, then an impartial witness should be present during the entire informed

consent process and must append his/her signatures to the

consent form.

An impartial witness is a person who is independent of the trial and cannot be unduly

influenced by the people involved with the trial and who attends the informed consent

process if the participant or the participant's LAR cannot read and who reads the ICF and

any other written information supplied to the participant.

In case of a matured minor (as defined by the ICMR guidelines as an individual <18 years

but ≥7 years), informed assent needs to be taken apart from parental/LAR consent.

30th August 2018 manifested as one of our special day for 2018 as our annual event, Hospital

Kuala Lumpur Research Day came into a success. As usual, Main Auditorium Hospital Kuala

Lumpur has been the witness for the event with a very nice decoration of “Traditional” theme. The

Hospital Kuala Lumpur Research Day 2018 was officiated by our Deputy Director General of

Health (Medical) YBhg. Dato’ Dr Hj. Azman Abu Bakar and had a very good attendance response.

Total estimation of 250 delegates which came from our hospital staffs, National CRC and other

hospitals CRC colleagues, Medical Research Review Committee (MREC) representatives, Institute

of Medical Research (IMR) representatives as well as volunteers support from few universities such

as University Kebangsaan Malaysia (UKM). We were lucky as apart from Clinical Research Malaysia (CRM) being our regular

sponsor for research day, we also have MAXIS and PHARMANIAGA that joined in collaborating and sponsoring the event for

this year.

Maintain the same theme as previous year “Research Today for the Best Healthcare of Tomorrow”, today HKL has

progressed to be the favourite site for clinical trials by major pharmaceutical companies worldwide as being the largest hospital in

the country with the biggest pool of patients. HKL is one of the national referral centres whereby multiple disciplinary specialties

are situated to offer the best services in the country. Thus, HKL has always been at the centre of research hub among MOH

facilities.

During the event, we had the opportunity to congratulate and acknowledge our Hospital Research Review Committee

member (HRRC HKL) who had given their continuous support and commitment in reviewing clinical research proposals together

with CRC HKL team before we make the recommendation to MREC. We were glad enough to see our old and new faces of

committees who joined us on board.

We were very honoured to have an inspiring plenary session entitled “My Research Journey” delivered by Professor Dr

Teo Soo Hwang who is currently serves as Chief Executive and Head of Breast Cancer Research Programme of Cancer Research

Malaysia. She had recently been awarded the OBE by Her Majesty Queen Elizabeth II for her contribution to medical research.

Her sharing session on the vast experience of her in clinical research work amazed the audience. We believed that she had instil

the passion and courage to our young researchers who attended the event.

The research day event continued with a forum session entitled “Bridging Public Health and Tertiary Care Through

Research” participated by experienced panels who tried to unravel the potential of HKL, and what challenges might be ahead in

our pursuit towards the seamless integration of research in the current clinical practice. For this, we invited YBhg. Dato’ Dr Goh

Pik Pin (Director of National Clinical Research Centre), YBrs. Dr Haji Tahir Bin Aris (Director of National Institute of Public

Health), YBrs. Dr Saiful Bin Md Sani (Consultant Internal Medicine, HKL) and YBrs. Professor Dr Sanjay Rampal A/L Lekhraj

Rampal (HOD of Social & Preventive Medicine, Faculty of Medicine, UMMC) to lead the hot one hour forum session.

Overall, we had a very good number of participations this year as there were 59 posters and 3 oral presentations were

contested. All the presentations were judged fairly by 8 experienced and dedicated judges. They split into 3 for Oral and 5 for

Poster presentation. Professor Dr Ng Chirk Jenn (Faculty of Medicine, University of Malaya), Dr Ho Tze Ming (MREC Scientific

Reviewer) and Dr Zuraidah Che Man (Clinical Epidemiologist, UKM Medical Centre) led the Oral presentation competition.

While Dr Hajjah Nor Fariza Binti Ngah (Head of Ophthalmology & CRC Hospital Shah Alam), Dr Wong Yoke Fui (Head of

CRC National Cancer Institute, Oncologist), Dr Zaleha Abd Manaf (HRRC member, Former Head or Radiology department

HKL), Dr Ngu Lock Hock (Chairperson of HRRC, Consultant Medical Geneticist) and Mdm Rohana Hassan (HRRC member,

Clinical Pharmacist) be the judges for the poster competition. We would like to congratulate to all the winners for their great

research work and presentation.

Our event was closed by our HKL new director, YBrs. Dr Herric Coray. CRC HKL would like to thank everyone who

has tirelessly contributed towards making this event a success. We hope that we continue to show our commitment in ensuring a

flourishing research culture in HKL and pave a distinctive path for our future generation. See you in the HKL Research Day 2019.
