National Implementation Plan 2015 - 2017

The plan does not represent the totality of

what is required to address the multi-faceted

challenges facing the health service.

Focuses on reducing the biggest risk to our population’s health and our

services – the burden of chronic disease

Capitalises on our large size, our influence, our workforce, and our day to

day relationships with our service users

Builds on what we are already doing – already leading this change –

bringing those things together

Will use exiting external partnerships and structures for implementation

e.g. children service committees, local authorities

Focuses on what will make this a sustainable priority over 5 – 10 years
