PERBEDAAN TROMBOSIT SEBELUM DAN SESUDAH
TRANSFUSI FRESH FROZEN PLASMA PADA
PENDERITA DEMAM BERDARAH DENGUE

ABSTRAK
Trombositopenia berat sering terjadi pada fase akut DBD dan menjadi dilema dalam
menangani pasien karena kekhawatiran terjadi perdarahan. Transfusi darah yang biasa
diberikan dalam penanganan kasus tersebut adalah transfusi suspensi plasma beku atau
Fresh Frozen Plasma (FFP) yang mengandung plasma dan faktor koagulasi labil (faktor
V dan faktor VIII). Setiap unit biasanya dapat menaikkan masing-masing kadar faktor
pembekuan sebesar 2-3% pada orang dewasa, dosis inisial adalah 10-15 ml/kg. Transfusi
plasma diharapkan meningkatkan trombosit karena fungsi plasma tersebut untuk
menambal atau menutup perembesan atau kebocoran pada penderita DBD. Pemeriksaan
jumlah trombosit dilakukan 6 jam setelah pasien selesai transfusi darah FFP. Tujuan
penelitian untuk mengetahui perbedaan jumlah trombosit sebelum dan sesudah transfusi
jenis Fresh Frozen Plasma pada penderita DBD. Jenis penelitian analitik pendekatan
cross sectional. Penelitian dilaksanakan di RSUD Tugurejo pada bulan Juni-Juli 2017
terhadap 16 pasien rawat inap diagnosis DBD dengan menghitung jumlah trombosit
sebelum dan sesudah dilakukan transfuse FFP. Hasil diperoleh variasi peningkatan
jumlah trombosit sesudah transfusi FFP, yang meningkat dibanding sebelum transfusi
FFP. Peningkatan bervariasi, antara 3,13%-75% atau rata-rata 28,32%. Uji beda Paired t
Test diperoleh hasil terdapat perbedaan bermakna antara jumlah trombosit sebelum dan
sesudah FFP.

DENGUE HAEMORAGIC FEVER PATIENTS

ABSTRACT
Severe thrombocytopenia often occurs on the DHF acute phase and becomes a dilemma
in dealing with patients because of worries about bleeding. The usual blood transfusion is
given in that case treatment are frozen plasma suspension transfusion or Fresh Frozen
Plasma (FFP) which is containing plasma and unstable coagulation factors (factor V and
factor VIII). Each unit usually can raise each freezing factor level by 2-3% on adults, the
initial dose is 10-15 ml / kg. Plasma transfusion is expected to increase thrombocyte
because that plasma function is to patch or close the infiltration or leakage on DHF
patients. The examination of thrombocyte amount was performed 6 hours after the
patients completed FFP blood transfusion. The research goal is to know the difference of
thrombocyte amount before and after Fresh Frozen Plasma type transfusion on DHF
patients. The research type is cross sectional approach analytic. The research was
conducted in RSUD Tugurejo on June-July 2017 of 16 towards hospitalized patients who
were DHF diagnosed by counting thrombocyte amount before and after FFP transfusion.
The result obtained variation of thrombocyte amount increase after FFP transfusion,
which is increased compared to before the FFP transfusion. The increase varies, between
3.13% -75% or average 28.32%. Different test of Paired t Test was obtained result that
there was significant difference between thrombocyte amount before and after FFP.
