Gen-Os® 100% collagenated bone mixA natural replica of autologous bone, Gen-Os® conserves the same intimate structures (matrix) and porosity; and has highly osteoconductive properties. Putty is a bone paste with 80% fine micronized heterologous bone (granulometry up to 300 μm) and 20% collagen gel. Gel 40 is made of 40% collagen matrix (type I and III) obtained using exclusive Tecnoss® process, loaded for 60% of its volume with micronized heterologous bone (granulometry up to 300 μm).

The product is in a gel state at temperatures below 30°C; at higher temperatures viscosity is reduced and Gel 40 can be mixed with hydro soluble and or liposoluble drugs. Thanks to its collagen component, Gel 40 facilitates the formation of primary blood clot and the subsequent invasion of repairing and regenerative cells; moreover the cortico-cancellous component provides the necessary scaffold function. The collagen gel component contained in Gel 40 is rapidly and totally resorbed; it is also endowed with exceptional anti-inflammatory, eutrophic and cicatrizing properties. This lipophilia is due mainly to a percentage of polyunsaturated fatty acids of the oleic-linoleic series (to which Omega 3 also belongs) directly derived from the raw material. Such components possess a valuable antioxidant action on the free radicals and therefore aid tissue regeneration.

HANDLING The distinctive characteristics of viscosity and density of Gel 40 facilitate the handling of the product by the operator, providing a glue-like support. If viscosity is excessive, add a few drops of sterile, lukewarm saline and then re-mix thoroughly to obtain the desired density.

Placed on the site Gel 40 combines with blood, contributing to the fast and compact formation of primary blood clot.

Sp-Block is a cancellous block of xenogenic bone produced with an exclusive Tecnoss® process which avoids ceramization of the hydroxyapatite crystals, thus accelerating physiological resorption. Sp-Block supports new bone formation; thanks to its rigid consistency it is able to maintain the original graft volume, which is particularly important in case of large regenerations. Moreover, its collagen content facilitates blood clotting and the subsequent invasion of regenerative and repairative cells, favouring the restitutio ad integrum of missing bone.

HANDLINGSp-Block must be hydrated before use for 5/10 minutes with sterile, lukewarm physiological solution or with antibiotics. Afterwards, it can be adapted to the receiving site; the block must always be fixed with osteosynthesis microscrews and should be protected with a resorbable membrane (Evolution)

HANDLING The membrane can be shaped with sterile scissors until the desired size is reached; unless the grafting site is already bleeding, the membrane should be rehydrated with lukewarm physiological solution. Once it acquires the desired plasticity it must be adapted to the grafting site.

Evolution membrane is obtained from mesenchymal tissue and is completely resorbable. Its structure is made of dense collagen fibres of high consistency and of extraordinary resistance.

Derma Membrane has two functions; covering dehiscences and augmenting soft tissue.Partially exposed mp3 or Gen-Os can be covered with Derma and the dehiscence will heal over it. Closure will normally take 3-4 weeks (second intention flap closure). During healing, while exposed, it is possible that membrane thickness be reduced slightly by enzyme activity. It is not usual that the membrane get infected and it will protect the underlying graft.Soft tissue augmentation: this requires secure suturing of the flap over the Derma. Mucosa infiltrates the membrane and thickens soft tissue over 4-5 months; this is very beneficial for implants in a site with thin soft tissue. Derma placed around the neck of an implant and with good closure will help produce thicker / stronger soft tissue which is important for future good maintenance and the life expectancy of the implant.

Heterologous collagenated cortical bone

HANDLING All Lamina can be shaped with sterile scissors until the desired size is reached, then it must be hydrated for 5/10 minutes in sterile physiological solution - Note do not hydrate Lamina Soft (Curved).

Once it acquires the desired plasticity, it should always be immobilised either with titanium microscrews or sutured (fine model) directly to the surrounding tissues with a triangular section non-traumatic needle.

Lamina Soft (Curved) should not be hydrated and must be fixated with osteosynthesis screws. Lamina should only be removed if there is a clear suprainfection, because its consistency is such as to allow it to achieve a complete second intention healing of the wound.

Luxator Periotomes are the preferred instrument for performing extractions. These instruments were invented by a Swedish dentist to make extractions as trauma free as possible. Choose the needle holder carefully to match the size and strength of the needle to be used. A larger needle holder than required can result in damage, in particular distortion of the curvature. Needles should be grasped firmly on the tip of the needle holder jaws. Needles should only be held in the flatted area provided and not on the attachment area or near the needle point. Needles which are not flat should be grasped for placement at a point approximately one third of the needle length from the butt.

Reverse Cutting NeedleTriangular in cross section, with the apical cutting edge on the outside of the curve improves strength and increases resistance to bending.

Conventional Cutting NeedleTriangular in cross section with the apex on the inside of the curve. The effective cutting edges are at front section and merge into a triangulated body which continues for half the length of the needle.

Tapercut NeedleCombines the initial penetration of a cutting needle with minimised trauma of a round bodied needle.

Prime Needle Cross geometry reduces cutting edge angles to give improved penetration and control. A square shank increases strength and improved grip in the needle holder. Available as conventional or reverse cut.

Bone Quantity At least 4 mm of bone in a facial-lingual direction and preferably 13 mm of bone in a crestal-apical direction provide relatively acceptable conditions. However, success can be achieved with as little bone as 3 mm and 10 mm respectively.

Bone QualityWhen screwing the SDI into place, at least 30 Ncm of resistance should be present. If that level of resistance is not present, implant failure is likely. Refer to Fig. 5 and note that the bone on both the maxillary and mandibular arches is not dense. This patient did not receive implants on the maxillary arch, but he had SDIs placed on the mandibular arch that have served for many years. As you know, the maxillary bone is usually not as dense as mandibular bone.

Width of ImplantThe diameter of the SDI should be as wide as the bone that is present will allow. Use of the smallest diameter SDI (1.8 mm) is not advisable if wider implants can be used.

Length of ImplantThe average most used length for SDIs is 13 mm as reported by companies selling SDIs. Observational data confirms the success of this length, although shorter SDIs can be used with caution.

OcclusionBruxers, either grinding or clenching, are not good candidates for SDIs. If SDIs are used on these patients, flexible attachments such as rubber “O” ring attachments should be used to provide resilience.

Soft-tissueSoft tissue more than 2 mm thick coronal to the bone should be removed to reduce excess stress on the SDIs.

Parallelism Place SDIs parallel to one another! More than 15 degrees of non-parallelism can be negative.

Separation of ImplantsDo not place implants too close together. Leave space for the housings or attachments, which are at least 4 mm in diameter.

Immediate Loading of ImplantIf the bone quality is questionable, place soft denture reline material in an indentation inside the denture over the implants and allow the implants to integrate into the bone for a few months be-fore loading. However, most SDIs used for support and retention of complete den-tures can be loaded the day they are placed.

Huddersfield’s First elexxion Pico Laser Dr Jones, Royd House Dental Practice, Huddersfield HD3 3NF

Huddersfield, is now well and truly on the map! The Yorkshire towns first elexxion Pico Laser has been installed at the Royd House Dental Practice, in East Street, which now adds laser treatment to the high quality dentistry previously offered. Dr Jones, who is no stranger to lasers said of the Elexxion Pico Laser: “This is a super unit, neat compact and easy to use.”The elexxion Pico Laser is very easy to operate with a user friendly, menu based, control system that is easy to navigate. The unit itself is super-compact and battery powered. For Dr Jones the Laser fibre delivery system allows complete and free movement of the hand-piece in hand. Treatment is without the noise and vibrations associated with other procedures.

Is ideal for soft tissue surgery – and does not interact with teeth or bone. In periodontology and endodontics–it can decontaminate and destroy bacteria. It can also be used for Tooth Whitening (of both vital and non-vital teeth). This allows superb results to be obtained in surgery, in relatively short times with low levels of sensitivity. The elexxion Pico Laser contains two lasers: a 5 Watt Gallium Aluminium Arsenate (GaAlAs) diode laser and a small laser pointer. The laser energy is fibre delivered - the smallest available fibre being 200 microns. The GaAlA’s laser has a wavelength that makes it an ideal way to do minor oral surgery. Using this laser, an area can be cut with localised haemostasis. Not only does the laser cut but it also cauterises the tissues as well making for good post-operative results. All this is done silently and in a tactile manner – giving plenty of control to the clinician.Our offering includes lasers for hard tissue as well as snoring treatment.

They were designed by Patrick Palacci who observed through many years of experience that:-

1. That optimum space (7mm approx.) is needed from a biomechanical and aesthetic point of view.

2. Ideally, each crown will be cylindrical emerging from the implant.

3. Pre-treatment bone loss will influence the number of implants needed to support a bridge. There is no relationship between the number of teeth to be replaced and the number of implants placed using the surgical guide.

Careful analysis of the positioning of implants is essential.
