Powered Haulage Safety

Powered Haulage accidents are low-frequency, high-consequence events• 50% of fatalities in 2017• 46% of fatalities so far in 2018• 5 fatalities in 2019 – two are powered

haulage

Two seatbelt fatalities in one weekOctober 19, 2018: 63-yr-old operator lost control of his haul truck while descending 18% grade, drove through berm; no seatbelt used

October 25, 2018: 29-yr-old driver died when pick-up flipped after climbing a berm; no seatbelt used

Fire Suppression Failures

• Fire suppression systems failed in all• One miner killed, another injured• Fire suppression equipment must be

properly installed• Miners should have a means to escape

Scofflaw Initiative

• Small share of operators are chronically delinquent and have poor safety records

• Monthly selection of the worst violators• Payment plans proposed/established with

total commitment of $5.5 million• 17 mines paid in full for $1.5 million• 38 104(a) citations issued; 16 104(b) orders

issued; all but 3 operations are now open
