Award for Clinical Excellence

Intermediate Care Social Work TeamThis team is made up of social workers and community assessment officers working city-wide to support discharges from the intermediate care units and they support colleagues from Rapid Response. The team work hard to reduce the likelihood of individuals being readmitted to hospital by ensuring the correct care, equipment and support is in place prior to going home; helping patients take control of their own lives.

Rapid Response / HomesafeOver the past year this team has worked with wards to develop the ‘discharge to assess’ model and allow patients to get home as soon as possible. They have embraced quality improvement methodology and have constantly adapted and challenged practice. Nothing has been too much trouble for them and they have embraced working in an integrated way with social work colleagues and the Salford Urgent Care Team, helping to keep patients in their own homes and out of hospital.

Pennine Lung Service Pennine Lung Service Oldham (PLS) provides community based respiratory services, including COPD admission avoidance and early supported discharge, consultant and nurse led outpatient clinics, pulmonary rehabilitation (PR), domiciliary rehabilitation, quality assurance of primary care spirometry and primary care education. The service also works with third sector organisations including the British Lung Foundation (BLF), Oldham Community Leisure Services (OCL) and Age UK. The service aims to deliver a patient-centred approach to care, which supports the concept of self-care and independence for respiratory patients. Education of patients and the development of individualised care plans are an essential part of achieving these aims.

Rachel Newport - Research Midwife, Research and Innovation Rachel is a rare find. Not only enthusiastic and passionate about research, but innovative and collaborative as well. Rachel works tirelessly on studies to help women have better pregnancy outcomes and improve not only their health, but that of their babies as well. She works across Oldham, North Manchester, Bury and Rochdale sites, going to where the patients are located to maximise recruitment into her studies. As a result of this commitment, 2017-18 was the best recruiting year for studies in this specialty area at Pennine Acute and 2018-19 looks like being even better, with this specialty reaching its target less than halfway through the year.

Catherine Hegarty - Practice Educator Midwife, MaternityCatherine Hegarty is a practice educator midwife. She provides a high standard of education, not only for midwives but for the whole multidisciplinary team. Every day, Cath demonstrates and promotes clinical excellence within the maternity unit. Her high standards and extensive knowledge ensure that women and babies are receiving safe, up to date evidence-based care. Not only is Cath an excellent educator, she is an integral member of the senior team. The staff within the unit truly value her role and the staff have no better role model.

Eve regularly meets with distressed families and works through very difficult inquest cases to ensure the optimum outcome for every family.

HMR Integrated Leads - Integrated Neighbourhood Teams, Rochdale InfirmaryAt a time of significant change and transformation the HMR Cluster and borough-wide lead teams have consistently maintained safe standards of service delivery, through driving change and supporting staff to develop innovative practice. They emit a strong team ethos through their close communication with their community teams as well as developing and maintaining close working relationships with partners and third sector agencies. On a daily basis they enable their teams to deliver high standards of care often with competing demands contributing to the positive service achievements seen across the HMR community.

Wolstenholme Unit, Rochdale Infirmary The Wolstenholme Unit team works together to ensure patients receive the best care. They embrace and engage with collaborations such as the deteriorating patient and End PJ Paralysis initiatives to help improve their service, the patient experience and patient safety. They encourage patients to undertake activities and encourage their families to get involved too. The team stimulate and support physical, psychological and social well-being of patients and look for innovative ideas like creating a tropicool juice bar to assist with hydration or introducing decaffeinated drinks during the evening to help reduce night falls. This has all helped to achieve a green NAAS on their first assessment.

Linda Applebee - Lead Specialist Midwife for Mental Health, MaternityLinda is an inspiration to others, always striving to improve the care and outcome for mothers who have a mental health diagnosis, and their families. Linda has excellent partnerships with community and hospital teams, GPs, social care and psychiatrists among others. She was instrumental in the coordination of the new perinatal psychiatry clinic. With Linda’s knowledge, experience and expertise, women and their families receive compassionate, inclusive evidence-based care.

Denise King - Clinical Governance Manager, MaternityDenise has been pivotal in leading the maternity team on a journey from ‘inadequate’ to ‘good’ in the CQC ratings. Much of the transformation has happened thanks to clear and robust processes to deal with reporting and managing incidents, investigations and complaints. Denise makes sure that staff are supported and is committed to help them progress to achieving an ‘outstanding’ rating in the future.

Quality Improvement and Innovation Award

Emergency Department – Urgent Treatment CentreThe emergency department has worked very closely with the QI department carrying out a large number of tests of change, which have resulted in changes in practice and improved patient experience. GP streaming has been introduced at the front door ensuring patients are being directed to the correct place and only appropriate patients are seen in the department. The introduction of a weekly 15 minute MDT communication cell keeps all staff updated with results from test of change and performance.

Gabriel Ford - Highly Specialist Physiotherapist and Clinical Lead for Go2physioGabriel is an integral member of the MSK outpatients team. She is described as a born leader and is at the forefront of driving the physiotherapy department forward in innovative ways. Most recently she has been dedicated to initiating and implementing the Go2Physio project – an online self-referral service for patients requiring musculoskeletal physiotherapy. She works tirelessly and can be relied on at all times. She is highly professional and is also quite a dab hand when it comes to baking!

Stacey Brown - NIPE Midwife, Postnatal Ward Stacey started working as a Band 6 NIPE midwife in March 2018. Her role involves performing the newborn baby examination prior to discharge and referral on to the wider team. Prior to Stacey taking on this role, baby checks were completed on an adhoc basis by paediatricians and this process had many complications, particularly surrounding flow. Stacey has set up a fully functioning baby clinic, which has received fantastic feedback from service users. Flow through the postnatal ward has improved since Stacey came in to post and discharges are now prompt and timely.

Ward F1 (Gynaecology) Ward F1 recognised that elective patients were having a poor experience while waiting for admission to the ward due to site pressures and availability of beds. Patients should be admitted straight onto the ward into various beds and when they weren’t, chaos and confusion ensued. A small change was tested and introduced, and patients were placed in another area away from the ward. A visual management system was also introduced. The process was refined and has now been embedded. Anaesthetists and theatre staff have commented how much better this works. Patient feedback has also been great.

Sarah Parker & Suzy Hilton - Dietetic Operational and Professional Leads Sarah and Suzy have been instrumental in designing a new inpatient pathway for oral nutritional support that aims to encourage ward nursing staff to offer first line support to poor feeders at the bedside and empower patients who are not eating well to take control of their own nutritional care once they are discharged. This approach has released dietetic resource, which can be focused on the clinically high-risk patients who are very unwell and who may need artificial feeding, allowing interventions to be actioned in a more timely and effective manner.

Care Home Extra Support Service, Rochdale InfirmaryThe Care Home Extra Support service is assisting nursing and residential homes across Heywood, Middleton and Rochdale to assess and treat residents in a ‘virtual bed’ within the home. Referrals to the service have steadily increased and the single point of access allows the nursing or residential home to phone for advice to provide the best possible journey for the patient. The service will also demonstrate a reduction in ambulance call outs and A&E attendances and is contributing to a reduction in hospital admissions.

HMR Integrated Neighbourhood Team, RochdaleWith 70% of sepsis patients falling ill at home the HMR Integrated Neighbourhood team developed a Community Observation Chart using NEWS2 and Sepsis Screening and Action tool to help identify acute illness in the community. They developed the tool in partnership with NWAS and the QI team and also developed a highly engaging and interactive e-learning tool with the digital learning team. Early results show that the document and e-learning tool has resulted in staff successfully identifying acutely unwell patients in the community and getting them into hospital quicker to receive the right treatment earlier.

Sally Hulmes, Lead Nurse for Falls Prevention, Fairfield General HospitalSally has been pivotal to the falls prevention quality improvement programme at Fairfield General and is an inspirational role model across the site with high clinical visibility and credibility. Sally has been focused on improving patient experience and safe care, and has supported several ward teams through bespoke education, working alongside clinical teams to challenge practice, change culture and embed sound fall prevention strategies. Sally encourages teams to identify issues and supports them to take on tests of change. Sally set up and leads a Falls panel to share learning, ideas and improve understanding.

Ward F3 Ward F3 are enthusiastic about innovation and change management. They volunteered to take part in the testing process for manual observation equipment within the clinical area and, following positive leadership and successful team engagement, F3 is now the first ward to be fully manual. They have managed their NAAS accreditation with a positive attitude, going from red to amber to green.

MSK Physiotherapy Team The outpatient physiotherapy department started a self-referral project funded by two North Manchester GP neighbourhoods. Patients refer themselves for physiotherapy and have treatment within the service or are referred to secondary care if required. The project has received positive feedback from patients and GPs which has led to a third GP neighbourhood asking to join. Staff have worked hard to build strong relationships with GPs and community groups.

Karen Hunter - Lipoatrophy Nurse, Infectious DiseasesKaren took over the lipoatrophy service for HIV patients a year ago, undertaking lots of additional training. The service provides patients with the opportunity to live normally and not feel ashamed of their status and this increases compliance with therapy. Karen takes medical photography to show patients the difference treatment makes; counselling them and reviewing them holistically. Karen has transformed the service, as seen in regular positive patient feedback.

Safe, Clean and Personal Every Time (SCAPE) and Nursing Assessment and Accreditation System (NAAS)Congratulations to the following teams which have achieved the SCAPE award or ‘green’ NAAS status within the last 12 months. The SCAPE award is given to wards/teams which have attained level 2 NAAS (green) for 24 months or a total of three NAAS assessments. This year marks 10 years since the introduction of the scheme and we are really proud of the way our wards and teams across the Northern Care Alliance have embraced this programme to ensure our patients are receiving safe, clean and personal care every time.Well done to all the staff working for the wards and teams below for making such fantastic progress on their SCAPE journey.
