1. Introduction Milvik Bangladesh have partnered with bKash, a mobile money provider, to launch two products
that will use mobile wallets as payment channels
The two products offered will be Milvik Life product and Milvik Health
2. Milvik Life
2.1. Insurance Partner Prime Islami will be the insurance partner for the Life product.
2.2. Insurance Benefits The product will pay a lump sum benefit in the event of death* (for any reason) and an additional lump
sum if the death is due to an accident.

2.3. Pricing A customer can subscribe for the product in two ways
i) One-time (Annual and 2-year)
ii) Subscription based (Daily and Monthly recurring) (for bKash customers only)
Note: 15% VAT will be applicable on the service charge portion on the prices
2.3.1. Upfront payment
A customer can subscribe with Milvik Life products by making one-shot payment for the next 1 year (for
all plans) or for 2 years (plan 3 and 4). Coverage for these policies will start immediately and will
continue for next 1 or 2 year based on the plan. For 2-year policies coverage will be as per annual
benefits and will renew after completion of 1 year.

2.3.2. Subscription based (Recurring payment)
A customer can subscribe with Milvik Life products by choosing to pay in easy daily and monthly
installments for Life 3 and Life 4 only. Only bKash users can avail this benefit. Payment must be made
either through bKash subscription payment gateway or Milvik microsite embedded inside the bKash app.
Coverage for these policies will start from the beginning of the next calendar month and coverage in a
month will be calculated based on the payment collected in the previous calendar month.

2.4. Eligibility Criteria
Customers aged 18 - 69 will be able to register for the product. If customer is aged more than
69 years, he cannot subscribe for the product. However, after registration, customers will
remain eligible for cover until age 70.
However, for 2-year policy, customer cannot be aged more than 68 during the time of
registration.
For subscription payment, as the membership is created for 2 years, it is expected that
customer is aged not more than 68 at the time of the registration. However, if customer
persists on subscribing for the package on subscription payment basis even though s/he is aged
more than 68, customer must provide declaration that s/he will deregister upon becoming 70
years of age by calling Milvik helpline.

2.6.2. Medical Test Discount
10% to 40% discount at around 150 Milvik Hospital and Diagnostic center partners across the
country
2.7. Waiting Period There will not be a waiting period for annual and 2-year policy subscribers. However, for daily and
monthly subscribers, cover will start from the next month of policy subscription.
In the event of natural death occurring within first 3 months from the date of registration a subscriber
can not avail any Insurance coverage benefits
2.8. Exclusions a) War, invasion, acts of foreign enemies, hostilities or warlike operations (whether war be declared
or not), civil war, mutiny, strike, riot, civil commotion, military rising, insurrection, rebellion,
conspiracy, revolution, military or usurped power, martial law, state of siege, any event or cause
that determine the proclamation or maintenance of martial law or state of siege;
b) Nuclear, Biological and Chemical Risks and losses;
c) Suicide or self-inflicted injury;
d) Natural death within 3 months from the date of registration
e) Pre-existing conditions within 3 months of policy registration; and
f) In case of physical injury, if the insured doesn’t die within 180 days after the date of receiving
injury, and the customer crosses the eligible age limit or ceases to continue the product
3. Milvik Health
3.1. Insurance Partner Protective Islami will be the insurance partner for the Health product.
3.2. Insurance Benefits The product will pay a fixed benefit per night spent in Hospital. The benefit paid and the number of
lives covered are dependent upon the plan selected.

3.4. Persons covered The product will provide coverage for the policyholder and additional family members as shown in the
benefits table.
3.4.1. Eligibility Criteria
Customers aged 18 - 69 will be able to register for the product. If customer is aged more than 69 years,
he cannot subscribe for the product. However, after registration, customers will remain eligible for
cover until age 70.
For main subscriber: 18 to 69 years
For family members: 3 months to 69 years
3.4.2. Beneficiary selection
Any of the following:
1. Father
2. Mother
3. Spouse 4. Son/ daughter
For the nominal complimentary Life benefit, brother or sister can also be nominees.
Annual policy subscribers will be able to review their additional lives covered once a year. Monthly/
Daily subscribers can update the additional people covered; cover period will start after 1 month waiting
period. For example, if anyone request to change beneficiary on 23 June, the new beneficiary will be
covered for 1 August.
3.5. mHealth Benefits
3.5.1. Tele-doctor consultation
Tele-doctor consultation (24*7 tele-doctor consultation) with call-back feature: Milvik Health customers
will be eligible for general tele-doctor consultation, and specialist consultation. Please note, doctor
consultation service is off on government holidays and only primary consultation can be provided over phone. For emergencies it is advised to visit a doctor directly.

5.1. Upfront payment For Upfront payment, service fee will be deducted from customer’s mobile wallet account at once for
the whole upcoming 1 or 2 year. After that, no further charging will happen during the coverage period.

5.2. Subscription based (Recurring payment) The deduction logic will depend on the payment frequency chosen by the customer.
For daily, there is no retrials, once payment missed, it is missed forever. Coverage will be
calculated based on the number of payments received by end of month.

For monthly, there are 2 retrials following the first attempt at 3 days interval. If all deduction
attempts are unsuccessful in month m billing cycle, then there will be no coverage provided in
month m+1. There are no catch-up deduction attempts for month m in month m+1. There will
be a total of 6 days grace period from the payment due date till retrial attempts are over
If the first payment for monthly subscription, was made on 30 March, the next payment date will
be 30 April. Even if customer pays during the grace period, 3 May, the next payment cycle will
be on 30 May. However, in February it will be 28 February, then in the next month it will be 28
March and so on.

6. Un-subscription For upfront payment, there will be no way to unsubscribe, but for recurring payments, based on
customer request, Milvik will initiate deregistration request to bKash and bKash will cancel the
subscription payment.
7. Medical Test Discount Both Milvik Life and Health customers will be eligible for Medical test discounts from partner hospitals
either through physical or digital health card or SMS. For customers in the field, we will be providing a
membership card and for tele-customers we will be sharing the membership number through SMS, which
they will use to avail discount from partner hospitals and diagnostics.
Field:
Customer will be handed over a membership card after you subscribe for the product. When they visit
any of our partner hospitals and take any service eligible for discount, please show the membership card
to the hospital/diagnostic counter before they produce the bill and ask them to provide a discount.
Salesperson will direct the customer to visit Milvik website to see the list of partners or provide the list.
Tele and bKash App:
Customer will be sent an SMS with his/her membership ID along with the link to the discount partner
list.
SMS Text:
“Dear customer, as a valued member of Milvik, your membership ID no. is …. You are eligible for
discount at Milvik partner hospitals. Hospital name and discount information on this link:
https://tinyurl.com/uvlsnho”
In addition, customer will be provided a digital version of the membership card over email or
WhatsApp.
8. Claim Process Customer will call the helpline 09614500599 and place their request. Claim team will call them back and
process for claim filing. Based on the submitted documents, UW will approve/decline claims. Claimed
amount will be disbursed through customer’s bKash account.
Notification of a claim must be received within 180 days of the event (death of customer or
admission in hospital)

Who can be the other family members for hospital cashback?
Ans: Family members can be your parents, spouse, and children
How will the shared coverage work?
Ans: Under each package, there is a maximum number of days for hospital cashback. All the members
can avail cashback provided that their cumulative hospitalized day is within that limit.
Is there a list of hospitals to get hospital cashback?
Ans: No, hospital cashback is available for any registered hospital. You need to get admitted and stay
there for at least 1 night
What happens if the subscriber becomes 70 years old?
Ans: The insurance cover will no longer be available for the subscriber and family members (for hospital
cash) as soon as the main subscriber become 70. However, they can get claim for incidents happening
before they became 70. If the family members want to continue to enjoy the benefits of hospital cash,
new registration must be done with the details of any of them who are within the age limit.
How do I continue service after service duration 1/ 2 year is over?
Customer has to renew the service for another period.
How does coverage under recurring payment work?
Based on your chosen payment frequency, your bKash wallet will be deducted either daily or monthly. if
you are on daily subscription mode, the number of total deductions at the end of the calendar month
will decide your coverage in the next month. For example, you subscribed for Health 6 on a daily
subscription basis. Your wallet was deducted on all 30 days. So, your coverage next month will be BDT
3000-taka hospital cash for 25 days. But if your wallet was deducted ‘x’ days instead, your daily cashback
will be calculated as (3000*x/30) for 25 days. So, the number of days will be unchanged, but the daily
cashback amount will vary based on your deduction rate.
The number of base days in a month will depend on the number of days in the deduction month and it
will be dynamically calculated by the system. For example, if the deduction month has 31 days, available
coverage in the next month will be calculated as (x/31) *Coverage
What if there is not enough balance in my wallet for recurring payment?
For daily, there will be only 1 payment, if enough is not available in the wallet at the time of deduction
attempt, the payment for that day will be missed.

For monthly, there will be 2 more deduction attempts, at 3-day intervals following the monthly due
date. For example, monthly due date is 4th May, but customer did not have enough balance in the wallet,
then the next deduction attempt will be made on 7th May, and if it fails as well, the next attempt will be
made on 10th May. So, customer will have 6-day grace period to make payment for the month and be
eligible for the coverage next month.
My first payment date was 31 January for monthly policy, when will be by next payment
date?
It will be on 28 February (29 if leap year), then 28 March and so on.
Can I pay after my deduction attempts fail?
No, there is no scope at present to catch up for any missed installment.
My monthly payment was deducted at 2nd retrial, what would be my benefit in the next
month?
You will have full coverage as payment was made within the grace period.
If I pay on 1st or 2nd retrial, will my payment cycle be changed?
No, Payment cycle will be as previous. For example, your original cycle was on 15 April, next month,
you missed the payment on 15 May, but paid on 18 May. The next month, first payment attempt will be
made on 15 June, not 18 June.
Will there be any reminder SMS before the recurring payment is due?
Yes, bKash will send the customer a pre-notification before payment date, so that they ensure enough
balance is available. If payment is successful, customer and merchant will be sent success notifications
(from bKash). If payment fails, customer will be notified of payment failure and reason i.e. insufficient
balance or account status (from bKash)
