The Department of Health advocated that treatment of tobacco dependence as an issue cutting across all specialties, and as such core competency in tobacco dependence treatment should be possessed by all specialists. DH has suggested that HKAM may contemplate the following areas of collaboration with DH: 1. Support the formation of a panel or a faculty of specialists on smoking cessation and tobacco control under HKAM. The faculty will comprise HKAM fellows, some of whom may have been conferred as Tobacco Treatment Specialists (TTS) via accredited training. The panel/ faculty will serve as a facilitators/trainers’ pool for training on smoking cessation in the postgraduate/ fellowship training programme of respective colleges. They may also be invited to join overseas assignments on training health care professionals in the region or the mainland, on behalf of a WHO collaborating centre. 2. Incorporate treatment of tobacco dependence into postgraduate training and fellowship examination. This will help enhance the knowledge and skills on smoking cessation and integrate smoking cessation into the daily practice of specialists. The Council of HKAM has endorsed the proposed collaboration. It will be an on- going working relationship with DH and series of events in terms of media promotion, training of TTS and future plans will follow. As the HKAM is the statutory body to set standards in training and assessment, there will be strong implications for all Colleges in these aspects. Members and especially trainees will need to acquire adequate knowledge and skills in the management of tobacco dependence. Here I would like to emphasize that not only tobacco, but all substance abuse or addiction should be covered along the same line of thought. Family Physicians have a point of vantage to deal with these patients, be it “healthy” smokers or those already suffering from CHD or COPD, or school children and teenagers who are vulnerable to the temptation of substance abuse. We can always help them during treatment or prevention, or take the role of educator/advocate. However, we have to ask ourselves whether we are well-trained to assume the responsibility, or are we only just doing case finding and referral out to other healthcare professionals, such as nurses or counselors. Therefore, the College will need to review the training programme to ensure that future trainees can satisfy the requirement of knowledge and skills in tobacco dependence, with proper assessment. FellowshipinHongKongAcademyofMedicineisthebeginningofalife-longprofessionaldevelopment.Itwasnevermeanttobetheend-pointofourtrainingandmedicaleducation.Whileotherspecialtieshaveevolvedanddevelopedvarioussub-specialtieswhicharemoretechnologyoriented,FamilyMedicinestandsoutuniquelyinitscorecompetency.Themostimportantattributesprofoundlyinvolveinter-personalcommunicationandinteractions,humanityandlifeexperiencethroughoutoureverydayclinicalpractice.Everypersonwemeetintheconsultationroomisdifferentinspiteofthefactthathe/shemayhavesimilarcomplaintsorsymptoms;thisneedsnofurtherelaborationtoourmembers.Thereisnoquestionaboutthebasiccontinuousupdateaboutmedicalandtechnologicaldevelopmentwhichisamustforeverydoctor,nomatterwhatspecialtyoneisin.

andfail foraparticulardomain ineachquestion.Thecandidatesweregivenachancetomarkthescenariowiththerespectivekeyfeaturechecklistthemselves,sothattheycouldunderstandhowtoassessaparticulardomainandhowtoachievethepasscriteriaineachparticulardomain.

Therewas lotsof interactionandtheatmospherewas friendly.Thecandidateswereenthusiasticand there were lively discussions. Looking atthe statistics from the feedback forms, mostcandidatesbenefitedfromthisseminarandfoundit veryuseful. The largemajorityof attendeesagreed that the seminarhelped in their OSCEpreparation, and provided useful informationregardinghowtheirperformancewillbeassessed.

Theafternoonwasfruitfulforboththecandidatesand Board Members . I t i s hoped that theadditional informationgainedbycandidateswillhelptranslatetheirhardworkintosuccessinafewmonthstime!

The informationseminaronOSCEsegmentwasheldon20June2010.Theroomwaspackedwith31candidates (Cat I:23,Cat II:8)andmembersfromtheBoard-Chairman,ChiefExaminer,OSCECoordinatorandSecretariat.

TheseminarstartedwiththewarmwelcomebyourChairmanDr.ChanHungChiu.Dr.Chanexplainedtothecandidatesoursettingof theexaminationand questions. He also explained the securitymeasuresinourexamination.

TheInternetispowerfulenoughtobankruptnewspapers.Thefirstpointofresearchforanyinformationisnowtheinternet,andisthenewbattlegroundforpowerandpublicinfluence.AneffectiveefforttowardshealthpromotionandoffamilymedicineontheinternetcanmeanjudiciousmonopolyofprimaryhealthcareinHongKongandAsia,inaveryshorttime.Whatshouldwebedoing?

1. Keywords and Links Promotion

Forastart,weshouldlandonthefirstpagewhenpeoplesearchfor“FamilyMedicine”and“FamilyPhysicians”,bothinHongKongSearchEnginesandaboard.SearchEnginesratetheimportanceofasitebyboththequalityofthesite, thenumberof importantsites linkingtothesaidsite,andthepresenceofcompetitors. Inordertoachievethis,wemusthaveacontentrichdisease-basedsite,a facebookpage,anarticleofourselves inwikipedia,manyarticleswrittenbymembersinvaroiusforums/onlinenewspaperswithalinkreferringbacktooursite,andadviseotherimportantsitesthatweexistandtheyshouldaddalinkreferringtousifpeoplesearchforfamilymedicine,the listgoesonandon. Ifwearecovetous,anyonesearchingforailmentterms(suchasasthma)shouldalsofindus.Thatmeanswehavetodotheabovealloveragainforeachailmentterms,inbothEnglishandthelocallanguage.

Weshouldalsorunadoctors’practisingsearchlistinHK,similartotheotherexistingsearchlists,butwithanaddonsearchtermforanyprimarycarephysicianswhohavecertifiedqualificationsinFamilyMedicine,beitdiplomaorfellowship.

Adisease-basedrichwebsite issorely lacking inHongKong.WecanbecometheforefrontofprimarycareinHongKongandChinaifwehaveanexternalsite insteadofaninternalone,whichisA.)diseasebased,B.)currenthealthaffairbased(withpublicstatementsconcerningdailyhealthcarenews),inEnglish,TraditionalandSimplifiedChinese.Itissimpletosay,butwillrequireconcertedefforts,jointvisions,quickresponse,unification,anda lotmoremedical inputcomparedtonow.However ifachievedtherewillbeamuchgreaterpublicinfluence,andmayevenbeeconomicgainsatpersonalandcollegelevels.

PromotionofourstatuscanbeachievedifwestartseedingarticlesandeducationvideosacrosstheinternetinHongKongandaboardinreputedwebsites.Thearticlesshouldbesignedbythedoctor’snameaswellasunderthecollege’sname.Theyshouldbeonvariouscommondiseasesthatthepublicareinterestedin,oftensearchedabout, incantonese,andnotaboutfamilymedicineconcepts.Theyarethekindthataresimpletowrite,underwellacceptedguidelines,butthepublicarehungryaboutsimplybecausetherearenosucharticleswrittenbyanyonewithauthorityincantonese.

5. Coming up

Onthepracticalside,thereareseveral internaldevelopmentsplannedbutnotyetexecuted, includingonlineregistrationof lectures,andonlinepayment ifsecurityconcernsareresolved.Thissupportsmorepaperlesscommunicationwithmemberswithagreener,fasterandcosteffectivesystem.

In Conclusion

Iguess itwill requireahugecollaborativeeffort,effortsthathaveno immediategains, ifwearetoachieveanyoftheabove.On the web,wecouldriseabovebeingjustoneofthespecialists,butastheprimarycareitself forHongKong.Tohelpthewebgrow,moreresources in termsof timefromvariousboardsandmemberswouldcertainlybenecessary.Onalighterside,havingaspecialinterestinprogramming,internetandmedicine,Ihopemyprogrammingofthenewhkcfpwebsitehasmadelifeeasierforeveryone.FeelfreetocontacttheWebCommitteeforanyrequests,jointventures,insightfulvisions,orsimplyifyouwanttojoinustohelpout.Alasthewebcommitteeconsistsofonlyafewpeople,weneedalotmorepeopletohelpout,theworkisnon-computerrelated.

一生不可少的八種朋友

  隨著生活節奏的加快,社會的浮躁和功利,人與人之間有著太多分不清的是非真偽,以至於我們對「朋友」

的稱謂產生了畏懼。那麼,真正的朋友究竟是什麼樣的,人的一生到底需要什麼樣的朋友呢?美國作家帕爾指

出,「不要指望一位密友帶給你所需要的一切。」另一位作家湯姆拉思則認為,以下八種朋友是必不可少的。

成就你的朋友 他們會不斷激勵你,讓你看到自己的優點。

  這類朋友也可稱之為導師型。他們不一定是你的師長,但他們一定會在某些領域具有豐富的經驗,能經常

在事業、家庭、人際交往等各方面給你提供許多建議。人生中擁有這種朋友會成為你最大的心理支柱,也常常

會成為能夠「左右」你的「偶像」。

支持你的朋友 一直維護你,並在別人面前稱讚你。

  這類朋友可謂是「你幫我,我幫你」,相互打氣,使得彼此成為對方成長的墊腳石。在一個人的成長過程

中,朋友的支持與鼓勵是最珍貴的。當你遇到挫折時,這類朋友往往可以幫你分擔一部分的心理壓力,他們的

信任也恰好是你的「強心劑」。

志同道合的朋友和你興趣相近,也是你最有可能與之相處的人。

  與他們在一起,會讓你有心靈感應,俗稱「默契」。你會因為想的事、說的話都與他們相近,經常有被觸摸

  有些朋友,當我們有了心事,有了苦惱時,第一個想要傾訴的對象就是他們。這樣的朋友會是很好的傾聽

者,讓你放鬆,在他們面前,你沒有任何心理壓力,總能讓你發洩出自己的「鬱悶」,讓你重獲平衡的心態。

開闊眼界的朋友能讓你接觸新觀點、新機會。

  這類朋友對於人生也是必不可少。他們可謂是你的「大百科全書」。這類朋友的知識廣、視野寬、人際脈

絡多,會幫助你獲得許多不同的心理感受,使你成為站得高、看得遠的人。

給你引路的朋友善於幫你理清思路,需要指導和建議時去找他們。

  這類朋友是「指路燈」。每個人都有困難和需要,一旦靠自己力量難以化解時,這類朋友總能最及時、最

認真地考慮你的問題,給你最適當的建議。在你面對選擇而焦慮、困惑時,不妨找他們聊一聊,或許能幫助你

更好的理順情緒,瞭解自己,明確方向。

陪伴你的朋友 有了消息,不論是好是壞,總是第一個告訴他們。他們一直和你在一起。

  這種朋友的心胸像大海、高山一樣寬廣,不管何時找他們,他們都會熱情相待,並且始終如一地支持你。他

IcannotbelievethatIamafamilymedicinetraineenow.Meetingthenewinternsduringmy firstday in thehospitalbasedtrainingletmerecognisethat Ihavegonethroughaharshyearofinternshipandhaveturnedanewpageofmycareer.

I am lucky that I canwork ina specialty Ilike.WhenIwent intothemedicalschool, Ireallywantedtobeafamilyphysician.Ihavebeen livingandstudying inYuenLong,aruralyetsimpleplace,formanyyears.WhenIwasakid,IwasworriedifIwoulddieoffluasIwasdebilitatedbyfever.Tome,doctorsarethemosttrustworthypeople.Ifeltbetterafterseeingadoctorevenwithouttakingmedications.Theircare,abilitytolistentomyproblem,comfortablevoiceandfirmreassurancemakemefeelbetter.‘Doctor’perseisamagicdrugaswell.

AfterIenteredthemedicalschool, IrealizedthattherearesomanydifferentspecialtiesotherthanMedicineandSurgery.Radiology,Anesthesiology,Pathology,Microbiology,A&E,Oncology,Familymedicine……IthoughtaboutwhichcareerpathIshouldchoosewhenIgraduate.

Communication, communication and communication

Iliketointeractwithpatients.Althoughmanyofthetimetheseinteractionsmaybringfrustration,disappointment,angerandothernegativefeelings,Ithinkgoodcommunicationcansolvemostproblems.Itisanarttomastersuchskill.Italsodrivesmetolearnmoreinmycareer.Ibelievepracticemakesperfect.SoIchooseFamilyMedicinethatIcaninteractwithpatientsmost.Iwillhavemanyconversationswithmypatientbythenatureofmypractice.Howtogainastranger’strustinsuchashortcontacttime?Theauthorizeddoctorcoatornicesuitisnotenough.IthinkIcanlearnmoreasafamilyphysician.

Long term relationships

Althoughallalongourtrainingwasfocusedonhospitalspecialties, I realizedthat,tomostofthepatients,hospitalmedicinecomposesonlyasmallpartofmedicalcare in their life.Forthosewithrelativelygoodpasthealth, theymaygotohospitalonlyat theveryendof theirlife.Nowadays,manyofthepatientswithchronicillnesseslikeDMandHTwill followupintheGOPC.Goodandcompetentfamilyphysiciansreallydoaffectthehealthstatusofgeneralpublicverymuch.Withoutus,themedicalsystemcannotbesustainable.Thepeopleandthesystemitselfwantgoodfamilydoctors.Bybuildingupthelongtermrelationshipwithourpatients,wegaintheirtrustandrapport.Wecangettoknowthepatientmore,understandtheir“RICE”,offertimelytreatment,educateonhealth issuesandmodifytheir lifestylegradually. Icherishsuchdoctor-patientrelationship.

“Minor” complaints

Duringmyinternship, Icanstillnoticetheverytrivialcomplaintsfromthein-hospitalpatients.It is trivial in thesensethatunlikely tokill,but theymaycausegreatdiscomfortorstress topatients. Intheacutehospitalsettings,weseldomseriouslyaddressedthesecomplaints.Wemay treat thesymptomsby reflex likepiriton to insomnia, triact todyspepsia, lactulose toconstipation,panadoltoheadache.Wedidnotstoptolistenforawhileanddigintothehiddenagenda.Iwon’tconsidermyselfas“smallpotato”todealwitheveryminorcomplaintbutinsteadtriedtolistenandthinkcarefullytodelineatetheunderlyingcausesandsolvetheirproblems.Hippocratesshouldbeagoodfamilydoctorashesaid“Curesometimes,treatoften,comfortalways”.

Holistic care

Iexperiencedthefragmentedcareduringthehospitalrotation.Ican’tevenrememberhowmanyconsultationreferralformsIhavetofill induringin-patientcare.It is idealtotreatapersonbutnotjustthedisease.IrememberonceintheundergraduateFMtutorial,mytutorpointedoutoneimportantthing:trytoknow“who”hasthedisease.Treatingpatientsfromdifferentwalksoflife,educationalbackground,co-morbidity,andsocialsupportscanbetotallydistinct.Besidesthat,theymayhavemultiplecomplaintsandtheirproblemscanbecomplicatedbybiopsychosocialfactors.FamilyPhysiciansarethetypeofdoctorwhoiswillingandcompetenttolookafteralltheseareas.

One of my seniors in the department whosharedwithus in the internsbriefingsessiononhisroughroadtobecomingaFMspecialistwhenhepassedhisexitexam lastyear. Iwasdeeplyimpressed.Nowadays,thereisalreadyawell-establishedstructuredtrainingprogramforus. If I really like it,whynot justgoahead?Therosyprospectofourselvesisnotdependentonthespecialtyitselfbuthowwecreateourpath.

Hot Drug Issue - Thiazolidinedione

Diabetesmellitus(DM)iscommoninHongKong,yetacomplicatedanddifficult-to-treatchronicdisease.Apartfromthetraditionaloralhypoglycaemicagentssuchassulfonylureaandmetformin,thiazolidinedionewasonceahopeforthosewithpoorlycontrolledDM.Manypatientswhoarereluctanttostartinsulininjection,orareintoleranttothesideeffectsofmetforminorsulphonylureaswouldopttotryrosiglitazoneorpioglitazoneasanalternative.

However,thereisnowgrowingconcernaboutthesafetyofrosiglitazoneespeciallyregardingthecardiovascular(CV)aspect,althoughthecurrentevidenceisstillinconclusive.Ameta-analysisbyNissenandWolskiin2007suggestedthatrosiglitazoneincreasedtheriskofmyocardialinfarction(MI)andCVdeath,butthevalidityofsuchevidencewasbeingquestioned.1Therewerealsostudies(namelyADOPT,DREAM,RECORD)comparingrosiglitazonetootheranti-diabeticagentsorplacebo;theylookedatacompositeofmajoradversecardiovascularevents(2.6%vs.2.4%),myocardial infarction(1.4%vs.1.2%)andstroke(1.8%vs.1.9%)betweenpatientstakingrosiglitazoneandpatientstakingacomparator,respectively.Forthesetrials,astatisticallysignificantdifferencewasnotdemonstrated.2-4

Newerdatain2010showedmoreevidencesuggestingthatrosiglitazoneposedsignificantCVriskstopatients.5Twometa-analysesincluding10newrandomizedtrialsshowthattheriskofacuteMIis30to80%higheramongpatientswithdiabetestakingrosiglitazonethanamongthosereceivingplacebo.6,7Whencomparingrosiglitazonewithotheranti-diabeticagents,suchasthesulfonylureasormetformin,thehazardratioislower,althoughitisstillgreaterthan1.IncomparisonwiththeFDA’s2007meta-analysis,theneweranalysesshowedhigheroddsratiosformajoradverseCVevents,myocardialinfarction,andseriousischemiceventsinpatientsreceivingrosiglitazone.

Severalnewdatasetsfrombothclinicaltrialsandobservationalstudiesshowthatpioglitazoneeitherconferrednoincreasedcardiovascularriskor, insomecircumstances,protectedagainstmyocardialinfarction.5Inonecase,FDAinvestigatorsperformedaretrospectivecohortstudyof225,000MedicarePartDbeneficiarieswhoreceivedeitherrosiglitazoneorpioglitazonefrom2006to2009;theyfoundthattheriskofacardiovasculareventordeathwasnearly25%higherifthepatientwastakingrosiglitazoneratherthanpioglitazone.8

Theassessmentofthecardiovascularsafetyofrosiglitazoneisstillongoing.FDAiscurrentlylookingattheprimarydata fromoneof theaforementionedstudies, theRECORDstudy,alongwithotherobservationalstudies.TheRECORDstudyassessedtheoverallCVsafetyofrosiglitazone,whenaddedtometforminorsulfonylurea,comparedtometforminplussulfonylurea.TheFDA’sreviewisexpectedtobecompletedsoonandthereshouldbesomenewinformationcomingoutinduecourse.Asofnow,FDAhasprovidedsomewarningsandprecautionsasfollows:

i) UseofrosiglitazoneinpatientswithestablishedNYHAClassIIIandIVheartfailureiscontraindicated.Thedrugisalsonotrecommendedinpatientswithsymptomaticheartfailure.

ii) Rosiglitazonecausesorexacerbatescongestiveheartfailureinsomepatients.Signsandsymptomsofheartfailure (i.e.excessive, rapidweightgain,dyspnoea,and/oroedema)shouldbemonitoredafter treatmentinitiationandafterdosage increase. Ifheart failuresignsandsymptomsoccur, theconditionshouldbemanagedappropriatelyanddiscontinuationordosereductionofrosiglitazonemustbeconsidered.

iii)Co-administrationofrosiglitazoneandinsulinisnotrecommended.iv)Co-administrationofrosiglitazoneandnitratesisnotrecommended.7

Interest Group in Dermatology – the 20th Meeting on 3rd July 2010

The 20th Dermatology Interest Group meeting washeldon3rdJuly2010.OurspeakerwasDr.ChanShuYu,adermatologist inprivatepracticeandoursponsorwasGaldermaHongKongLtd.The topicwas“SkinTumours”.

Inthemanagementofskintumours,wefamilydoctorshavean importantrole in identifyingthetumoursandtheirprecursorsintheirearlystagesandmakingsuitablereferralforearlytreatment.Wealsohavearoleinpatienteducationoncarcinogenavoidance.

Numerousagentsare identifiedascarcinogensofskincancers,bothendogenousandexogenous.Themostimportant one is UV radiation: chronic exposure isrelatedtosquamouscellcarcinoma (SCC)andactinickeratosis whereas intense intermittent exposure isrelatedtobasalcellcarcinoma (BCC)andmelanoma.TheamountofUVradiationreceivedofcoursedependsongeographical latitude, timingof theday, individualskinthicknessanddegreeofpigmentation,andphoto-protectivemeasuresetc.

There are a numberofpre-cancerous keratinocyticlesionsandthefollowingwerediscussedinthelecture:

• Ac tinic keratosis usually appears as ir regularbut sharply defined erythematous, scaly “roughspots”onsun-exposedskinofelderly.Theriskofdevelopmenttoskincancerrangesfrom1%to20%andthecourse isunpredictable.Modernconceptfavours early treatment, either by cryotherapy,electrocauteryortopical5-flurouracil.

• Bowenoidpapulosis iscausedbyHPVinfectionandappearsaspigmentedwartyplaquesatgenitalia.TheyareessentiallySCC-in-situ.

• Epidermodysplasia verruciformis isanautosomalrecessivedisorderwith increasedsusceptibility totype5&8HPVinfectioncausingwidespreadplanewart-likeorpityriasisversicolor-likemaculopapulesatextremities,withahighchanceofdevelopingintoSCC.

• Bowen’sdiseaseappearsassolitary,sharply-defined,pinkorredmaculopapulesatsun-exposedpartsofbodyandareSCC-in-situwith3%to5%chanceofdevelopingintoSCC.

There are three common types of skin cancers inhumans, SCC ar ises f rom suprabasal epidermalkeratinocyteswhileBCCandmelanomaarederivedfrombasallayers.

SCCareusuallyprecededbypre-cancerouslesions.Oneshouldalwaysremainvigilantforchangesinsymptomslikepain, itchiness,bleedingandsigns liketenderness,erosion,induration,increasedsize,increasedscales,‘uglylooking’etc.Thefollowingaresomeoftheriskfactorsofmetastasisand recurrence inSCC:diameter>2cm,depth>4mm,involvingbone/muscle/nerve, locationonlip/ear,arisingfromoldscars,and immunosuppressedpatients.

BCCisoneof thecommoncancers inhumansand itsincidence is increasing inyoungpopulations. Inmostcasesthetumourhasarolledborder.Itislocallyinvasiveanddistantmetastasis isuncommon.MorpheaformBCCisusuallymoreaggressivethantheothersubtypesbecauseittendstobemoreinfiltrative.Thebestchanceofcureisbyadequateexcisionoftheprimarytumourasrecurrentdiseaseismoredifficulttotreat.

The incidenceofcutaneousmelanomahas increasedsignificantlyworldwideduring the last fewdecadesand itaccounts for80%ofallskincancerdeaths.Sunexposureisagainthemost importantriskfactor.Thereisalso increased risk for thosewith largequantitiesof melanocytic naevi (>100), those with atypical ordysplasticnaevi,andthosewith largecongenitalnaevi(>20cm). However, overall only 20% of melanomasare derived from pre-existing melanocytic naevi.In western societies, the commonest melanomais superficial spreading type in sun exposed areaswhi le the commonest melanoma in or ientals i sacrolentiginous type. A high index of suspicion isimportant forearlydiagnosis -observe theABCDEchecklist,namelyAsymmetry,Border,Colour,Diameter/Difference,Elevation/Evolving.Breslow thickness isthemost importantprognosticfactor inearlystagesofmelanomas.

It isunnecessarytoadvisepatientstohavetotalsolaravoidance.Behaviouralmodifications likeavoidanceofpeakUVBperiodsandphoto-protectivemeasures(wide-brimhat,sunscreen,sunglasses,long-sleevedandlight-colouredclothings)aremorepractical.
