On behalf of the Board of Trustees, I am pleased to present the Trust's Annual Report and activities for the year 2015.

This has been a year of coming together, of cohesion for Fatimid Foundation. We have begun to change the structure of our operations to concentrate on expertise and capacity to prepare for growth.

Fatimid Foundation have begun to develop a multi disciplinary research consortium and enhance donor and patient care. We collaborated with reputable hospitals and overseas bodies to bring about individual strength and expertise to fully round out our strategic abilities.

We have throughout our history taken bold steps to fulfill our mission to make a difference for our underprivileged patients and blood donors who depend on us. This would not have been possible without the generous and continuous support of its donors which includes individuals, corporate sector, local and foreign Governments.

By the grace of Allah, donation, zakat, and grant-in-aid have excelled this year as compared to last year by 19% to Rs. 301.0 million. We plan to construct new Centres at Peshawar, Karachi and Hyderabad in the coming years.

In addition, substantial overseas donations from American and European citizens of Pakistani origin alongwith local, corporate and individual donors have seen value addition for the creation of new projects, thus placing trust and confidence with us in order to serve the underprivileged sick children patients in our society.

I would like to thank the Federal and Provincial Government of Pakistan, the Government of Japan, our Trustees, Mr. S. M. Muneer, Sardar Yasin Malik, Mr. Arif Habib, Mrs. Nasreen K. Kasuri, Dr. Gaffar Billoo, Dr. Mohammad Khurshid and our eminent Board of Trustees for their continuous and generous philanthropic donation to Fatimid Foundation this year to enable us to provide the necessary support and services to our less deprived sick children patients of genetic blood disorders.

Finally, my special thanks go to our dedicated employees for their commitment and efforts without which none of this would have been possible. In all humbleness we bow down to God Almighty and look forward to your continuous support in the coming years in order that we are able to serve and promote our noble cause for these underprivileged children patients of genetic blood disorders in our society.

During the year in review Fatimid Foundation have achieved quite a few milestones in partnering ourselves globally and within the country on our resource mobilization strategy, health, safety, environment, systems, controls and facilities i.e.

a. Internal audit for renewal of ISO 9001:2008 certification and preparation for 9001:2015.

b. Successful installation of a first class modern PCR lab at the Head Office with special reference in undertaking CVS (Chronic Villous Sampling) and DNA testing for Thalassaemia major foetus in order to avoid and reduce the number of Thalassaemia major patients in Pakistan. Hopefully we would also undertake PCR for Hep. 'B' and Hep. 'C' reactive patients in our PCR lab.

c. Installed and implemented the grant agreement ceremony against GGP (Grant Assistance for Grassroots Human Security Project) by the Government of Japan for the sum of US $ 83,755/- with a Cell Separator (to provide mega units), Automated Coagulation Analyzer for the diagnosis of blood coagulation defects in patients, Ultra Low Temperature Freezer -86 C° and a Mobile Blood Donation Vehicle.

d. Increased collection of donation, Sponsor-a-Child, Grant-in-Aid , Zakat and other services and income during the year by over Rs. 48.2 million or 19.1 % on all Pakistan basis. Fatimid Foundation increased its patient's strength from 12,000 to 14,000+ during the year under review.

e. Repairs and renovation of the Head Office Karachi for improvement of health, safety and environment for its clinical and operational needs and comfort for our underprivileged sick children patients e.g. brand new thalassaemia chairs and blood collection camp cots.

f. Audit in process with A.F.Ferguson and Co. Chartered Accountants (PwC) for the year ended June 30th, 2013 and year ended June 30th, 2014.

g. Continuous training and development of our staff's education both in clinical and administrative management.

h. Approved annual recurring grant-in-aid by The Punjab Government of Rs. 25.00 million to be shared equally by Lahore and Multan Centres.

i. Installation of Cobas E411 screening ( a ECLIA system) for Hyderabad and Quetta Centre and a

brand new donated Cryofuge equipment of Thermo Fisher HERAEUS 5500i brand for Quetta Centre.

j. Though our total blood collection vis-à-vis last year decreased from 56976 pints to 53034 pints - a decrease of 7% over the previous year, its manufacturing treatment of its blood and blood products in real terms have increased from 93964 to 100226 pints in volume or by 6% during the year under review. In spite of security issues, numbers of blood banks, Thalassaemia Centres and fly by night sub standard blood banks in the past few years, Fatimid Foundation have still been able to step up its manufacturing treatment and maintain its blood collection through road side and corporate blood camps. Fatimid Foundation also participated in the Shaheed Benazir Bhutto birthday blood transfusion campaign and transfused 500 pints of blood to the underprivileged sick children patients in Karachi.

K. With these above achievements, Fatimid Foundation have been able to build on its capacity of blood banking and haematological services effectively and efficiently to carry out its tasks in hand.

l. In October 2016 , Dr Shabneez Hussain , Consultant Haematologist, went to Montreal , Canada to

attend the “9th Global Forum on Research and Treatment Products for Bleeding Disorders”, a

conference organized by the World Federation of Haemophilia (WFH) . Another purpose was to

create a liaison between Fatimid Foundation and WFH. She also visited the haemophilia treatment

center at McMaster University and met Karen Strike (physiotherapist), Dr Alfonso Lorio

(Haematologist) and Dr Anthony Chan (Pediatric Haematologist) who are willing to have a good

collaboration with Fatimid foundation.

Future Outlook:In order to bring about an increased level of efficiency, facilities, services, FF have approached local corporate donors as part of their CSR policy to seek help and support for the improvement of our facilities and services in order to provide free of cost medical and clinical treatment to our underprivileged sick children patients of various genetic blood disorders.

Further it intends to collaborate with overseas donors in the coming year, enhance mobilization of resources, sponsorships and enable us to also create an effective endowment fund for balancing, modernization and replacement of equipments, and providing relief and support in the event of natural disasters in the coming years.

To approach the KPK Government for constructing a state-of-the-art Centre through its own resources on a lease of the BHU premises. This will go a long way to benefit our underprivileged sick children patients of Thalassaemia and Haemophilia in the province of Khyber Pakhtun Khwa (KPK).

On completion of the construction of our Centre, to follow up with the KPK Government for an Annual Recurring Grant-in-Aid to support its Centre since facilities are not being provided by Government hospitals towards these clinical and health care services.

To collaborate with various hospitals and clinics to offer mega units to the general public if and when required and CVS sampling/DNA testing to expectant mothers to detect thalassaemia major foetus in minor couples.

Fatimid Foundation was established as a Non-Profit Organisation by late Mr. Nazim Jiwa in 1978 in

Karachi. The major aim of establishing this organisation was to provide free of charge blood and

blood components to patients suffering from various chronic blood disorders requiring blood/blood

component therapy as an essential part of their treatment. With the passage of time, the

Organisation has grown in stature to" National" level with the opening of treatment Centres in

Lahore, Peshawar, Multan, Quetta, Hyderabad, Tando Allahyar and Khairpur in addition to Karachi

Centre which also serve as its Head Office. Fatimid Foundation is the largest Blood Bank &

Haematological Services provider in the NGO sector, which is providing free of charge treatment to

poor & needy patients. It was the first blood bank in Pakistan to introduce the concept of voluntary

blood donors, and discourage professional donors. It has been collecting and distributing approx.

100,000 pints of blood and blood components annually from healthy voluntary donors.

Fatimid Foundation- Pakistan was established under a trust deed registered in Karachi. The trust deed vests powers to a "Board of Trustees" (BOT) to run the affairs of the Foundation. The Foundation provides specialized services for patients with blood disorders nationally and transfuses around 8,200 bags of healthy, screened blood and blood products every month to its registered patients, who number more than 14,000.

Fatimid Foundation Centres are equipped with the latest blood screening state-of-the-art equipments on CMIA/MEIA system, being operated with high quality blood screening kits of Abbott laboratories USA. Fatimid is also the pioneer of voluntary blood transfusion service in Pakistan and in terms of quantitative output of blood and blood products it is the leader amongst blood transfusion services in the country. It has recently been accredited an ISO-9001:2008 certification for Quality Management System and has certified as a CSO with PCP and approved charity with The i-Care Foundation

What is Thalassaemia?Thalassaemia is an inherited genetic blood disorder in which there is an abnormality in one or more of the globin genes. Thalassaemia is caused by variant or missing genes that affect how the body makes haemoglobin. Haemoglobin is the protein in red blood cells that carries oxygen. People with thalassaemia make abnormal haemoglobin and that results in breakdown of red blood cells immediately after formation, which results in mild or severe anaemia.Children with thalassaemia major are normal at birth but become anaemic between the ages of three months to eighteen months. They become pale, do not sleep well, do not want to eat, and may vomit their feeds. If children with thalassaemia major are not treated, they have miserable lives and usually die between one to eight years of age.

How widespread is Thalassaemia in Pakistan? 5-7% of our population (approx 9 ~ 13 million) carries genes of thalassaemia minor. They do not need blood transfusions themselves but marriage of two such persons may give birth to children with thalassaemia major who would need regular blood transfusion for the rest of their life. Due to the carrier rate in our society, more than 6,000 children with thalassaemia major are born every year in Pakistan. More than 110,000 thalassaemia major children may be found in our country, getting half hearted treatment with infrequent blood transfusions and inadequate iron chelating therapy.

Can it be treated?The only known treatment for thalassaemia major is regular blood transfusions, usually every two or three weeks. Most children, who have these transfusions, grow normally and live quite happily into their early twenties. But to live longer, they need other treatments as well. After each blood transfusion the red cells in the new blood are broken down slowly over the next four months. The iron from the red blood cells stays in the body. If it is not removed, it builds up and damages the liver, the heart and other parts of the body. If this damage is not prevented most people with thalassaemia major die in their teens. The only known cure for thalassaemia major is bone marrow transplant. It is very expensive (approx. Rs. 2.0 million) and results have not been encouraging in Pakistan.

Can it be prevented? Yes, it can be prevented by

• Detection of carrier

• Pre-marital screening

• Avoid marriages between two carriers• Discourage consangeneous marriages

• Pre-natal diagnosis

The treatment of Thalassaemia without an attempt to control numbers of patients born with thalessaemia major, will result in unavoidable drain on family and national economy, that is why we have to focus on preventive management. Treatment of Thalassamia includes repeated Blood Transfusion and Chelation therapy to reduce iron over load. Permanent cure of Thalassaemia is Bone-Marrow Transplantation, expensive, with not so encouraging results in our setup, further more only 12% of Beta-Thalassaemia Patients have access to Blood transfusion Centers and remaining balance, remain undiagnosed and die at an early stage, hence to tackle this menace effectively the only measure is prevention.

Do you care?

It is an irony of fate that the vast majority of thalassaemic belong to low income groups. Often the cost of

treating one child is 2 or even 3 times the monthly earning of the child's parents. should then such children

be left to die a painful death?

All religions of the world teach us that compassion and charity are amongst the greatest of virtues. We

beseech you to spare a thought for these children. We beseech you to express your gratitude for all

kindness the Creator has bestowed upon you by helping us ease the pain and suffering of thalassaemic

children. Surely you will be rewarded many times over.

What Can be done?

There is really only one long-term solution and that is to stop birth of Thalassaemia Major children. This is

possible. Countries like Italy, Greece & Cyprus where carrier rates were very high and 18% of population

carried different genes of haemoglobinopathies, have completely stopped birth of new Thalassaemic

children. In Iran, compulsory pre-marriage blood tests and genetic counseling have resulted in very high

success rate.

In our country, illiteracy and poverty makes this problem harder to tackle. Lack of understanding of

genetic disorders due to inter-marriages and resistance to pre-marital screenings also makes it difficult to

control such deadly disorders. There is a strong need to educate our people both in rural and urban areas

about these disorders and ways to stop their occurrence.

Till we achieve this level of awareness and response in Pakistan, we need to continue treatment of

thalassaemic children. For optimum treatment every thalassaemic child needs frequent transfusions of

screened packed red cells and regular iron chelation therapy. This costs more than USD 100 per child per

month.

What is Thalassaemia Major?

The child who has it, cannot make enough haemoglobin in his or her blood, So he or she would need

transfusion.

We suffer from Thalassaemia because by chance both our parents were carrying the genes of

Thalassaemia Minor.

Blood is made of 3 types of cells.

· Red Blood Cells

· White Blood Cells

· Platelets

The Red Blood Cells are little bags full of little bits called Haemoglobin which is red. That's what makes our

blood red. Each bit of Haemoglobin has iron granules attached to it.

Red Blood Cells are made in the bone marrow, which is a factory inside the bone. It is responsible for

the production of the blood cells. They go through the lungs and collect oxygen which sticks to the

iron granules and they carry it to our tissues. Oxygen gives us energy.

What is Anaemia?

Anaemia means there are not enough red blood cells and less haemoglobin. You are ill because your bone marrow can't make haemoglobin and red cells properly. In fact, these red blood cells are smaller with less haemoglobin and don't collect much oxygen.

The best ammunition you have against anaemia due to thalassaemia is blood transfusion.

Blood transfusion of Red Blood Cells will replace the empty Red Blood Cells with new healthy ones andyour anaemia will go away and you will feel better.

Now be careful! The red blood cells are broken down slowly over the weeks but the granules of ironwhich were attached to bits of haemoglobin remain in the body.

What shall we do?

Fortunately, there is a drug used which is called DESFERRIOXAMINE, Desferrioxamine is like a ringmagnet, it picks up all the excess iron and carries it out of the body in the urine and the faeces.

The best way to inject it is from a small portable pump five to seven (5-7) nights a week.

Hi! I am the pump. I am here to help you. If you operate me correctly, you can receive Desferrioxamine perfectly and we can be life-time friends.

The pump is a little machine which helps push the syringe a little at a time and makes the injection last as long as 8-12 hours.

Overnight is the best time to use it and the most convenient, therefore, I sleep with my friend the pump every night and we are “ Friends for life”.

The needle is small and it is put under the skin, it hurts a little bit. You can put it in at night-time and take it out in the morning. That way the medicine is taken a drop at a time and moves around the body to catch the iron granules.

Anaemia

What is Haemophilia?

Haemophilia is an inherited genetic bleeding disorder. The defect lies in the clotting mechanism of the

blood and is characterised by a life long tendency to excessive internal or external bleeding. This bleeding disorder can be classified into: Haemophilia 'A' due to the deficiency of factor VIII, Haemophilia 'B' also known as Christmas disease due to the deficiency of factor IX, the third kind 'C' due to the deficiency of factor XI. Another common bleeding disorder is Von Willebrand's disease, which is due to deficiency of Von Willebrand factor.

Symptoms:Patients with haemophilia very often have internal or external bleeding, both spontaneously or on trauma i.e. gums bleeding, bleeding from nose, bleeding in large joints of the body such as elbows, knees, ankles and shoulder joints, bleeding in urine, bleeding in stool or they may even get bleeding in the brain. These internal bleedings can vary in frequency from thrice a week to three or more times a year.

Consequences:If haemophilic patients remain untreated, the result would be prolonged excessive bleeding which may prove fatal or cause severe anaemia. Bleeding in joints causes severe deformities of joints, flexion/extension deformities, weakness of legs, shortening of leg and so on, which makes a patient crippled. Bleeding in the brain and abdomen are very dangerous and may even cause death.

Cost of treatment: Cost of treatment varies with degree of deficiency from mild to severe. In Pakistan the cost of one injection of Factor VIII in the open market is approximately USD 80/- If a severely affected patient needs 12-16 injections per month, the cost of treatment would be approximately USD 12,240 ~USD 16,320 per

patient per year. All Centres of Fatimid Foundation are equipped to make various blood components itself, which includes factors VIII, IX and other factors as well and therefore are able to shoulder part of the above cost through its fund raising efforts. Fatimid Foundation treats around 3200 registered haemophiliac patients free of charge but substantial government support is needed to subsidise this cost of treatment.

Haemophilia

Physiotherapy is important for the bleeding disorder patients. Patients with bleeding disorder like Haemophilia bleed in their joints and muscles and experience severe pain in the affected area and movements are restricted. Factor replacement therapy controls the bleeding and accompanying pain. After the pain is relieved, physiotherapy plays a very important role and helps to restore muscle power and function, restores motion, blood circulation, improvement of muscle mass and power and helps to prevent deformities of the joints and improves pain.

Fatimid Foundation's physiotherapy department was started alongwith its haemophilia department in 1984. Today Fatimid Foundation's physiotherapy department has all the instruments required for modern physiotherapy treatment . Under the supervision of senior well experienced Physiotherapists, this department is catering to the needs of more than 2000 Haemophilia A & B patients. The complications and outcome of repeated joint and muscle bleed is much controlled and patients are now experiencing great benefits of a well equipped department.
