This document identifies steps in organizing andconducting a community survey and provides materials for assembling awell-focused survey form. Planning and conducting a successful surveyinvolves the organization of a planning group, selection of relevanttopics and information to be collected on each topic, preparation ofa survey form, identification of local organizations willing toconduct the survey, tabulation of survey responses, reportpreparation, and communication of survey results to the community.Appendix A provides a case study example of a community needs andpriorities assessment developed and carried out by citizens in afictional community in Nebraska. It includes a brief synopsis of thesituation in this community and describes community leaders whodecided to take action when the future of the community wasthreltened. Surveying CommunityNeeds and Priorities:The Nuts and Bolts
In any given year many community surveysare carried out. Most generate informationon locally important topics, problems, and
issues. When well-designed and appropriatelycarried out, a community survey provides authen-tic and timely information that's not availablefrom any other source.
Authentic and timely information can be animportant stimulus for local problem solving. Itoften provides a mandate for decision makerswho must make difficult choices. Almost always,residents' perceptions of community situationsand concerns as reported through a carefully fo-cused and well-conducted community survey, areimportant data and help decision makers plancommunity improvements and/or resolve com-munity issues.
Valid community surveys require relativelylarge investments of residents' time andresources. The nature of the survey form, theapproach used in conducting the survey, and thedata tabulation and reporting methods areprincipal determinants of the validity of surveyfindings. It's very important that the surveyform be carefully focused and that good proce-dures be followed by those who conduct thesurvey.
This document will help you assess commu-nity needs and priorities. It identifies the steps tobe followed in organizing and conducting a com-munity survey, and provides materials you canuse in assemblit g a well-focused survey form. Inshort, this kit gi yes you the tools needed to con-duct an effective community needs and prioritiesassessment.

Community surveys go by many names.Depending on the primary purpose of the sur-vey, it may be called a "Needs Assessment," a"Housing Study," a "Development Capacity As-sessment," an "Environmental Scan," or bysome other name that carries local meaning. Re-gardless of the survey's name or the sponsors'goals, a successful community survey requirescareful organization and focused effort. It's im-portant that the survey be designed to generateresponses containing the needed information andthat the responses are compiled, analyzed, andeffectively communicated. Planning and con-ducting a successful survey generally requirescompletion of these steps:
I. A planning group that's interested in con-ducting the community survey is organized.
2. In consultation with interested decisionmakers, the planning group selects topics to beincluded and the information to be collected oneach topic.
3. A survey form is prepared by using thistool kit to develop a new survey form or byadapting a survey form that previously was usedin the same or another community.

4. Local organizations or groups agree totake responsibility for conducting the survey. Or-ganizations or groups that have conducted com-munity surveys include: a local school orcommunity college; a local government unit,agency or office; a local bank or other local busi-ness; a service club or other community organi-zation; a group of local volunteers; or anoutside-the-community agency or firm. Tasks tobe completed are:
Distributing survey forms to the intendedrespondents,
Reminding recipients to return completedsurveys,
Collecting or receiving the completedsurvey forms,
Other survey responsibilities requested bythe planning group.
5. The planning group tabulates the surveyresponses and prepares a report. Sometimes thereport contains only tables listing the number ofresponses in each category. However, a morecomplete report with response tables and narra-tive interpretations of the survey responses usu-ally is more useful in conveying the findings tothe community. Response frequency graphs alsocan be used with the narrative descriptions ofsurvey responses.
6. The survey report and insights drawn fromthe response pattern(s) are reported to the com-munity. This can be done by publishing the re-port in the local paper, by distributing a writtenreport, or by presenting a verbal report to per-sons attending a public meeting. The effective-ness of the verbal report usually is greater if awritten summary is made available to those whoattend the community meeting.

A team of people sharing in the effort canhelp to move the survey activities ahead. It isbest if persons in a number of community rolesare active in planning and conducting a commu-

nity survey. Time and resource commitments canbe kept at a reasonable level if the survey has aclear and limited focus.
It is only human to want to learn everythingthrough a community survey. Typically, eachtopic and question that is suggested to beincluded in the survey is focused on an authenticinterest of one or more persons. However, a sur-vey that addresses many topics and containsmany statements is likely to end up beingineffective because:
the survey form becomes so long that manyrespondents do not complete it.
the accuracy of the responses drops offrapidly because the time required to com-plete the survey is more than 10 to 12minutes.
data tabulations become so difficult andtime consuming that they are not com-pleted for weeks or months and by thattime local interest has faded away.
the volume of data tabulations becomes solarge that interested persons are over-whelmed and cannot decide what to dowith the data.
Following the suggested procedures andusing the forms provided in this document willhelp you avoid these problems by:
helping you to focus your efforts on a care-fully targeted and relatively short survey.
providing survey form originals and topicand statement listings from which you canselect items appropriate to your surveyform.
including both needs and prioritiesassessments in one survey form. Thishelps you identify urgent and importantneeds, and also helps to separate priorityitems from "pie-in-the-sky- dreams andinterests.
allowing you to "customize- your surveyso it fits local interests and informationneeds.
This document provides you with an oppor-tunity to customize your survey by providing a"laundry list" of topics and related statementsfrom which you can select the topics and state-ments you want to use. Each statement is care-fully worded for brevity, clarity and focus. Afterselecting topics and statements that address yourinterests and needs, you can add statements spe-cific to your local situation and communityinterests.
For example, suppose your planning groupdecides that decision makers would benefit fromknowing residents' needs and priorities for themanagement of community parks. One topic foryour survey might be "Public Parks and GreenAreas," a topic that appears in the topics andstatements listings. When you read the state-ments listed under the "Public Parks and GreenAreas" topic you know the swimming pool andbike path statements are not needed for your sur-vey. You do not include them in the list of state-ments in your survey form.
At the same time, the planning group wantsto gather opinions about the need to renovate the"Prairie Pioneer" statue.lhat has been a Parklandmark for many years. The planning groupwrites one or more statements that a respondentcan use to indicate his/her assessment of theneed and level of priority given to renovating thestatue. The statements are included in the state-ment list of your survey form.
Requesting Demographic andFinancial Data
A ready-to-use page of demographic andfinancial questions is included with the topicsand statements listing. Use these questions inyour community survey only if you have a clearlyidentified need for the responses. These ques-tions ask for confidential information infor-mation that must be kept confidential to ensurethat the respondents' privacy is protected. Besure to ask for demographic and financial infor-

mation onlywhen there is adefinite needand suitabledata are notavailable fromother sources.
If circum-stancesrequire thatyou collectdemographicand finan-cial datafrom respondentsfor example, the community must have thesedata for a Community Development Block Grantor a housing grant application be certain thatthe privacy of respondents is preserved whentheir responses are collected, tabulated, and re-ported. In doing so you will want to use proce-dures that protect the privacy of individuals,families, and a9y identifiable subgroups of re-spondents. Heft are some ways of ensuring thatthe rights of respondents are protected and theirprivacy is maintained:

I. Design the survey form so it does not askfor information that will indicate or give cluesabout any respondent's identity.
2. Ensure that the survey forms are free ofidentifying characteristics that can be linked toany particular respondent, family unit, or sub-group of the potential respondents. (Avoid plac-ing numbers or colored markings that giveindividual identity to each survey form or groupof forms. Copy all forms on the same color andtype of paper. Keep the order of topics, state-ments, and questions the same on all surveyforms, etc.)
3. Be certain that all persons who receive thesurvey form know their participation in the sur-vey is completely voluntary. (This means thatthey have the right not to respond to the surveyand they have the right not to return a completedform to the survey sponsors.)
4. When tabulating and reporting the surveyresponses, do so in ways that prevent identifica-tion of the responses of any individual, family, orother community subgroup.
Maintaining the privacy of respondentsrequires that each person involved in conductingthe survey, and those who tabulate and report theresponses, treat each individual's surveyresponses as completely confidential information.
Before printing and distributing your surveyform in the community, print a few copies andpretest them with a half-dozen people. In yourpretest use a letter and cover page like theexamples in Appendix C, and let the respondentsuse only that information as their instructions.(Do not coach them on how to respond to the sur-vey.) By doing so you can pretest both theinstructions and the survey form.
Ask each pretest respondent to individuallycomplete the survey form. Then, discuss the formand the responses with each to determine whetherthe instructions and statements caused the respon-dents to focus on the needs, issues, or problemsfor which the sponsors wanted needs and priorityassessment information. If the pretest indicatesmany or most respondents m I understood borneof the statements, reword or replace the problemstatements. If needed, rewrite any portion of theinstructions that caused misunderstandings. Pre-test again with a different group of respondents.
When the pretest demonstrates that theinstructions and survey form will generate theneeded information, you are ready to print copiesof the form and cover letter and to move ahead inconducting the survey.
Getting Started
No two communities are alike. Local history,the values of residents, the nature of groups andorganizations in the community, and many otherfactors shape community characteristics. Ethnic

and cultural awtors often influence the willing-ness of residents to respond to a communityneeds assessment survey. Whatever the natureof local conditions, a sense of humor, tolerancefor other points of view, persistence, andpatience are needed.
Start by talking with local leaders. Tellthem what you want to do and ask their help inidentifying five to seven persons who will plan,guide, and sponsor the needs and prioritiesassessment efforts. Bring the planning grouptogether. Through discussion, generate answersto these important questions:
What topics, problems, and issuespresently are important to the community?
What do residents want to know and needto know about each topic or issue?
What approach is most likely to result ingood participation, accurate responses.and rapid return of completed surveyforms.
Which individuals, organizations, andgroups might be willing to help carry outthe survey?
How can the survey responses betabulated and reported in no more thantwo or three weeks following the dead-line for returning completed surveyforms'?
What marketing approach will help toensure that: 1) interested persons andgroups will be involved in conducting thesurvey: 2) time, effort, and funds areavailable to support a successful survey:and 3) survey responses are rapidlytabulated and reported to all interestedpersons?
Have someone keep a formal or informalrecord of the discussion of these and otherquestions. The record will help the group tocapture and use good ideas that emerge in thediscussion, but are not developed at that time.Possibly you will want to name a secretary whowill keep minutes of the meetings.

The survey form is the cornerstone of a suc-cessful needs and priorities assessment survey. Itdetermines the content, nature, and amount of in-formation generated by the survey. It is to youradvantage to include only priority topics in yoursurvey as a short survey will improve the returnrate, the accuracy, and the completeness qf theresponses.
Even if your community has a real need fordata on a relatively large number of topics, giveserious consideration to having two or moreshort surveys held a few months apart. A seriesof short surveys usually will result in better datathan you will get from a long survey. If you planto have two or more short surveys, be sure toinform residents of your intention to do so beforethe first survey starts. Otherwise, you may getunhappy feedback from persons who are disap-pointed that their favorite topics were not in-cluded in the first survey and more of thatfeedback from those who say, "We've alreadydone this!" when the second or later surveys aredistributed.
When designing your short survey(s),include only topics and issues for which youhave a clearly evident information need. Avoidincluding topics when the only reason for doingso is someone saying, "It would be nice toknow. ...." By keeping the survey as short andsimple as possible, your chances for success areincreased.
At the meeting when you design the surveyform use the listing of topics and statements inAppendix C of this document as a focal pointfor discussion. Quickly eliminate from consider-ation any topics that are not appropriate to thecommunity. From the remaining topics, plusany others that the group agrees are priorityconcerns, select the three or four that the plan-ning group believes are most important tocommunity well-being. These become the topicsaddressed in your needs and priorities assess-ment.

Organized effort is vitally important to carry-ing out a successful survey. After the topics havebeen selected, the survey form must be prepared,pretested, copied, and distributed to residents.Completed survey forms must be gathered andthe responses tabulated in a timely manner. Then,the findings are reported to the community asquickly as is feasible after the tabulations arecompleted. If possible, this should be accom-plished within two weeks following the deadlinefor return of the completed survey forms.
Have the planning committee decide on theapproach to be used in distributing and collectingthe survey forms. This usually is a choicebetween having volunteers deliver and pick upthe survey forms in a door-to-door effort vs. send-ing the forms out by mail or in the newspaper andasking that they be delivered to or mailed to thecollection point. In most cases, door-to-door de-livery and pick up will result in more completeresponses and a higher return rate. However, itwill require large amounts of volunteer time ofyouth and/or adults in the community.
Mail or newspaper delivery with a mail returnwill require less volunteer time but may result ina smaller number of surveys being returned, andthe survey responses may be less complete. Ap-pendix B contains a sample cover page and asample cover letter for use in each of the twomost frequently used approaches.
Decisions also are needed with respect to theapproaches to be used in tabulating surveyresponses, preparing the report, and presentingthe report to the community. When theseapproaches have been decided on, divide thetasks among planning committee members inkeeping with their interests and available time.
As previously emphasized, a report of the sur-vey responses should be presented to the commu-nity as soon as possible after the survey forms aredistributed preferably within two weeks of thedeadline for return of the completed surveyforms.

The Survey Materials:Step-by-Step Assembly

After you have selected the design foryour needs and priorities assessmentsurvey (the design is determined by the
topics, problems, and issues to be examined),you are ready to start assembling the surveyform and tally sheets. The materials you willneed are included in Appendix C where you willfind blank ORIGINALS for survey form pages,blank ORIGINALS for tally sheets, and severalpages of topics and statements listings.
As the first step, make at least a half-dozenPHOTOCOPIES of a SURVEY FORM PAGEORIGINAL and a half-dozen PHOTOCOPIESof a TALLY SHEET ORIGINAL. Also make atleast two PHOTOCOPIES of the topics andstatements listing for topics you will address inyour community survey.
Examine the photocopies of the topics andstatements listing and identify any duplicatestatements. (Some statements appear under morethan one topic heading.) If you find duplicatestatements, decide on the topic heading underwhich you want these statements to appear. Thenremove those statements where they appear un-der other topic headings. Also remove from bothcopies any statements that do not "fit" your com-munity.
Add any needed statements (include reword-ed statements that don't "fit" your communitysituation) by typing them on white paper using afour-inch line. You will need two copies of eachstatement. If you use a computer and word pro-

cessing program, a Times Roman 12 point fontwill be very similar in appearance to the state-ments in Appendix C. Remember to print onwhite paper at least two copies of each statementwritten on the computer.
Work with the two PHOTOCOPIES of thetopics and statements listing and the two copiesof your list of added statements as you assemblethe master copies of your survey form and tallysheets. Using a paper cutter or sharp scissors, cutout the topic headings and the statements to beincluded in your survey form. Take an identicalsequence of topic headings and statements fromeach photocopy of the listing and from your listof added statements you will need two copiesof each.
"Paste up" pages of the master copy of yoursurvey form using PHOTOCOPIES of the pageoriginals. For each survey form page, also "pasteup" a tally sheet master copy with identical topicheadings and statements using PHOTOCOPIESof the tally sheet original. (NOTE: Cut out andattach one topic heading or one statement orgroup of statements at a time to ensure you donot lose items or attach them in the wrongorder.) Generally, it works best to use transpar-ent tape when attaching the topic headings andstatements. Make very effort to assemble mastercopies that are very neat. The appearance of thesurvey form you distribute is determined by theneatness of the master copy. If disaster occurs inthe form of a mistake or a messy master copypage, make more photocopies and try again.
When your master copies have been assem-bled, make a photocopy of each page. Look at

the photocopies very carefully to identify errorsor defects. If you find a line is missing, use aruler and black pencil or pen to draw it in. Ifshadows show at the edges of the statementsmake sure that they are tightly taped down. Usetypewriter correction fluid to "white out" persis-tent shadow lines or type or printing marks thatshow on the photocopies. When you are satisfiedwith the quality of photocopies made from yourmaster copy of the survey form, you are ready torun the copies for the pretest. Make changes asneeded until the pretest indicates the survey formis ready to use. Then. you will be ready to makecopies for your community survey.
Check the tally sheet copies to ensure theyduplicate topics and statements in the after-pretest version of the survey form. Also checkfor defects and errors and remove any you findbefore running tally sheet copies as needed. Ifpossible, run the tally sheet copies on a copymachine that will enlarge them to fit on 11" x17" paper. Doing so will give you more space fortallying the responses to each statement.
The cover letter explains why the survey isbeing conducted and what is expected from therespondents. To prepare your cover letter, selectthe example letter from Appendix B thatcorresponds to the survey distribution and returnprocedures being used in your community.Retype the example letter adding the appropriatedates, times, and/or addresses.
Appendix A provides a hypothetical caseexample of a community needs and prioritiesassessment as carried out in the fictional commu-nity of Good Life, Nebraska. This hypotheticalexample demonstrates the process by which agroup of community leaders can react to chang-ing community circumstances and illustrates thedesign and use of a community needs and priori-
ties assessment survey. It demonstrates the useof this publication and its "tool kit" approach bysponsors of the fictional community's survey,and illustrates customizing the cover page andthe cover letter so they are suitable for use in thecommunity.
A well-planned and well-conducted commu-nity needs and priorities assessment will help tocreate awareness ot local problems and issues,will stimulate discussion among decision makersand other citizens, and will help to start identifi-cation of desired outcomes. It also will supportcommunity improvement by increasing the infor-mation base for future decisions.
Planning, organizing, conducting, tabulating,and reporting a community needs and prioritiesassessment can be a major undertaking. How-ever, it is a feasible undertaking when volunteersand community organizations contribute theirtime and skills to the survey effort. Keys to suc-cess include:
A well-organized approach
Broad-based community involvement
Speedy tabulation and reporting of results

Customizing A Needs Assessment
The Good Life, Nebraska Example
Appendix A provides a case study type example of a communityneeds and priorities assessment developed and carried out by per-sons active in the life of the mythical community of Good Life,Nebraska. It includes a brief synopsis of the situation in Good Lifeand describes the group of community leaders who decided to takeaction when the future of the community was threatened. Anexample cover letter and cover page were developed from the mate-rials in Appendix B and C to demonstrate how the materials can becustomized for any community.
In a manner similar to that used in the example, you can use thispacket to prepare a needs and priorities assessment for your com-munity. It can focus on whatever topics are of particular importanceto your community. Narrative for your cover letter and your surveyform (including the survey cover sheet and instructions for complet-ing the survey) can be developed directly from the sample pagesfound in Appendix B.

The mythical village of Good Life, Nebraskaand the actions of a committee with membersfrom the Good Life community are describedhere as an example of the local activities and thedocuments required for a community Needs andPriorities Assessment. The Good Life commu-nity is typical of many small farming communi-ties, and the committee members could be theconcerned persons living in almost any smallfarming community. The example survey docu-ment was easily assembled using the informationavailable in this publication. It demonstrates thenature of a short survey targeted on a few highpriority community concerns. Please notice thatthe turn-around time for this example survey isvery short which is desirable and possiblewhen the survey is relatively short and simple.
The mythical village of Good Life (popula-tion 664 persons at the time of the 1990 Census)is located about 20 miles from the county seat(1,800 population), and about 65 miles from aregional trade center with over 17,000 residents.For several decades the village had little change.Farm failures, rapidly declining business activ-ity, and out-migration resulted in a populationdecline of 113 persons between the 1980 Censusand the 1990 Census. Several businesses closed.Their buildings are empty today and the businessdistrict looks partly abandoned.
The village is surrounded by an attractiverural countryside with farms of modest size thatproduce grain, livestock, hay. and milk. Severaldairies are located within a few miles of the vil-lage along the highway between Good Life andthe regional trade center. Their milk goes bytruck to a cheese factory, a haul of nearly 200miles. Recent comments by a dairy owner indi-cate he is about to close down his dairy andretire. (No one has been both willing and finan-
cially able to purchase his dairy operation.)Other dairy owners are concerned. If one dairycloses, the cheese factory might stop haulingmilk from the Good Life area. If milk haulingstops, all operating dairies are likely to go cut ofbusiness.
If the dairies close, eight village residentswho now are employed as milkers, farm labor-ers, or managers of these dairy operations willlose their jobs. The local feed dealer will have animmediate reduction in sales, and will let oneemployee go. Over the long-term the feed dealermay go out of business. People in and near GoodLife are worried becawe they feel the alreadyprecarious future of the village would be furtherthreatened by these changes.
While residents' social loyalties to the vil-lage are very strong, they are like most people inour society. If they have cars or can ride withfriends, many of their dollars are spent in thecounty seat and regional trade center stores. Ifasked why, the replies often sound like this: "Wewere going anyway because .... Or, "prices arebetter there, so we did our shopping while wewere there." This causes local stores a large lossof business.
Concerned over possible further decline, thecafe's coffee shop group that meets each morn-ing decides it is time to "do something." Theytalk over the possibilities. No one knows whatresidents would like to have done. The villageboard president says, "Whatever we do. it can'traise taxes." The owners of local businesses saythat they could provide a little support for a vil-lage activity if they knew the people wanted it.An informal committee of interested persons isnamed and given a charge: Find out what thepeople want. Members include: the village clerk,the grocery store owner, the bank president, theextension educator, a retired farmer and coffeegroup member for many years, and the con-venience store owner (she has the only new storein town.)
The committee meets in late October anddecides to use a needs assessment survey toreveal local needs and priorities. Each memberhas a favorite topic on which he or she wants

more information. However, they decide to keepit simple by focusing on local economic condi-tions. (The extension educator said it would takemany hours to tally the responses if most resi-dents completed and returned their surveys.)Four topics were selected:
The appearance of the village business dis-hict,
Possibilities for expanding the localeconomic base,
Conditions for small-scale entrepreneurs,and
Community tourism. (One member of thecoffee group has argued for years that thearea and village are so attractivt that theyshould "pull in" the tourists.)
They decide to distribute the surveys door-to-door because the Extension educator said itwill increase participation and provide moreaccurate responses. They ask members of severallocal organizations to help distribute and pick upthe survey forms, and set a deadline for them-selves for reporting back to the community infive weeks. The following copy of their surveydocuments provides an example of a short andsimple community needs and priorities assess-ment.
