Berdasarkan penjelasan yang telah disampaikan oleh peneliti, tentang penelitian yang akan dilaksanakan sesuai dengan judul di atas, maka saya mengetahui tujuan dari penelitian ini, yaitu untuk menjelaskan faktor-faktor yang berhubungan dengan kepuasan kerja perawat.
Saya memahami bahwa keikutsertaan saya dalam penelitian ini sangat besar manfaatnya. Saya juga mengerti bahwa catatan mengenai penelitian ini akan dijaga kerahasiaannya dan berkas yang mencatumkan identitas saya hanya digunakan untuk keperluan pengolahan data saja dan bila sudah tidak digunakan lagi, data tersebut akan dihilangkan.

Peneliti adalah mahasiswi FIK-UKSW yang sedang melakukan penelitian dengan tujuan untuk menjelaskan faktor-faktor yang berhubungan dengan kepuasan kerja perawat
Peneliti mengharapkan kesediaan Bapak/Ibu untuk berpartisipasi dalam penelitian ini. Jika Bapak/Ibu bersedia, maka peneliti akan memberikan pertanyaan kepada Bapak/Ibu tentang faktor-faktor yang mempengaruhi kepuasan kerja perawat..
Peneliti mengharapkan Bapak/Ibu menjawab pertanyaan tersebut dengan jujur, apa adanya dan sesuai dengan apa yang dialami saat ini. Partisipasi Bapak/Ibu dalam penelitian ini bersifat sukarela, sehingga Bapak/Ibu bebas untuk mengundurkan diri tanpa sanksi apapun.Jika bersedia menjadi partisipan dalam penelitian ini, silakan menandatangani formulir persetujuan ini.
