Dear fellow community members,

On December 8, 2021, Cottage Health marked an important milestone. Since the opening of Santa Barbara Cottage Hospital in 1891, the doors have never closed. Through floods and fires, earthquakes and pandemics, Cottage has served those in need in our community, every single day, for 130 years. And though much has changed in healthcare and in our world, one thing has remained constant: Cottage Health has cared for this community with excellence, integrity, and compassion.

The past two years have brought challenges unlike any previously experienced. With the announcement in January 2020 of confirmed COVID-19 in the US, Cottage physicians, nurses and other professional and support staff took steps to assure the safety of patients, the community, and each other. Remarkably, over the past two years, at times when facing incredible personal hardships, our caregivers continued to place patients as their priority, resulting in the highest national patient experience ratings in our history. We can never thank them enough for their dedicated service and for all they have done.

Many thanks to all of you who have contributed to our COVID-19 response efforts – making a difference for patients and the staff who are providing care. As we continue to navigate the rapid changes this virus continues to bring, your support and encouragement have helped sustain us. You’ll read more about COVID-19 response in this issue of Cottage Magazine.

While assuring the highest quality and safety of patient care has been our focus during this pandemic, we continue to pursue other important advancements in the health of our community. Recent examples include the launch of Cottage Virtual Care, the opening of Cottage Urgent Care centers

throughout the region, the decision to add a pediatric residency to our medical education program, and our efforts to provide vaccinations to all populations we serve.

Looking ahead, with your help we will not only meet the immediate healthcare needs but also invest in the future, assuring medical excellence close to home. We will continue to provide the latest medical advancements. We will invest in our future workforce to assure the highest levels of medical, nursing, and other clinical professionals for the region. We will expand our research efforts to identify new treatments and interventions to improve health. We will improve the health of our communities by identifying and acting on the challenges and needs of the most vulnerable. And we will always be prepared for the unexpected, whether it is in an immediate emergency or injury or a complex public health crisis or natural disaster.

JANUARY 24, 2020 Within 24 hours of the World Health Organization (WHO) convening to assess the novel coronavirus outbreak, Cottage Health leadership convenes an incident response team to begin daily meetings, monitoring the virus and its spread.

COTTAGE HEALTH CARES FOR THE COMMUNITYCOVID-19 Pandemic Response Timeline

MARCH 1, 2020 With no cases yet confirmed in Santa Barbara County, Cottage Health actively prepares with education, updates, training and protocols.

MARCH 11, 2020 WHO declares COVID a pandemic. The following day, Cottage Health implements remote work for employees whose roles could be moved offsite, to help protect patients and staff and keep the hospitals safe during ongoing care.

MARCH 13, 2020 With school closures, Cottage Health develops options to help families with childcare subsidies and ensure continuity of patient care.

MARCH 16, 2020 Cottage Health hospitals in Santa Barbara, Goleta and Santa Ynez prepare for a potential surge in COVID patients while continuing to meet existing patient care needs. Surge tents are installed in the ER parking lots for screenings of emergency patients. Surge planning includes converting patient rooms to negative pressure for the safety of patients and staff. Cottage Health develops a dashboard of safe elective procedure metrics. This dashboard is later published by the American Hospital Association, to help other hospitals respond with safe, proven methods to manage hospital capacity.

MARCH 18, 2020 COVID is confirmed in Santa Barbara community, and systems at Cottage Health are in place. Visitor restrictions are implemented to protect patients and staff. Supply chain teams secure PPE for pandemic preparedness. Ongoing surge planning and staff flexing is established. COVID updates are provided for staff and community.

MARCH 20, 2020 Cottage Health quickly adapts to the evolving situation. A new system of COVID symptom screenings at entrances is developed. A labor pool matches available employees to departments needing extra staff. Employees with health risks or with immune-compromised children or family members are offered alternate work – remotely or in other areas of the hospitals.

MARCH 22, 2020 Cottage Health establishes new programs as needs are identified. Local lodging is provided at no cost to employees concerned with exposing loved ones to the virus. Two separate patient care units are converted to negative pressure isolation units.

The Cottage Emergency Response Fund is created to support patients, staff and our community. An outpouring of community support leads Cottage to open a community supply donation center in the Goleta Valley Cottage Hospital parking lot where individuals and organizations bring medical supplies. Community members arrive by truck, car and even bicycle to donate everything from face masks to gloves to isolation gowns for healthcare workers. Local restaurants provide meals for Cottage Health staff. Others donate funds to help support staff needing childcare and other services.

MARCH 25, 2020 For staff and dependents, Cottage Health joins with local therapists to provide online counseling for stress management, and expands childcare services. Emergency childcare for first responders and healthcare workers is established. For employees with unexpected childcare expenses, Cottage Health reimburses up to $100 per week. A collaboration of Cottage clinicians, UCSB and local entrepreneurs produces locally-made powered air-purifying respirator (PAPR) shields to better protect staff.

MARCH 26, 2020 Three patients at Santa Barbara Cottage Hospital (SBCH) test positive for COVID. To prepare for a potential patient surge, four patient care units are equipped and ready to be negative pressure isolation units. New protocols are developed by infectious disease specialists and nursing staff.

MARCH 30, 2020 SBCH has 16 patients in isolation, six of whom test positive for COVID and four of whom are in critical condition. To prepare more bed capacity, the Eye Center moves to the Outpatient Surgery Center and Antepartum 2, 3 and 4 are converted to negative pressure rooms for childbirth deliveries when isolation is required for a patient with COVID.

APRIL 1, 2020 22 patients are hospitalized at SBCH with COVID; seven are in critical care. SBCH provides regular census reports to media. On this date, 26 patients are at SBCH with confirmed COVID.

APRIL 13, 2020 Free Cottage Virtual Care visits are offered to the community for individuals with COVID symptoms.

APRIL 15, 2020 14 patients with confirmed COVID at SBCH.

APRIL 16, 2020 Cottage launches Stay Safe campaign to help share messages on how to stay safe from COVID.

APRIL - AUGUST, 2020 The labor pool provides financial stability for staff. Education department springs to action to cross-train 70+ staff for critical care. The education and infection control departments collaborate to train staff on PPE protocols and specimen collection. Facilities and support services teams work with clinical teams to relocate entire units for effective isolation care during surges.

Pacific Diagnostic Laboratories (PDL) successfully implements two new COVID testing instruments to provide quicker results for hospital patients.

New ways to care for employees are established due to many staff being financially impacted as spouses lose jobs due to business closures. The Cottage Assistance and Relief for Employees (CARE) Fund is modified to provide monthly support for employees in need.

JULY 15, 2020 30 patients with confirmed COVID at SBCH.

JANUARY 29, 2020 The state of California reports two cases of novel coronavirus, with none in Santa Barbara County. The same day, Cottage Health issues precautions and procedures for the rapidly developing situation.

SEPTEMBER 2020 When the new school year starts, Cottage Health updates its childcare resources to ensure employees receive the care they need for their families with flexible work schedules and additional temporary childcare centers.

SEPTEMBER 15, 2020 Summer surge subsides. Eight patients with confirmed COVID at SBCH.

NOVEMBER 19, 2020 Two patients with confirmed COVID at SBCH.

DECEMBER 15, 2020 Winter surge in cases. 39 patients with confirmed COVID at SBCH.

DECEMBER 17, 2020 Cottage receives its first shipment of Pfizer’s COVID vaccine. Later this same day, the first frontline staff members are vaccinated. The Southern California region reports ICU capacity of 0.5%.

DECEMBER 30, 2020 SBCH is caring for 65 patients with confirmed COVID.

JANUARY 5, 2021 California reports a statewide 7-day average positivity rate of 15%. Locally, Pacific Diagnostic Laboratories records 17%. SBCH converts an additional unit for negative pressure isolation, to respond to increasing needs for COVID care.

JANUARY 11, 2021 Local COVID cases near the peak of the winter surge. SBCH is caring for 93 patients with confirmed COVID, nearly a quarter of these are in critical care.

JANUARY 15, 2021 The Cottage Drive-Up COVID Vaccine Clinic opens at Goleta Valley Cottage Hospital and administers 500 vaccines the first day, to community healthcare workers and individuals age 75+. More than 1,200 Cottage staff and community volunteers will run this clinic for a total of 68 days over the next six months.

FEBRUARY 9, 2021 COVID hospitalizations decrease. SBCH converts a temporary isolation unit back to normal operation. 67 patients are in isolation with COVID; 13 are in critical care.

FEBRUARY 12, 2021 COVID hospitalizations decrease steadily. SBCH converts a second temporary isolation unit back to normal operation. 58 patients are in isolation with COVID; eight are in critical care.

MARCH 2, 2021 SBCH is caring for 18 patients with COVID; eight are in critical care.

APRIL 6, 2021 The Cottage Drive-Up Vaccine Clinic surpasses 50,000 doses administered, with capacity expanded to administer 10,000 doses a week.

MAY 13, 2021 The Cottage Drive-Up Vaccine Clinic begins offering vaccine for 12-15 year olds. Nearly half of the eligible population in Santa Barbara County is fully vaccinated.

JULY 13, 2021 SBCH is caring for three patients with COVID; none are in critical care.

JULY 30, 2021 The effects of the Delta variant are seen in rising case numbers. With data showing high transmissibility of the variant, and asymptomatic individuals spreading the virus, hospitals resume routine testing of all patients.

JULY 29, 2021 The Cottage Drive-Up Vaccine Clinic ends on July 29 after administering nearly 100,000 doses of vaccine since January.

AUGUST 5, 2021 California releases a State Public Health Officer Order with updates on vaccination or testing requirements for all who work in and visit healthcare facilities.

AUGUST 20, 2021 11 patients are in isolation and none are in critical care. Hospitals once again limit the number of visitors.

AUGUST 22, 2021 Hospitalization numbers continue to rise. A second COVID isolation unit reopens at SBCH.

SEPTEMBER 1, 2021 22 patients are in isolation and five in critical care.

“I had a patient who was so critically ill that I was assigned to be responsible for only her care that day, what we refer to as a 1:1. She had been on the ventilator for several weeks, requiring sedation. Her family sent voice recordings to the social worker through our hospital iPads, and I played them every time I saw her.

“She had several children and grandchildren, and they left sweet messages encouraging her to kick the virus and come home, just to do the everyday

things like enjoy breakfast together. These messages brought me to tears every time.

“I can still hear the recordings in my mind when I think about it. When I had this patient, I couldn’t help thinking of how we all cope with things differently and how everyone in this family had their own way of cheering on their mother/grandmother/wife.”

SBCH AWARDED ENVIRONMENTAL SERVICES DEPARTMENT OF YEAR

Santa Barbara Cottage Hospital’s Environmental Services Department recently earned the nation’s top award from the Association for the Health Care Environment (AHE) of the American Hospital Association for hospitals in the 500+ beds category. The criteria included high patient survey scores for cleanliness of environment, cleaning and disinfection protocols during COVID-19 surges, and high employee retention. The honor recognizes outstanding achievements of a cutting-edge environmental services team in maintaining high levels of performance in cleaning, disinfecting, infection prevention, environmental sustainability and stewardship, technology use, patient satisfaction, education and training in critical areas.

“Our team of 151 professionals in the EVS department at SBCH is committed to the highest standards for protecting the environment and health of all patients, staff and visitors,” said Nick Henderson, Vice President, Support Services. “Despite all the challenges from COVID-19, our EVS team has consistently set the bar higher to create new ways to enhance its performance for the benefit of our community.”

If you meet Henry, his smile will make you smile right back. You will notice his can-do attitude, and you will see he is smart and is an amazing athlete in all sorts of sports – baseball, skiing, hiking, skateboarding and rollerblading. You would never know that Henry faced a long, unimaginable challenge, overcame it and recently completed his last round of chemotherapy at Cottage Children’s Medical Center.

When Henry was three years old, he began experiencing symptoms like bruising easily and feeling very fatigued after riding his bike for just a minute or two. He also started having nosebleeds. Henry’s pediatrician referred him to the ER at Santa Barbara Cottage Hospital. At Cottage, Henry was diagnosed with high-risk B-cell acute lymphoblastic leukemia (ALL). This disease affects B-cells, which are essential in the immune system. The disease starts in bone marrow, where new blood cells grow. The leukemia cells grow very fast and interfere with bone marrow making enough normal cells.

“Henry was diagnosed on May 16, 2018, and stayed at Cottage Children’s Medical Center for the next 14 days to get his initial

chemotherapy and steroids,” said Henry's father, Andrew. “The staff was amazing, and they made everything so much easier on us during the darkest times of our lives. We will forever be grateful for their compassion.”

It’s often the small moments of kindness that remain in family memories: A pediatric nurse using tape to create blue and red combinations, because Spider-Man was Henry’s favorite character. An employee who cleaned his room sharing smiles and making him laugh every morning.

In all, Henry’s treatment lasted three and a half years and included numerous hospitalizations. “Henry was brave and strong through it all,” said Giana, Henry’s mother. Then on October 8, 2021, the big day came. Henry had completed his treatments. Soon after staff gathered with his family to cheer as Henry rang the bell at Cottage Children’s Medical Center – a joyful tradition that signifies a child no longer needs chemo treatments.

From the beginning, Henry was no stranger to challenges. He was born 10 weeks premature and spent five weeks in the Haselton Family Neonatal Intensive Care Unit (NICU) at Cottage Children’s Medical Center. “The NICU nurses were angels,” Andrew said. “They took amazing care of Henry and helped keep Giana and me calm.”

These days, Henry is full of life and enjoying sports and school, where math is his favorite subject. Henry will be representing Cottage Children’s Medical Center as the 2022 Children’s Miracle Network Champion. Beating cancer amidst a pandemic will be always be part of Henry’s childhood story – and his champion spirit.

To help reach everyone in our community, the Cottage Center for Population Health (CCPH) developed new initiatives and partnerships to serve various populations including Spanish-speaking communities, the indigenous migrant community, and people without housing living in Santa Barbara County. The Community Health Ambassador program launched a pandemic response to meet needs for COVID-19 information and convenient vaccination options.

A Community Health Ambassadors (CHA) is an individual dedicated to improving the health of their community through education, navigation, referral and support. Throughout the year, CHAs have helped provide information and create easier access to vaccines by addressing barriers in areas of transportation, language and scheduling.

REACHING HIGHER TOGETHERIf you walk through the main concourse in the lobby of Santa Barbara Cottage Hospital, you’ll see a new donor recognition installation designed to honor early leadership contributors to the SBCH Foundation’s Reaching Higher Together Campaign. These generous contributors to the $130M effort are partnering with Cottage to ensure the people of Santa Barbara have the best healthcare in the nation – right here at home. They are helping us to envision bold new ways to promote excellence and innovation to strengthen patient care, education, research and the health of our community today and into the future.

The Campaign will enter its public phase in 2022 and the entire community is encouraged to participate.

INVESTING IN COTTAGE'S FUTURE

Amidst the immediate impact of the pandemic, donors have continued to make significant gifts to strengthen the long-term capacity of Cottage Health to care for the community.

These investments in Cottage's future assure that medical excellence, innovation, and our ability to respond to the unexpected will continue to thrive on the Central Coast.

One of these forward-thinking commitments is from David and Anna Grotenhuis, Wayne and Sharol Siemens, and Virginia Grotenhuis. Their contribution, valued at $14M, will create a consolidated Cottage business services campus in the years ahead – bringing together non-clinical departments including Advancement, Finance, Human Resources, Information Technology & Services, Marketing, Supply Chain, Research and Revenue Cycle for more efficient and collaborative workflow among these departments currently in multiple locations across Santa Barbara and Goleta.

David and Anna have been long-time supporters through their giving to the Grotenhuis Pediatric Clinic and critical care services.

“Santa Barbara Cottage Hospital has provided extraordinary care for our community for 130 years,” Wayne said. “We share a feeling of appreciation and pride in the care Cottage provides to our friends and neighbors, and we are excited to support Cottage into the future.”

Another important commitment comes from Virgil Elings, PhD, an entrepreneur and UCSB professor of physics for 20 years. Dr. Elings has become one of the region's most recognized philanthropists through his giving to education, park lands, research and healthcare. By designating his gift as unrestricted, Dr. Elings enables Cottage to apply the funds where they will have the most impact in providing patient care.

PHILANTHROPY SUPPORTS GREAT NURSINGMelissa Aldecoa Fayram serves in the medical intensive care unit at Santa Barbara Cottage Hospital. She is a Santa Barbara Cottage Hospital Foundation nursing scholarship recipient currently working on her Master of Science in Nursing at CSU Channel Islands and will graduate in summer of 2022.

Melissa’s connection to Cottage – and its philanthropy – have been present from the very start of her developing healthcare career. She served as a volunteer in the ED at Santa Ynez Valley Cottage Hospital, and she received a SYVCH Auxiliary Award scholarship in 2013 while pursuing her BSN degree.

EXCELLENCE IN NURSING EDUCATION

Supporting nurses and nursing education is a priority for Cottage Health and for its fundraising program.

• The Gunner Scholarship was established by Richard and Mimi Gunner to support nurse leaders pursuing advanced degrees. The first scholarships totaled $107,000 and were awarded to 11 candidates for the 2020-2021 academic year.

• George Burtness is a longtime volunteer and supporter of nursing scholarships at Cottage. He has made a new $1M pledge to establish the George L. Burtness Endowment in Nursing Education. When established, the new fund will support and promote excellence and innovation in nursing education in perpetuity. This gift celebrates a legacy of service and dedication to Cottage by Mr. Burtness and his family.

• Grateful patient, Mr. Angelo Mozilo, recently made a $1M gift to create the Phyllis and Angelo Mozilo Endowed Scholarship in Nursing. This extraordinary gift celebrates a meaningful commitment to Cottage by supporting the education of promising nursing students who face financial challenges, yet remain committed to improving their life – and their community – by pursuing a career in nursing at Cottage.

“The Cottage scholarship has been so helpful during a stressful time balancing school and work. The scholarship has allowed me to progress through school quicker. I’m very grateful for the opportunities that Cottage and the MICU have provided for me to grow professionally as a registered nurse. The Santa Barbara Cottage Hospital Foundation established its first Endowed Chair in October 2020 honoring a visionary physician leader in medical education and two extraordinarily forward-thinking donors. Andrew Gersoff, MD, FACP, was named the Craig Malley and Louise Stewart, MD, Chair of Graduate Medical Education. The philanthropic investment of Craig Malley and Louise Stewart, MD, will allow Cottage to maximize opportunities to cultivate and train the best and brightest young physicians who will care for people throughout the Central Coast. Cottage’s long-standing Graduate Medical Program includes residencies in Internal Medicine, Radiology and Surgery. A new program, the Cottage Children’s Medical Center Pediatric Residency, will begin in June of 2022.

Endowed chairs for physician leaders enhance the reputation of the organization, which helps attract and retain renowned clinicians, educators and leaders essential to providing exceptional care to our community.

With this gift, Cottage has launched an important new endeavor to establish endowed chairs in critical specialties including neurosciences, pediatrics, emergency, trauma and critical care medicine. Endowed Chairs produce a steady stream of funding essential to support clinical excellence and the highest levels of patient care. They may also be named by the donor. BUILDING A NEW HOSPITALITY HOUSE To care for patients and their families, Cottage Health provides accommodation for families who need lodging near the hospital and who have been referred by Cottage’s medical social services, case management staff or spiritual care team. Frequently, these families have a newborn in the Neonatal ICU, they’ve traveled from out of town or they need intensive guidance and training time to learn how to sustain their child’s care. The requests have exceeded the current space at the Little Cottages. Fortunately, building is in progress for a new Hospitality House which will double capacity, and Cottage Health will be able to accommodate more than 4,000 requests from families each year. These Hospitality House apartments will have children’s play equipment and social areas, and will offer support and meals to families to help ease the stress of the hospital stay.

The $200,000 cost of this special ambulance has been provided to Cottage through the Children’s Miracle Network and its retail sponsors including Costco, Panda Express, Radiotón, Credit Unions for Kids, Walmart, Ace Hardware, Dance Marathon and Rite Aid, as well as individual donors. Staff and maintenance of the ambulance is an ongoing expense.

DRIVING CHILDREN’S HEALTHThanks to the generosity of our Childrens Miracle Network Hospitals (CMNH) partners and caring community donors, Cottage Children's Medical Center has a new 'ICU on wheels.' This new vehicle is a specialized ambulance to transfer newborns and children from throughout the region to receive the advanced medical care provided at Cottage Children’s Medical Center. With each transfer, a team including a nurse, respiratory therapist – and, if the baby is very small, a physician – provides care on board during transit. For newborns, the ambulance is equipped with isolettes, which control temperature, humidity, and oxygen levels throughout this important first journey.

TIARA CAMPAIGN BREAKS RECORDWhile the annual gala was paused in 2021 due to the pandemic, Cottage care never stops. The 2021 Tiara Campaign volunteer committee and many generous donors set a new record by raising over $650,000 in vital funds to ensure everyone in the community has access to exceptional emergency, trauma and critical care.

Special thanks to past and current Tiara Ball chairs who reunited to lead the campaign: Alex Nourse, Betsy Turner, Jelinda DeVorzon and Debbie Mackall.

Since 2005, the Tiara Ball has raised $6,395,240 to help treat thousands of children and adults who are critically ill or injured. RESPONDING IN EMERGENCIESWith the COVID pandemic, everyone was learning together about this virus we’d never seen before. For hospitals, the emergency loomed larger, because the community was at great risk, and the healthcare staff providing care were on the frontlines, facing risks no one yet fully understood. They relied on training and preparation – and sometimes sheer grit – to serve, to care, to save lives on the toughest of days.

The Cottage Emergency Response Fund (CERF) – established through the generous support of community members along with our own employees – helped the hospital adapt and:

• Acquire vital equipment and supplies, including COVID Response Kits for patients who test positive for the virus. The kits help patients recover safely at home

• Provide emergency preparedness training and tools to respond to the pandemic and stand ready for future situations

• Assist employees facing COVID-related financial hardships with housing, food, childcare, transportation and other basic needs

Total giving to the Cottage Emergency Response Fund now exceeds $2.9M and has helped save countless lives.

Cottage Rehabilitation Hospital offers the only accredited comprehensive, traumatic brain injury and spinal cord injury rehabilitation programs between Los Angeles and the San Francisco Bay Area. Now, with increasing patient demand, the need for more advanced state-of-the-art facilities and a deadline to meet seismic safety standards, Cottage Rehabilitation Hospital (CRH) Foundation and Goleta Valley Cottage Hospital (GVCH) Foundation are collaborating on a new fundraising campaign to support CRH’s move to GVCH. Already more than $6.85M has been pledged or contributed. This move ensures residents of the Central Coast will have a full scope of rehabilitative specialties and services and a building designed to support the most advanced rehabilitative therapies therapies prescribed after routine orthopedic surgery or to promote recovery from complex illness or trauma.

SANTA YNEZ VALLEY COTTAGE HOSPITAL FOUNDATION RAISING FUNDS FOR NEW CT SCANNER In the Santa Ynez Valley Cottage Hospital ER, the CT (computerized tomography) scanner is essential diagnostic equipment to support lifesaving treatment and provides thousands of CT scans each year. The new CT scanner will advance the ability to quickly diagnose disease and injury using lower levels of radiation. Santa Ynez Valley Cottage Hospital is recognized for outstanding care with honors including:

• Stroke-Ready Certification by the The Joint Commission and Santa Barbara County Emergency Medical Services. The ER includes a Telestroke Program with neurologist on call 24/7 and a telemedicine robot to support rapid care.

• Top 20 Critical Access Hospitals in the nation for Best Practice in Patient Satisfaction by the National Rural Health Association in 2019 and 2020.

• Guardian of Excellence Award for Patient Experience from Press Ganey

2021 JUNIOR WHEELCHAIR SPORTS CAMPSponsored by the Cottage Rehabilitation Hospital Foundation, the Junior Wheelchair Sports Camp is an annual event created to inspire remarkable achievements by children who use wheelchairs. Due to the COVID pandemic, the 2021 camp was a four-day virtual event with fun, interactive, empowering activities for both campers and their families, and the event was expanded to include young adults up to age 29. The camp’s theme was “I Am My Power.”

EVENT HIGHLIGHTS:

• Special guest instructor, Wesley Hamilton, an award-winning athlete, advocate, and CNN Hero, is the founder and CEO of Disabled But Not Really (DBNR), a non-profit with a mission to advance courage and confidence for those who live with disabilities. He shared his own inspiring story and offered sessions on fitness and nutrition.

• Activity boxes mailed to campers included items to participate in every activity. This year dancing and boxing were two of the campers’ favorite activities. Other activities included an obstacle course, table tennis, power wheelchair sports, fitness, Frisbee golf, tennis, yoga, nutrition, cooking, games and more.

• Campers enjoyed a special feature on filmmaking and content creation and shared their own short film with others.
