It gives me great pleasure to write this message for the 12th Asian & Oceanic Society of Regional Anaesthesia and Pain Medicine Congress to be held on 19th to 22nd June 2013 in Kuching, Sarawak.

I note that the Organising Committee has chosen the theme “Exploring the World of Regional Anaesthesia and Pain Medicine” which is apt and timely as exchanges of ideas, techniques and current practices in this rapidly evolving field can be discussed and viewpoints shared. It is noteworthy that an eminent faculty of world-renowned speakers has been assembled to deliver the state-of-the-art lectures and to provide the latest updates in these two specialties. I observe that the scientific programme is packed and has something exciting for everyone.

Sarawak has plenty to offer the visitors including the rich cultural diversity with colourful ethnic communities. The Organising Committee has worked hard to ensure that all those who set foot on Sarawak will not only improve their clinical knowledge but also have the chance to experience the unpredictable yet enchanting tropical rainforest (one of the oldest in the world) as well as the unique variety of flora and fauna.

This will also be a good time for you to renew old ties and make new ones.

Message from the President Asian & Oceanic Society of Regional Anaesthesia

and Pain Medicine

I would like to welcome you to the 12th Biennial Meeting of the Asian & Oceanic Society of Regional Anaesthesia and Pain Medicine.

The discipline of Regional Anaesthesia and Pain Medicine has advanced so much in the past decade.

We have witnessed the introduction of ultrasound as a guidance for not only peripheral nerve blocks but also for pain and central neuraxial blocks as well. New Regional Anaesthesia techniques have now become the main mode of postoperative analgesia.

Pain Medicine has widened its scope to include not only pharmacological agents and block procedures, but also non-pharmacological therapies including psychological, behavioral and physical interventions.

Ultrasound has also become an essential tool for perioperative diagnosis as well as therapy guidance for the anaesthetist.

I believe the Scientific Committee has drawn up a programme to incorporate all the knowledge and practice of the above.

The social events planned by the Organising Committee will make this meeting a memorable one where old ties can be strengthened and new ones can be made.

On behalf of the Organising and Scientific Committees, it is with great delight and pleasure that I welcome you to the 12th Asian & Oceanic Society of Regional Anaesthesia and Pain Medicine Congress from the 19th to 22nd June 2013 in Kuching, Sarawak.

This event promises you an enlightening scientific programme whilst being enveloped by the mystic charm and various attractions that Sarawak has to offer.

The scientific programme will offer a comprehensive coverage of the current advances in Regional Anaesthesia as well as highlighting its importance in Paediatric and Obstetric Anaesthesia. Our internationally renowned faculty will deliver state-of-the-art lectures and conduct a variety of interactive symposia. The programme will be appropriate for both the doctors and allied health personnel and is approved for continuous medical education credit points.

Budding researchers now have an avenue to present their findings from the multitude of innovative studies that they may have conducted in the recent past and stand a chance to win some really attractive awards.

The Organising Committee assures a variety of social events that will complement the diverse scientific programme. A grand gala night filled with fun and frolic besides other activities are being lined up for you. Befittingly, the Organising Committee has decided on the theme “Rendezvous in Gold” to celebrate the 50th Golden Anniversary of the Malaysian Society of Anaesthesiologists. Come and join us for this fun filled evening with a dash of gold and black, the colours for the event.

Kuching itself has a number of tourist attractions and you will be spoilt for choice. Please come prepared to thoroughly enjoy yourselves and we will ensure that you have a great time basking in the warm hospitality which will be offered by us.

Message from the President Malaysian Society of Anaesthesiologists

On behalf of the Malaysian Society of Anaesthesiologists, may I extend a warm welcome to all invited guests, speakers and delegates to the 12th Asian & Oceanic Society of Regional Anaesthesia and Pain Medicine Congress incorporating the Annual Scientific Meeting of the Malaysian Society of Anaesthesiologists (MSA), the College of Anaesthesiologists and the Malaysian Association for the Study of Pain (MASP) in Kuching, Sarawak.

The Scientific Committee has worked hard to put forth an attractive and comprehensive scientific programme, both broad and varied, encompassing eminent invited plenary speakers, cutting-edge symposia, and interactive workshops which will arm participants with new visions, knowledge and motivation for their future work either in practice or in the academic life.

The Organising Committee has planned an outstanding social programme. I thank Datin Dr V Sivasakthi, Chairperson, Organising Committee and Professor Dr Marzida Mansor, the Scientific Chairperson, and their teams for a job well done.

It is indeed an honor that the trail of the development of anaesthesia in Malaysia has been finally captured in a wonderful effort by Dr A I Gurubatham in his book entitled ‘History of Anaesthesia in Malaysia’ which will be launched on the Gala Night. This year also marks the 50th year for the Malaysian Society of Anaesthesiologists. I do hope all of you will join us at the Gala Night in celebrating the Golden Jubilee of the Malaysian Society of Anaesthesiologists!

Incredible Kuching is a mystical city seeped in natural wonders, packed with lush rainforest, green national parks and is rich in cultural heritage with colourful ethnic tribes. Sarawak, also known as the Land of the Hornbills, offers you a wide range of tourism activities to see and enjoy; the longhouses, the Mulu caves in Miri just to name a few. Please do take some time off to savour these beautiful sights.

Enjoy the Congress, not only the academia but also the networking with colleagues and catching up with old acquaintances.

Message from the President Malaysian Association for the Study of Pain

It gives me great pleasure to welcome you to the 12th AOSRA-PM Congress in Kuching Sarawak. The Malaysian Association for the Study of Pain is honored to be given this opportunity to co-host this important biennial event together with the Malaysian Society of Anaesthesiologists and the College of Anaesthesiologists, Academy of Medicine Malaysia.

Pain Medicine is a relatively young field in Malaysia and many of our neighbouring countries, but is rapidly gaining popularity especially in the past decade. New analgesic drugs, new equipment and techniques for neural blockade and other pain relieving procedures as well as a better understanding of basic pain physiology has resulted in many developments in the field of Pain Medicine and more treatment options for patients especially those with severe cancer pain and with chronic non-cancer pain. Advances in techniques of regional blockade also allow us to provide effective analgesia for a greater variety of surgery in a wider range of patients. All these developments make Pain Medicine a growing field which offers exciting possibilities for young doctors. Furthermore, the multidisciplinary nature of Pain Medicine lends another interesting facet to this field as one has the opportunity to work closely with specialists in other fields as well as healthcare professionals from other areas.

Having a conference like the AOSRA-PM gives a great opportunity to Malaysians as well as doctors and health professionals in the region to get together to update themselves on the latest developments in the field of Regional Anaesthesia and Pain Medicine, and also provides an opportunity for our doctors to present some of the work that they have done through posters and oral free paper presentations. I hope that all participants will benefit from the scientific sessions as well as enjoy the social events, and also take some time to see a bit of beautiful Kuching and its surroundings.

Keynote Speakers

RAMANI VIJAYAN

Ramani Vijayan is a Senior Consultant and Professor in the Department of Anaesthesiology, Faculty of Medicine, University Malaya Medical Centre, Kuala Lumpur. Her main interests are in the management of pain (acute, chronic and cancer), anaesthesia for neurosurgery, regional anaesthesia and clinical pharmacology. She also has a keen interest in pain education. She continues to be an examiner for both the part I and Part II of the Conjoint Masters in Anaesthesiology examinations of University of Malaya (UM), National University of Malaysia (UKM) and University of Science Malaysia (USM).

She helped set up the first Pain Clinic in Malaysia in the then University Hospital, Kuala Lumpur in 1988 and the first Acute Pain Service in Malaysia in October 1992. She is a founding member of the Malaysian Association for the Study of Pain (MASP) and served as the President from 2001 to 2013. She is a member of the International Association for the Study of Pain (IASP) and has served as president of the College of Anaesthesiologists, Academy of Medicine.

She has authored numerous peer-reviewed articles in journals and is the author of “Handbooks on Pain Management”. She has been an invited speaker at National, Regional and International Conferences, including the World Congress on Pain and World Congress of Anaesthesiologists.

She is the Founder Editor-in-Chief of the “ASEAN Journal of Anaesthesiology” (1999-2005), which was established in 1999 with the first issue in 2000. She continues to serve on the editorial board of the journal.

MANOJ KARMAKAR

Dr Karmakar is an Associate Professor and Director of Paediatric Anaesthesia at the Chinese University of Hong Kong, Prince of Wales Hospital, Shatin in Hong Kong. Dr Karmakar’s area of research interest include regional anaesthetic techniques in adults and young children and in particular thoracic paravertebral block, spinal sonography, ultrasound guided central neuraxial blocks and local anaesthetic pharmacology. Dr Karmakar has published extensively in various peer-reviewed journals, been invited to speak at numerous international meetings around the world, been a faculty of countless workshops, and has lectured as a visiting professor at various reputable international universities. He is also the past president of the Asian Society of Paediatric Anaesthesiologists (ASPA). Dr Karmakar currently serves as an editorial board member of the “Journal of Pediatric Anesthesia” and is a reviewer for most of the major journals in anaesthesiology.

SRINIVASA N RAJA

Srinivasa N Raja is professor in the Department of Anaesthesiology & Critical Care Medicine, Professor of Neurology, and Director of Pain Research and the Division of Pain Medicine at the Johns Hopkins University School of Medicine.

Dr Raja received his early medical training in India, his residency training in Anaesthesiology at the University of Washington, Seattle, and post-doctoral training at the University of Virginia School of Medicine. He was awarded the Diplomate of the American Board of Anesthesiology in 1982, and added qualification in Pain Management in 1993 and was recertified in 2002.

Clinical interests of Dr Raja include management of chronic neuropathic pain states, such as sympathetically maintained pain and complex regional pain syndromes (CRPS, causalgia), postherpetic neuralgia and post-amputation pain. His recent research efforts are aimed at understanding the peripheral and central mechanisms of neuropathic pain and in determining the role of opioid and adrenergic receptor mechanisms in mediating or maintaining chronic neuropathic pain states. He has also conducted controlled clinical trials to develop better evidence-based practices for the pharmacological treatment of neuropathic pain.

In 1993, Dr Raja joined the editorial board of the journal, “Anesthesiology” as an Associate Editor and subsequently served as an Editor from 1998-2006. He established the section “Classic Papers Revisited” in 2000, for which he served as the Section Editor. He was an Associate Editor for the journal “Pain”, a member of the FDA Scientific Advisory Panel for CDER, and a member of the Executive Committee of IMMPACT. He is presently in the editorial board of “Pain Medicine”, “Pain Management”, and the “Scandinavian Journal of Pain”. He is a Section Editor for the journal “PAIN”. Dr Raja has been a member of the Scientific Advisory Committee for the World Congress on Pain from 2008-2012. He was elected the Vice-Chair of the Neuropathic Pain Special Interest Group (NeuPSIG) of the International Association for the Study of Pain and appointed as the Chair of the Scientific Programme Committee for the 15th World Congress on Pain, Buenos Aires, 2014.

Dr Raja and his collaborators have been funded for over 20 years by the National Institutes of Health for their basic science and clinical research and have published more than 130 original articles in peer-reviewed journals such as: “Anesthesia and Analgesia”, “Anesthesiology”, “Brain”, “European Journal of Pain”, “Journal of Neurophysiology”, “Journal of Neuroscience”, “Lancet”, “Neurology”, “Pain”, and “Science”. He is an Editor of five books and has written numerous book chapters. He has been invited to lecture both nationally and internationally, and as a Visiting Professor to several universities. His honors include the 14th Annual Benjamin G Covino Lecture at the Harvard University and the John J Bonica Lecture at the University of Washington. He was the recipient of the Wilbert E Fordyce Clinical Investigator Award from the American Pain Society in May 2008 and the prestigious John J Bonica award from the American Society of Regional Anesthesia and Pain Medicine in November 2010.

Mike Paech is a Chair of Obstetric Anaesthesia in the School of Medicine and Pharmacology of the University of Western Australia, in Perth. He has longstanding clinical, research and educational interests in obstetric anaesthesia and analgesia and postoperative pain management.

He is an editor of the journal “Anaesthesia and Intensive Care”; an editorial board member of many journals, including the “International Journal of Obstetric Anesthesia and Obstetric Anesthesia Digest” and reviews for another 30 journals. He has written two text books; published over 170 peer-reviewed articles, mostly original research studies, and lectured at 70 international meetings.

He is a past-president of AOSRA-PM.

ANDREW MOORE

Andrew was educated at Balliol College in the University of Oxford. Originally intending to study medicine, Andrew quickly changed to Biochemistry under the then Professor, Sir Hans Krebs. After his first degree, he rejected merchant banking to do research, being awarded his Doctorate in Philosophy in 1976.

He then became a clinical biochemist in the Nuffield Department of Clinical Biochemistry, where his role was to integrate clinical biochemistry into clinical medicine. This led to considerable research activity, including working with Henry McQuay on analgesic drugs, particularly opioid pharmacokinetics, and later clinical trials of analgesics.

This was followed by a spell in the biomedical industry, in which he formed a diagnostic company, and it grew to around 300 employees in about 10 years.

But his real love was research, and so in 1993 he was back in Oxford, working in pain and EBM, founding Bandolier – an EBM journal, and researching into systematic reviews and meta-analyses.

In over 30 years of biomedical research he has published over 500 scientific papers, including many systematic reviews, and approaching 100 Cochrane reviews. He has a H-index of over 70.

Andrew was awarded the Doctor of Science degree by the University of Oxford in 1997, made an Honorary Professor, School of Health Sciences, University of Wales at Swansea in 2005, was elected to Fellowship of the Royal College of Anaesthetists in 2009, and made an Honorary Member of British Pain Society in 2012. He is the Chairman of the International Association for Study of Pain Special Interest Group on Systematic Review and Evidence.

Dato’ Dr Say-Wan Lim (1939-2008)

DrSay-WanLimisbestrememberedforfosteringinter-institutionalrelationshipsamongtopmedicalorganisationsaroundtheworldandputtingMalaysiaonthemapinthearenaofinternationalmedicine.

BorninPenang,heobtainedascholarshiptostudymedicineinSingaporeandreturnedtoservetheMinistryofHealthin1963.In1965hejoinedthenewlycreatedDepartmentofAnaesthesiologyintheFacultyofMedicine,UniversityofMalaya(UM),KualaLumpur.DrLimundertookhisanaesthesiatraininginLiverpool,UKandbecameaFellowoftheEnglishandIrishFacultiesofAnaesthetists,RoyalCollegeofSurgeons.HecamebacktoMalaysiatoserveasalecturerinUMtillheleftforprivatepracticeinlate1969.

However,working as anAnaesthetist onlywas not enough forDr Lim.Hewas amanwithboundless energy, avisionarywho saw the tremendousbenefitsof unitingASEANAnaesthesia societies andwasoneof thedrivingforcesinestablishingtheConfederationofASEANSocietiesofAnaesthesiologists(CASA).ThebiennialcongressesofCASAhavebeengoingfromstrengthtostrengthwitheverincreasingcooperationbetweenthemembercountries.Thishaslednotonlytoacademicexchangebuthasresultedinforgingnetworksandlastingfriendshipsbetweenindividualmembers.

Inasimilarvein,DrLimwentontogetagroupoflike-mindedAnaesthetiststogetherfromtheAsia-PacificregiontoestablishtheAsianOceanicSocietyofRegionalAnaesthesia(AOSRA)in1989.AOSRAhadasimilarvisionandgoalstothemoreestablishedAmericanSocietyofRegionalAnaesthesia(ASRA)andEuropeanSocietyofRegionalAnaesthesia (ESRA) i.e. to revitalise the science and art of regional anaesthesia in this region.ThefirstAOSRACongresswasheldinTaipeiin1991withDrLimastheCongressPresidentandFounderPresidentoftheBoardofDirectors.Thesebiennialmeetingshavebeenheldeversinceandthiscurrentmeetingisthe12thintheseries.

In1992,DrLimwaselectedPresidentoftheWorldFederationofSocietiesofAnaesthesiologists(WFSA),asingularhonourashewasonlythesecondAsianandfirstMalaysiantoholdthehighestofficeinthisfield.Hehadtheskillandleadershipforthisoneroustask,whichalsoputMalaysiaontheWorldAnaesthesiamap.Besidesthis,hehadnumerousotherachievementsinhisdistinguishedcareer,withthenotableonesbeingPresidentoftheMalaysianSociety ofAnaesthesiologists (1974), President of theMalaysianMedicalAssociation (1882-1989),Master of theAcademyofMedicine,Malaysia(1984-1990)andPresidentoftheConfederationofMedicalAssociationsofAsiaandOceania(1983-1985).Hewasalsonotonetoholdofficeinnameonly,buteveryorganisationthathewasinvolvedwithcameoutstrongerandmorerespected.

Despiteallhisimpressiveachievements,DrLimremainedaveryapproachableman,whowouldbeeverwillingtohelpoutyetanothercommittee.Accordingtohisclosefriend,DrYeohPohHong,hehadtheuncannyabilitytoremembernamesandthisendearedhimtobothfriendsandacquaintances.InadditiontoAnaesthesiaandMedicine,hehadalotofotherinterests.Heexcelledinseveralsportsincludingbadminton,(MalayanSchoolboysDoublesChampionin1956and1957),squashandgolf.HisothergreatlovewasFreemasonryinwhichhewaspassionatelyinvolvedforthegreaterpartofhislifeandwhenheretired.

Life dealt himadoublewhammy.Hewasdiagnosedwithoralmalignancy in late 2001 forwhichheunderwenttreatment.Havingsurvivedthathewasdiagnosedwithanotherunrelatedmalignancyofthelungwhicheventuallyoverwhelmedhim.Throughouthisordealshemaintainedaninnerstrengthandacceptedhisfatewithquietdignity.

Dato’DrLimissurvivedbyhiswife,JeannieLim,whostoodbyhimthroughallthegoodandbadtimes.Hisfamilywhoincludesthreesonsandtwograndchildren,gottoseealotmoreofhimwhenheretired.

Withhispassingin2008,Malaysiahaslostachampion.Butwewillbeforeverindebtedtohimforhisvision,passionandgumptiontotakeupsomanychallenges.OurthoughtsarewithDatinJeannie.

TheSay-WanLimOrationatthisandallsubsequentAOSRA-PMCongresseshasbeenestablishedinhismemoryasatributetohistremendouscontributionstotheSociety.

Sarawak Attractions

Bako National Park

Get close to nature in Sarawak’s oldest national park, the Bako National Park. Commonly spotted Bako inhabitants include the rare Proboscis Monkey, the Silver Langur and the Bearded Pig. The largest

reptile here is actually the Monitor Lizard – and not crocodiles!

Mulu National Park

No visit to Sarawak is complete without visiting the Mulu National Park, a UNESCO World Heritage Site, also a popular site for expeditions, most notably the Royal Geographical Society Expedition of 1977–1978. It encompasses caves and karst formation in a mountainous equatorial rainforest setting. Mount Mulu has it all;

the world’s largest cave chamber, the Deer Cave that can contain London’s St. Paul Cathedral five times over and accommodate 40 Boeing 747s side by side; and the world’s most extensive cave system with razor sharp limestone pinnacles, The Pinnacles.

Gunung Gading National Park

For a possible glimpse of the world’s largest flower, the rare Rafflesia, come to Gunung Gading National Park. Have an exciting adventure through the mountains to view beautiful cascading waterfalls and

rare foliage.

Kubah National Park

A must-see for bird-watching enthusiasts. This park is situated on a sandstone plateau, is the home of mammals, birds and many species of reptiles and amphibians. Target to spot some endemic species like the Blue-banded Pitta, Bornean Blue-flycatcher, Yellow-

rumped Flowerpecker, Bornean Black Magpie and others. With a bit of luck, you may encounter some other resident birds.

Semenggoh Orang Utan Rehabilitation Sanctuary

This facility is not a zoo but it is home to some Orang Utans, Hornbills, Honey Bears, Sea Eagles, Storks and Gibbons. The Orang Utans, some of which are taught survival skills and released back to

their natural environment, still return for a free meal.

“The Entire Village Under One Roof” – The Longhouses

Bidayuh, the second largest ethnic group in Sarawak, is formerly known as the ‘Land Dayak’ and was once also known as the ‘Engineer of Bamboo’. Annah Rais is one of the most famous Bidayuh longhouses in Sarawak. This village is well known for its unique architecture

and culture of the traditional bamboo longhouse which is home to dozens of families. Grab the opportunity to witness traditional lifestyle of the villagers. Do not miss “Baruk”, the head house, once used as a fort to protect the whole village, and contains real human skulls hanging around the fire place around their sacred war drum.

Visit the Iban community and their longhouses in the Batang Ai area. The Iban longhouses are guarded by the spirit world of hornbills, orang utans and gibbons. Enjoy ‘tuak’ (rice wine) with the longhouse chief.

Sarawak River Cruise

A Cruise on this main waterway is a fun way to see the different faces of Kuching. This cruise takes you past a host of fascinating sights. On one bank is the hustle and bustle of

a modern city while on the other lies the more relaxed kampungs (Malay Villages). The boat glides past colourful Malay Villages, the elegant City Mosque, the Astana, Fort Margherita and the Kuching Waterfront.
