In the past 18 months, across our broad
portfolio of activities, we have embarked on an
ambitious set of initiatives that offer new and
groundbreaking opportunities for the patient
and research communities to come together and
be part of the momentum we’re building in our
urgent pursuit of a Parkinson’s cure.
The Foundation is now approaching $300
million in research funded — all for projects
targeted to push the most promising therapeutic
breakthroughs closer to pharmacy shelves. In
2010, we received nearly 65,000 contributions
and reviewed over 600 PD-specific grants from
research teams all over the world. Our in-
house staff of PhDs and business strategists
worked directly with hundreds of researchers
and clinicians from academia and industry to
manage our portfolio of over 300 active grants.
In our short history we have earned tremendous
credibility with the research community. And
today we can say with confidence that the model
we have built is speeding progress:
● We have enabled outcomes-focused work
on over 100 therapeutic targets and invested
more than $100 million in the development of
disease-modifying therapies. On the back of this
investment, we have identified and advanced
several leading targets — including genetic
targets LRRK2 and alpha-synuclein as well as
dopamine-protecting neurotrophic factors —
for prioritized action.

● Building on significant and systematic
biomarker investments of $28 million from 2002–
2010, our landmark $45-million biomarker study,
the Parkinson’s Progression Markers Initiative, is
now under way and enrolling PD patients and
controls in 21 sites in the United States and
Europe, with nine industry funding partners in
place and expansion to Australia in the offing
for 2012.
● MJFF funding since 2005 has validated no
fewer than five promising drug targets for
dyskinesia — the debilitating side effect of long-
term dopamine replacement — with two already
in clinical testing and the remainder advancing
there rapidly.
● The Foundation has forged paradigm-shifting
processes for developing critical research tools
and then distributing them at low or no cost to
academic and industry researchers — including
pre-clinical models and basic laboratory reagents
that allow scientists to efficiently study cellular
functions at work in Parkinson’s pathology and
test new treatment approaches.
● We are now seeing greater than ever interest
and investment in PD by industry players —
from small and midsized biotechs to the giants
of Big Pharma — whose resources, expertise
and business incentive are non-negotiable
requirements to take promising new treatments
through clinical testing and regulatory approval.

Throughout this report, you’ll read about the
strategic roadmap developed by MJFF to
orchestrate and streamline Parkinson’s
therapeutic development, providing both financial
and intellectual leadership that has galvanized
the field and moved us tangibly closer to definitive
clinical trials and breakthrough treatments.
This is exciting. Yet it also poses new challenges
for our problem-solving, which we are meeting
head-on with fresh, collaborative platforms
designed to inspire the Parkinson’s community
— and the general public — to action in greater
numbers than ever:
● As resource-intensive as pre-clinical research is,
the clinical projects we see on the horizon come
with exponentially steeper price tags. The Brin
Wojcicki Challenge creates a critical opportunity
for the Foundation to double the capacity-building
funds (up to $50 million) we can raise by the end of
2012. With your help in earning match dollars, we
will ensure that no promising clinical study goes
unfunded due to lack of resources.
● Underenrollment in clinical research, by
people both with and without PD, slows research
progress — and we all pay the price in terms of
higher costs and longer time horizons to treatment
breakthroughs. The Foundation has stepped up to
answer this challenge with the development of a
new Web-based tool, Fox Trial Finder, a smart and
user-friendly solution to connect willing volunteers
with the specific trials that need them.
● Our grassroots fundraising network, Team
Fox, has grown exponentially since its founding
in 2006. With more than $15 million raised, this
group remains the passionate standard-bearer
for our message of can-do optimism and action
across the country and around the world — and
its numbers only continue to increase.
No doubt the road ahead remains challenging,
but we are gaining real traction. We know we are
on track to achieve our shared goal of eradicating

Parkinson’s patients have access only to treatments that
temporarily alleviate symptoms. If a biomarker is found through
PPMI, researchers will have a critical tool to use in the quest
for disease-modifying therapies that can slow or stop the
progression of the disease. The existence of a biomarker would
de-risk PD drug development by increasing the certainty
of research results, likely enabling smaller, shorter, more
predictable and potentially less expensive clinical trials.
Yet the development of reliable and valid measures of PD
requires such a major systematic infrastructure investment
that no single company has the incentive to do it alone.
By establishing a consortium of industry partners to help
fund the study, PPMI offers companies the opportunity
to help move the field forward while taking on only a
fraction of the risk.
PPMI exemplifies MJFF’s commitment to streamlining
the scientific process, increasing collaboration/data-
sharing, and putting resources where other funders
can’t or won’t in order to speed life-transforming
treatments to patients.

Clearly, the need for high-impact investment in research has
never been more urgent. But while financial investment is
critical, dollars alone will not take us across the finish line in
pursuit of therapeutic breakthroughs. The active involvement
of Parkinson’s patients and their loved ones in clinical research
is vital to finding the cure. Yet today, as many as 80 percent of
clinical trials face recruitment challenges, which significantly
increase costs and delay progress toward breakthroughs.
In summer 2011, MJFF launched Fox Trial Finder
(www.foxtrialfinder.org), a simple Web-based solution to
address these challenges head-on. With the single-minded
goal of bringing more willing volunteers to the PD clinical trials
that urgently need them, Fox Trial Finder combines robust
and relevant content — the Foundation is making a concerted
effort to include all Parkinson’ s trials in the tool’s database —
with a smart system focused on matching volunteers to trials
and developing relationships that persist beyond an initial
search. An expanded definition of the critical stakeholders is
also key; the tool proactively engages clinical coordinators,
physicians, and caregivers as well as patients themselves.
While it remains early days for this tool, an enthusiastic
response so far reinforces our instinct that a tool leveraging
technology, education and effective communication can
inspire the Parkinson’s population to action in significant
numbers. Within two months of the site’s initial launch, nearly
1,000 potential research volunteers had created profiles;
103 members of trial teams had registered, and personnel
from seven trials had contacted the Foundation after learning
of Fox Trial Finder from patients at their clinics.

The Foundation is sponsoring a $1-million clinical study to
validate clinical scales that measure dyskinesia (a must for
designing effective clinical trials of potential new treatments).
And we continue to drive research toward increased
understanding of the cellular mechanisms underlying
dyskinesia to ensure that every promising therapeutic avenue
is identified and pursued.
Toptherapeutictargetsfordyskinesia:
● GlutamatereceptormGluR5.In five years the Foundation’s
de-risking efforts chaperoned mGluR5 from basic discovery to
clinical testing and attracted industry interest in the target. In
August 2010, Addex Pharmaceuticals of Switzerland received
Foundation funding for a Phase 2 clinical trial of an mGluR5-
targeting agent that has shown promise in pre-clinical trials
and a Phase 1 clinical study. Additionally, Novartis is pursuing
clinical testing of mGluR5.
● Muopioidreceptors.Scientific evidence suggests that
increased opioid peptide transmission in a part of the brain
called the basal ganglia might underlie dyskinesia after long-
term levodopa treatment. Agents that reduce this increased
activity might benefit patients experiencing dyskinesia as a
result of taking levodopa for Parkinson’s disease.
● Serotoninreceptors.Researchers at Lund University in
Sweden have been funded by MJFF since 2005 to develop a
dyskinesia treatment targeting the serotonin system, better
known for its role in clinical depression. With Foundation
funding, the researchers are now clinically testing a drug
that has been shown to act on two specific receptors, known
as 5-HT1A and 5-HT1B, located on the serotonin neurons.
Clinicaltrialinfrastructure:
● Validationofclinicalratingscalesfordyskinesia.Significant challenges in measuring dyskinesia and potential
new therapies have created roadblocks to clear-cut trial
results and, therefore, industry investment in dyskinesia.
The establishment of validated scales for dyskinesia clinical
trials would significantly reduce the barrier for biotech and
pharmaceutical companies to invest in this debilitating
aspect of Parkinson’s.

opportunities for collaboration. MJFF is
orchestrating efforts to encourage
sharing data and resources early in the

toward practical drug therapy faster.
DYSKINESIAA debilitating side effect of current “gold –standard”
treatment for PD, dyskinesia is difficult to study
because it varies in intensity, frequency and duration.
MJFF is pushing dyskinesia treatments toward
patients by working on biomarkers, new drugs and
drug targets and clinical scales simultaneously to
optimize dyskinesia clinical trials. There is great
optimism in the PD scientific community that we will
see successful dyskinesia treatments come to market

KeyMJFF-ledrepositioningeffortsinParkinson’sdisease:
● With MJFF funding since 2005, the FDA-approved diabetes drug
pioglitazone has now advanced to an NIH-supported Phase 2 clinical
study in Parkinson’s.
● Approved high-blood-pressure drug isradipine is now being tested in
an MJFF-funded Phase 2 clinical study of 100 people with Parkinson’s with
final results expected by year-end 2011.
● In September 2011, the Foundation launched its first repositioning-
specific RFA, funding about $3 million to investigate seven known drugs
that are potentially effective against Parkinson’s.

De-risking Parkinson’s Research for Industry
Repositioning is just one of a broad portfolio of MJFF approaches to lessen the risk for industry players of investment in Parkinson’s disease drug development. As we’ve taken on more projects targeted toward industry, our role in the field has expanded, as has our influence on the agendas of other critical players. In 2010, we formally established an on-staff Research Partnerships team charged with engaging industry leaders across the board, whether in our brainstorming and planning meetings, as funding partners to our significant research initiatives such as PPMI, or in special efforts such as our LRRK2 industry working group.
Today, MJFF has funded over 135 unique projects led by industry, with investments of over $65 million. Our funding to small and mid-sized biotechs has helped move drug targets toward the clinic. Seed grants to large pharma have given PD a seat at the table — and as those grants have become successful the companies themselves have funded their furtherance.
MJFF also has become increasingly involved in developing and distributing tools that make PD drug development more effective and efficient. These include laboratory reagents, pre-clinical models, and biomarkers. And our PD Therapeutics Conference has become the most significant Parkinson’s drug development meeting in the world.

The Challenge comes at an opportune moment.
As we look ahead toward the next phase of
our work, several conclusions emerge. First and
foremost, our model is working. Due in no small
part to over $260 million in MJFF investments
since 2000, the stage of Parkinson’s drug
development as a whole is moving forward.
We have funded work on over 100 therapeutic
targets, and, with our backing, several have taken
significant steps toward clinical testing and
practical patient relevance.
This is exciting. Yet it also poses new challenges
for our problem-solving. As resource-intensive
as pre-clinical research is, the clinical projects
we see on the horizon come with exponentially
steeper price tags. New tools and strategies
are required to rally the patient community to
volunteer to take part in studies that urgently
need them. And we must continue to vet new
and existing potential therapies at every stage of
development to identify the most promising and
push them closer to pharmacy shelves.
If we can raise the $50 million we need, the Brin
Wojcicki Challenge will expand our capacity to
streamline and fund targeted research toward
Parkinson’s breakthroughs. It will allow us to bring
our successful out-of-the-box strategies into critical
areas that remain underfunded. It will aid our
continuing efforts to drive pre-clinical research,
lead groundbreaking public-private partnerships,
and attract broad expertise to address the unmet
medical needs of Parkinson’s patients.

2010FinancialHighlights
In 2010, despite a challenging economy, MJFF
funded just under $50 million — our greatest
annual commitment to date — in research
targeted to speed treatment breakthroughs into
Parkinson’s patients’ hands. Yet we measure our
success not by dollars raised or spent, but by
scientific advances achieved.
We’re proud of what we’ve accomplished to
date. But we stand on the precipice of a new
day in Parkinson’s research, and we will never
be satisfied until we have attained our goal of
eradicating PD. As we look to the future, we
continue to refine our approach to solve the
problems and address the challenges standing in
the way of progress now.
One element of our strategy that will reliably not
change is our insistence on fiscal accountability
and transparency. These are core elements of
our organizational values, particularly in how we
spend supporters’ contributions. We’re proud of
our track record of efficiency: Since inception,
87 cents of every dollar spent has gone straight
to our research efforts. And we intentionally have
no endowment or excessive reserves. Every dollar
we raise is immediately deployed to advance
research with the best chance of leading to new
PD therapies and, ultimately, a cure.
The American Institute of Philanthropy rates
MJFF as an “A” in its list of Top-Rated charities,
based on MJFF’s organizational and financial
efficiency. In 2010 we were included for the
first time in the Chronicle of Philanthropy’s
“Philanthropy 400,” an annual ranking of the
400 U.S. charities that raise the most funds
from private sources. And our recent keynote at
the annual meeting of the Center for Effective
Philanthropy was the highest-rated conference
session in the organization’s history.
