This Public Facilities Report is being prepared at the request of Anthem Park Community Development District (the District) to comply with Florida Statute 189415 This report contains a general description of existing public facilities owned by the District and facilities expansion or replacement currently purposed for the next seven years

The District encompasses 2106341+i- acres of land located in Section 9 Township 26 South Range 30 East in Osceola County Florida The District is within the City of St Cloud (the City) limit

The District owned and maintained facilities include but not limited to the following

bull Stormwater Management System

bull Wetland

bull Entrance Monuments

bull Parameter Wall

bull Recreation Facilities

e Parks and Trails e Limited Portion of the Roadways

bull Alley Ways

bull Landscapes and Hardscapes

Water distribution system and wastewater collection system is owned and maintained the City

Exhibit C is a list of parcels owned by the District obtained from Osceola County Property Appraisers Website on December 8 2014

As stated in Section 1 Anthem Park owns and maintains certain pubilc facliities beiow ls a ilst of such facilities

Stormv1ater Management Svstem

The stormwater management system was designed and constructed to Osceola County City of St Cloud and South Florida Water Management District (SFWMD) standards The stormwater management system consists of three (3) wet detention ponds integrated with an existing onsite wetland and the associated collection and routing system (storm inlets storm manholes and storm pipes)

The District is responsibie for maintaining those portions of the system that are outside the City and Homeowners Association owned right-of-ways

Wetlands

There is one (1) wetland within the District The District is responsible for the maintenance of onsite wetlands in accordance with the requirements of the SFWMD permit

Entrance Monuments

Subdivision signs and flag poles at the entrance on Lexington Boulevard and Capital Boulevard are owned and maintained by the District

Perimeter Walls

There are approximateiy 3553 linear feet of perimeter wall aiong portions of the eastern boundary of the District These perimeter walls are owned and maintained by the District

Recreation Facilities

The recreation faciiities consist of an amenities center (clubhouse and pooij The clubhouse includes restrooms a meeting room with a small kitchen and a workout room

The recreation faciiities are owned and maintained by the District

bull two (2) active recreational playgrounds with childrens play structures

bull one (1) fenced-in dog park (to be converted to active recreational playground with childrens piay structure)

bull one (1) sand volleyball court

one (1) park containing a non-functional vater fountain feature as a flower planter flag pole and park benches

bull one (1) park containing a functional water fountain feature park benches and access gates

bull two (2) passive recreational parks

The District is responsible for maintaining these parks and the features within them as well as the associated landscape within the parks

There are approximately 72887 square feet of concrete walking trails throughout the community that are owned and maintained by the District Please note that this amount does not include sidewalks within the Citys right-of-way which are owned and maintained by the City

Limited Portion of the Roadwavs

Roadways within the Citys right-of-way are maintained by the City with the exception of any brick pavers and concrete bands which are maintained by the District

AiievWays

The District has approximately 2992 linear feet of paved alleys that are owned and maintained by the District

Landscapes and Hardscapes

This District owns and maintains landscapes within the common areas that are not owned by the Homeowners Association as well as the irrigation systems

Landscapes and irrigation system within privately owned parcels are the responsibilities of the parcei owners

Hardscapes and fixtures such as aforementioned entrance monuments flag poles gazebos fences etc within common areas owned by the District are owned and maintained by the District

Please note that street lights are owned and maintained by Orlando Utilities Commission (OUC) The District pays a monthly lease amount to OUC for the street lights

The curient fenced-in dog park is proposed to be converted to an active recreational playground with childrens play structures

Also one of the two tennis court is scheduled to be converted to a basketball court

This Public Facilities Report is being prepared at the request of Anthem Park Community Development District (the District) to comply with Florida Statute 189415 This report contains a general description of existing public facilities owned by the District and facilities expansion or replacement currently purposed for the next seven years

The District encompasses 2106341+i- acres of land located in Section 9 Township 26 South Range 30 East in Osceola County Florida The District is within the City of St Cloud (the City) limit

The District owned and maintained facilities include but not limited to the following

bull Stormwater Management System

bull Wetland

bull Entrance Monuments

bull Parameter Wall

bull Recreation Facilities

e Parks and Trails e Limited Portion of the Roadways

bull Alley Ways

bull Landscapes and Hardscapes

Water distribution system and wastewater collection system is owned and maintained the City

As stated in Section 1 Anthem Park owns and maintains certain pubilc facliities beiow ls a ilst of such facilities

Stormv1ater Management Svstem

The stormwater management system was designed and constructed to Osceola County City of St Cloud and South Florida Water Management District (SFWMD) standards The stormwater management system consists of three (3) wet detention ponds integrated with an existing onsite wetland and the associated collection and routing system (storm inlets storm manholes and storm pipes)

The District is responsibie for maintaining those portions of the system that are outside the City and Homeowners Association owned right-of-ways

Wetlands

There is one (1) wetland within the District The District is responsible for the maintenance of onsite wetlands in accordance with the requirements of the SFWMD permit

Entrance Monuments

Subdivision signs and flag poles at the entrance on Lexington Boulevard and Capital Boulevard are owned and maintained by the District

Perimeter Walls

There are approximateiy 3553 linear feet of perimeter wall aiong portions of the eastern boundary of the District These perimeter walls are owned and maintained by the District

Recreation Facilities

The recreation faciiities consist of an amenities center (clubhouse and pooij The clubhouse includes restrooms a meeting room with a small kitchen and a workout room

The recreation faciiities are owned and maintained by the District

bull one (1) fenced-in dog park (to be converted to active recreational playground with childrens piay structure)

bull one (1) sand volleyball court

The District is responsible for maintaining these parks and the features within them as well as the associated landscape within the parks

There are approximately 72887 square feet of concrete walking trails throughout the community that are owned and maintained by the District Please note that this amount does not include sidewalks within the Citys right-of-way which are owned and maintained by the City

Limited Portion of the Roadwavs

Roadways within the Citys right-of-way are maintained by the City with the exception of any brick pavers and concrete bands which are maintained by the District

AiievWays

The District has approximately 2992 linear feet of paved alleys that are owned and maintained by the District

Landscapes and Hardscapes

This District owns and maintains landscapes within the common areas that are not owned by the Homeowners Association as well as the irrigation systems

Landscapes and irrigation system within privately owned parcels are the responsibilities of the parcei owners

Hardscapes and fixtures such as aforementioned entrance monuments flag poles gazebos fences etc within common areas owned by the District are owned and maintained by the District

Please note that street lights are owned and maintained by Orlando Utilities Commission (OUC) The District pays a monthly lease amount to OUC for the street lights

The curient fenced-in dog park is proposed to be converted to an active recreational playground with childrens play structures

Also one of the two tennis court is scheduled to be converted to a basketball court

Anthem Park CDD was established on mAY 4 2004 by Osceola County Ordinance

As stated in Section 1 Anthem Park owns and maintains certain pubilc facliities beiow ls a ilst of such facilities

Stormv1ater Management Svstem

The stormwater management system was designed and constructed to Osceola County City of St Cloud and South Florida Water Management District (SFWMD) standards The stormwater management system consists of three (3) wet detention ponds integrated with an existing onsite wetland and the associated collection and routing system (storm inlets storm manholes and storm pipes)

The District is responsibie for maintaining those portions of the system that are outside the City and Homeowners Association owned right-of-ways

Wetlands

There is one (1) wetland within the District The District is responsible for the maintenance of onsite wetlands in accordance with the requirements of the SFWMD permit

Entrance Monuments

Subdivision signs and flag poles at the entrance on Lexington Boulevard and Capital Boulevard are owned and maintained by the District

Perimeter Walls

There are approximateiy 3553 linear feet of perimeter wall aiong portions of the eastern boundary of the District These perimeter walls are owned and maintained by the District

Recreation Facilities

The recreation faciiities consist of an amenities center (clubhouse and pooij The clubhouse includes restrooms a meeting room with a small kitchen and a workout room

The recreation faciiities are owned and maintained by the District
