MESSAGE FROM THE CHAiRMAn The year under review has been a landmark year for HelpAge India. The organization carried out a visioning exercise on a pan-India format for the coming five years; this was to address the reality that the 100 million senior citizens of the country were poorly served in areas that needed critical and urgent attention, even after 68 years of Independence. The result: the charting of a set of mission objectives and goals to reach the un-served or under-served millions of needy senior citizens. The areas have been identified. The numbers to be impacted have been arrived at. The implementation processes have been laid out. Over the next five years, HelpAge India will be focused on achieving the maximum impact in selected

areas of health interventions, livelihood generation and most importantly, in creating a national platform that allows Indian elders voice their issues, their concerns in a way that is heard by all - the society, the policy makers and the implementers.

Lack of access to primary healthcare is a key factor in pushing thousands of elderly into a spiral of declining productivity, diminished income and poverty, all due to their deteriorating health. The HelpAge Mobile Healthcare delivery model will aim to reach 5,00,000 elderly by 2020, with an expanded network of paramedics, empanelled community health professionals and support from expert doctors through the use of telemedicine.

Cataract is a leading cause of blindness amongst elders causing loss of livelihoods, especially amongst the disadvantaged who need to work to survive. HelpAge in partnership with international and national organizations such as HelpMeSee and Satguru will work towards helping a million elders with cataract surgeries, in the coming 5 years.

For any movement to succeed, the stakeholders themselves must take ownership of it. HelpAge India has evolved the mechanisms to empower ten million seniors to participate in the demand for inclusion in the Government Pension and Food Security Schemes, Health Insurance and the National Livelihood Missions. Further, the HelpAge AdvantAge programme is being strengthened to enable seniors in peer bonding to combat loneliness and diminishing social relevance.

There are many other critical areas that seniors need assistance in. Of these, HelpAge is mandating itself to creating a nationwide network in setting up an Elders’ Helpline and Elder-Self-Help collectives engaged in providing livelihoods. Finally, to ensure that senior citizens receive their dues, they will be enumerated, identified and linked to national welfare projects.

The challenges are formidable. HelpAge is committed to realising the goals that have been set. We need the help of all our donors and pledge the support of each of our employees in this endeavor.

CEORights of older persons have been nebulous from the beginning. Very often it is considered as not necessary and is subsumed within an overall human rights framework. This endeavor to empower them takes on greater urgency when those that are denied are not only weak and vulnerable but have the added challenge of their advancing age. HelpAge India pledges itself to be a part of the national collective effort towards achieving three key rights and entitlements for the 100 million senior citizens of India.

90% of Indians work in the unorganised sector and older workers have no social security by way of health schemes or provisioning for pensions. This has led to many workers across the country slip into ill health, poverty and neglect. HelpAge India has been actively working to demand for a universal, non-contributory old age pension with a built-in provision for a minimum assured pension of Rs.2000 a month. However, results obtained have been uneven and inadequate, which indicates a long struggle ahead for pensions for all.

A key factor in the rising indebtedness of millions of low income families is a result of loans taken to meet the costs of treating serious illnesses. Though large numbers of government hospitals exist, their infrastructure cannot cope with demand and their locations are often too distant to be accessible by all. Private medical care is available but costs here are prohibitive. For many, these services are simply beyond their means. HelpAge India has partnered the Jan Swasthya Abhiyan in raising the demand to enact a Right to Health Act that would enable a public system to provide universal, free healthcare, including a full range of free, essential medicines and diagnostics along with a Universal Health Insurance scheme, specifically covering the elderly.

The Right to Food should be a fundamental right ensured by every government whether in the state or the Centre. The campaign for the Right to Food specifically demands the increase in food grain provided from 5 kg per person per month to 10 kg per person per month and to be supplemented with the additions of pulses and edible oil. HelpAge India along with its partners is actively engaged in advocating for these. For the millions of senior citizens whose survival to the very next day depends on this, there is not a day to lose. Apart from all this, we continued to provide more than a million treatments and livelihood opportunities for over 1,00,000 elders last year. We still strive to do more for the shelter of older people in our Homes and for the sponsored Grans.

TRAnSFORMinG livES Working on the groundUltimately, the true worth of a NGO’s work shows in the lives of its beneficiaries. In this respect, HelpAge India has heartwarming stories to share. In state after Indian state, case after case, beneficiaries narrate how their lives have changed for the better, as one program or another impacted their lives by bringing hope, reviving failing health, restoring a lost livelihood and, in the process, enabling each to live a life of dignity.

Each happy story has received help in its making from numerous, generous donors of HelpAge India. That is why, while this Annual Report honors the indomitable courage of otherwise helpless elders, it also salutes all of you who through your continuous support have helped HelpAge India to fulfill its mandate. It is this fact that emboldens HelpAge India to reach even more of those that wait in yet to be served areas. The HelpAge Vision 2020 is dedicated to this task. In the years to come, subsequent Annual Reports will bring you more such stories of elders finding a reason to welcome life again.

Affordable, accessible healthcare at their doorsteps

Mohanlal’s tiny provision shop in a Delhi slum barely sustains him. Disabled, unable to walk properly and abandoned by his son, he is the sole supporter of an ailing wife and responsible for his daughter’s education. He is left with nothing to take care of his own deteriorating health. For Mohanlal, the Mobi-health program has emerged as a savior, bringing him the medical care and attention that had been out of his reach.

“Whether it’s the rainy season or winter, the MMU comes right to my doorstep and continues to give me my medicine. This is a great benefit, they are always ready to help us”, says Mohanlal.

About the ProgramIndia has close to 100 million elders. Of these, the needy elderly, especially those over the age of 80, virtually have no access to the specialized medical care and attention that they sorely require. Most elders living in remote locations without any basic, modern healthcare have to walk long distances to health centres, which is often not possible. HelpAge India’s nationwide Mobi-health program provides regular and almost free healthcare to these and other under-served sections of society. A team consisting of a doctor, a pharmacist and a patient facilitator accompanied by a trained social worker, traveled to serve those who cannot visit stationary medical units (a local area medical centre) or live in very remote locations.

Each MMU covers an area of around 200 km every week. The mobile medical unit can be a four wheeler or even a two wheeler vehicle. Where no roads exist, medical workers cover the distance on foot. HelpAge India currently serves 1,255 sites across 23 states through 98 mobile medical units. Each MMU provides approximately 30,000 treatments annually. The goal is to serve17lakheldersoverthenextfewyears.

CATARACT SURGERiESRestoring vision, enabling livelihoods

RamKumar,atage76,stillhastoworktosurvive,thoughhebarelymanagestomakeendsmeet. Sadly, a few months ago he was about to lose his source of livlihood.

“My eyes were paining and I was losing my vision due to cataract. I had to shut my shop and stay at home. I could no longer earn and that led to problems within the family”.

Four months ago HelpAge India intervened with a corrective cataract surgery and Ram Kumar could once again open his shop.

About the programIn India, 62.60% are blind due to cataract. HelpAge steps in to help those needy elderly affected by cataract. Under a program that screens elders for cataract and conducts remedial surgeries where needed. All surgeries are performed under medical supervisions in top class facilities and never in make-shift camps. This ensures aseptic conditions and prevents post-operative infections. After an overnight stay for most patients, it is almost like seeing the world with a fresh pair of eyes. They no longer have to depend on family support and can begin to live a life of self respect and dignity.

So far, over 9,00,000 surgeries have been conducted in partnership with 400 credible eye hospitals, Trusts and NGOs in 25 states across India. Through cataract surgeries, HelpAge India wishes to restore the vision of one million needy elderly by the year 2020. The ultimate aim is to create cataract-blindness-free zones. In the effort to eradicate preventable blindness, HelpAge has partnered with the international NGO, HelpMeSee.

Treatment and palliative care

RamPiyari,67yearsofage,wasdiagnosedwithcancer.Herson,atailoratagarmentfactoryhad no means to afford her treatment.

“My condition is getting worse, where do I get the money to afford such expensive treatment?” a forlorn Ram Piyari had asked.

HelpAge came to her aid. After helping Ram Piyari receive radiotherapy and chemotherapy, she is now provided palliative care. A professional team of a doctor, a trained nurse and a counselor visit her regularly. The team treats her pain, infections and the side effects of chemotherapy, providing her relief and easing her suffering.

About the programPoverty forces many elderly patients to leave the costly treatment incomplete. Many others do not even start the treatment. HelpAge India comes to the aid of such needy elders in partnership with a number of credible and competent cancer hospitals and organizations in 11 states across India. Cancer treatments include surgeries, radiation and chemotherapy. In association with these partners, HelpAge also conducts cancer awareness and cancer detection camps. It has supported more than 99,000 treatments since 1998.

ElDER-SElF-HElP-GROUPSFrom dependence to interdependence

Located in the foothills of the Himalayas, on the banks of Kamala River lies the sleepy hamlet of Jhanjhaarpur. The elder women in this community led impoverished lives. They were powerless in the face of natural calamaties, till a handful of them were mobilized by HelpAge into an Elder-Self-Help-Group (ESHG). One morning they walked all the way to the Area Collector’s office to persuade him to lease them a pond. The group has now developed an integrated and sustainable fish-farming system, enabling each member to earn and pay for his or her needs.

“Every member here earns 10,000 rupees monthly, something which we could never imagine”, smiles Roopmati, a member of the group.

Members of these Elder-Self-Help-Groups have realized that it is easier to work together as a community. Group efforts have led to self-reliance and paved the way to a life of dignity. This initiative by HelpAge India has helped elders set up innumerable ESHGs across the country.

About the program71millioneldersinIndiahavetoworktosurvive.Sidelined,oftenneglected,theseelders live lonely, impoverished and almost invisible lives. It is this vulnerable population that HelpAge India directs its resources to. In order to provide them with income and a self-sufficient life, HelpAge organizes them into viable and sustainable ESHGs. The elders in the group pool their resources to support each other.

These groups of elders need special support to match the productivity of younger groups. Besides helping them access funding and special training, HelpAge India facilitates these groups to federate at local and district levels.

HelpAge India is currently working closely with more than 5,400 Elder-Self-Help-Groups in different states and these numbers are on the rise.

Relief to rehabilitation

Dayal Singh used to run a water mill that could grind wheat, till the 2013 flash-floods of Uttarakhand damaged the mill severely and took away his only source of livelihood. As a rehabilitation measure, HelpAge not just replaced the mill with a modern substitute but also transformed it into a community managed, multi-purpose system of livelihood generation which today serves as a flour and oil mill, a spice grinder, and also supplies electricity.

Life for Singh has changed dramatically for the better since the installation of the new mill.

“There has been a manifold increase in income. Earlier, in one day we used to grind 80 to 90 kilos of grain, today we grind 300 – 350 kilos”, says Singh.

Residents of over 100 villages in Rudraprayag and Chamoli in Uttarakhand have benefited from HelpAge India’s Relief to Rehabilitation program.

About the programDuring natural calamities, the elderly usually fall far behind in the struggle for survival and relief measures. HelpAge India brings succor to the elderly by first building long lasting rehabilitation programs for the entire community. It enables disaster victims like Dayal Singh to form groups and create sustainable ways of livelihood. Master trainers and designers train groups in using available natural resources to create cost-effective livelihood solutions. HelpAge also sets up health and diagnostic centres for immediate relief and keeps them running, thereby building long-term community assets.

SPECiAliZED GERiATRiC CAREback on their feet

Keshav Dutt is suffering from Alzheimer’s, a disease he could neither understand nor afford to treat. It was only when he was brought to HelpAge’s Geriatric Care Services Research and Training Centre (GRTC) based in Pitampura, Delhi, that he started receiving the required rehabilitation measures.

“Earlier he was very irritable, here people talk to him and that makes a difference to him. His condition is fast improving and he can now walk without a stick”, says his wife.

About the programLike Keshav, thousands of elderly Indians want to live life unassisted - with dignity and independence. Thousands more suffer from chronic arthritis, pain and immobility. They are unable to work or even carry out their basic day-to-day activities without support. This lack of mobility and functionality imposes a heavy burden on their family and caregivers, which in turn often leads to elder neglect, abuse and a slow erosion of self-confidence and self-worth. A few sessions of physiotherapy often work wonders and helps put them back on their feet.

HelpAge India supports many such specialized geriatric physiotherapy clinics like the Delhi based GRTC, across the country. Physiotherapy services are provided to the elderly – free for the needy and at subsidized fees for others. Professionally qualified physiotherapists guide people to achieve independence and mobility.

More than shelters, havens

Each morning Krishna’s voice resonates at a local temple in Cuddalore with her melodious prayers, drawing people towards the temple. But not many know about the pain she hides behind these prayers. She lost both her children early in life. At their old age, Krishna and her husband lost all support, with no roof over their heads.

Today, Krishna is a resident of Tamaraikulam Elders Village or TEV, the first Home for the Aged constructed by HelpAge India and her life has taken a three sixty degree turn.

“There was nobody to look after us. We were totally alone without any support and didn’t know whom to turn to, till TEV took us in. It has been 6 years since we came to this HelpAge home. Today it is more than home - in fact it is the best home I have known”, says Krishna.

Krishna is not just another resident of the home, but also the main store manager and accountant, managing the home’s finances. It keeps her happy, active and gives her a sense of self-worth.

About the programSituated at Cuddalore, about 20 km from Puducherry, TEV provides an end-to-end solution for elders with multiple age-friendly facilities. The home is self-sufficient in terms of energy and food, and has adequate medical facilities. The most unique aspect of the home - it encourages active ageing to keep the elders physically active, mentally alert, as well as financially secure, through livelihood-building schemes like farming and making handicrafts. Built by HelpAge, through donations made by the viewers of NDTV, the village is recognized as a model old age residential project by the Government of Tamil Nadu. This free stay facility for the rural poor can accommodate 100 residents and serves as a safe haven for many others like Krishna who have found the warmth of a home again.

PROGRAM DETAilS How we made a differenceIn the year 2014-15, the focus was to increase health outreach, respond to disasters by way of extending relief, continuing with rehabilitation and the inclusion of more and more beneficiaries in the National Rural Livelihoods Mission.

Health outreach increased The health outreach was increased, with 98 mobile medical vans delivering primary healthcare in 110 new locations. Increased numbers also meant that a larger number of elderly and community members benefitted. Many donors, from the public as well as private sector continue to support the program - such as SJVN, CAIRN India Energy Ltd., Asian Paints etc. New donors such as Adani Foundation and Sun Pharmaceuticals added their support.

The HelpAge India - HelpMeSee campaign for ‘Eradicating Preventable Blindness’ restored sight to 35,776 elderly affected by cataract.This was achieved through 18 partner hospitals in 13 states across the country. It is aimed that by the year 2020, at least 1 million elders would be helped with cataract surgeries. The campaign has set out to achieve entire zones free of cataract-caused blindness.

Help Desk for Elders at AiiMS, DelhiThe Geriatric Help Desk at AIIMS, Delhi was started as a pilot, with the intent to facilitate easy access for the elders who visit the Institute. The General OPD of the hospital caters to more than 5,000 patients in a day, of which approximately 10% are elders. Those elderly who are not accompanied by attendants usually need help in scheduling appointments with doctors and availing lab facilities. The Geriatric Help Desks with a ‘Single Window Counter’ based at Raj Kumari Amrit Kaur (RAK) OPD and Geriatric OPD (4th floor) help such elderly who need assistance with appointments, diagnostics, pharmacists or even to be physically escorted to the doctor or another department. The Help Desk is managed by four dedicated elder assistants, monitored and trained by HelpAge, who helped more than 24,400 patients during the year.

Disaster Relief and Rehabilitation efforts continueHelpAge India, responded to natural disasters that affected different parts of the country - Jammu & Kashmir, Odisha, Andhra Pradesh, Assam and Meghalaya floods. The disasters led to loss of life and property in these areas, leaving behind communities vulnerable to outbreaks of disease. HelpAge India was amongst the first organizations to intervene with health relief programs where Mobile Medical Units were deployed to provide primary healthcare, create awareness about key measures for health & sanitation, and prevention of outbreak of water-borne diseases. The mobile units provided more than 35,000 treatments to the affected population.

HelpAge also continued with its rehabilitation program for the elderly affected in Uttarakhand flash-floods, 2013. Under its various initiatives to rehabilitate elders, HelpAge designed and carried out programs to encourage livelihood activities for people from locally available resources such as wool-craft. It also restored sources of livelihoods damaged in the floods, organized women and elders to form community self-help groups and supported them to graduate to the inter-loaning stage. The Health and Diagnostic Centre at Phata village continued to provide primary, secondary as well as tertiary levels of health service to the community. During the year, nearly 11,000 treatments were provided. The treatments ranged from diagnostic tests, specialized advice on gynaecology, ultrasound scanning, geriatric physiotherapy and eye screenings.

livelihoods, Rights & EntitlementsThe Livelihood program of HelpAge India witnessed an increase in coverage of community based organizations of elderly,wheremorethan1,750newElder-Self-Help-Groups were formed. Today, there are nearly 5,400 such groupswithamembershipof71,272eldersin17states.Trainingswereheldfor inclusion of elders in the State Rural Livelihoods Missions.

For the first time, HelpAge engaged with the urban elderly for livelihood programs in Delhi and Bhopal, where 16 Elder-Self-Help-Groups were formed with 192 elders as members. Some of these elders have been linked with income generation activities and old age pension schemes.

Nearly 13,500 elders were linked to the Pradhan Mantri Jan Dhan Yojana within a month of the drive for financial inclusion. This also means that these elders now have access to banking-savings and deposit accounts, remittance, credit, insurance and also pension.

Organic agricultural movement and livelihoods generation were initiated in Bihar where nearly 500 vermi-composting pits were developed to yield organic fertilizer for use in their own farms as well as for sale in local markets.

Another HelpAge India – NDTV partnershipEnthused by the successful implementation of the model home for the elderly in Cuddalore, Tamil Nadu, NDTV once again moved in to support HelpAge India in the construction of a model rehabilitation village complex for elderly in Uttarakhand. This is primarly for those elders who have been severely affected by the flash-floods disaster that devastated the state in 2013. A suitable piece of land on NH 109 in Rudraprayag district was identified and purchased. The Bhoomi Pujan was done by officials from NDTV and HelpAge, and the construction of the facility will begin in 2015-16.

Other programsHelpAge’s geriatric physiotherapy centres, elders’ helplines, homes for the aged continued to make a positive difference in the lives of the elderly. These services enable the elderly to lead a dignified life where they are less dependent on others for their activities of daily living, psychosocial support, shelter etc. The geriatric physiotherapy programprovided 1,02,785 physiotherapytreatments in 23 states.

Elder Helplines were successful in rehabilitating more than 850 elders who were rescued either from the roadside in a debilitating state or had other reasons forcing them to look for shelter outside their homes.

Empowering through informationMost elders in India are unaware of not just their rights and entitlements, but also of the existing government welfare schemes. HelpAge organized numerous State Level Meets of Elders to generate awareness about various government schemes they are entitled to such as those related to old age pension, health insurance and food security. More than a thousand meetings were organized with Senior Citizen’s Associations in urban areas. In the rural areas, interactions of elders with Block Development Officials were also arranged to enable elders access their due entitlements from the government.

Ms. Anuradha Sen, Joint Director - Advocacy (East), listens attentively to the concernsof75 yearold ‘Boro Dada’ as he is popularly called in Labpur village, Birbhum District, West Bengal. This interaction took place during the State Level Meet of Elders where they were given information about their rights and entitlements.

Ms. Anita Gupta, Advocacy Officer, HelpAge India, Mumbai, Maharastra, explains to elders how to access the State Pension and Health Schemes, and also how to use the HelpAge Elder Helpline (1800-180-1253) to service their queries.

Ms. Meera Midha, a young volunteer from HelpAge India teaches senior citizens how to use Google maps at the HelpAge AdvantAge Centre in Neb Sarai, New Delhi. This is part of the Digital Empowerment drive by HelpAge India, to enable elders access information about their rights and register for government schemes online.

YEAR HiGHliGHTS 2014 -15HelpAge India launched report - ‘State of India’s Elderly’HelpAge India released the ‘State of India’s Elderly – 2014’ in New Delhi. The report launched by Mrs. Justice (Retd.) Leila Seth is a compilation of articles and factoids that serve to highlight the dichotomy between an apparently happy picture of increased longevity and the reality of long years of hopelessness, bereft of family, society or state support that our elderly face today.

Today, India is home to one out of every 10 senior citizens of the world. The nationwide dependency ratio of elderly to the general population is 13.1%. There are numerous problems that need to be addressed urgently, particularly those that affect the underprivileged sections. Some need immediate intervention by State and Central governments. Others, such as ‘Elder Abuse’ highlight a national shame that requires both introspection and action from society at large.

HelpAge released a disturbing report on Elder Abuse in india HelpAge India released a nationwide report on “Elder Abuse in India (2014)” marking ‘World Elder Abuse Awareness Day’ (June 15). Released nationally, the aim of the report was to bring to light the alarming growth of Elder Abuse in the nation, with the scale of abuse increasing drastically to 50% from 23% the year before, when a similar study was conducted.

The report spanned 12 cities across 8 states, covering both metro and non-metro cities. What was most disconcerting, was that the abuser was a trusted source from within the elder’s family, with the daughter-in-law (61%) and son (59%) emerging as the topmost perpetrators.Notsurprisingly,77%ofthosesurveyed,livedwiththeirfamilies.

Various events were held across the country to raise awareness about the issue and urge people to come forward and show their support. Signature campaigns, candle light marches, interaction with Senior Citizen’s Associations and students were held across the nation to garner support for the fight against Elder Abuse.

The youth celebrate international Day of Older PersonsInternational Day of Older Persons was marked by HelpAge India on October 1, 2014 across the nation. In Delhi, 5,000 senior citizens marched proudly at India Gate during the annual HelpAge India - Ministry of Social Justice & Empowerment inter-generational walkathon. Similar walks were held across the country in 22 locations.

Elders support EldersSenior citizens who are relatively better off are among the first to help and support the cause of disadvantaged and needy elders. During the year, there were several occasions when HelpAge enabled Senior Citizen’s Associations to collectively lend their might to provide relief to the less privileged seniors.

When floods hit the state of Jammu & Kashmir, Senior Citizen’s Associations of Triveni Senior Citizen’s Forum, Sheikh Sarai (Phase-I) and Senior Citizen’s Society of Freedom Fighters Enclave, Neb Sarai, both in Delhi, came forward to support HelpAge India’s flood relief work in the state.

Over 500 senior citizens along with students join the inter-generational Walkathon sponsored by the Ministry of Social Justice and Empowerment with the theme ‘Get Active, Stay Active’ in Guwahati, Assam.

School children take the SAVE pledge to love, respect and care for the elderly in Dehradun, Uttarakhand. Students of more than 23,500 government schools in the state pledged their support to the cause.

HelpAge India MMUs reached many faraway locations and in total covered 85 sites in Srinagar, Shopian, Baramulla, Bandipora, Pulwama, Budgam, and Jammu.

FOR HElPAGE inDiAOver the years the work of HelpAge India has earned multiple awards and citations in recognition of its service to the community of senior citizens, particularly the disadvantaged. These accolades are received with all humility by the organization on behalf of its hard working staff who work in difficult circumstances and often, in inhospitable environments.

Such awards serve to encourage HelpAge India to strive for even more daunting tasks and attempt to reach far-flung corners of the country, wherever the elder community finds itself helpless and without hope. HelpAge thanks the various organizations and institutions for reposing their faith and belief in the cause and the services it renders. This underlines the fact that working for the elder cause is both a crying necessity and a great responsibility that the organization must continue to address with all earnestness.

Asian Paints continues its extensive supportAsian Paints, Khandala Unit supported one Mobile Medical Unit (MMU) in Khandala, Maharashtra while the Kasna Unit supported two in Rohtak, Haryana and Kasna, Uttar Pradesh. The Kasna Plant also supported the specialized physiotherapy centre at Pitampura, New Delhi. Asian Paints has also supported MMUs at Patancheru, Andhra Pradesh and Sriperumbudur, Tamil Nadu.

Larsen & Toubro, through its charitable Trust, has provided support towards one Medical Mobile Unit in Bengaluru, Karnataka in 2014-15, and has recently renewed their support for another year.
