2. Medical Insurance - Preface

Your company has chosen NLGIC to manage your health care, so you can enjoy medical care when you really need it. The Scheme

provides insurance cover for the benefits detailed below, subject to the terms, limits, conditions and exclusions of AL BADI GROUP medical

insurance policy.

This Benefit Guide sets out the benefits and rules of your healthcare cover. Please read this guide in conjunction with your Table of Benefits,

Medical Card and Insurance Certificate (available on request).

This guide details the geographical area of cover that your company has chosen for you and your dependents (if applicable) as well as the

start date and renewal date of your cover.

Your Table of Benefits outlines the plan(s) selected by your company and the associated benefits available to you. In addition, it specifies

any benefits/treatments which require submission of a Pre-authorization and confirms any benefits to which specific benefit limits, waiting

periods, deductibles and/or co-payments apply. Your Table of Benefits is issued in Omani Rials.

For full details of your company’s insurance contract, please contact your company’s HR. Please note that the terms and conditions of your

membership may be changed from time to time by agreement between your company and us.

Important Note:

This booklet is just a brief reference guide regarding the benefits available under the policy and the claims procedure for

availing the benefits. For a full list of the Benefits, Exclusions and Definitions; please refer to Medical Claims Administration

Service Agreement (TPA). This guide does not replace or override the original policy and the contract signed between the

insurance company and your organization.

Territorial limits

Elective treatment: The principal residence of covered persons under policies covered under this scheme must be

CAT A – Worldwide excluding USA & Canada

CAT B – Oman, Arab Countries, SEA, Home Country excluding USA & Canada

CAT C – Oman, India, Bangladesh, Pakistan & Home Country excluding USA & Canada

Emergency treatment: Worldwide excluding USA & Canada during emergency whilst the member is on business or holiday trip. (Maximum

Inpatient stay – 45 days outside Oman).

CAT A & B - Inside Oman outside Network & Home country- 10% co-pay

CAT C - Inside Oman outside Network - 10% co-pay

Eligible Members

Primary Insured:

All full-time employees and trainees of AL BADI who have not reached their sixty-fifth (65th) birthday in the Insured’s offices in the country of

coverage, Members under Cat A & Cat B can be covered up to 70 years subject to providing their proof of coverage under the previous year

policy/Medical card copy. No coverage above 70 years.

Eligible Dependents can also be covered under the Medical Benefits with the same terms and conditions. Eligible dependents include:

Spouses up to 65 years

Unmarried dependent children between the ages of 14 day (subject to baby born healthy) and 19 years residing permanently with

the insured person in Oman.

Coverage in respect of the unmarried dependent children shall be extended up to 23rd birthday subject to full time student in an

accredited College or University.

The term dependent shall mean legal wife or husband of an employee or the person living with an Employee in a recognized husband and

wife relationship, who is registered as such in the records of the Insured and Employee’s unmarried children, step children and children

legally adopted, who are within the age limits.

Please note that brothers, sisters, parents, cousins, nieces, nephews and other relatives are not eligible for inclusion under the policy.

The eligibility for the cover shall be guided by AL BADI’s Human Resources policies, within the criterion as mentioned above.

3. Claims Process / Procedure

Direct billing (Cashless Facility) Procedure

Direct billing or Cashless Facility is a facility offered to the insured members whereby the provider network hospitals will provide the

treatment and claim the bills directly from National Life. In other words, the employee need not pay and seek reimbursement.

List of Network hospitals/clinics in Oman is attached as per Annexure 1

Procedure for Treatment in Network Hospital

Present your medical identity cards at the reception

The provider will complete the relevant sections of the claim form

Sign the claim form

Pay the deductible excess if applicable

Consult the Physician/specialist

Carry the relevant portion of the claim form for the procurement of medicines

Reimbursement Procedure (With prior approval only for planned/non-emergency treatment)

In case you have visited a doctor outside the provider network, you have to make payment for the treatment and submit the following to

National Life within 60 days from the date of treatment.

Original claim form duly filled, signed & stamped

Doctor’s Prescription for the medicines

Original detailed and dated receipt

Pre-authorization form, if applicable

Detailed medical and diagnostic and lab reports

Detailed Discharge Summary (for Inpatient Treatment)

Copy of Membership card

All the above documentation should be submitted to NLGIC who will in-turn arrange for the reimbursement directly to the

employee concerned by way of bank transfer, as per the Table of Benefits / Term Sheet as attached in Annexure II, the Policy Contract

applicable to your membership and specific insurance scheme, within 14 working days from receipt of all necessary claim documentation.

Important: Please note that well documented claims, received within the reporting period, will assist us to settle claims in a faster and

more efficient way.

Claim Denials

Insurers have the right to decline or return submitted Claims, under the following conditions:

Submitting incomplete claim form.

Attaching photocopies of receipts.

Tests, drugs and treatments not prescribed by Physicians.

Diagnosis and Treatment are not medically relevant. Any decision of what constitutes diagnosis and treatment not being medically

relevant rests with your insurer and any such decision shall be final.

Tests, drugs or treatments for which pre-authorization is required in accordance with the terms and conditions of this policy, but

for which the pre-authorization has not been obtained

Services received are excluded under the policy

Expenses in excess of the Reasonable and Customary charges

Claims are submitted after ninety (60) days from the date of treatment

Expenses exceeding annual maximum limits

Treatments after the policy has expired

Treatment was before the insured member’s joining date or before the effective date of the Policy.

Appeals on Claim Denials

Settlement of eligible claims shall be considered final unless objections along with supporting justifications are received in writing along with

relevant reports and facts, within a maximum of one (1) month from the date of receiving the payment/denial. The insurer reserves the right

to deny any objections received after the said period. Insurer will review the justifications received and payments for any approved claims will

be made within thirty (30) days from the date the justifications were received.

4. Pre-authorization of Treatment

This is the validation process where NLGIC determines if your treatment costs are covered and to what limits, as per the Table of Benefits

and Policy Contract applicable to your membership and specific scheme.

Preauthorisation must be obtained for the following conditions:

All Inpatient Treatments (Hospital)

Major investigations such as MRI Scans, CT Scans, and Physiotherapy etc. (List attached)

Who will obtain Pre-authorization?

1- When using Designated Providers - The Designated Provider will seek Pre-authorization on your behalf.

2- When using Non-Designated Providers - You or your Non-Designated Provider must seek pre-authorization from NLGIC.

How?

(1) Call the Emergency Assistance no. at the back of the Member Identity Card

(2) Inform the NLGIC or its representative as mentioned above (Claim registration number need to be provided)

Important: An emergency condition does not require any pre-authorization. However, such conditions are to be reported to NLGIC

within 48 hours.

5. Important definitions

Annual

maximum limits Total amounts that may be claimed in any one (1) policy period by an Insured member. These limits are shown in the

Table of Benefits

Chronic

condition An Illness that persists, or would persist over a long period of time, which is not curable and requires regular

maintenance treatment.

Coinsurance An insurance participation percentage borne by the Insured Member towards the cost of Eligible Medical Expenses

after he/she has paid his/her applicable Deductible amount. The Coinsurance is specified in the Table of Benefits and

is expressed as a percentage of Eligible Medical Expenses after deduction of applicable Deductible amount.

Deductible A specific amount of Eligible Medical Expenses that must be incurred and paid for by the Insured member before

benefits are payable under this Policy. This amount is payable by the Insured member per each Outpatient

Consultation per visit, per illness, except for a follow-up outpatient visit within seven (7) days for the same Illness and

at the same Physician or Consultant. If the Eligible Medical Expenses are less than the Deductible amount, then the

Insured member will be liable to pay all the expenses incurred. The Deductible amount is stated in the Table of

Benefits and Membership card.

Emergency Emergency can be affirmed in case of an accident, a disaster or any sudden onset or worsening of a severe illness

resulting in a medical condition that presents an immediate threat to the insured member and therefore requires

urgent medical measures. Only medical treatment through a physician, medical practitioner, or specialist and

hospitalization that commences within 24 hours of the emergency event will be covered.

Nursing at

Home Rendering the medical services of a nurse in the Insured member’s home in the country of residence when

prescribed by a Consultant and related directly to an illness for which the Insured member has received inpatient

treatment in accordance with the terms and conditions of this Policy. This benefit is provided in lieu of a hospital

admission where a skilled nurse, under the supervision of the treating consultant, can provide the necessary care at

home for the remaining length of stay of a particular admission or procedure.

Pre-existing

condition Any bodily injury or Illness or its related condition that medically exists prior to the enrolment date of the Insured

member, whether it is known or not known to him, and necessitates the insured member to receive care and

treatment.

Reasonable

and customary

charges

The amount NLGIC recognize for payment for a particular medical procedure/treatment/medication. It is based on

what is considered “reasonable”

for that procedure/treatment/medications in accordance with the market price, in the country where the treatment (if

eligible as per table of benefits) was provided or in the applicable band of designated providers (as specified in the

table of benefits) in the country of residence, whichever is less.

6. Frequently Asked Questions

1. What happens if I exceed my Benefit Limit(s)?

Your scheme only covers you to the maximum financial limits indicated in the Term Sheet. You will not be reimbursed for any costs

which exceed your Annual Maximum Limit, or the benefit sub limits.

Where you incur costs at one of NLGIC’s Designated Providers and the cost of treatment is in excess of the remaining balance of your

benefit limits, you will be responsible for payment of the excess cost amount to either NLGIC or the treating Designated Provider.

2. What shall I do if I lose my Membership Card?

NLGIC will provide you with a membership card which identifies you as a member of the NLGIC Medical Insurance Scheme. Your

membership card must be produced as proof of membership when seeking medical treatment from our Designated Medical Providers.

Should you lose your membership card, inform your HR to request NLGIC to issue a replacement card. NLGIC will issue a

replacement card, which you should receive within one week from the date that the replacement card was requested.

3. Why should I seek treatment from the Designated Providers?

Designated Providers participating in NLGIC’s Network are committed to quality care.

NLGIC constantly assesses new hospitals and medical facilities for inclusion in the Network. Where any providers meet NLGIC’s

quality criteria, they will be added to the Network and you will be regularly updated with the network lists.

NLGIC is endeavouring to manage your insurance programme, professionally and efficiently. NLGIC has better control with the

Designated Providers, which will eventually be favourable to your company.

Kindly note that, as per policy terms and conditions, if an insured member pays for the treatment at a designated provider

the insurer will only reimburse the insured member the agreed charges between the insurer and the designated provider.

• Organ transplantation shall cover the organ transplantation as recipient excluding any cost related to donor, and

excluding the acquisition and organ cost

• Treatment outside Sultanate of Oman is subject to reasonable & customary charges in Sultanate of Oman.

• Hormonal Replacement therapy shall be covered excluding infertility related hormones and growth hormones

• Acts of terrorism are not covered

Notes

1. Full Refund is subject to annual maximum limit and Sub-limits per person

2. Treatment for emergency conditions shall not require pre-authorization, but such cases are to be notified to the

company within 48 hours of the emergency treatment

3. Utilization of medical expenses above the Policy Annual limit or any of the Benefit Limit will be borne or recovered

from the member or policy holder

4. Reimbursement claims (with prior approval only) should be submitted within 60 days from the Date of treatment in

medical Reports & medical Bills. Resubmission and submission of additional requirements should be submitted within

30 days from the date intimation from claim team.

5. Members not using Medical Card in our Network panel Hospital / Clinic / Medical Centre will be reimbursed with our

agreed tariff rate with that center.

6. Insurer reserves the right to include / exclude / upgrade banding / degrade banding of any clinic or hospital at any

time from the designated provider network list.

7. Any Upgradation of Network in between the Policy period is not allowed or will be charged full Contribution.

8. In case of any deletion after which card is not returned to the insurance company and is utilized, the claimed

amount will be debited to the Insured/Company.

9. All other Terms and conditions are as per Insurer’s standard policy wordings and conditions mentioned in accepted

Quotation is also applicable.

10. Covid-19 is covered within the territory of Sultanate of Oman and is subject to the guidelines issued by Capital

Market Authority and Ministry of Health.

8. Annexure III – Exclusions

The items procedures and medical conditions listed below and their related or consequential expenses are excluded from the coverage

provided under this policy unless specifically stated to be included in the table of benefits or endorsement(s) to this policy:

1. All pre-existing conditions, unless agreed otherwise in the schedule of benefits. Unless otherwise provided for under the plan and listed in

the table of benefits, treatment of chronic conditions including palliative treatment and routine follow up consultations.

2. Services, accommodation or treatment charges incurred in health hydras, spas, nature cure clinics, rest homes or any similar place even if

it is registered as a hospital, residential stay in a hospital or any similar institution arranges wholly or partly for domestic reasons and which is

not directly related to treatment, or beyond the period required for recovery from treatment ,Services received before the effective date of

coverage or during an inpatient stay that began before the effective date or services received after coverage ends.

3. Routine ear examination, routine foot care, Routine I Preventive medical examinations or check-ups, vaccinations, inoculations, medical

certificates and examination for residence, employment or travel.

4. All Dental, Optical and Maternity benefits, unless listed in the table of benefits.

5. Any pharmaceutical products which are not on the approved list of drugs and which are not considered to be medically necessary for the

specific treatment of the medical condition or bodily injury as per the list in Annexure-II attached to the policy contract.

6. Cosmetic treatment or circumcision unless medically necessary and pre-authorized by the insurer.

7. Tests or treatment related to contraception or sterilization, infertility, impotent, sexual dysfunction or any similar condition.

8. Birth defects, developmental disorder, congenital illness or hereditary conditions, deviated nasal septum.

9. Treatment of mental illness, psychiatric, behavioural disorders, psychological disorders including but not limited to anxiety, insomnia,

homesickness, unless listed as covered in the table of benefits.

10. Any treatment or test for acquired immune deficiency syndrome (AIDS), HIV and all sexually transmitted diseases, its complications and

consequences.

11. Self - inflicted I intentional injury, suicide, alcohol or drug addiction/abuse or use of intoxicating/narcotic substances, impairment of an

insured persons intellectual faculties by abuse of stimulants or depressants or by the illegal use of solid, liquid or gaseous substances.

12. Treatments resulting from racing of any form, treatments to injuries arising out of participation on behalf of organized amateur or

professional sports team and participation in hazardous sports/activity including but limited to scuba diving, dune bashing/biking,

parachuting, hand gliding, rock or mountain climbing etc.,

13. Treatment for any illnesses or injuries resulting from active participation in war, riots, illegal activity, civil disturbances, terrorism, acts

against any foreign hostility, whether war has been declared or not treatment for any medical conditions arising directly or indirectly from

chemical contamination, radioactivity or any nuclear material whatsoever, including the combustion of nuclear fuel/weapons.

14. Treatment received outside the territorial limits described in the table of benefits and/or expenses incurred where the insured has

travelled against medical advice. Elective Treatment unless pre-authorized by the insurer.

15. Costs incurred in connection with locating or the acquisition of a replacement organ or any costs incurred for removal of the organ from

the donor, transportation costs of same and all associated administration costs.

16. Prosthesis or implants /devices (including corrective) which are used for replacing a functional or structural loss of a particular body part

or an organ and medical appliances that are not surgically required.

17. Any Treatment not considered as medically necessary or any substance not considered as medicine such as but not limited to tonics,

slimming pills. Reconstructive/Plastic surgery unless necessitated due to accidents, Aesthetic/Cosmetic Treatment/Preparations including

but not limited to hair removers, skin care products, moisturizing lotion, creams and other similar products, scalp and hair lotions, alopecia,

wigs/ toupee, shampoos.

18. Any medical prescription relative to a special diet, weight control/modification/maintenance, children's food, baby supplies, milk formulas,

food supplements etc.

19. Vitamins unless prescribed along with antibiotics or during antenatal period (if maternity benefit is covered for the member).

20. Treatment not ordered or received from a registered medical practioner or physician, complementary medicine applications such as

Chinese acupuncture, chiropractic and osteopath

21. Treatment of any allergic condition or disorder or tests, however, the initial visit to diagnose an allergy will be covered.

22. All substances which are not considered as medicines such as but not limited to mouthwash, toothpaste, lozenges, antiseptic solutions

etc.

23. Home visits unless it is an emergency as defined in the policy definitions.

24. Hormone replacement therapy (HRT) unless carried out as part of or immediately after a surgical procedure which is covered under the

table of benefits to this plan.

25. Skin disorders like warts, keloid, acne and Mollusum contagiosum.

26. Any treatment or test, second or subsequent opinion for which the required insurer's pre -authorization is not obtained, stay in hospital for

domestic reason where no active regular treatment is given by a specialist.

27. Benefits recoverable under other insurance policies like workmen's compensation act insurance, Motor insurance, social insurance,

travel insurance and personal accident.

28. Kidney dialysis.

29. Claims directly or indirectly occasioned by happening through, or in consequence if, aviation, other than as a fare paying passenger in a

fully certified passenger carrying aircraft, flown in the course of licensed operation for the transportation of passengers by properly licensed

crew.

30. Consultations or Treatment of speech and voice problems.

31. Treatment for Loss of hearing unless caused by a medical condition covered under the policy, hearing aids, ear and body piercing.

32. Experimental unproven treatment or drug therapy, Bariatric Surgery, Sleep Study, drugs/medical supplies not forming part of diagnosis.

33. Claims for Genetical disorders, cryopreservation, implantation of living cells or living tissue (including but not limited to stem cell

treatment), whether autologous or provided by a donor.

34. Treatment for anorexia nervosa, Bulimia, loss of appetite and other such eating disorders or any form of treatment by a psychologist.

35. Medical practitioner fees for the completion of a claim form or other administration charges.

36. Investigations into and treatment of obesity

37. Sex change operations and related treatments.

38. Expenses incurred because of complications directly caused by an illness, injury or treatment for which cover is excluded or limited under

your plan.

39. Diseases defined by the WHO as epidemic

40. Expenses incurred in relation to treatment for Systemic Lupus Erythematosus (SLE), Multiple Sclerosis, Ulcerative Colitis, Psoriasis,

Crohn’s disease, Immune Therapies (Immune Suppressants, Modulators and Stimulators).

41. Charges by a provider for telephone consultations.

10. Annexure IV – Treatments which require Pre-Authorization

Preauthorization is required before the insured undergoes any of the following medical services:

1. Inpatient treatments implying all treatments where the Insured member requires hospitalization for a minimum of one (1) night, for

specialized medical attention and care, before, during and after the treatment. Such hospitalization can be for medical

management or surgical interventions which cannot be confined within the purview of outpatient / day care management.

2. Day care management or procedures wherein neither a continuous 24 hours hospitalization is involved nor is within the scope of

Outpatient treatment.

3. All Outpatient surgical procedures, including but not limited to:

a) Incision and drainage

b) POP application

c) Chalazion excision

d) Sebaceous Cyst/ Dermoid Cyst excision.

4. All medical imaging studies including but not limited to the following:

a) MRI (Magnetic Resonance Imaging)

b) CT (Computerized Tomography)

c) IVP (Intravenous Pyelogram)

d) Mammogram

e) Hysterosalpingogram

f) Bone Densitometry

g) Doppler Studies

h) Barium Studies

i) MCU (Micturating Cysto Urethrogram)

Please note that routine X-rays do not require preauthorization.

5. All endoscopic procedures including but not limited to :

a) Gastroscopy

b) Colonoscopy

c) Sigmoidoscopy

d) ERCP

e) Cystoscopy

6. Cardiac Studies including but not limited to:

a) Echocardiogram

b) Stress Echo

c) TMT – Tread Mill Test

d) Holter Monitoring

e) Ambulatory Blood Pressure Monitoring

7. Investigations and treatment for Oncology and related diagnostic investigations including but not limited to:

a) Fine Needle Aspiration Cytology

b) Surgical biopsy

c) Histopathology

d) Pap Smear

8. All pre-procedural serology tests including AIDS and all tests related to viral serology including but not limited to:

a) Rubella

b) CMV (Cytomegalovirus )/ Toxoplasma/ Malarial parasite

c) Herpes

d) Viral Hepatitis

9. All hormonal tests including but not limited to:

a) Thyroid Function Tests

b) Follicle Stimulating Hormone

c) Luteinising Hormone

d) Prolactin

e) Testosterone

10. Neurological investigations including but not limited to:

a) Electroencephalography

b) Nerve Conduction Studies

11. Intra Articular Injections including but not limited to:

a) Injection Hyalgan

b) Injection Depomedrol

12. All vitamin and mineral estimations including but not limited to:

a) Vitamin D

b) Vitamin B 12

c) Calcium

d) Magnesium

13. Physiotherapy.

14. Nursing at Home

15. Long term medications for more than 30 days.

Note: Treatment for emergency conditions shall not require pre-authorization, but such cases are to be notified to the company within 48

hours of the emergency treatment.

Emergency shall mean: medical treatment following an accident, a disaster or any sudden/unexpected beginning or worsening of a severe

illness resulting in a medical condition that presents an immediate threat to the insured member and therefore requires urgent medical

measures by a doctor to prevent long term impairment of the insured health.

12. Introduction to National Life Mobile App

NLGIC Mobile insurance application is a comprehensive Reimbursement and Pre-approval claim management solution that connects the

members, clients and brokers. Users can view Reimbursements, Pre-approval & relevant status.

1. Download NLGIC Mobile App

NLGIC Mobile App can be downloaded from http://www.nlicgulf.com/Nlgic-apps.html or from Google Play, App Store or Windows Mobile.

2. Registration

In order to use NLGIC Mobile App, Registration is required for the first time. Provide Name, Member ID (as per NLGIC Medical Card), Date of

Birth, Mobile No, Email and Password. Please note that the password must have at least one numeric character. Member and Policy must

be active during the registration. All fields on the page are mandatory for registration.

Please refer to your NLGIC Medical Card for your Member ID. On successful registration, you will receive an email notification to your
