Nursing care about Hypertension

The patient is Mr “F” , he is 49 years old , he’s Male and he’s Married , the religion is Islamic the address is Batanghari street .General condition of the patient looks Medium sickness , bed rest in bed and installed infusion as 20 drops/minute in his left hand . From the subjective data the patient said the leg and arm felt pain. limp on the right foot and sometimes cannot moved and objective data the patient, patient appears week , sometimes patient week get angry , cause the pain and the vital signs of the patient are described as follows the blood pressure is 150/mmHg , the temperature is 36 Celsius , pulse is 80x/mmHg and respiratory is 24x/minute.

The result of the medical checkup showed that suffered from hypertension. It was indicated from the fact he felt week and headache. Theoretically, hypertension is caused by the increase of the blood pressure. Of the patient 150/100 mmHg. It indicates that is less than the normal limit of blood pressure 120/80mmhg.

The next phase of nursing care is intervention phase , I needs to plan any treatment of patient in the case of the patient initially Mr.F, I should examine his general condition , monitoring vital sign capital refill , furthermore I should raise the head position in bed .in addition , I also needed to implement give cold compress for relaxation techniques . Furthermore for the better health improvement, I should collaborate with his family and the medical team of the hospital. Related to the medicine, there was kind of the medicine that would be given. I.e. tolozoline 3x/days for headache.

The next phase of nursing care implement, in the implement phase, I should implement intervention that have been made for the treatment of patients .the schedule of the treatment were described as follows. In the morning I should check the patients general condition , next I should give patient eat , after that I must give oral medication to patient ,

furthermore , I measured vital signs , and patient had fever , so I collaborate to give cold compress , next I observed another patient’s general condition .

The last stage of nursing care is evaluation; I must conduct an evaluation after the implementation of the Mr.F. From the subjective data after intervention the patient said the pain and headache were little left , as well objective data after the intervention , in the patient looked bed rest in bed , the patient relaxed in bed . The vital signs showed blood pressure was 140/100mmhg, temperature was 36 Celsius , respiratory was 24x/minute , phase was 86x/minute . Furthermore last Assessment phase the patient still felt headache and planning before patient left intervention was continued.
