Permohonan PENDAFTARAN PENANAMAN MODAL ini diajukan kepadaKantor Pelayanan Terpadu Satu Pintu (PTSP) untuk rencana penanaman

modal dalam rangka Undang-Undang Nomor 25 Tahun 2007.This INVESTMENT REGISTRATION is herewith submitted to the One DoorIntegrated Services Office for the purpose of investment under theInvestment Law No. 25 of 2007.

Permohonan ditandatangani di atas materai cukup oleh seluruhpemohon (bila perusahaan belum berbadan hukum) atau olehdireksi perusahaan (bila perusahaan sudah berbadan hukum)dilengkapi Surat Kuasa bermaterai cukup untuk pengurusanpermohonan yang tidak dilakukan secara langsung olehpemohon/direksi perusahaan. (ketentuan tentang surat kuasa

diatur pada Pasal 63 Peraturan ini).

This application should be properly and duly signed with sufficientstamp duty by all applicants (if the company is not yetincorporated) or by the companys Board of Directors (if thecompany is already incorporated) attached with Power of Attorneywith sufficient stamp duty from whom signs and/or submits theapplication if the applicant is represented by another party(provisions concerning the Power of Attorney is strictly regulated inthis regulation (article 63).
