Welcome Parents!

Congratulations! By enrolling your child in the DAFFODIL Pre-K at

Memorial Drive you have taken a very important step in your child’s education.

You are encouraged to continue to share in your child’s education this year.

Preschool is a time to develop a firm foundation to ensure success in elementary

school years. It is a time of action and discovery with many experiences planned to

help your child develop positive self-concepts, accept responsibility, follow

directions, work and share with others, and develop good habits in personal

hygiene.

We are looking forward to working with your family and your child at

school, in the community, and in your home. Together we can make this

experience for your child successful and happy, and ensure that your child will

enter elementary school ready to learn.

Remember that we share a very important concern: Your Child.

Enrollment/Eligibility/Withdrawals

Enrollment Policy Enrollment for DAFFODIL Pre-Kindergarten Program is open and nondiscriminatory. All

children living in Ware County who are four years of age on or before September 1 of the current

school year are eligible to attend. No one is denied participation on the basis of race, color,

national origin, sex, or disability.

When the number of enrolled students reaches the Program’s capacity (approximately 330),

the names of students applying after reaching initial capacity are placed on a waiting list in the

order in which they applied. When vacancies occur among the enrolled students, students are

transferred from the waiting list to program enrollment in the order in which they applied for

DAFFODIL.

Enrollment Requirements For enrollment into DAFFODIL each child must have the following:

A certified birth certificate with the state file number.

A certificate of immunization (Form 3231).

A child’s social security card for school records.

Proof of Residency (electric bill, water bill, cable bill).

Withdrawal Procedures In order to withdraw a student from DAFFODIL, the school is required to:

Get prior approval of the Department of Early Child Care and Learning

Request in writing to the Pre-K Division Director, along with documentation of attempted

interventions used to help the child benefit from the program. The report must also include

meeting notes showing ongoing involvement of parents.

Attendance The Georgia Pre-Kindergarten program provides an opportunity for all Georgia four-year-olds to

attend. The DAFFODIL at Memorial Drive school day begins at 7:45 AM and ends at 2:15 PM.

Attendance is crucial to the Pre-K program. The DAFFODIL program has 14 full classrooms at the

main site and one at Memorial Drive with a limited enrollment 330 students; therefore it is very

important that students attend on a regular basis and for the full day. Families enrolling children

in the program must agree to send the child for the full school day, five days a week for the entire

school year. After the 2nd

consecutive absence, the teacher will call the parent and submit an

attendance referral to their principal. After the 3rd

consecutive absence, the principal will follow-

up.

5 or more excused or unexcused absences, tardies, and or early check-outs will result in an

attendance warning letter to be sent to the parent/guardian.

10 or more excused or unexcused absences, tardies, and or early check-outs will result in a

2nd

letter that will require a school conference with the date and time attached. At the

conference the principal, teacher, social worker and parent/guardian will discuss attendance

problems and resolve any issues.

15 or more excused or unexcused absences, tardies, and or early check-outs may consist of

contacting Bright from the Start to determine possible dis-enrollment.

Perfect Attendance

A student qualifies for a perfect attendance award when he/she has not been absent and has not

accumulated seven (7) unexcused tardies and/or seven unexcused early checkouts for the calendar

year.

The DAFFODIL school calendar follows the Ware County School System calendar with

the same starting and ending dates, holidays, early-dismissal dates, and teacher workdays.

Arrival and Dismissal Procedures

DAFFODIL Pre-K at Memorial Drive school hours for children are from 7:45 AM-2:15

PM. The car rider area is open in the front of the school from 7:10 AM until 7:40 AM. Students

arriving after the tardy bell must report to the school office.

Special Note: For the first and last weeks of school, parents will be allowed to walk their

child/children to the classroom in the mornings while the children are making adjustments.

After this time, parents need to park and walk into the building to sign your student in. This

will help establish a routine and assist with the safety of your child.

The safety and welfare of our students and staff are our district’s highest priority. We employ a

variety of security measures in our daily efforts to provide safe and secure learning environments.

These measures reflect our community’s high expectations for school safety. As a result, the Ware

County Schools, in an effort to increase the safety of our children, will implement the following

changes:

1) Students arriving after the tardy bell must be escorted into the building by an adult. The

adult must sign the student in as tardy (Pre K, Elementary and Middle School students).

2) Student transportation changes and student early checkouts will no longer be conducted or

accepted over the telephone. Any request for a change in student transportation or student

attendance must be presented in the form of a written note from home to his or her

homeroom teacher. The note must contain the following specifics:

Parent name and phone number of person requesting the transportation change or

early check-out so that verification can be made (All notes must be verified prior

to granting the request).

Full name and address of person picking up student.

Address to where the student will be taken (must be within the attendance zone).

Changes for students being transported by the district’s transportation department

must be authorized at the building level. Please do not call the transportation

department.

3) The parent or designated person MUST present a picture identification before the student

will be released (ex. Driver’s License). NO EXCEPTIONS!

4) Any designated person seeking to sign out a student must be listed on the School’s

Registration Form as an individual authorized to pick up the child. Please make sure

this information is always current and accurate.

Parents of children who are car riders in the afternoon MUST have their child's Car TAG

visible in window or you will be asked to park and show identification.

Special Note: Teachers will bring the students to the front and staff members will put

them into cars to go home. Please make sure you have the child’s car tag or you will be asked

to go to the front office to show identification.

Car Tags / Bus Tags All students will be given a car/bus tag at orientation. The classroom teacher will make a

total of THREE tags for your child throughout the school year. If your child loses his/her tag, a

new one will be issued until the third one is used. As tags are left at home, please return them back

to school so that the teacher will have them on hand in case one is misplaced another day. Each

student will be given two car tags. When picking students up as car riders or walking up, please

make sure that the tag is visible (hang from your rear view mirror). If you do not have a car tag,

you must park and walk in to check out your student.

Student Discipline No employee shall use any form of corporal punishment (which includes, but is not limited to,

paddling, striking, or hitting) as a method of discipline at DAFFODIL. This applies to parents

also. Pre-K teachers are encouraged to involve the (1) parents; (2) principal; and (3) community

resources in assisting with discipline, if needed. All teachers are trained in Handle with Care. It

is a positive protective restraint program used if a child is hurting himself or others. If a

child has to be restrained, the parent will be called immediately.

Suspension Procedures

Immediate suspension can be made at any time there is a situation when a child is causing harm

to himself/others or exhibits disruptive behavior in class.

Parental Involvement We believe that parent participation is essential for a successful preschool experience.

Therefore, the DAFFODIL Preschool Center strongly encourages parents to be actively involved in

our program.

We encourage parental involvement in the following ways:

Attending parent education sessions.

Attending scheduled parent/teacher conferences.

Reinforcing at home what your child learns at school.

Participating in field trips and classroom activities.

Visiting and working with your child in the classroom.

Checking out learning materials from the Parent Lending Library

Classroom Visitation

ALL VISITORS MUST SIGN IN WITH THE SCHOOL OFFICE. Visitors (including parents) will be asked to show a valid ID. All who visit will be issued a

visitor’s badge that must be worn while in the center. Badges should be returned to the office

when leaving.

Each teacher has a “sign-in” sheet in the classroom. Please remember to always sign in as

you enter the room. This helps us to document parent participation.

Health and Safety The school system works cooperatively with other government and community agencies

to prevent, assess, manage, and follow-up on any threat to the health or safety of the students,

faculty, or staff. At various times throughout the school year students will participate in drills

such as fire and tornado. Please make sure that students’ emergency numbers are updated

periodically and school checkout procedures are adhered to.

The following regulations have been adopted for the protection of all preschool children in

the center. The cooperation of each parent is vital in making these rules effective.

Safety 1. Car riders will enter and exit from the front of the school. Parents must escort their child in

and out of the school building.

2. Parents are encouraged not to check students out after 1:00, unless absolutely necessary.

3. Students who are car riders must be picked up by 2:30 each day

4. Transportation in back of the school (paved area) is for buses only.

5. Students will not be allowed to leave campus with anyone not listed as parent/guardian or

emergency contact on the registration sheet or who does not have the school’s car-rider tag.

Transportation Students may be transported by parents or ride a Ware County School System bus. All

students riding the bus must abide by rules designated by the bus driver. Busing schedules will be

given to parents at Orientation before school starts. Parents or caregivers must meet the bus at

the scheduled time each day, or the child will be delivered back to school.

Health A child who is ill should be kept at home. The following factors should be used by parents to

determine whether or not a child should be kept at home and will be used by the DAFFODIL staff

to determine if a child should be sent home:

1. When a child’s illness is suspected to be contagious;

2. When a child has had a fever, vomiting or diarrhea within the past twelve hours;

3. When a child shows evidence of potential illness, such as severe runny nose, red eyes, rash,

sore throat, or feverish skin.

If a child becomes ill during the day, the parents must make provisions to pick up the child from

school. If the parent cannot be reached, persons listed for emergency contacts will be called and

asked to pick up the child.

Please notify the school at once if a child has a communicable disease or has been exposed to one.

The nurse, teacher, or school designee may administer drugs prescribed by a doctor only if all of

the following requirements have been met:

The parent must bring the medication to the school. Students are not allowed to bring

medication on campus.

All medicine is in the original labeled container bearing the name of the student, the name

of the medication, and the dosage.

There are written instructions from the doctor explaining how to administer, amount, time,

etc.

A written request is made by the parent or guardian to the school to administer the drug

including time and dosage. This request will be made on the school’s “Medical

Authorization and Release” form. Each teacher will keep a written daily log of drugs

administered.

Lice/Scabies/Ringworm/Impetigo: Occasional checks for head lice and scabies will be

made in the classroom. Parents of any child found to be infected will be notified

immediately. A letter giving information about the treatment and preventive measures will

be given to parents when they come to school to pick up the child. Upon readmission to

school, the parent must provide proof of treatment. All children with whom the infected

child has had contact will be checked for possible infestation. If this problem occurs more

than three times, a statement from the Health Department will be required. When a child is

suspected of, or has, ringworms, this procedure will be followed:

o Parents will be asked to contact the child’s primary healthcare provider for

diagnosis and treatment.

o Child will be encouraged to use good hand-washing techniques.

o Sharing of personal items such as hair care articles, towels and clothing, including

the “dress-up” corner for young children, will be prohibited.

If a child has impetigo, his/her parent will be asked to take the child to the doctor to get an

antibiotic ointment to put on the sores. After 48 hours of treatment, a child can usually return to

school.

Curriculum As part of the Georgia Pre-Kindergarten program, DAFFODIL offers one age-appropriate

curriculum: SPLASH with GELDS (Georgia Early Learning Standards). The GELDS ensure that

there are child-centered activities and open-ended, active learning experiences based on the

individual needs and abilities of the students. The learning program is designed to provide

children with an opportunity to explore, to discover, to create, to learn, and to develop their own

knowledge about their world in a way that is meaningful to them.

The preschool curriculum promotes development in the following areas:

The Learning Environment The learning environment at the DAFFODIL Preschool is designed to encourage active

learning experiences where teachers provide the stimulation, attitudes and insight that support the

individual development of each child’s potential.

While every classroom is a little different, each includes many of these experiences:

Field Trips Field trips expose your child / children to the exciting world around them. We incorporate

several field trips throughout the year which are aligned to our thematic units. Some field trips are

appropriate for parents to accompany us on while others are open to students only. You will

receive information from your child's teachers as each field trip approaches.

We request that you do not bring any other children with you when you attend field trips

with your DAFFODIL child/ children. We want this to be a special experience for your

child/children and you. Families of twins are taken into consideration, so that their field trips are

scheduled at the same time.

A Typical Day

During a day at DAFFODIL, your child will take part in many of the following activities:

Arrival:

Arrival is between 7:10 AM to 7:40 AM by bus or car. Children can take part in learning

center activities, look at books, or share experiences with their teachers and classmates during this

time.

Group Activities:

Children take part in large group and small group activities. During large group time,

children have an opportunity for music and creative movement. During small group time, the

children take part in individually planned activities that will enable them to have hands-on

experiences in language concepts, math concepts, fine motor skills, and art.

Outdoor Activities:

The children are provided a minimum of thirty minutes of outside playtime each day. The

only exception is rain or severe weather. The playground is designed to provide a variety of large

and small motor activities in a safe environment where the children can interact with other

children and adults.

Center Time:

Center time is uninterrupted work time that lasts for a minimum of one hour at different

times of the day in each classroom. Children plan with the teacher where they will work for that

day, and together they review their activities when center time ends. Learning centers provide

children with the opportunity to explore ideas, learn to play with others, and be actively involved

with their environment by using a variety of materials and activities.

Quiet Time:

Each day children will be given an opportunity for rest and quiet time. At this time, they

lie on mats and take part in quiet activities, such as looking at books, listening to soft music, and

napping.

radKIDS Program:

Throughout the year, teachers will teach a safety program called radKIDS. It teaches

students to develop a plan to resist aggression defensively and many other safety plans at home

and at school.

Helpful Hints for Parents In order to make your child’s preschool experience as rewarding and enjoyable as possible, we

provide the following suggestions:

Make sure DAFFODIL Pre-K has a current address and phone number so you may be

reached in case of an emergency. DAFFODIL Pre-K should have a minimum of three

emergency numbers. These numbers should be persons who may pick-up your child from

school in case you cannot be reached.

Remember to send a clean towel each week for nap time.

Please send a complete change of clothing (including underwear and socks) for your child.

Remember that these clothes should be appropriate for the current weather conditions.

Please do not allow your child to bring toys or candy from home without the teacher’s

permission.

Riding the school bus is an opportunity for DAFFODIL Pre-K students. All bus rules

must be obeyed.

School pictures are taken several times during the year. Purchase is voluntary.

Please dress your child for appropriate weather conditions.

DAFFODIL Pre-K participates in the Ware County School System school lunch program.

Students are served free breakfast and lunch while at school.

Students will not be allowed to receive flowers, balloons, etc. at school.

Invitations for any special event will not be distributed at school unless there is an invitation

for each child in the class.

We encourage all students to eat in the lunchroom. You may send a nutritious lunch to

school in a lunchbox. No sodas or fast food meals will be permitted unless earned by

PBIS.

At the end of the year, please make sure that you have enrolled your child in

kindergarten at his/her home school. DAFFODIL cannot register your child. You must

go to the home school to register for the upcoming year.

Meals

Students will not be charged for school meals.

Ware County School District is participating in a new nutrition program, Community Eligibility

Provision (CEP). No student will be required to pay for breakfast or lunch, and parents will not be

required to complete a school meal application.

In the operation of the child feeding programs, no child will be discriminated against because of

race, sex, color, national origin, age or handicap. If you believe you have been discriminated

against, you may write to the Secretary of Agriculture, Washington, D.C. 20250.

L.A.M.B.S. is a Waycross-area children's week-end food program that originated at and is

coordinated by Waycross First United Methodist Church. Many children depend on two school-

provided meals each weekday. However, week-ends are difficult for many of them because of

parents working, numerous children in one household, or having limited food on hand. We supply

a L.A.M.B.S. bag each Friday that is filled with child-friendly basic foods as a supplement for the

week-end. Included are healthy foods for 2 breakfasts, 2+ additional meals, and a snack. We

occasionally add hygiene or school supplies. These bags are delivered to the school on Thursday

each week and are given to students selected by each school counselor based solely on obvious

need. Parents are asked to fill out permission stating any allergies, etc. This is a HUGE

commitment!!! At Christmas and other times, the church will provide each child a treat containing

a hand knitted cap, socks, book, hygiene items or candy. We are committed to and believe in the

success of this mission again this year.

Dress Code

All students are asked to dress in an age appropriate manner. Clothing must be conservative and

cover the student’s stomach and back. Students may not wear flip flops, wedge heels, high heels,

or tops with spaghetti straps. Outfits such as overalls with shoulder tabs are not appropriate

clothing because they are difficult for the child to maneuver. Students’ shoes must be comfortable

on their feet to ensure their safety while running and playing on the playground and other

activities. Tennis shoes or sandals with a strap should be worn at all times. Please make sure that

your child wears a jacket in cool weather. ALL Jackets must be labeled with child’s name.

Parents will be called to bring their child an outfit if he/she is not dressed appropriately.

Teacher/Student/Parent Compact

As an individual school in the Ware County System, DAFFODIL Pre-K is dedicated to providing

high quality curriculum and instruction in a supportive and effective learning environment that

enables children to meet Georgia’s academic achievement standards.

We believe that the parent is the child’s first teacher and expect DAFFODIL parents to become an

active member of their child’s public education team.

We require all parents, teachers, and principals to sign a teacher/student/parent compact to ensure

that all involved parties pledge to do their best to ensure that all DAFFODIL Pre-K students obtain

their optimal functioning capacity.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT

Notice to Parents/Guardians and Eligible Student of Rights Under Family Educational

Rights and Privacy Act

1) Inspect and review, within 45 days of a request, the education records of a student who is

your child, or in the case of a student who is eighteen (18) or older, or those who are

emancipated, your own education records. Parents or eligible students should submit to the

Superintendent a written request identifying the record(s) they wish to inspect. The

Superintendent will make arrangements for access and provide notice of such

arrangements.

2) Request the amendment of the student’s education records to ensure that they are not

inaccurate, misleading, or otherwise in violation of the student’s privacy or other rights.

To request the school district to amend a record, parents or eligible students should write

the school principal, specify the part of the record they wanted changed, and specify why it

is inaccurate, misleading or otherwise in violation of the student’s privacy or other rights.

If the district decides not to amend the record, it will notify the parents or eligible student

of the decision and inform them of their right to a hearing. Additional information

regarding the hearing procedure will be provided with the notification of the right to a

hearing.

3) Consent to disclosures of personally identifiable information contained in the student’s

education records, except to the extent that the Act and the regulations promulgated

pursuant to the Act authorize disclosure without consent. One exception, which permits

disclosure without consent, is to school officials with legitimate educational interests. A

school official is a person employed by the district as an administrator, supervisor,

instructor, or support staff member; a member of the school board; a person with whom the

district has contracted to perform a specific task (such as an attorney, auditor, or therapist);

or a parent or student serving on an official committee (such as a disciplinary or grievance

committee). A school official has a legitimate educational interest if the official needs to

review an educational record in order to fulfill his/her professional responsibility. Upon

request, the school district forwards educational records without prior consent to another

school, in which the student seeks or intends to enroll.

4) The Ware County School District has designated the following information as directory

information:

a. Student’s name, address and telephone number;

b. Students date and place of birth

c. Student’s participation in official school clubs and sports;

d. Weight and height of student is he/she is a member of an athletic team;

e. Dates of attendance at the Ware County School System; and

f. Awards received during the time enrolled in the Ware County School System;

g. Video, audio or film images or recordings;

h. Photograph

i. Grade level; and

j. The waiting list containing names of student seeking enrollment in Ware County’s

Magnet School

Unless you, as a parent/guardian or eligible student, request otherwise, this information may be

disclosed to the public upon request. You have the right to refuse to allow all or any part of the

above information to be designated as directory information and to be disclosed to the public upon

request. If you wish to exercise this right, you must notify the principal of the school at which the

student is enrolled in writing within 15 days after officially enrolling in school.

You are also notified that from time to time students may be photographed, video taped, or

interviewed by the news media at school or some school activity or event; unless you, as a

parent/guardian object in writing to your student being photographed, video taped or interviewed,

to the principal of the school where your student is enrolled. You must notify the principal of your

objection by the date specified above.

Notice to Parents/Guardians and Eligible Students of Rights Under

Protection of Pupil Rights Amendment (PPRA) 1. Parents and eligible students (18 or older or emancipated minors) shall be notified at the beginning

of the year of the approximate dates during the school year when any of the activities below are

expected to be scheduled. The Board of Education has developed and adopted policies, in conjunction

with parents, regarding the activities described in paragraph (1). In accordance with Board policies,

prior written consent must be obtained from parents before students are required to submit to any

survey that contains questions about one or more of the areas listed in subparagraph (1) (A) and that is

funded in whole or in part by the U.S. Department of Education. You have the right to inspect any

survey or instrument used in the collection of information under subparagraphs (1) (A) and (1) (B)

before the instrument is administered or distributed to a student and to opt out your student of

participation in any activities described in paragraph (1) in accordance with regulations developed by

the Superintendent.

(A) The administration of any survey containing one or more of the following items:

i. Political affiliations or beliefs of the student or the student’s parent;

ii. Mental or psychological problems of the student or the family;

iii. Sex behavior or attitudes;

iv. Illegal, antisocial, self-incriminating, or demeaning behaviors;

v. Critical appraisals of other individuals with whom respondents have

close family relationships;

vi. Legally recognized privileged or analogous relationships, such as

those of lawyers, physicians, and ministers;

vii. Religious practices, affiliations, or beliefs or the student or the

student’s parent; or

viii. Income (other than that required by law to determine eligibility for

participation in a program or for receiving financial assistance under such program),

without prior written consent of the parent eligible students.

(B) Activities involving the collection, disclosure, or use of personal information

collected from students for the purpose of marketing or for selling that

information (or otherwise providing that information to others for that

purpose).

(C) Any non-emergency, invasive physical examination or screening that is

required as a condition of attendance, administered by the school and scheduled by the

school in advance and not necessary to protect the immediate health and safety of the student, or of

other students.

2. You may, upon request, inspect any instructional material used as part of the educational

curriculum for your student.

Local Educational Agency Parent Involvement

Every Student Succeeds Act (ESSA)- Parent Involvement

PARENTAL INVOLVEMENT The Ware County Pre-K/Elementary Schools affirm and assure the right of parents and legal

guardians of children being served in activities funded by Title I the opportunities to participate

in the planning, design, and implementation of the Title I programs and its activities. The Ware

County School District Pre-K/ Elementary Schools shall involve parents in the joint

development of its plan, and in the process of school review and improvement through input

received from the school improvement team, school council meetings with parent membership,

and through input received from other parent meetings or written surveys. It shall also establish

expectations for parent involvement through written information distributed to parents through

school and classroom newsletters, parent contracts, automated phone system, the local media,

and school websites.

The Ware County School District Pre-K/Elementary Schools shall build the capacity for strong

parental involvement through a variety of activities. (Listing of activities is not all inclusive.)

Provide information to parents in written form or through meetings on topics such as the

State’s academic content standards, state and local student achievement

standards/assessments, and the requirements of parent involvement under the law.

Provide materials and training to help parents work with their children and monitor their

progress to improve their children’s achievement.

Train educational staff in building relationships.

Coordinating and integrating parent involvement strategies with the feeder elementary

schools and other programs, to the extent feasible and appropriate by communicating

directly and indirectly with these agencies through verbal and written information when

needed.

Provide information to parents in a format and to the extent feasible, in a language that

parents can understand.

Provide reasonable support for parental involvement activities as parents may request or

as the school deems appropriate or necessary.

The Ware County School District Pre-K/Elementary Schools shall conduct, with the

involvement of parents, an annual evaluation of the content and effectiveness of the parent

involvement policy to determine whether there has been increased participation and whether

there are barriers to greater participation, particularly by parents who are economically

disadvantaged, disabled, have limited English proficiency have limited literacy, or are of any

racial or ethnic minority background. This data will be obtained from parent committees or

school council meetings or through parent surveys.

The Ware County School District Pre-K/Elementary Schools shall, to the extent possible,

implement activities to meet the federal requirements under Title I, Part A. The school shall

encourage and support activities that effectively involve parents and that place a positive

impact on student success.

Parent Information-Title I Federal Programs

In compliance with the requirements of the Every Student Succeeds Act, parents may request the

following information:

1. Whether the student’s teacher—

o has met State qualification and licensing criteria for the grade levels and subject

areas in which the teacher provides instruction;

o is teaching under emergency or other provisional status through which State

qualification or licensing criteria have been waived; and,

o is teaching in the field of discipline of the certification of the teacher.

Whether the child is provided services by paraprofessionals and, if so, their qualifications.

STATEMENT OF NONDISCRIMINATION

activities, including its athletic programs. Any student, employee, applicant for employment,

parent, or other individual who believes he or she has been subjected to harassment or

SCHOOL MEDICATION ADMINISTRATION

If medications can be given at home before or after school hours, please do so. However, if

medications must be given during school hours, the following procedures apply.

A parent or legal guardian must complete and sign a School Medication Authorization (form SHS01)

in addition to SHS-00 (School Clinic Permission Form) for all medications given at school. For

prescription medications that are to be administered for more than 2 weeks, a medical doctor must also

complete and sign the School Medication Authorization. A copy of this form is available on the web

site, from your school clinic and at most physicians’ offices.

A school nurse or staff member designated by the principal may administer drugs prescribed by a

doctor if all of the following requirements are met:

Students should not be in possession of medications at any time on a school campus, on any school

transportation vehicle, or at school sponsored functions with the exception of physician prescribed

inhalers for respiratory disorders of physician Epinephrine for severe allergies.

A parent/legal guardian or other designated adult must bring all medication with the signed School

Medication Authorization to the school clinic. Prescription medications must be clearly labeled

with the physician’s name, name of medication, strength, dosage, date, time for administration,

and dispensing pharmacy. When medication is to be given at home and at school, ask your

pharmacist to fill the prescription in 2 labeled containers, one for home and one for school.

Enough medication for one month should be furnished at the time.

All over-the-counter medications and prescription medications must be in the original

containers and be FDA approved. If a child requires an over the counter medication which is

supplied by the school clinic more than two times, the parent must furnish the medication. If a

child requires an over the counter medication that is not listed on the standard school health

permission form, the parent must furnish the medication with an individual parent permission for

that medication.

If your child has a life-threatening condition (i.e. asthma, diabetes, or allergic reaction), permission

may be granted to carry medication (such as inhaler, glucose tablet, Epi-pen, or internal Insulin

pump) on his or her person. Such permission will require a completed and signed School

Medication Authorization from the child’s physician and parent/guardian. Students with asthma or

severe allergic reactions are encouraged to keep a second inhaler or Epi-pen in the clinic for

emergency use during school even if they do carry such emergency medication. This will help

ensure the ability of the staff to assist the student in the event the student does not have their

medication on their person and is unable to instruct others where to find it. If a student has these

conditions, a parent conference with the school nurses and other staff should be conducted to

develop a Health Plan for the student.

Parents and guardians are responsible for replenishing medication supplies at least six days prior to

the student’s medication running out.

Unused or discontinued medication must be picked up by the parent from the school clinic prior to

the end of the school year. All medication left at school more than one weekday following the last

day of school will be disposed of.

ILLNESSES OR ACCIDENTS

Students who are ill or injured will be checked out of school under regular check-out procedures. The

school will take steps to care properly for the student in either case. If a student is ill, he/she must

report to the clinic. If a student is involved in an accident while at school, the student and his/her

parents are financially responsible for needed medical treatment unless otherwise determined.

The school officials should be made aware of any unusual health problems that your child may

have which might affect his/her learning. No one will be excused from physical education class

unless a statement is received from a doctor specifying that there is a problem that prevents the

child from participating in physical education.

Students with contagious infections need to stay home so they will not expose others. If your

child is home with a communicable illness, please contact the school nurse so the parent(s) of

other classmates may be alerted of symptoms.

Children must be fever free, temperature below 100.4 degrees, for 24 hours without fever

medication before returning to school.

Parents/guardians of any child found to be infected with head lice will be notified immediately

to pick up their child. A letter giving information about the treatment and prevention measures

will be given to parents when they come to the school. If you find head lice in your child’s

hair, please notify the school nurse. The school nurse can advise you how to treat

appropriately, how to manually remove nits, and treat your household before returning to

school. Upon return to school, bring your child to the clinic first so the school nurse can help

you be sure your child is free of nits before returning to class.

A child with chicken pox may return to school when all of the lesions have scabbed (Usually

5-7 days).

Impetigo, ringworm, shingles, & scabies must be under treatment to return to school. In some

cases lesions may have to be covered.

If your child has reoccurring vomiting and/or diarrhea, they must stay home.

A child with bacterial Pink Eye (thick drainage and redness of the whites of the eyes

throughout) may return to school after 24 hours of physician prescribed treatment.

If a child becomes ill during the day, the parents must make provisions to pick up the child

from school. If the parent cannot be reached, persons listed for emergency contacts will be

called and asked to pick up the child.

TELEMEDICINE IN THE SCHOOL

Ware County Schools has a telemedicine service called the WARE (Wellness and Resource

Education) Center provided for students. The services offered are:

Care for acute illnesses (i.e., sore throat, earache, colds, rashes, eye infections)

Minor injuries (i.e., scrapes and muscle strains)

Management and ongoing care of existing medical conditions (i.e., asthma, diabetes, sickle

cell)

Lab tests (i.e., drawing blood, strep tests, flu screens, urine analysis)

Professional counseling in regard to nutrition and personal hygiene

Mental health, substance abuse, and family centered case management

A packet giving consent must be completed before the student can receive these services. This form

must be completed only once and is different from the yearly clinic permission form. To receive

information about these services, you can check with your school nurse or school guidance counselor.
