I Tóm tắt chung về trang thiết bị y tế
1.1 Mô tả tổng quan *Tóm tắt và giải thích:
Chất chuẩn BD Calibrite có hai dạng gồm bộ hai và ba màu. Bộ hai màu chứa
3 loại hạt Calibrite: một hạt không gắn nhãn, hạt gắn nhãn gắn fluorescein
isothiocyanate (FITC), và hạt gắn nhãn gắn phycoerythrin (PE). Bộ 3 màu chứa
các loại hạt này cùng với hạt gắn nhãn có bổ sung thêm peridinin chlorophyll
protein (PerCP). Hạt màu allophycocyanin (APC)-có sẵn riêng biệt và có thể
sử dụng với bộ 3 màu để cài đặt 4 màu. Hạt Calibrite PerCP và PerCP -Cy5.5*
cũng có sẵn riêng biệt Hạt PerCP-Cy5.5 có thể sử dụng cho hạt PerCP đối với
ứng dụng có gắn kết kháng thể với PerCP-Cy5.5 chất tạo màu.
Máy Phân tích dòng chảy tế bào có các bộ phát hiện riêng biệt và các ống nhân
quang điện (PMTs) phát hiện tín hiệu ánh sáng. Cả hai tín hiệu ánh sáng tán xạ
và huỳnh quang đều được phát hiện. Bởi các hạt chuẩn BD Calibrite mô phỏng

các tế bào chưa nhuộm màu và các tế bào đã nhuộm màu (dán nhãn) với các
kháng thể đơn bào liên kết với fluorochrome, các hạt được sử dụng để điều
chỉnh các cài đặt thiết bị trước khi chạy mẫu tế bào trên máy phân tích dòng
chảy tế bào.

Mỗi fluorochrome phát ra ánh sáng trên một dải bước song khi bị kích thích
bởi chùm tia laze. Do đó một phần của tín hiệu FITC được phát hiện bởi FL2
PMT, một phần của các tín hiệu PE được phát hiện bởi các FL1 và FL3 PMTs;
một phần của các tín hiệu PerCP hoặc PerCP-Cy5.5 được phát hiện bởi các FL4
PMT (tín hiệu PerCP và PerCP-Cy5.5 không được phát hiện bởi các FL2 PMT);
và một phần của tín hiệu APC được hiện bởi các FL3 PMT. “Chồng lấp quang
phổ” phải được khắc phục bằng bù điện tử . Các hạt chuẩn BD Calibrite được
sử dụng để xác định các cài đặt bù thích hợp.
Sau khi các thiết bị đã được cài đặt, các hạt chuẩn BD Calibrite được sử dụng
để đánh giá độ nhạy của máy. Tia tán xạ thẳng (FSC) và tia tán xạ bên (SSC)
của máy được đo lường bởi việc tách kênh giữa các tín hiệu ánh sáng tán xạ
của các hạt và tín hiệu nền (tín hiệu điện tử và quang học). Độ nhạy huỳnh
quang FL1, FL2, và FL3 được xác định bằng cách đo các sự phân tách trung
bình giữa các tín hiệu của các hạt gắn nhãn và các hạt không gắn nhãn . Độ

nhạy FL4 được xác định bằng cách đo sự phân tách giữa các hạt PerCP hoặc
PerCP-Cy5.5 và APC. Một sự phân tách kênh tối thiểu phải được đáp ứng cho
các tham số tán xạ và huỳnh quang cho phép các tế bào để được phân biệt với
các mảnh vỡ mẫu hoặc tín hiệu nền và cho các màu mờ hơn giữa các tế bào
nhuộm để được phân biệt từ các tế bào không nhuộm
1.2 Lịch sử đưa sản phẩm ra thị trường Sản phẩm được cấp phép lưu hành đầu tiên tại Mỹ năm 1998
1.3 Mục đích sử dụng Chất chuẩn BD Calibrite™ được thiết kế sử dụng cho phần mềm BD
FACSComp™ hoặc AutoCOMP™ và dòng máy phân tích dòng chảy tế bào
BD FACS™ (BD FACSCalibur™, BD FACSort™, BD FACScan™, và BD
FACStrak™). Các hạt này sử dụng trong việc cân chỉnh cài đặt cho máy, cài
đặt bù trừ quang phổ và kiểm tra độ nhạy của máy. Khuyến cáo sử dụng thường
qui nhằm đánh giá hiệu suất của máy theo thời gian. Chất chuẩn BD Calibrite
được sử dụng cho chẩn đoán in vitro.

đến sự an toàn/ hiệu quả của sản phẩm
Từ ngày 01 tháng 10 năm 2015 tới ngày 21 tháng 12 năm 2020, không có bất
kỳ ghi nhận nào về tác dụng bất lợi liên quan tới việc sử dụng sản phẩm, cũng
như không có bất kỳ hành động thu hồi sản phẩm nào diễn ra trong khoảng
thời gian trên.

II Mô tả trang thiết bị y tế
2.1 Mô tả trang thiết bị y tế *Nguyên lý của quy trình
Để chuẩn bị cho việc sử dụng các hệ thống phân tích dòng chảy tế bào, phần
mềm BD FACSComp (hoặc AutoCOMP ), sử dụng các hạt Calibrite chưa gắn
nhãn , chọn một cổng FSC để tách các hạt bead đơn khỏi các mảnh vỡ và các
hạt bead dính với nhau, sau đó điều chỉnh điện áp PMT.
Điện áp huỳnh quang PMT được điều chỉnh cho đến khi các giá trị kênh cho
các hạt không gắn nhãn tương ứng với giá trị mục tiêu xác định trước (đối
với các FL4 PMT, phần mềm BD FACSComp đặt các kênh giá trị trung
bình cho các hạt BD Calibrite APC đến một giá trị mục tiêu xác định trước).
Điện áp SSC PMT được điều chỉnh để xác định vị trí các hạt tại kênh mục
tiêu SSC. Ngưỡng FSC được điều chỉnh đến một mức độ giảm thiểu tín hiệu
nền (nếu có).
Tiếp theo, các phần mềm điều chỉnh bù trừ huỳnh quang bằng cách sử dụng
một hỗn hợp hạt chứa một lượng bằng nhau của các hạt Calibrite phù hợp.
Đối với cài đặt thiết bị hai màu, các điều chỉnh bù trừ huỳnh quang là FL1-
% FL2 và FL2, -% FL1. Đối với cài đặt cho ba màu, điều chỉnh bù trừ
huỳnh quang là FL3-% FL2. (Vì PerCP và PerCP-Cy5.5 không được phát
hiện bởi các FL2 PMT, bù trừ FL2-% FL3 là không cần thiết.) Đối với cài
đặt cho bốn màu, điều chỉnh bù trừ huỳnh quang là FL3-% FL4 và FL4-%
FL3.
Điều chỉnh bù trừ cho FL1, FL2 và FL3 một cách chính xác cho sự chồng
lấp quang phổ bằng cách dịch chuyển các quần thể hạt có nhãn để chúng
được căn chỉnh với các quần thể hạt không gắn nhãn tương ứng. FL4 bù trừ

được thiết lập bằng cách đặt APC trong một kênh mục tiêu dọc theo các FL3
trục và PerCP hoặc PerCP-Cy5.5 trong một kênh mục tiêu cùng các FL4
trục. Tham khảo Hướng dẫn người sử dụng phần mềm FACSComp để biết
thêm chi tiết.
Theo PMT và điều chỉnh bù trừ, các phần mềm thực hiện đánh giá độ nhạy
bằng cách sử dụng các hỗn hợp hạt thích hợp
LƯU Ý: Vì bạch cầu có tính hướng quang hơn các hạt Calibrite, mẫu máu
ngoại vi của người cho bình thường được khuyến nghị để tối ưu hóa mức
tăng FSC và SSC, ngưỡng FSC và mức bù trừ huỳnh quang. Xem Tối ưu
hóa và kiểm soát chất lượng trên trang 3. Hình 1-4 cho thấy ví dụ của tối ưu
hóa cho ứng dụng hai, ba, và bốn màu.
*Thuốc thử:
Các hạt Calibrite chứa các lọ riêng biệt các vi cầu polymethylmethacrylate
có kích thước xấp xỉ 6 micromet: 2,5 ml hạt không gắn nhãn, 1,25 mL hạt
gắn nhãn FITC, 1.35 mL hạt gắn PE, 1,35 mL hạt gắn PerCP, 1,35 mL hạt
gắn nhãn PerCP-Cy5 .5 và 2,7 ml hạt gắn APC. Tất cả được đựng trong
dung dịch muối sinh lý ổn định 0.1% sodium azide (Xem phần Thận trọng).
Thuốc thử đủ dùng cho 25 xét nghiệm.
Đệm pha loãng các hạt bead chứa100mL dung dịch đệm ổn định 0,1%
sodium azide (Xem phần Thận Trọng). Một chai đủ dùng cho 25 xét
nghiệm.
*Thiết bị
FACSCalibur™, FACSort™, FACScan™, hoặc FACStrak™
Các hạt bead Calibrite được chỉ định sử dụng trên các dòng máy phân tích

dòng chảy tế bào FACS™ được trang bị cho phát hiện hai-, ba-, và bốn màu
huỳnh quang và phát hiện hai thông số tán xạ ánh sáng. (FACStrak™ chỉ
được trang bị cho phát hiện hai màu huỳn quang. Các thiết bị BD
FACSCalibur và FACSort được trang bị với tùy chọn FL4, cho phép phát
hiện 4 màu huỳnh quang). Để sử dụng, tham khảo hướng dẫn sử dụng thiết
bị tương ứng.
Thiết bị phân tích dòng chảy tế bào phải được trang bị với phần mềm BD
FACSComp hoặc phần mềm AutoCOMP, phiên bản 2.0 hoặc cao hơn.
Thông tin chi tiết về sử dụng, tham khảo hướng dẫn sử dụng phần mềm
tương ứng.
LƯU Ý: Phần mềm BD FACSComp phiên bản 4.2 hoặc cao hơn là cần thiết
để sử dụng với các hạt beads PerCP-Cy5.5.
2.2 Hướng dẫn sử dụng
*Chuẩn bị huyền phù:
Chuẩn bị huyền phù tất cả các hạt ngay trước khi sử dụng. Trộn các lọ
hạt bằng cách đảo ngược nhẹ nhàng hoặc trộn nhẹ trước khi sử dụng
1. Dán nhãn hai ống polystyrene 12 x 75 mm Ống A và Ống B.
2. Cho 1 ml dung dịch đệm hoặc đệm pha loãng vào ống A.
3. Cho 3 ml dung dịch đệm hoặc đệm pha loãng vào ống B.
THẬN TRỌNG: Chỉ sử dụng dung dịch đệm pha loãng để hiệu chuẩn
với các hạt bead PerCP-Cy5.5; không sử dụng dung dịch đệm (sheath
fluid)

4. Trộn nhẹ các hạt Calibrite trong lọ, sau đó thêm 1giọt của các hạt này
vào mỗi lọ như bảng bên dưới
LƯU Ý: Đảo ngược hoàn toàn lọ hạt khi thêm một giọt vào ống. Đảo bảo thu
được đầy đủ các hạt. Giọt phải có màu đục, cho thấy cá hạt đã được trộn đúng
cách.

a. Dùng ống A cho điều chỉnh PMT, Ống B cho việc bù trừ huỳnh quang
và kiểm tra độ nhạy

5. Giữ các huyền phù hạt bead đã chuẩn bị trong đá hoặc ở điều kiện
2°C đến 8°C và luôn tránh ánh sáng trực tiếp

• Các huyền phù hạt bead BD Calibrite đã chuẩn bị trong dung dịch pha
loãng được ổn định trong 8 giờ ở nhiệt độ 2°C đến 8°C hoặc 1 giờ ở nhiệt độ
20°C-25°C
• Các huyền phù hạt bead BD Calibrite đã chuẩn bị trong dung dịch đệm
FACSFlow sheath fluid được ổn định trong 8 giờ ở nhiệt độ 2- 8°C hoặc trong 1
giờ ở điều kiện 2° đến 8°C nếu có hạt PerCP. Không pha loãng các hạt bead
PerCP-Cy5.5 trong dung dịch sheath fluid.
*Cài đặt hệ thống
Thông tin chi tiết sử dụng phần mềm BD FACSComp hoặc AutoCOMP với các
hạt bead BD Calibrite cho cài đặt thiết bị, tham khảo Hướng dẫn sử dụng phần
mềm BD FACSComp hoặc Hướng dẫn sử dụng phần mềm AutoCOMP.
1. Điều chỉnh cài đặt điện áp PMT bằng ống A.
2. Điều chỉnh bù trừ quang phổ bằng ống B.
3. Thực hiện kiểm tra độ nhạy sử dụng ống B.
4. In một bản kiểm tra kết quả độ nhạy và giữ 1 bản in. Ghi PMT điện
áp và kênh tách thu được cho mỗi tham số hàng ngày trong sổ theo dõi
5. Tối ưu hóa cài đặt cho mẫu của bạn, khi cần thiết. Cài đặt thiết bị có thể cần
được tối ưu hóa thủ công trước khi chạy.
Kiểm tra trực quan để đạt được mức tăng PMT, bù trừ và ngưỡng FSC thích
hợp (xem từ hình 1 -hình 5 trong phần sau, Tối ưu hóa và Kiểm soát chất lượng).
*Tối ưu hóa và kiểm soát chất lượng

Bởi vì bạch cầu có sự khác nhau về thuộc tính quang hơn các hạt bead BD
Calibrite, việc tối ưu hóa các cài đặt thiết bị với mẫu tế bào là quan trọng. Chuẩn bị
một mẫu máu hàng ngày từ người cho bình thường. Sử dụng cùng phương pháp
nhuộm và chạy song song với các mẫu xét nghiệm. Tối ưu hóa các cài đặt thiết bị
sau cài đặt hai màu bằng cách sử dụng mẫu máu được nhuộm với bất kỳ sự kết
hợp nào của các kháng thể đơn dòng xác định các quần thể tế bào riêng biệt không
chồng chéo, chẳng hạn như kháng thể đơn dòng được gắn nhãn FITC và PE. Tối
ưu hóa sau thiết lập ba và bốn màu có thể khác nhau tùy thuộc vào các ứng dụng.
(Xem Hình 1đến Hình 5 để biết các ví dụ. Luôn luôn tham khảo các ghi chú ứng
dụng thích hợp hoặc hướng dẫn sử dụng chất thử)
LƯU Ý Các phương pháp chuẩn bị mô hình miễn dịch khác nhau có
thể yêu cầu các quy trình tối ưu hóa khác nhau. Có thể cần phải điều chỉnh các
bộ khuếch đại FSC và SSC để tất cả các quần thể bạch cầu trên quy mô, và để
điều chỉnh bù trừ và thiết lập ngưỡng (xem Hình 1).
*Tối ưu hóa tán xạ
Hình 1 hiển thị một mẫu máu toan phần đã ly giải (LWB) từ một người cho bình
thường trước và sau khi tối ưu hóa. Lưu ý những quần thể có tín hiệu FSC thấp
hơn so với các tế bào lympho (ví dụ: mảnh vụn) có thể được loại trừ bằng
cách tăng các mức ngưỡng FSC.

Các hạt bead BD Calibrite được khuyến nghị sử dụng với phần mềm
FACSComp hoặc AutoCOMP trên máy phân tích dòng chảy tế bào
FACSCalibur, FACSort, FACScan hoặc FACStrak.
Trong một số trường hợp, phần mềm có thể không được tự động cài đặt
trên thiết bị. Nếu điều này xảy ra, điều chỉnh thủ công các cài đặt. Tham
khảo Hướng dẫn sử dụng phần mềm FACSComp
*XỬ LÝ SỰ CỐ
Nếu các hạt bead BD Calibrite không đáp ứng các thông số độ nhạy tối
thiểu cần thiết cho máy phân tích dòng chảy tế bào được sử dụng, hãy kiểm
tra như sau:
• Hãy chắc chắn rằng các hạt bead chưa hết hạn sử dụng in trên bao bì
• Nếu huyền phù đã chuẩn bị không mới, tạo huyền phù mới và lặp lại
quy trình. Các hạt bead được sử dụng vượt quá sự ổn định bắt đầu cho thấy
sự giảm phân tách giữa các quần thể không được gắn nhãn và được dán
nhãn, dẫn đến thất bại trong Thử nghiệm độ nhạy.
• Sử dụng dung dịch đệm pha loãng hạt bead (Bead Dilution Buffer) thay
vì dung dịch đệm (sheath fluid) để chuẩn bị huyền phù hạt. Các huyền phù
ổn định trong một thời gian dài hơn đệm pha loãng hạt.
• Sử dụng dung dịch đệm hoặc dung dịch đệm pha loãng hạt bead mới để
pha loãng hạt nếu bạn nghi ngờ nhiễm vi khuẩn.
• Mở lọ hạt mới nếu bạn nghi ngờ hạt đã bị nhiễm.

• Kiểm tra hệ dẫn dịch của máy có bọt khí hay cặn bẩn. Nếu việc vệ
sinh thiết bị là cần thiết, hãy tham khảo hướng dẫn sử dụng phân tích dòng
chảy tế bào để biết hướng dẫn.
2.3 Chống chỉ định Không có
2.4 Cảnh báo và thận trọng Thận trọng
• Sử dụng cho chẩn đoán trong ống nghiệm (in vitro)
• Không đóng băng các hạt bead BD Calibrite hoặc để tiếp xúc trực tiếp
với ánh sáng trực tiếp trong thời gian lưu trữ hoặc sử dụng.
• Hạt PerCP-Cy 5.5 là dạng dưới của hạt PerCP, không để chung hai loại
hạt này cùng nhau trong cùng một ống.
• THẬN TRỌNG: Chỉ pha loãng các hạt bead BD Calibrite trong dung
dịch FACSFlow (Mã sản phẩm 342003) hoặc dung dịch đệm pha loãng hạt
(Mã sản phẩm 345035) như hướng dẫn tại Phần 6. Không pha loãng các hạt
PerCP-Cy 5.5 trong dung dịch đệm (sheath fluid)
• Không sử dụng các hạt BD Calibrite đã hết hạn sử dụng hoặc ngoài giai
đoạn ổn định được mô tả trong phần Lưu trữ và xử lý. Các hạt ngoài độ ổn
định bắt đầu cho thấy sự giảm phân tách giữa các quần thể không được gắn
nhãn và được gắn nhán, có thể đẫn đến thất bại trong thử nghiệm độ nhạy.
• Hạt hết hạn có thể làm giảm việc phân tách huỳnh quang khoảng 2,5
kênh mỗi tháng
• Thay đổi quá mức trong kết quả có thể cho thấy sự suy giảm của các
hạt hoặc thay đổi về điều kiện dụng cụ. Nếu nghi ngờ suy giảm, hãy chuẩn
bị một hạt mới và kiểm tra điều kiện dụng cụ

• Đối với tối ưu hóa của các máy phân tích dòng chảy tế bào trước khi
chạy các mẫu, xem tối ưu hóa và kiểm soát chất lượng ở trang 3.

After all risk control measures defined in the risk management plan have been implemented and verified , the risk management team evaluated the residual risk, and performed the overall risk-benefit analysis (weighing all risks combined against the benefit) in all cases. The residual risks are judged acceptable by established criteria , and no new hazards were introduced by any implemented risk control measures.
7. POST-PRODUCTION INFORMATION
The post-launch risk management of BO Calibrite APC beads reagent followed the established post-market surveillance process for collecting and reacting to post-production information as documented in current BO-SanJose procedures (SOP5011 , SOP5413, SOP5420). Post-launch information consists of customer complaints, product incident reports, requests for changes, and may include additional methods/information as appropriate regarding BO Calibrite APC beads reagent (or similar reagents in the field) .
If there is any information or potential that the residual risk(s) or its acceptability has changed, the impact on previously implemented risk control measures will be re-evaluated.

Manufacturing & Quality Control Process Aminated polymer beads are purchased as raw material from a BDB certified supplier. The raw material beads are tested for autofluorescence and percent singlets. In addition, pilot conjugation titrations are completed to determine appropriate fluorophore labeling. Upon approval, the raw material beads are then counted, washed and, if applicable, labeled with a fluorescent dye, per the manufacturing specifications. In-process checks are performed during the manufacturing process to ensure the product quality. The bulk beads are counted and formulated to their final bottling concentration in a buffer containing phosphate buffered saline, 0.2% gelatin as a stabilizer, sodium azide as a bacteriostatic agent and tween-20, used for even bead dispersion within solution. After formulation a sample is submitted to Quality Control for final testing. Bead counts, fluorescence, compensation, and percent CV are confirmed by QC testing per the QC specifications. Diluted product is filled in a validated container / closure system. Environment throughout the manufacturing area is controlled and monitored routinely to minimize the risk of microbial contamination. 8.1 This testing is valid on only FACSCalibur s/n E0158. 8.2 Set up FACSCalibur instrument with 4-color option per WI7173, immediately prior (within 1/2
hour) to bead testing. All minimum separation specs. must be met before beginning.
8.3 Set all settings per WI7173
8.4 Acquire single bead samples uncompensated using CellQuest (channel values) software. Set
threshold to ensure that all bead events are being acquired.
8.5 Collect 5000 events.
8.6 Store data list mode.
8.7 After data analysis and suffix calculation, run FACSComp Lyse/Wash and Lyse/No Wash using
test APC beads

9.1 Set a gate on bead singlets FSC vs SSC. Use template in BD Apps: Bead Analysis FL1, FL2, FL3 R1 (make adjustments as needed).
9.2 Pull single FL3 and FL4 histograms.
9.3 Set marker from Channel ~500 to Channel ~1000 around FL4 histogram.
