Fall protection is a complicatedissueandoneof themost impor-tantfacedbyanemployerorwork-sitesupervisor.Withover100,000reported incidents per year, fallsfromheightsalmostalways resultin serious injury. In the construc-tionindustry,fallsarethenumberone cause of worker death. DBI/SALA and Protecta understandsthese issues and offers its exper-tise on-site with specialized dem-onstrationvehicles.

A • Anchorage Anchoragemeansasecurepointofattachment(structure)forthefallarrestsystem.Thetypeofanchoragevarieswiththeindustry,thejobbeingperformed,thetypeofinstallationandmostimpor-tantlythestructureavailable.Anchorageconnectorsprovideameansofattachingthesystemtotheanchorage.DBI/SALA&Protectaofferawidelineofanchorageconnectorsforallapplications.

THE ABCD’s of Fall Arrest

OSHA says…[1926.502(d)(15)(i)-(ii)]: “Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 lb. (22kN) per employee attached, or shall be designed, installed and used as follows: …as part of a complete personal fall arrest system which maintains a safety factor of at least two; …under the supervision of a qualified person.”

OSHA says…[1926.502(d)(16)(iii), (iv), (v)]: “Personal Fall Arrest Systems, when stopping a fall, shall limit maximum arresting force on an employee to 1,800 lbs.(8 kN) when used with a body harness; be rigged such that an employee can neither free-fall more than 6-feet (1.8 M), not contact any lower level.”

Whensettingupandusingapersonalfallarrestsys-tem,fallclearanceandswingfallhazardsarecriticalissues.Shouldafalloccur,theremustbesufficientclearancebelowtheusertoarrestthefallbeforetheuserstrikesthegroundoranyotherobject.Theuserof theequipmentmustdetermine if thesystemwillarrestthefallwithintheavailableclearance.Somefactorsthataffectthisdeterminationincludeanchor-age location, typeand lengthof connecting system(lanyard, self-retracting lifelines, rope grab), decel-eration distance (the elongation of the deceleratingdevicewhendeployed--allow3.5’),workerheight,movement of harness attachment element (allow asafetyfactorof2.5ft.).

(d)Personalfallarrestsystems.Personalfallarrestsystemsandtheiruseshallcomplywiththeprovisionssetforthbelow.EffectiveJanuary1,1998,bodybeltsarenotacceptableaspartofapersonalfallarrestsystem.Note:Theuseofabodybeltinapositioningdevicesystemisacceptableandisregulatedunderparagraph(e)ofthissection.

(5)Snaphooksshallbesizedtobecompatiblewiththemembertowhichtheyareconnectedtopreventunintentionaldisengagementofthesnaphookbydepressionofthesnaphookkeeperbytheconnectedmem-ber,orshallbealockingtypesnaphookdesignedandusedtopreventdisengagementofthesnaphookbythecontactofthesnaphookkeeperbytheconnectedmember.EffectiveJanuary1,1998,onlylockingtypesnaphooksshallbeused.

(15)Anchoragesusedforattachmentofpersonalfallarrestequipmentshallbeindependentofanyanchoragebeingusedtosupportorsuspendplatformsandcapableofsupportingatleast5,000pounds(22.2kN)peremployeeattached,orshallbedesigned,installed,andusedasfollows:(i)aspartofacompletepersonalfallarrestsystemwhichmaintainsasafetyfactorofatleasttwo;and(ii)underthesupervisionofaqualifiedperson.

(16)Personalfallarrestsystems,whenstoppingafall,shall:(i)limitmaximumarrestingforceonanemployeeto900pounds(4kN)whenusedwithabodybelt;(ii)limitmaximumarrestingforceonanemployeeto1,800pounds(8kN)whenusedwithabodyharness;(iii)beriggedsuchthatanemployeecanneitherfreefallmorethan6feet(1.8m),norcontactanylowerlevel;(iv)bringanemployeetoacompletestopandlimitmaximumdecelera-tiondistanceanemployeetravelsto3.5feet(1.07m);and,(v)havesufficientstrengthtowithstandtwicethepotentialimpactener-gyofanemployeefreefallingadistanceof6feet(1.8m),orthefreefalldistancepermittedbythesystem,whicheverisless.

(e)Positioningdevicesystem.Positioningdevicesystemsandtheiruseshallconformtothefollowingprovisions:(1)Positioningdevicesshallberiggedsuchthatanemployeecannotfreefallmorethan2feet(.9m).

(2)Positioningdevicesshallbesecuredtoananchoragecapableofsup-portingatleasttwicethepotentialimpactloadofanemployee’sfallor3,000pounds(13.3kN),whicheverisgreater.

(OSHA 1926.503 - Training requirements)

(a)TrainingProgram.(1)Theemployershallprovideatrainingprogramforeachemployeewhomightbeexposedtofallhazards.Theprogramshallenableeachemployeetorecognizethehazardsoffallingandshalltraineachemployeeintheprocedurestobefollowedinordertomini-mizethesehazards.

Fixed Ladders (OSHA 1910.27)

(d.5)Laddersafetydevicesmaybeusedontower,watertankandchim-neyladdersover20feetinunbrokenlengthinplaceofcageprotection.Nolandingplatformisrequired.Allladdersafetydevices,suchasthosethatincorporatelifebelts,frictionbrakes,andslidingattachments,mustmeetthedesignrequirementsoftheladderstheyserve.

Powered Platforms For Building Maintenance (OSHA 1910.66)

(j)PersonalFallProtection.Employersmustprovidepersonalfallarrestsystemsmeetingtherequirementsoutlined.Requirementsincludethefollowing:

Beforeusingapersonalfallarrestsystem,andafteranycomponentorsystemischanged,employeesshallbetrainedinaccordancewiththerequirementsofparagraph1910.66(i)(1),inthesafeuseofthesystem.

Personalfallarrestsystemsshallbeinspectedpriortoeachuseformil-dew,wear,damageandotherdeterioration.Defectivecomponentsshallberemovedfromserviceiftheirstrengthorfunctionmaybeadverselyaffected.

Permit-Required Confined Spaces (OSHA 1910.146)

(a)Scopeandapplication.Thissectioncontainsrequirementsforprac-ticesandprocedurestoprotectemployeesingeneralindustryfromthehazardsofentryintopermit-requiredconfinedspaces.(k)(3)Tofacilitatenon-entryrescue,retrievalsystemsormethodsshallbeusedwheneveranauthorizedentrantentersapermitspace,unlesstheretrievalequipmentwouldincreasetheoverallriskofentryorwouldnotcontributetotherescueoftheentrant.Retrievalsystemsshallmeetthefollowingrequirements:(i)Eachauthorizedentrantshalluseachestorfullbodyharness,witharetrievallineattachedatthecenteroftheentrant’sbacknearshoulderlevel,abovetheentrant’shead,oratanotherpointwhichtheemployercanestablishpresentsaprofilesmallenoughforthesuccessfulremovaloftheentrant.Wristletsmaybeusedinlieuofthechestorfullbodyharnessiftheemployercandemonstratethattheuseofachestorfullbodyharnessisinfeasibleorcreatesagreaterhazardandthattheuseofwristletsisthesafestandmosteffectivealternative.(ii)Theotherendoftheretrievallineshallbeattachedtoamechani-caldeviceorfixedpointoutsidethepermitspaceinsuchamannerthatrescuecanbeginassoonastherescuerbecomesawarethatrescueisnecessary.Amechanicaldeviceshallbeavailabletoretrievepersonnelfromverticaltypepermitspacesmorethan5feetdeep.

Safety Requirements for Personal Fall Arrest Systems, Subsystems and Components

(ANSI Z359.1-1992)

1.1Scope-Thisstandardestablishesrequirementsfortheperformance,design,marking,qualification,instruction,training,inspection,use,maintenanceandremovalfromserviceofconnectors,fullbodyhar-nesses,lanyards,energyabsorbers,anchorageconnectors,fallarresters,verticallifelines,andselfretractinglanyardscomprisingpersonalfallarrestsystemsforuserswithinthecapacityrangeof130to310lbs.(59to140kg.).

1.2.1Thisstandardaddressesonlypersonalfallarrestsystems(PFAS)incorporatingfullbodyharnesses.Whenevertheterm“system”isusedinthestandarditreferstoapersonalfallarrestsystem.

3.1.2Whensubjectedtotestscontainedin4.2,apersonalfallarrestsysteminwhichafullbodyharnessisusedshallproduceamaximumarrestforce(MAF)ofnotmorethan1,800pounds(8.0kN)andshallbringthefalltoacompletestopwithadecelerationdistanceofnotmorethan42inches(1,067mm).Insuspension,afterthefallisarrested,theangleatrestwhichtheverticalcenterlineofthetesttorsomakeswiththeverticalshallnotexceed30degrees.

3.2.1.4Snaphooksandcarabinersshallbeselfclosingandself-lockingandshallbecapableofbeingopenedonlybyatleasttwoconsecutivedeliberateactions.

3.2.2.4Theharnessshallprovidesupportforthebodyacrossthelowerchest,overtheshouldersandaroundthethighswhenatensileloadisappliedtothefallarrestattachmentelements.Theharness,whenproperlyfittedandused,shallpreventfall-out.Thefallarrestattachmentshallbelocatedattheback(dorsal)position.

3.2.4.7Whenenergyabsorbersaredynamicallytestedinaccordancewith4.3.5.2,themaximumarrestforceshallnotexceed900lbs.(4kN).

3.2.8.7StaticStrength-Whentestedinaccordancewith4.3.7.3,theSRLshallwithstandatensileloadof3,000pounds(13.3kN)staticallyapplieddirectlytothepointofSRLlineconnectiontotheSRLdrum.

3.2.8.9DynamicPerformance-Whentestedinaccordancewith4.3.7.1,theSRLshalllockandremainlockeduntilreleased.Thearrestdistanceshallnotexceed54inches(1,372mm).Maximumarrestforceshallnotexceed1,800pounds(8kN).

5.1.2Thelegibilityandattachmentofrequiredmarkingsshallendureforthelifeofthecomponent,subsystem,orsystembeingmarked.

5.3.1Instructionsshallbeprovidedtotheuser,printedinEnglish,andaffixedtotheequipmentatthetimeofshipmentfromthemanufacturer.

Ladder Safety Devices (ANSI A14.3-2002)

7.1.3Theladdersafetysystemshallbedesignedtoabsorbtheimpactofasolidobjectweighingatleast500poundsinafreefallof18inches.

7.1.4Designandinstallationofmountingsshallnotreducethestrengthofthefixedladder.

7.3.1Thesafetysleeveshallbeatypewhichcanbeoperatedentirelybythepersonusingtheladdersafetysystem.Itshallpermitthepersonusingtheladdersafetysystemtoascendordescendwithouthavingtocontinuallymanipulatethesafetysleeve.
