Your letter referenced the information provided in response to InformationNotice No. 89-25 regarding corporate reorganization. The changes in yourorganization are being reviewed as a separate action. You will be notified ofthe results of that review.

Please be advised that you must conduct your program involving radioactivematerials in accordance with the conditions of your NRC license,representations made in your license application, and NRC regulations. Inparticular, note that you must:

1. Operate in accordance with NRC regulations 10 CFR Part 19, " Notices,Instructions and Reports to Workers: Inspection and Investigations,"10 CFR Part 20, " Standards for Protection Against Radiation," and otherapplicable regulations. ,

2. Possess radioactive material only in the quantity and form indicated in'|

your license.

3. Use radioactive material only for the purpose (s) indicated in your |license. '

4. Notify NRC in writing of any change in mailing address (no fee requiredif the location of radioactive material remains the same).

5. Request and obtain written NRC consent before transferring your licenseor any right thereunder, either voluntarily or involuntarily, directlyor indirectly, 'through transfer of control of your license to any personor entity. A transfer of control of your license includes not only atotal change of ownership, but also a change in the controlling interestin your company whether it is a corporation, partnership, or otherentity.

In addition, appropriate license amendments must be requested andobtained for any other planned changes in your facility or program thatare contrary to your license or contrary to representations made in yourlicense application, as well as supplemental correspondence thereto,which are incorporated into your license. A license fee may be chargedfor the amendments if you are not in a fee-exempt category.

6. Submit a complete renewal application with proper fee, or terminationrequest at least 30 days before the expiration date on your license.You will receive a reminder notice approximately 90 days before theexpiration date. Possession of radioactive material after your licenseexpires is a violation of NRC regulations.

7. Request termination of your license if you plan to permanentlydiscontinue activities involving radioactive material.

You will be periodically inspected by NRC. A fee may be charged forinspections in accordance with 10 CFR Part 170. Failure to conduct yourprogram in accordance with NRC regulations, license conditions, andrepresentations made in your license application and supplementalcorrespondence with NRC will result in enforcement action against you. Thiscould include issuance of a notice of violation; imposition of a civilpenalty; or an order suspending, modifying, or revoking your license asspecified in the General Policy and Procedures for NRC Enforcement Action,10CFR Part 2, Appendix C. Since serious consequences to employees and thepublic can result from failure to comply with NRC requirements, prompt andvigorous enforcement action will be taken when dealing with licensees who donot achieve the necessary meticulous attention to detail and the high standardof compliance which the NRC expects of its licensees.

Item 9 - Facilities and Equipment

Add: A Temporary Storage Site Only - Corner of Drill Site Drive and Sag RiverDelta Road in Deadhorse, Alaska on the North Slope. This is a Baker HughesINTEO location which will be used as a temporary storage site betweenperforming well logging services for the customer.

Item 10.4 - Radiation Detection instruments and Calibration

Change: As per the requirements of 10 CFR 39.33 (b), Baker Hughes INTEO hasacquired additional instrumentation sensitive enough to detect low radiationlevels that could be encountered if a seal source ruptured. We refer to theseinstruments as " mud monitors" and are/will be used as specified by 10 CFR39.69 (c). Once the respective instrument is purchased it is package in aspecial container and prepared for shipment to the jobsite in the event INTEO'slogging tool containing a sealed source (s) becomes lodged in the well. Thespecial water proof and padded container (s) provides protection from damageto the instrument (s). Therefore, because of the low usage rate, special caregiven to the instruments and the fact that 10 CFR 39.33 (c) refers to the surveyinstruments used regularly in 10 CFR 39.33 (a) only. We request that INTEObe allowed to have the " Mud Monitors" calibrated annually versus every 6months. An INTEO technician will perform a physical inspection and batterycheck after each use and/or every 6 months.
