FOREWORD

It gives me immense pleasure to write the foreword for the “Textbook on Geriatric Medicine”published by the Indian Academy of Geriatrics. With the longevity of life getting increased, the populationof elders in our country is also mounting steadily.

The whole world is witnessing a steady growth of the ageing population. This stresses theimportance of intensified geriatric service to take care of the elderly in India. This will need thefunctioning of more and more geriatric care units across the nation. To train doctors in this newdiscipline, postgraduate course in Geriatric Medicine (MD Geriatric Medicine) has been started inmany states in India.

Although a lot of books in geriatrics have been authored and published all over the world, theneed of the hour is a comprehensive geriatric textbook based on the Indian scenario. Such a textbookwould serve as a wonderful basic study material for the postgraduate medical students in geriatrics.

Since family physicians and primary healthcare physicians come across a large number of elderlypatients, a basic knowledge on geriatric care is very essential for them.

My hearty congratulations to the Indian Academy of Geriatrics for its wonderful pioneeringefforts in bringing out the first textbook on Indian Geriatrics.

This voluminous textbook with around 150 chapters has been divided into 21 sections. The contentsof this book have been broadly classified under the categories of Gerontology, Geriatric Medicine,Systemic Disorders, Rehabilitation and Preventive Geriatrics. The sections on Clinical Geriatrics andSyndromic Management are sure to help physicians get a clearer outlook on geriatrics. "Caregiverstress and elder abuse" and "Euthanasia" would provide a new outlook to the Indian physician. Geriatricsand stem cell research is a fresh avenue in geriatrics. My personal favourite being the "Preventivegeriatrics" section, for "Prevention is always better than cure," more so when it comes to geriatriccare.

Enormous efforts have been taken to put a complex and complicated subject like geriatrics in asimple, easy to assimilate format. This makes this book a ready reckoner for the paramedical personsas well. The highlight of this book is the inclusion of the latest and most recent developments, particularlyin the therapeutic side.

My heartfelt appreciation to all the editors and authors for having put in their precious time andsincere hardwork in shaping this book. My best wishes to the whole team.

PREFACE

Geriatric care has become more relevant in the past few decades than it has ever been in the humanhistory. Unfortunately, there has been a lack of good comprehensive reference material for the same.This book attempts to fill the gap by introducing the intricacies in geriatric care particularly those whichare relevant for developing and underdeveloped areas across the globe.

It does so by playing three roles - as a basic textbook, as a reference source on the myriad issuesrelated to aging and as a supporting material for research. It has been written primarily for postgradu-ates, teachers, researchers and professionals in various discipline of geriatrics.

Scientific writing is a craft and I am proud to be a student of it. The rewards of editing textbookslike this are invaluable and intrinsic and I am indeed fortunate to have been given this task and respon-sibility. The task had its share of challenges too and a risk of losing focus and direction because of thelarge number of authors, and section editors. Anticipating them, I had set a simple and clear goal toserve as a beacon for myself and the team to reach the end avoiding the pitfalls on the way and I hopeit has served the purpose. The goal was and is to - "Provide a sound and comprehensive understandingof geriatric care and medicine and stimulate the reader's mind towards innovative ideas & methodol-ogy."

While aging is an inevitable event after birth, most people dread old age. This is more importantin view of increase in aging population, their diverse medical needs, advances in technology and anincrease in number of related health personnel. The elderly are often challenged by health complica-tions, difficulties in obtaining quality medical care, financial and legal hurdles and psychological orsocial issues. While momentum has picked up in developed countries, in developing countries it is stillin infancy.

To live with dignity and with optimum health are fundamental rights of an individual and elderlyare no exception. Unfortunately, little has been done by medical, social, political and pharmaceuticalcommunities to address this issue. Collapse of conventional social network, rising cost of living andhealth care and changes in family support have further complicated the issue.

This book contains a tremendous amount of information. This is not just a narration of academicdescription. It is a well baked dish with flavour of personal experience, wisdom and newer perspec-tives. The version that you hold in your hands today has reached its current state after careful andinspired reviews and critique by a host of talented individuals.

All authors, eminent experts in their field, have been identified with the guidance of sectioneditors. A typical chapter begins with a historical background followed by the main text discussingimportant issues on the subject and ends with future perspectives and key messages. Focus has beenon basic principles and how it differs in elderly compared to young and adults. Each author has pro-vided his viewpoint and writing style will differ from chapter to chapter while conforming to this overallstructure and theme. While each patient is unique and requires an individualized treatment, we haveattempted to give a more holistic approach and view. Those readers who want to learn more have beenprovided with suggested reading at the end of chapter.

May this book provide all elderly hope, healing and a healthy life. I also pray that all personnelconnected with care of elderly will have new and innovative ways of treatment and this book is able tohelp in a small way towards achieving that vision. I look forward to seek your patronage, suggestionsand critique.

Quest for a good textbook on geriatric medicine which is directed towards problems in developing andunderdeveloped countries is a long pending dream for academician across the globe. Perhaps keepingthis in mind, Dr AB Dey proposed, at a general body meeting of Indian Academy of Geriatrics (IAG)at Jodhpur in October 2012, that the association must take the lead and bring out a textbook on thesubject. He also proposed my name as the editor. Before I could react and put my concerns, everyonepresent in the meeting approved it with an assurance of extending all the help I would need.
