Judul

• Flash Fire Incident on ORF during Condensate Pipeline repair work between ORF and Semen Gresik Jetty (Field Operation) – Feb 24th 2016 – 3 First Aids & Fire < USD 1 M

• Root Causes: a. No procedure and/or rule that regulate work duration limitation. b. Inadequate compliance to Confined Space Entry Procedure. c. Inadequate competency requirements and HSE responsibilities on supervisor role profile. d. Inadequate personnel data base on Control of Work key personnel so that PA & AA assignment did not match with

the authorization. e. Inadequate lead by example of stopping unsafe work from leadership (safety leadership). f. Inadequate compliance to Control of Work Cycle. g. As built drawing of underground pipeline was not match with the actual condition on site.

Kontraktor mengajukan surat permohonan kepada

PHE WMO untuk tes kompetensi bagi personil HSE yang akan dikirim ke

lapangan

Surat permohonan dikirim melalui user departemen

ke HSSE Department dilengkapi dengan CV

personil yang ikut

HSSE Department akan membuat jadwal tes yang

kirimkan via email ke perwakilan kontraktor dan user department

Personil HSE Kontraktor mengikuti tes di tempat yang di tunjuk oleh PHE

WMO (PHE TOWER/ ORF Gresik)

Pengumuman lulus atau tidak lulus akan

dikirimkan melalui email ke perwakilan kontraktor

dan user department

Bagi yang lulus akan di berikan HSE Id Badge Competency

(berlaku 2 thn)

Bila tidak lulus diberikan kesempatan re-test 1 kali lagi setelah mendapatkan pembinaan

Proses Tes Kompetensi Kontraktor dilakukan dengan Online Sistem Melalui Intranet PHE WMO dengan materi pertanyaan mengenai pengetahuan HSE di Industri Minyak dan Gas.

Nilai Kelulusan Minimal adalah 60, apabila lulus diberikan kartu yang berlaku selama 2 tahun.
