Indikasi peeling TCA
Indikasi utama peeling TCA adalah terapi photoaging kerut, actinic
keratoses, lentigines serta kelainan pigmentasi lainnya (Fig. 2). Selain itu dapat juga
digunakan untuk feeling superfisialis skar acne. (Figs. 3 and 4) , hiperpigmentasi
paska inflamasi, melasma, mengecilkan pori-pori, vitiligo, rosasea dan kondisi kulit
lainnya (2). Dibandingkan dengan krim 5-fluorouracil (5-FU) pada terapi actinic
keratoses peeling TCA lebih unggul. (Fig. 5).TCA akan menyebabkan garis-garis
kerut menjadi samar termasuk kerut perioral dalam, tetapi TCA tidak diindikasikan
untuk kerut yang diakibatkan kelainan otot. Sehingga secara umum peeling TCA
akan membuat kulit lebih halus, kekeringan berkurang.2, 3
Mekanisme kerja dan efek histologis TCA
Aplikasi TCA pada kulit menyebabkan presipitasi protein dan koagulasi
nekrosis sel di epidermis. Pada konsentrasi tinggi menyebabkan nekrosis kolagen di
papila dermis sampai dermis reticularis. Dalam beberapa hari jaringan nekrotik
exfoliasi dan terjadi reepitalisasi kulit dari stratum germinatum follikel rambut yang
tidak rusak pada saat peeling kimiawi. (Fig. 8). Remodeling kolagen dermis paska
peeling medium bisa berjalan sampai beberapa bulan. Konsentrasi TCA 40-60% (
medium-dalam) dapat menyebabkan luka papila dermis sampai retikular dermis.
Jenis-jenis peeling TCA
Superfisial TCA
Peeling TCA superfisial dilakukan dengan TCA 10-30%
Peeling Medium
Peeling medium biasanya dilakukan dengan konsentrasi TCA 30-50%. TCA
akan menyebabkan koagulasi protein membran dan merusak sel-sel di
epidermis sehingga keratinosit sehat akan mengganti sel abnormal dan
menstimulasi kulit untuk memproduksi kolagen baru.
Peeling Dalam
Konsentrasi TCA diatas 50%. Peeling dalam akan menyebabkan koagulasi protein
sampai retikular dermis yang secara klinis terlihat frosting dan menyebabkan
epidermolisis lengkap.

Prosedur peeling TCA
Pembuatan larutan TCA
Kristal TCA dicampur dengan air sesuai dengan konsentrasi yang diinginkan.
Misalnya untuk membuat TCA konsentrasi 35% maka 35 gram kristal TCA
dilarutkan dengan 100 cc air. Larutan ini stabil pada temperatur ruangan atau
refrigerator. Sedian ini sebaiknya disimpan dalam kontainer kaca karena bersifat
korosif dan harus ditutup sebab evaporasi dapat meningkatkan konsentrasi larutan.
Proteksi Mata
Bahan peeling kimiawi umumnya bersifat kaustik terhadap mata, sehingga
penting dilakukan proteksi mata dengan ointment, gauze, pat, tap sebelum peeling
dimulai. kepala pasien dielevasi 30 0 dan aplikasi daerah sekitar mata dengan solusio
yang agak kering dengan jarak 2-3 mm dari bats kelopak mata.
Precleansing
Sebelum dipeeling wajah dibersihkan dari minyak dan keratin yang akan
menghalangi penetrasi peeling. Agent pembersih yang sering digunakan adalah
asetone, alkohol atau scrub.
Aplikasi TCA
TCA diplikasikan perlahan ke kulit dengan menggunakan aplikator cotton tip
atau kasa dengan lembut. is applied evenly with cotton-tip applicators or saturated 2
in. 3 2 in. Gauze (kasa) pads with smooth strokes (Figs. 12 and 13). The pads
(bantalan) should be wrung out sufficiently to prevent a splash (percikan) effect that
could be injurious to the eyes or cause treatment of unintended areas of skin. The
hand that is holding the TCA gauze should be gloved and dry gauze should be held
in the other hand to absorb any excess drip and to avoid inadvertent (tdk sengaja)
application of the chemical. Since the patient will feel a moderate sting (menyengat)
when TCA is applied, cool compresses may be applied to each area as it is treated
and a small fan may be held and directed by the patient to the uncomfortable area (6).
To relieve some anxiety, the patient can be reassured that once the stinging and
burning begins, the sensations are already nearing their peak and a denouement
(peleraian) should gradually begin. Preoperative sedation is generally not necessary,
though light sedation with a benzodiazepine or narcotic can be used preemptively for
an apprehensive(gelisah) patient.

Anatomic subunits of the face are peeled sequentially, with feathering( bulu2)
into the hairline and at the treatment margins. One pattern may begin from the
forehead(dahi) to the temples and cheeks, followed by the lips, nose, and eyelids
(kelopak mata) (4). Often, broad strokes of 2 in. 3 2 in. TCA-soaked gauze are used
for even application to larger regions, and wide strokes with cotton-tip applicators
are used for smaller areas (4).
The endpoint (titik akhir) of treatment
As the skin is treated, a white color change or “frost” (tetesan embun) results
from epidermal and dermal protein coagulation (Fig. 14). Some authors feel that
monitoring this frost (e.g., color intensity and time to clear or fade) is a reliable
indicator of peel depth. Others suggest that the depth of injury cannot be appreciated
from color alone, and that this indicator is unreliable (1). Nevertheless, diffuse
redness with light (mendung) cloudy-white frosting that clears in 10–15 minutes
indicates superficial injury (epidermis) and is generally the desired endpoint of TCA
concentrations of less than 35%. A more uniform. white frost is indicative of
epidermal and papillary dermal injury and is the endpoint for 35–40% TCA peels,
and also clears over 10–15 minutes. A dense, white, uniform frost is indicative of
deep papillary to reticular dermal injury and is the endpoint for medium-depth peels
with 50% TCA alone. This deeper frost usually clears in 40–45 minutes (9). A
yellow-gray (kuning abu2) frost represents deep injury to the mid- to deep reticular
dermis and forecasts(perkiraan) a prolonged healing time. It generally does not clear
even after 40 minutes. After TCA is applied, a dry gauze pad should be used to blot
(aib) (not wipe) the excess from the skin.
Reapplication of TCA during the same and subsequent(berikut) patient visits
When using 50% TCA as a sole agent (agent tunggal) for medium depth
peels, reapplication during the same patient visit is usually not recommended, since
the effects are already somewhat unpredictable. Although the decision is sometimes
made to reapply TCA to areas with less apparent frost, this results in increased
protein coagulation and wounding, and is generally unnecessary if the area was not
missed with the first application (1). Reapplication can be useful, however, in
selected sun-damaged areas with impaired penetration, such as those areas with
actinic keratoses (1). Reapplication on a subsequent visit should not be performed for
at least 6 months, and most patients do not require re-treatment for several years.

For superficial peels using 10–35% TCA, an even redness or light whitish
film is sought(dicari), and reapplication to uneven(merata) or unresponsive areas
during the same visit is appropriate (1). One practitioner suggests that several repeat
applications of 15–25% TCA may be performed at 3-minute intervals during the
same visit to induce more erythema and frost, and improved clinical results (1).
These superficial peels may safely be repeated at 1- to 2-week intervals, and are
often applied in a series of about six treatments, beginning with low concentrations
and increasing the strength of TCA as tolerated.
Perawatan post peeling
Perawatan post peeling pada TCA medium meliputi penggunaan vaselin atau
antibiotik 3-5 kali perhari sesudah cuci muka. Kemudian pada hari ke 4-5 dilakukan
kompres basah untuk mempermudah eksfoliasi dan hindari pengelupasan krusta
sebelum saatnya. Sesudah reepitalisasi tercapai pada hari 7-10 ointmen dihentikan.
Selanjutnya dilakukan perawatan berupa cuci muka, moisturizer dan eksfoliasi
dengan rutin menggunakan krin tretinoin atau glikolik acid dan tabir surya untuk
meminimalkan resiko hiperpigmentasi paska inflamasi.
Perawatan post peeling pada peeling TCA superfisial biasanya tidak
diperlukan kecuali cuci muka rutin. Pasien dapat beraktivitas normal dan dapat
menggunakan moisturizer dan kosmetik untuk menyamarkan eritem. Hindari
penggunaan tretinoin pada hari ke 3-7 untuk menghindari iritasi. (1).
Kepuasan pasien dan konsultasi sebelum peeling
Sebelum tindakan peeling dilakukan sebaiknya dilakukan penjelasan tentang
tindakan dan efek samping tindakan.
Komplikasi peeling TCA
Penggunaan peeling TCA superfisial biasanya efek samping minimal, tetapi
pada konsentrasi diatas 35% bisa timbul komplikasi berupa perubahan tekstur kulit
,hipopigmentasi ataupun skar hipertrofi (3). Peeling TCA Peeling 60– 75%
sebaiknya dihindari kecuali untuk terapi aktinik keratosis atau lesi hipertrofik
lainnya.. Efek samping TCA dapat dikurangi dengan penggunaan peeling kombinasi.
Jika dengan perawatan tetap terjadi parut maka diberikan terapi injeksi intralesional
triamcinolone acetonide 5–20 mg/ml setiap 3 minggu. Adanya eritem persisten dan
indurasi kulit merupakan pertanda munculnya parut dan perlu diberikan terapi

topikal kortikosteroid topikal. Sehingga penting dilakukan pengawasan ketat. Peeling
TCA merupakan kontra indikasi relatif pada penggunaan isotretinoin dalam 12 bulan
terakhir akibat resiko timbulnya parut.(6). Komplikasi tersering peeling tCA adalah
kelainan pigmen berupa hiperpigmentasi , hipopigmentasi ataupun eitem menetap.
Hiperpigmentasi biasanya temporer dan berespon baik dengan tretinoin, pemutih dan
tabir surya. Hipopigmentasi pada peling TCA umumnya didapatkan pada peeling
dalam dan tipe kulit Fitzpatrick IV, V, dan VI . Eritem menetap lebih dari 90 hari
dpat diterapi dengan hydrocortisone 2.5% topikal, kortikosteroid sistemik jangka
panjang, antihistamin. Milia dapat terjadi paska peeling medium bisa dienukleasi.
Infeksi bakteri jarang terjadi dan hanya terjadi jika perawatan luka tidak adekuat dan
jika terjadi sebaiknya dilakukan kultur. Untuk mencegah reaktivasi herpes diberikan
profilaksis acyclovir 400 mg 2 kali/hari 2 hari sebelum peeling dan 5-10 hari paska
peeling. (6).

Kombinasi peeling TCA untuk peeling
Penggunaan peeling TCA konsentrasi tinggi berhubungan erat dengan
resiko,sehingga sebaiknya peeling TCA dapat dikombinasi dengan bahan peeling
superfisial lain yang tidak terlalu iritatif. Peeling superfial bersifat keratolitik dan
merusak kohesi epidermis sehingga penetrasi TCA lebih dalam.
1. Kombinasi peeling TCA 35% dengan CO2
Brody and Hailey melaporkan penggunaan C02 padat dapat merusak
permukaan epidermis sehingga peeling TCA 35% dapat mencapai dermis atas. (13)
Kombinasi peeling CO2 dan TCA diawali dengan mencelupkan es kering kedalam

aseton dan alkohol dan diaplikasikan ke kulit untuk menyebakan trauma epidermis
dan selanjutnya diikuti aplikasi 35% TCA untuk memperdalam trauma sampai
dermis. Pemeriksaan histopatologi menunjukkan bahwa peeling ini mencapai papila
dermis. Peeling ini efektif pada moderate actinic damage, hiperpigmentasi
superfisial, kerut ringan. Because of the ability to manually focus injury with the
CO2 pretreatment at the perimeter of depressed scars, this is one of the few peel
techniques that can also offer focal control of peel depth, thereby improving the

2. Kombinasi TCA 35% dengan Jesner’s solution
Jesner’s solution dibuat Dr. Max Jessner dari 14 gram resorcinol, 14 gram
asam salisil dan asam laktat 85% dan etanol sampai volume100 ml, larutan ini
merupakan bahan peeling superfisial poten. Kulit sebelumnya di peeling dengan
Jesner’s solution kemudian diaplikasihkan TCA 35%. Kombinasi ini efektif pada
moderate actinic damage, pigmentary dyschromias, kerut ringan dan juga bagus
digunakan untuk memudarkan batas jika digunakan dengan peeling dalam maupun
laser (4). Pemeriksaan histopatologi Jessner’s dan 35% TCA menyebabkan
penipisan grenz zone dan pemendekan serabut elastin. (2). Studi sebelumnya
melaporkan kombinasi Jessner’s solution dengan 35% TCA tampa komplikasi.
3. Kombinasi TCA 35% dengan Aasam glikolat 70%
A 70% glycolic acid superficial peel, followed by 35% TCA to extend injury
into the dermis, is another combination peel that is effective for medium depth
penetration. It has also been found useful for moderate actinic damage, pigmentary
dyschromias, and minor wrinkles. Histologic studies show comparable
improvements with the Jessner’s plus TCA peel, with a more narrow grenz zone and
elastotic band (2). The technique of application is different, however, in that no
cleansing or degreasing procedure is used prior to the application of the glycolic
acid, and the glycolic acid is neutralized with water prior to the application of TCA
(4).
Conclusion
TCA is a valuable agent for both superficial and medium-depth peels, and its
effectiveness has been well demonstrated. Used properly, TCA peels continue to be

a safe and reliable method for rendering(menberikan) histologic and clinical
improvement to the skin. TCA is particularly safe when used as a superficial peel or
in “combination peels” of medium depth. As a sole agent in higher concentrations, its
predictability (diramalkan) is less certain and should generally be avoided by the
novice (orang baru) practitioner. Since all resurfacing techniques are quintessentially
(dasarnya) operator dependent, it is important to develop a thorough (teliti)
understanding of and comfort with the peeling agent.
