Jan Brown is a 32-year-old Native American woman who is in her26th week of pregnancy. Dring her !rst prenatal visit" #rs.
Brown$s rine was tested for glcose and protein% &oth tests werenegative. At her last prenatal visit" the reslts of her rine test forprotein and glcose were
Mrs. Brown is overweight and has a family history of diabetes. Her age, ethnicity, family
history, and weight put her at risk for gestational diabetes mellitus. Mrs. Browns
physician uses the recommendations of the National Diabetes ssociation for laboratory
testing for diagnosis of gestational diabetes. !he criteria for diagnosing gestational
diabetes are based on the original research of "#ullivan and Mahan$% and modified by
&arpenter and &oustan.$' !he National Diabetes (roup protocol for diagnosis of
gestational diabetes is outlined in !able ')*. +n this protocol, the physician first orders a
-g glucose challenge to screen for diabetes. /or this screening test, the patient need
not be fasting. Blood is drawn 0 hour after the glucose solution is consumed. Mrs.
Browns results are shown below1
Because the screening test is positive, the physician asks Mrs. Brown to return to the
laboratory after an 2hour fast. Mrs. Brown is instructed to refrain
