ABSTRACT
The proponents built this project to create a system forDental Reservation with the use of Internet and New Technologies. To develop a website would facilitate appointment reservation andsave a large of customer’s information through an effective yethelpful and simple system using SMS Notification to confirm thatthe client request has been reserve.
Basically, the study is an action research. Unstructuredinterviews and document analyses were used as a data gatheringtools. Wherein proponents a questionnaire for the saidrespondents, and observation based on what proponents observed.
And based on the summary findings in terms of Functionalityif the system is functioning well according to its specific task,Reliability if its reliable to manage patient’s account,Usability if its usable for both personnel’s and clients,Efficiency on retrieving patient’s record, the Portability if itsrun in any operating system and can be viewed in any kind ofbrowser using Internet Connection, and last the Maintainabilityif the system is maintainable.
The “Online Dental Reservation with SMS Notification” willbe a big improvement over the manual system of the clinic.Patients will be able to access the page as long as there is an internet connection for reservation inquiries. Theproponents conclude that the Online Dental Reservation with SMSNotification should be enough once it is fully completed overtime. It is a system that will be having a great improvement tothe patient and Dr. Ricky Salvador relationship and will take alot of work off from the patient and to Dr. Ricky Salvador. Andalso the proponents then conclude that the patients of Dr. RickySalvador will have a great benefit from the system because of itsauthentication from the site this will help Dr. Ricky Salvadoridentify the patient even if she/he set an appointment online.
The proponents recommend that the company should considerthe system to easily reserve their patient thru online and to thefuture researchers to continue improving the system
CHAPTER I
INTRODUCTION
BACKGROUND OF THE STUDY
In the present generation, computers are widely used
in a way that it makes a job faster and easier. As the
population grows the need for dental care services is to be
in equal or at least add some new way to simply some
approach. Manual procedures may no longer be effective in
handling or accommodating patient, that is why the clinic
owner is find an alternatives with the use of new
technology and techniques to assist the patients.
Dr. Ricky Salvador used a Manual Services in almost 10
years, maybe he maintain his profession but not his profit
or at least increase of it. Dr. Ricky encountered many
difficulties during his services to manage his time in
order to reach maximize profit. There are some patients who
already have an appointment to Dr. Ricky but it doesn’t
assist it, because of many reasons. Reasons like, conflict
or its hard to accommodate more patients in one day. He
didn’t know how to manage more patients because of long
queue sometimes.
And there are times that patients have a contact to Dr.
Ricky but they didn’t know where the exact location of his
clinic. Unlike Online Dental Reservation with SMS
Notification has information on how to contact Dr. Ricky
and where his clinic can locate.
The Online Dental Reservation with SMS Notification
will give opportunity for Dr. Ricky Salvador to encourage
more customers and earn more profit with the use of their
clinic posted in the website. In that simple way the client
can be easily seen the clinic hours and schedule, it may
also give them information about Dr. Ricky Salvador as well
as the scope and limitation of his practice and to
advertise about his expertise. In that situation the client
can set an appointment without any hesitation and most
important is the assurance that they already have
appointment for that day by sending them a SMS Notification
for confirmation.
OBJECTIVE OF THE STUDY
GENERAL OBJECTIVE:
The proponents aim to develop a website which would
facilitate appointment reservation and save a large of
customer’s information through an effective yet helpful and
simple system.
1. To avoid patients to wait/ overlapping of schedule.
2. To reduce the assistant’s time to record patients.
3. To make the business hassle free.
4. To help patients get the schedule they want.
5. To help Dr. Ricky Salvador organize his schedule.
6. To send confirmation if the said schedule is available
or not.
7. To reduce the use of bond paper and other hard copies,
materials, instead the data will be saved in the database
and it will secure.
8. To have Dr. Ricky Salvador the ability to confirm
clients request anytime and anywhere as long as has a
computer with internet connection.
Specifically, this study aims to:
1. Develop a Online Dental Reservation with SMS
Notification using the Waterfall Model with phases of
development such as:
1.1 System Planning
1.2 System Analysis
1.3 System Design
1.4 Implementation
1.5 System Testing

variety of products to customers and clients. Although
their products, services and office procedures vary widely,
there is one thing most businesses have in common:
competition. Unless a service is completely unique and
unavailable from any other providers, businesses must
continuously look for new ways to stay ahead of the
competition. Whether it's through marketing, the creation
of new products or services, or added benefits for its
clientele, rising above competitors can help ensure both
short- and long-term success. The thought of staying
competitive can make some owners and operators cringe, as
they may believe it will cost a substantial amount of
money. However, there are many inexpensive actions a
business can take to keep pace and pass their competitors.
One of these is online reservation system.
New technologies especially computers are very
convenient for community nowadays. And for the past 5 years
the percentage of computer users has increased very fast.
And as technology growth increase, Dentist use different
strategies to improve patient engagement as well as the
satisfaction of the patient and the quality of the care.
Using technology can organize things in fast and convenient
way. The researcher will use a functional model and
conceptual schema of the system. A program’s input and the
process that is required to transform inputs into outputs.
And as far as I can observe in this generation, people
can rely on what is new and adopt what is useful. It is
because everything now is going to be generated and
practical.
SIGNIFICANCE OF THE STUDY
This part of the study explained the importance of
developing the system to the following:
1. Dentist. Online medical appointments can benefit Dr.
Ricky up to 50% increase of patients per day. Since
walk in or scheduling over the phone is limited to
patients who have their numbers saved and know their
clinics. In addition, Mr. Ricky can benefit by means
of extra advertisement. For other patients who do not
know his clinics, it might serve them as a reference
map.
2. The Patients. It will allow the patient to
conveniently and securely book their appointments and
reservations online through any Web connected device,
such as a computer, laptop, smart phone or tablet with
internet connection.
3. The Clinics. Free advertisement as their location is
shown on the web.
4. Clinic Personnel. Assistant or Secretary can conduct
their task more efficiently and accurately.
5. Community. Although we have different types of
patients, younger patients and clients may have easy
transition, as this group is typically savvy and
actually prefer to schedule appointment online. For
our large number of senior citizen patients, there
still exists a significant percentage in this group
that may not be comfortable scheduling online, which
would be consideration when determining how to best
utilize online scheduling.
6. Future Researchers. The proposed study will serve as a
reference for other or future researchers with the
same study or system.
SCOPE AND DELIMITATION
The scope of the system “Online Dental Reservation
with SMS Notification” is can accommodate only three (3)
patients per hour and every patient can only reserve two
(2) services. And if the time has already taken the clinic
personnel will not going to confirm the request reservation
instead it will going to cancel and find another time which
is available. The clinic hours will start at exactly 7:00
in the morning and it will end at exactly 9:00 in the
evening. So there is enough time for the patients to see
Dr. Ricky Salvador and achieve the service they need
whether it is for cleaning, tooth conserving, etc.
This system can provide SMS Notification for those
clients who take their schedule as a confirmation that they
already have an appointment to Dr. Ricky Salvador. And if
the patient has wrong information by inserting it in the
corresponding field it will send to the admin but it will
consider it as Cancel and the SMS Notification will
automatically send to the client. But the system can
provide only one (1) gateway that is why the client cannot
allow canceling their reservation through responding using
SMS Notification that they received. However if there are
some clients who want to cancel their appointment the
system will provide the site or another contact number that
the client can visit or send a message if ever this
situation happen.
In terms of not attending to the corresponding
reservation, Dr. Ricky Salvador will give only 15 minutes
for those who have a reservation. And if the 15 minutes
ended and the patient is no appearance at all the clinic

DEFINITION OF TERMS
The following terms are described for the purpose of
clarity:
Database - is basically a collection of information
organized in such a way that a computer program can
quickly select desired pieces of data. You can think of a
database as an electronic filing system.
SDLC - also referred to as the application development
life-cycle, is a term used in systems
engineering, information systems and software
engineering to describe a process for planning, creating,
testing, and deploying an information system.
SMS - is a text messaging service component of phone, Web,
or mobile communication systems. It uses standardized
communications protocols to allow fixed line or mobile
phone devices to exchange short text messages.
System – is a group of interaction, for an interrelated or
interdependent elements complex whole.
SQL – (Structured Query Language) is a standardized query
language for requesting information from a database.
Technology – is the scientific method and or materials used
to achieve a commercial industrial objective. IT is the
making, modification, usage, and knowledge of tools,
machines, techniques, crafts, systems, and methods of
organization, in order to solve a problem, improve a
preexisting solution to a problem, achieve a goal, handle
an applied input/output relation or perform a specific
function.
WEBSITE - written as web site, or simply site, is a set of
related web pages typically served from a single web
domain. A website is hosted on at least one web server,
accessible via a network such as the Internet or a
private local area network through an Internet address
known as a Uniform resource locator. All publicly
accessible websites collectively constitute the World Wide
Web.
CHAPTER II
Review of Related Literature and Studies
This chapter refers to the information and data about
related topics and articles gathered from books and
internet that are used to justify and to support the
importance and ideas of the proposed system.
Through studying the related literature, either
local or foreign, it can lead the researchers for
better understanding of the study. This can give them
a broader knowledge about the research.
Foreign Literature
The web has become an opportunity for the
marketers to add value to products and services. The
phenomenal growth and rising of the popularity of the
internet and the World Wide Web has become a key to
attract more consumers and businesses to engage the
benefits of Electronic Commerce (E-commerce). This E-
commerce is sited as any form of business transaction
in which the parties interact electronically rather
than by physical exchanges or direct physical contact
(Aldin, Brehmer & Johansson, 2009). This has
transformed the traditional commerce and enhanced
sales and exchanges of
merchandise and information. It is not just considered as
single entity of technology but a combination of
technologies where applications, processes, business
strategies are necessary to do business electronically. The
availability of goods and services with the click of a
mouse is changing the global setting.
Consideration of the websites design and
operations are dependent upon the nature of business
activities and target consumers. According to
(Burleson 2010), a website must be simple and focused
site to succeed. One that is easy to build,
maintenance-free, low cost, trustworthy, and a
powerful traffic-builder and customer-converter.
Having the right tool and the right product alone
doesn’t ensure the success of the website. To be
effective, the Website must be designed with the
target audience as foremost consideration. Website
designers must balance design considerations and
capabilities with client objectives and the consumer’s
level of understanding in the modern technology
(Geissler, 2010).
Internet users will be unlikely to use the
Internet on a routine basis for commerce unless they
have confidence that their communications and data are
safe from unauthorized access or modification.
Confidence in the system itself depends on the
availability of effective means both for protecting
information systems attached to telecommunications
networks and for authenticating and ensuring
confidentiality of electronic information to protect
data from unauthorized use. It also requires well
educated users who understand how to protect their
systems and their data. Unfortunately, as noted in the
US Framework document, there is no single "magic"
technology or technique that can ensure either
security or reliability. Instead this requires the use
of a range of technologies including encryption,
authentication, password controls and firewalls as
well as a trustworthy key and security management
infrastructures. (Patterson,2010).
Local Literature
In the Philippines, e-commerce is mostly being
implemented by major retailers and multinational
corporations for bank-to-bank exchange. A number of
Business-to-consumer transactions have emerged through
the years such as auctions, online shopping, and
online banking (Lacson, Pasadilla, 2009). This just
shows that Filipino businesses welcome this new
opportunity in selling goods since 16% of the
population is using Internet. This new marketing
strategy will not only benefit those big companies but
also the small businesses who cannot afford to
advertise their products. Just by creating a website
in a very affordable cost would make the business grow
in terms of sales and enhance the company image as
well.
“Philippine firms have yet to fully tap the
potentials of e-commerce that will help them to compete in
domestic and global markets. In the process of preparing
themselves to conduct e-commerce, these firms will be
changing the way they do business. These changes are
expected to have an impact on the welfare of Philippine
workers. Most efforts to measure the economic effects of e-
commerce—including those on workers’ welfare—have focused
on the U.S. and Europe, who are considered to be the
leaders in e-commerce. In general, it discuss the changes
in the nature of work from the front to back office is
likely within the sectors performing services Employment
demand for these industry, they may be focused away from
agents and blue collared workers, toward higher management
and executive staff”, stated by Roberto.
Developing a technology for the
companies/countries productivity should be the main
concern of the developer. As stated, the state shall
give priority to research and development, invention,
innovation, and their utilization; and to science and
technology education, training, and services. It shall
support indigenous, appropriate, and self-reliant
scientific and technological capabilities, and their
application to the country's productive systems and
national life. (Article XIV Section 10, Science and
Technology ,2012)
As Filipinos entered the 21st century, local e-
commerce transaction reached Php 1 billion. It shows that
there’s at least one billion transaction sales growth
annually. If this trend continues, figures can reach up to
Php 20 billion as more businesses conduct online
transactions. If significant economic growth takes place in
the next 10 years, these estimates may even double or
triple (Toral, 2011).
Foreign Study
E-commerce has become a standard method of
providing distributed information to many different
platforms. It is being implemented by a growing number
of companies to be more completive in corporate world.
All companies need to make a step forward towards a
new technology. To be able to compete with the global
market, e-commerce was a good answer (Lanvin and
Maggarqul, 2010).
The company’s websites seem to have significant
influence on sales and corporate image, and are
expected to contribute to overall customer
satisfaction. The easiest way to be reliable to the
customer is to maintain an easy and simple image in
the company’s website, which created positive web
experience to the customer. This can be done by having
transparent interface, rich content, easily accessible
information and having a design that facilitates
multiple audiences. This is simply emphasizing the
importance of knowing the target visitors as they have
different tastes in terms of colour and design as a
whole. Also, a good design is not enough to make your
customers stay in the website; it must be informative
as well especially on the product details. As what
Efendioglu and Igna stated that companies may have
great content, dynamic and attractive design on their
website, but the customer may not be aware of that
website because of wrong or inadequate online
marketing (Hossain, 2010).
Website security also refers to the protection of
confidential information of the consumers. This is one
way of gaining the customers trust because customers
want to believe that the information they provide will
be protected and used only in appropriate ways
(Mansoorian, 2009). Some of the important details to
be protected are Address and Contact Number which has
to keep in private. Passwords must also be encrypted
to protect the consumer’s account from hackers.
Local Study
Businesses who engaged into Electronic Commerce
(e-Commerce) have been observed to gain a competitive
edge in the field of Marketing in terms of
accessibility, convenience and availability. Since the
internet is accessible by people globally, the
customers are no longer limited within the vicinity of
the store. Anyone can visit the store anywhere,
anytime without hassle. Just by navigating the store,
the customer can choose the products and purchase it
without going to the store directly .E-commerce proves
to enhance the way usual business transactions occur
and helps lessen the time and effort consumed by the
company and customers as well (Deanna and Fritz,
2009). With these, a good relationship with their
consumers will be established and their objective of
increasing their sales will be achieved (Kim and
Katherine, 2011).
“Through internet, E-commerce offers a
simplified approach in business deals and gives a new
option for selling and buying the company’s products and at
the same time not only selling and buying of products but
also advertising the site. (Roso and Navarro 2012). Since
the products are available in the web, the more chances of
gaining exposure to target customers and can eventually
encourage them to purchase an item. It’s the most
affordable way of advertising compared to some paid
commercials that even small businesses can easily
implement.”
Since most of the companies employ websites as one of
their marketing strategies, competitors are also spreading.
The company must take into considerations the key factors
in order to have a successful online marketing system
starting from the target audience, web design, interface,
security in terms of payment and customer information,
content and even the smallest details of a website.
SYNTHESIS
The proponents will be expecting to develop a
business that can be the best alternatives for
existing common manual procedures of services. A
system that can provide high definition in terms of
advancement and benefits from it that can assure the
clients a very well set appointment to Dr. Ricky
Salvador, a message that will be given for those who
send a request reservation, a system that can provide
daily, weekly, and monthly reports of the business
income. That is why the proponents want to help to the
said common problems, by building this Online Dental
Reservation with SMS Notification for Dr. Ricky
Salvador to be a very helpful but effective system.
CHAPTER III
MATERIALS AND METHOD
The purpose of these studies is to examine the
usefulness of online reservation of patients using a
friendly user and reliable web design that will also show
significant changes in the development of a system to
improve Dr. Ricky Salvador and his patient’s interactions.
This chapter discusses the procedure in gathering
data, statistical tools, materials and methods used in
developing software.
Answering phone call, listing by pen to paper is a
common way of accommodating patients for appointment
reservation, common errors like spelling of name, addresses
and other error information are inevitable. Though in the
past this was a tried and true method, the rise in its
customer population, and the further increase in the
complexities of the reservation and how they gather
information, has made their methods crude and obsolete.
With the use of online based appointment scheduling,
information is more secure, more reliable and more
accessible. By modernizing their appointment system with
the use of internet, the proponents hope to ensure the
security and validity of scheduling
made by the customers by storing them in a more up to date
way, and to make it fast and more systematic as well. The
proposed system is critically analyzed and designed to
lessen or even eliminate the common errors and hassles in
gathering information in the existing system. Unique
username and password are generated to provide the best
security. The customer will have an easier and lesser time
to book anytime and anywhere with Internet access.
Research Design
Research design considered as the ‘blue print’ or the
skeleton of the study since it serve as the basis of the
study conducted. The Developmental Research is use in this
study. These designs will help the researcher to gather
data, and solicit responses some valid explanation that
will help in the development of the study.
Sampling Design
Sampling may be defined as measuring a small portion
of something and then making a general statement about the
whole thing. It enables the study of a large, heterogeneous
population more economic wise, meaning not too costly, and
more realistic and possible to make. The population to be
researched upon is quite small; hence scientific sampling
is implied to each member of the population since the
number of population is attainable for sampling. The
researchers decided to use simple random sampling technique
since the population is well in the reach of statistical
evaluation.
These are the possible person that is being conducted.
1. IT Professionals – they are important in evaluating
the system because they already have the knowledge and
experiences as a professional that can help the
proposed system to be improved.
2. Dentist – they are the people that will use the
proposed system that is why they are also concern in
evaluating it.
3. Clinic Personnel – they will also use the proposed
system that is why their feedbacks are also important.
4. Patients – they will also use the proposed system that
is why their feedbacks are also important.

Data Gathering Tools
Data Gathering instruments refers to the device
used to collect data, such as paper questionnaire and
interviewing. In order to support the validity of the
data, interview has been conducted to gather
information. The proponents were also consulted in
the internet and books that are conducive in
developing the system. And later chose the best-
printed materials from those gathered information
that would best serve in helping out the proponents
in developing the website.
1. Questionnaire List of
a research or survey questions asked to respondents,
and designed to extract specific information.
It serves four basic purposes: to (1) collect the
appropriate data, (2) make data comparable and amenable
to analysis, (3) minimize bias in formulating and asking
question, and (4) to make questions engaging and varied.
2. Interview is a conversation between two or more people
where questions are asked by the interviewer to elicit
facts or statements from the interviewee. Interviews are a
standard part of journalism and media reporting, but are
also employed in many other situations,
including qualitative research.
3. Observation is the active acquisition of information from
a primary source. In living beings, observation employs
the senses. In science, observation can also involve the
recording of data via the use of instruments. The term may
also refer to any data collected during the scientific
activity.
4. Survey to ask a question or a series of questions in
order to gather information about what most people do
or think about something. The researchers are able to
gather data on the behavior, practices, attitudes,
opinions, interest and perceptions of the users on the
existing and the proposed system, and then such data is
analyzed, organized and interpreted.
Statistical Tools
It is a type of mathematical analysis involving the
use of quantified representations, models and summaries for
a given set of empirical data or real world observations.
Statistical analysis involves the process of collecting and
analyzing data and then summarizing the data into a
numerical form.
Statistics is one way of getting the information’s
organized. To have a general view of the whole scenario
study, statistical tool will used. This also includes the
scaling system, which will use by the researchers as a
technique to monitor the respondent’s interpretation of
facts. The Likert scale will use to interpret items in the
questionnaire. These responses were based on the
respondents. The range and interpretation of five point
scale are shown in the table.
Range Scale Rating
4.50 – 5.00 Strongly Agree 5
3.50 – 4.49 Agree 4
2.50 – 3.49 Neutral 3
1.50 – 2.49 Disagree 2
1.00 – 1.49 Strongly Disagree 1
Table 3.2Likerts Scale
The Five Point Likert’s Scale
Likert Scale
A Likert’s scale is a psychometric scale commonly
involved in research that employs questionnaires. A
Likert’s Scale is a rating scale that requires the subject
to indicate his or her degree of agreement or disagreement
to a statement. In this type of questionnaire, the
respondents were given five response choices. These options
served as the quantification of the participants' agreement
or disagreement on each question item.
Weighted Mean
Weighted Mean is an average computed by giving
different weights to some of the individual values. If all
the weights are equal, then the weighted mean is the same
as the arithmetic mean. Whereas weighted means generally
behave in a similar approach to arithmetic means, they do
have a few counter instinctive properties. Data elements
with a high weight contribute more to the weighted mean
than do elements with a low weight. The weights cannot be
negative.
Weighted Mean Formula
The Weighted mean for given set of non-negative data
with non-negative weights can be derived from the formula:
The collection of tools employs the study of methods
and procedures used for gathering, organizing, and
analyzing data to understand theory of Probability and
Statistics. The set of ideas which is intended to offer the
way for making scientific implication from such resulting
summarized data. In many applications it is necessary to
calculate the weighted mean for a set of data with
different individual errors.
System Design
System design is a phase where researchers use the
gathered information on requirement analysis to build up
possible techniques for system. The technique would then
evaluate if it’s feasible so that the client can be
notified when some changes occur.
METHODS USE IN DEVELOPING THE SYSTEM
System Planning
This kind of method is used to help the researcher to
formulate and design a system that is effective and satisfy
the need of the client.
System Analysis
This method is used by the researcher to analyze or
evaluate the main goal of the study if feasible to put into
function and apply for actual operation.
System Design
This method is used by the researcher to visualize and
develop the desired features, processes and documentation
of the analyzed plan for this study.
System Developing
The Researchers used PHP, HTML, JQuery and MySQL, API
Gateway in developing the system. The main concern of the
unit testing is to verify the internal logic of code done
by the programmer by testing every Unit testing, test all
possible execution of the system.
Integration Testing
In integration testing the modules will be tested so
that errors can be corrected. These errors can consider as
a feedback for the system evaluation.
Acceptance Testing
Acceptance testing or client acceptance testing is the
phase of testing used to determine whether a system
satisfies the requirements specified in the requirements
analysis phase. This phase is used by the clients to
determine whether to accept the proposed system or not.
Thus this help whether the system satisfies the requirement
or not and to ensure if the system will work properly when
deployed.
