Did you mean: LAPORAN PENDAHULUAN LEUKIMIA TINJAUAN TEORITIS 2.1 ANATOMI SISTEM HEMATOLOGI Sistem hematologi tersusun atas darah dan tempat darah diproduksi, termasuk sumsum tulang dan nodus limpa. Darah merupakan medium transpor tubuh, volume darah sekitar 7%-10% berat badan normal dan berjumlah sekitar 5 liter. Darah terdiri dari atas 2 komponen utama, yaitu sebagai berikut. 1.Plasma darah, bagian cair darah yang sebagian besar terdiri atas air, elektrolit,dan protein darah. 2.Butir- butir darah (blood corpuscles), yang terdiri dari komponen-komponen berikut ini. Eritrosit : sel darah merah (SDM- red blood cell) Leukosit : sel darah putih (SDP- white blood cell) Trombosit : butir pembeku darah – platelet. 2.2. STRUKTUR DAN FUNGSI NORMAL SEL DARAH PUTIH Pada keadaan normal, darah manusia mengandung 4000 - 11.000 sel darah putih per mikroliter. Dari jumlah tersebut, jumlah tersebut, jumlah sel terbanyak adalah granulosit (leukosit polimorfonukleus, PMN). Sel granulosit muda memiliki inti berbentuk seperti kuda, yang akan berubah menjadi multilobular dengan bertambahnya umur sel. Sebagian besar sel tersebut mengandung granula neutrofilik (neutrofil), namun sebagian kecil mengandung granula yang dapat diwarnai dengan zat warna asam (eosinofil), dan sebagian lagi mengandung granula basofilik (basofil). Dua jenis sel yang lazim ditemukan dalam darah tepi adalah limfosit, yang memiliki inti bulat besar dan sitoplasma sedikit, dan monosit, yang mengandung banyak sitoplasma tak berglanula dan mempunyai inti yang berbentuk ginjal. Kerja sama sel tersebut menyebabkan tubuh memiliki sistem pertahanan yang kuat terhadap bebagai tumor, infeksi virus, bakteri, dan parasit (Ganong,2008). Fungsi Sel Darah Putih adalah sebagai serdadu tubuh yaitu membunuh dan memakan bibit penyakit/bakteri yang masuk ke dalam jaringan RES (sistem retikuloendotel), tempat pembiakannya di dalam limpa dan kelenjar limfe; sebagai pengangkut/ membawa zat lemak dari dinding usus melalui limpa terus ke pembuluh darah. Sel leukosit disamping berada di dalam pembuluh darah juga terdapat di seluruh jaringan tubuh manusia. Dua jenis sel yang lazim ditemukan dalam darah tepi adalah limfosit, yang memiliki inti bulat besar dan sitoplasma sedikit, dan monosit, yang mengandung banyak sitoplasma tak berglanula dan mempunyai inti yang berbentuk ginjal. Kerja sama sel tersebut menyebabkan tubuh memiliki sistem pertahanan yang kuat terhadap bebagai tumor, infeksi virus, bakteri, dan parasit (Ganong,2008). Fungsi Sel Darah Putih adalah sebagai serdadu tubuh yaitu membunuh dan memakan bibit penyakit/bakteri yang masuk ke dalam jaringan RES (sistem retikuloendotel), tempat pembiakannya di dalam limpa dan kelenjar limfe; sebagai pengangkut/ membawa zat lemak dari dinding usus melalui limpa terus ke pembuluh darah. Sel leukosit disamping berada di dalam pembuluh darah juga terdapat di seluruh jaringan tubuh manusia. Pada kebanyakan penyakit disebabkan oleh masuknya kuman/infeksi maka jumlah leukosit yang ada di dalam darah akan lebih banyak dari biasanya. Hal ini disebabkan sel leukosit yang biasanya tinggal di dalam kelenjar limfe, sekarang beredar dalam darah untuk mempertahankan tubuh dari serangan penyakit tersebut. Jika jumlah leukosit dalam darah melebihi 11.000/mm3 disebut leukositosis dan kurang dari 4000mm3 disebut leukopenia. Macam-macam leukosit secara jelas meliputi : 1.Agranulosit. Sel leukosit yang tidak mempunyai granula di dalamnya, yang terdiri dari: a. Limfosit, macam leukosit yang dihasilkan dari jaringan RES dan kelenjar limfe, bentuknya ada yang besar dan ada yang kecil, di dalam sitoplasmanya terdapat granula dan intinya besar, banyaknya 20%-25% dan fungsinya membunuh dan memakan bakteri yang masuk ke dalam jaringan tubuh. b.Monosit. Terbanyak dibuat di sumsum merah, lebih besar dari limfosit, fungsinya sebagai fagosit dan banyaknya 34%. Di bawah mikroskop terlihat bahwa protoplasmanya lebar, warna biru sedikit abu-abu mempunyai bintik-bintik sedikit kemerahan. Inti selnya bulat atau panjang, warnanya lembayung muda. 2.Granulosit disebut juga leukosit granular terdiri dari : a.Neutrofil atau polimorfonuklear leukosit, mempunyai inti sel yang kadang-kadang seperti terpisah-pisah, protoplasmanya banyak bintik-bintik halus/granula, banyaknya 60%-70%. b.Eusinofil. Ukuran dan bentuknya hampir sama dengan neutrofil tetapi granula dalam sitoplasmanya lebih besar , banyaknya 24%. c. Basofil, sel ini kecil dari eusinofil tetapi mempunyai inti yang bentuknya teratur, di dalam protoplasmanya terdapat granula-granula besar. Banyaknya setengah bagian sumsum merah, fungsinya tidak diketahui (Syaifuddin,2006). 2.3 LEUKEMIA 1. DEFINISI Leukemia mula-mula dijelaskan oleh Virchow pada tahun 1847 sebagai darah putih, adalah penyakit neoplastik yang ditandai oleh proliferasi abnormal dari sel-sel hematopoietik (Price, 1994). Leukemia adalah proliferase leukosit yang tidak terkontrol di dalam darah, sumsum tulang, dan jaringan retikuloendotelial (Tuker, 1998). Leukemia merupakan suatu penyakit yang ditandai dengan proliferasi dini yang berlebihan (sel muda) dari sel darah putih (SDP) (Engram, 1998). Leukemia merupakan proliferatif neoplastik dari perkusor sel darah putih, yang menyebabkan penggantian difus sumsum tulang normal oleh sel leukemia dengan akumulasi sel abnormal pada darah tepi dan infiltrasi organ misalnya hati, limpa, kelenjar limfe, meningen, dan gonad oleh sel leukemi (Underwood, 1999). Leukemia adalah proliferasi tidak teratur atau akumulasi sel darah putih dalam sumsum tulang, mengganti elemen sumsum tulang normal. Juga terjadi proliferasi di hati,limpa dan nodus limfatikus dan invasi organ nonhematologis, seperti meninges, traktus gastrointestinal, ginjal dan kulit (Smeltzer, 2001). Leukemia adalah penyakit mengenai sel darah putih yang mengalami pembelahan yang berulang-ulang.penyakit ini semacam kanker yang menyerang sel-sel darah putih. Akibatnya fungsi sel darah putih terganggu, bahkan sel-sel darah merah dapat terdesak karena pertumbuhan yang berlebihan ini jumlah sel darah merah menurun (Irianto,2004). Leukemia (kangker darah) merupakan suatu penyakit yang ditandai pertambahan jumlah sel darah putih (leukosit). Pertambahan ini sangat cepat dan tak terkendali serta bentuk sel- sel darah putihnya tidak normal (Yatim, 2003). Leukemia merupakan suatu penyakit yang ditandai dengan proliferasi dini yang berlebihan dari sel darah putih (Handayani, 2008) Dari beberapa pengertian di atas dapat disimpulkan bahwa Leukemia adalah suatu penyakit sistem hematologi yang ditandai dengan proliferasi yang berlebihan dan tidak normal pada sel darah putih yang mengakibatkan fungsi sel darah putih terganggu. 2.4 KLASIFIKASI LEUKEMIA Leukemia dapat diklafikasikan ke dalam : 1. Maturitas sel : Akut (sel-sel asal berdiferensiasi secara buruk) Kronis (lebih banyak sel dewasa) 2. Tipe-tipe sel asal Mielositik (Mieloblast yang dihasilkan sumsum tulang) Limfositik (limfoblast yang dihasilkan sistem limfatik) Normalnya, sel asal (mieloblast dan limfoblast) tak ada pada darah perifer. Maturitas sel dan tipe sel dikombinasikan untuk membentuk empat tipe utama leukemia : 1. LEUKEMIA MIELOGENUS AKUT (LMA) Leukemia Mielogenus Akut (LMA) atau leukemia mielositik akut atau dapat juga disebut leukemia granulositik akut (LGA), mengenai sel stem hematopetik yang kelak berdiferensiasi ke semua sel mieloid; monosit, granulosit (basofil, netrofil, eosinofil), eritrosit, dan trombosit. Dikarakteristikan oleh produksi berlebihan dari mieloblast. Semua kelompok usia dapat terkena; insidensi meningkat sesuai dengan bertambahnya usia. Merupakan leukemia nonlimfositik yang paling sering terjadi. 2. LEUKEMIA MIELOGENUS KRONIS (LMK) Leukemia Mielogenus Kronis (LMK) atau leukemia mielositik kronis atau leukemia granulositik kronis (LGK), juga dimasukan dalam keganasan sel stem mieloid. Namun, lebih banyak terdapat sel normal di banding pada bentuk akut, sehingga penyakit ini lebih ringan. Abnormalitas genetika yang dinamakan kromosom Philadelpia ditemukan 90% sampai 95% pasien dengan LMK. LMK jarang menyerang individu di bawah 20 tahun, namun insidensinya meningkat sesuai pertambahan usia. Gambaran menonjol adalah : - adanya kromosom Philadelphia pada sel sel darah. Ini adalah kromosom abnormal yang ditemukan pada sel sel sumsum tulang. - Krisis Blast. Fase yang dikarakteristik oleh proliferasi tiba-tiba dari jumlah besar mieloblast. Temuan ini menandakan pengubahan LMK menjadi LMA. Kematian sering terjadi dalam beberapa bulan saat sel sel leukemia menjadi resisten terhadap kemoterapi selama krisis blast. 3. LEUKEMIA LIMFOSITIK AKUT (LLA) Leukemia Limfositik Akut (LLA) dianggap sebagai suatu proliferasi ganas limfoblas. Paling sering terjadi pada anak-anak, dengan laki-laki lebih banyak dibanding perempuan,dengan puncak insidensi pada usia 4 tahun. Setelah usia 15 tahun , LLA jarang terjadi. 4. LEUKEMIA LIMFOSITIK KRONIS (LLK) Leukemia Limfositik Kronis (LLK) cenderung merupakan kelainan ringan yang terutama mengenai individu antara usia 50 sampai 70 tahun. Negara-negara barat melaporkan penyakit ini sebagai leukemia yang umum terjadi. LLK dikarakteristikan oleh proliferasi dari diferensiasi limfosit yang baik (mudah dikenali sel-sel yang menunjukkan jaringan asal). Kelompok Klasifikasi Leukemia Akut Menurut French-American-British (FAB) Leukemia Limfositik Akut L-1 pada masa kanak-kanak: populasi sel homogen L-2 Leukemia limfositik akut tampak pada orang dewasa: populasi sel heterogen L-3 Limfoma Burkitt-tipe leukemia: sel-sel besar, populasi sel homogen. Leukemia Mieloblastik Akut M-1 Diferensiasi granulositik tanpa pematangan M-2 Diferensiasi granulositik disertai pematangan menjadi stadium promielositik M-3 Diferensiasi granulositik disertai promielosit hipergranular yang dikaitkan dengan pembekuan intra vaskular tersebar (Disseminated intravascular coagulation). M-4 Leukemia mielomonositik akut: kedua garis sel granulosit dan monosit. M-5a Leukemia monositik akut : kurang berdiferesiasi M-5b Leukemia monositik akut : berdiferensiasi baik M-6 Eritroblast predominan disertai diseritropoiesis berat M-7 Leukemia megakariositik. 2.5 ETIOLOGI Penyebab leukemia belum diketahui secara pasti. Diperkirakan bukan penyebab tunggal tetapi gabungan dari faktor resiko antara lain : Terinfeksi virus. Agen virus sudah lama diidentifikasi sebagai penyebab leukemia pada hewan. Pada tahun 1980, diisolasi virus HTLV-1 dari leukemia sel T manusia pada limfosit seorang penderita limfoma kulit dan sejak saat itu diisolasi dari sampel serum penderita leukemia sel T. Faktor Genetik. Pengaruh genetik maupun faktor-faktor lingkungan kelihatannya memainkan peranan , namun jarang terdapat leukemia familial, tetapi insidensi leukemia lebih tinggi dari saudara kandung anak-anak yang terserang , dengan insidensi yang meningkat sampai 20% pada kembar monozigot (identik). Kelainan Herediter. Individu dengan kelainan kromosom, seperti Sindrom Down, kelihatannya mempunyai insidensi leukemia akut 20 puluh kali lipat. Faktor lingkungan. - Radiasi. Kontak dengan radiasi ionisasi disertai manifestasi leukemia yang timbul bertahun-tahun kemudian. - Zat Kimia. Zat kimia misalnya : benzen, arsen, kloramfenikol, fenilbutazon, dan agen antineoplastik dikaitkan dengan frekuensi yang meningkat khususnya agen-agen alkil. Kemungkinan leukemia meningkat pada penderita yang diobati baik dengan radiasi maupun kemoterapi. 2.6 PATOFISIOLOGI Jika penyebab leukemia virus, virus tersebut akan masuk ke dalam tubuh manusia jika struktur antigennya sesuai dengan struktur antigen manusia. Bila struktur antigen individu tidak sama dengan struktur antigen virus, maka virus tersebut ditolaknya seperti pada benda asing lain. Struktur antigen manusia terbentuk oleh struktur antigen dari berbagai alat tubuh, terutama kulit dan selaput lendir yang terletak di permukaan tubuh (kulit disebut juga antigen jaringan ). Oleh WHO terhadap antigen jaringan telah ditetapkan istilah HL-A (Human Leucocyte Lucos A). Sistem HL-A individu ini diturunkan menurut hukum genetika sehingga adanya peranan faktor ras dan keluarga dalam etiologi leukemia tidak dapat diabaikan. Leukemia merupakan proliferasi dari sel pembuat darah yang bersifat sistemik dan biasanya berakhir fatal. Leukemia dikatakan penyakit darah yang disebabkan karena terjadinya kerusakan pada pabrik pembuat sel darah yaitu sumsum tulang. Penyakit ini sering disebut kanker darah. Keadaan yang sebenarnya sumsum tulang bekerja aktif membuat sel-sel darah tetapi yang dihasilkan adalah sel darah yang tidak normal dan sel ini mendesak pertumbuhan sel darah normal. Proses patofisiologi leukemia dimulai dari transformasi ganas sel induk hematologis dan turunannya. Proliferasi ganas sel induk ini menghasilkan sel leukemia dan mengakibatkan penekanan hematopoesis normal, sehingga terjadi bone marrow failure, infiltrasi sel leukemia ke dalam organ, sehingga menimbulkan organomegali, katabolisme sel meningkat, sehingga terjadi keadaan hiperkatabolik. 2.8 MANIFESTASI KLINIS Gejala yang khas leukemia secara umum : Pucat Panas Splenomegali Hepatomegali Limfadenopati Perdarahan dapat berupa ekimosis, petekia, epitaksis, dan perdarahan gusi Gejala yang tidak khas Sakit/ nyeri sendi atau sakit tulang disalahtafsirkan sebagai reumatik Lesi purpura pada kulit Efusi pleura kejang Leukemia Mielogenus Akut Kebanyakan tanda dan gejala terjadi akibat berkurangnya produksi sel darah normal. Peka terhadap infeksi akibat granulositopenia, kekurangan granulosit Kelelahan dan kelemahan terjadi karena anemia Kecendrungan perdarahan terjadi akibat trombositopenia, kurangnya jumlah trombosit. Proliferase sel lukemi dalam organ mengakibatkan berbagai gejala tambahan : nyeri akibat pembesaran limfa; sakit kepala atau muntah akibat leukemi meningeal (sering terjadi pada leukemia limfositik); dan nyeri tulang akibat penyebaran sumsum tulang belakang. Leukemia Mielogenus Kronis Gambaran klinis LMK mirip dengan gambaran LMA, tetapi tanda dan gejalanya lebih ringan. Banyak pasien yang menunjukkan tanda dan gejala selama bertahun-tahun. Terdapat peningkatan leukosit, kadang sampai jumlah yang luar biasa. Limpa sering membesar. Leukemia Limfositik Akut Limfosit imatur berploriferasi dalam sumsum tulang dan jaringan perifer dan menggangu perkembangan sel normal. Akibatnya: Hematopoesis normal terhambat, mengakibatkan penurunan jumah leukosit, sel darah merah, dan trombosit. Eritrosit dan trombosit jumlahnya rendah dan leukosit jumlahnya dapat rendah atau tinggi tetapi selalu terdapat sel imatur. Manifestasi infiltrasi leukemia ke organ-organ lain lebih sering terjadi pada LLA daripada jenis leukemia lain dan mengakibatkan : - Nyeri karena pembesaran hati dan limpa - Sakit kepala - Muntah karena keterlibatan meninges, dan - Nyeri tulang. Leukemia Limfositik Kronis Kebanyakan pasien tidak menunjukkan gejala dan baru terdiagnosa pada saat penanganan fisik atau penanganan untuk penyakit lain. Manifestasi yang mungkin terjadi adanya : Anemia Infeksi Pembesaran nodus limfe dan organ abdominal Jumlah eritrosit dan trombosit mungkin normal atau menurun. Terjadi penurunan jumlah limfosit (limfositopenia) 2.9 KOMPLIKASI Komplikasi leukemia meliputi perdarahan dan infeksi, yang merupakan penyabab utama kematian. Pembentukan batu ginjal, anemia dan masalah gastroentestinal merupakan komplikasi lain. Risiko perdarahan berhubungan dengan tingkat defisiensi trombosit (trombositopenia). Angka trombosit rendah ditandai dengan memar (ekimosis) dan petekia (bintik perdarahan kemerahan atau keabuan sebesar ujung jarum di permukaan kulit). Pasien juga dapat mengalami perdarahan berat jika jumah trombositnya turun sampai di bawah 20.000/mm3 darah. Dengan alasan tidak jelas, demam dan infeksi dapat meningkatkan kemungkinan perdarahan. Karena kekurangan granulosit matur dan normal, pasien selalu dalam keadaan terancam infeksi. Kemungkinan terjadinya infeksi meningkat sesuai dengan derajat netropenia, sehingga jika granulosit berada di bawah 100/ml darah sangat mungkin terjadi infeksi sistemik. Disfungsi imum mempertinggi resiko infeksi. Penghancuran sel besar-besaran yang terjadi selama pemberian kemoterapi akan meningkatkan kadar asam urat dan membuat pasien rentan mengalami pembentukan batu ginjal dan kolik ginjal. Maka pasien memerlukan asupan cairan yang tinggi untuk mencegah kristalisasi asam urat dan pembentukan batu. Masalah gastrointestinal dapat terjadi akibat infiltrasi leukosit abnormal ke oran abdominal selain akibat toksisitas obat kemoterapi. Sering terjadi anoreksia, mual, muntah, diare, dan lesi mukosa mulut. 2.10 PEMERIKSAAN DIAGNOSTIK a. Pemeriksaan laboratorium Gejala yang terlihat pada darah tepi berdasarkan pada kelainan sumsum tulang berupa pansitopenia, limfositosis yang kadang-kadang menyebabkan gambaran darah tepi menoton dan terdapat sel blas. Terdapatnya sel blas dalam darah tepi merupakan gajala patognomik untuk leukemia.kolesterol mungkin rendah, asam urat dapat meningkat , hipogamaglobinea. Dari pemeriksaan sumsum tulang akan ditemukan gambaran yang menoton, yaitu hanya terdiri dari sel limfopoietik patologis sedangkan sistem lain terdesak (aplasia sekunder). Pada LMA selain gambaran yang menoton, terlihat pula adanya hiatus leukemia ialah keadaan yang memperlihatkan banyak sel blas (mieloblas), beberapa sel tua (segmen) dan sangat kurang bentuk pematangan sel yang berada di antaranya (promielosit, mielosit, metamielosit dan sel batang). b. Biopsi Limpa Pemeriksaan ini memperlihatkan proliferase sel leukemia dan sel yang berasal dari jaringan limpa yang terdesak, seperti limfosit normal, RES, granulosit, dan pulp cell. c. Pungsi Sumsum Tulang Pungsi sumsum tulang merupakan pengambilan sedikit cairan sumsum tulang, yang bertujuan untuk penilaian terhadap simpanan zat besi, mendapatkan spesimen untuk pemeriksaan bakteriovirologis (biakan mikrobiologi), untuk diagnosa sitomorfologi/ evaluasi produk pematangan sel asal darah. Tempat yang biasanya digunakan aspirasi untuk pungsi sumsum tulang adalah spina iliaka posterior superior (SIPS), krista iliaka, spina iliaka anterior superior (SIAS), sternum di antara iga ke-2 dan ke-3 midsternal atau sedikit di kanannya (jangan lebih dari 1 cm), spina dorsalis/prosesus spinosus vertebra lumbalis. d. Cairan Serebrospinal Bila terdapat peninggian jumlah sel patologis dan protein,berarti suatu leukemia meningeal. Kelainan ini dapat terjadi setiap saat pada perjalanan penyakit baik dalam keadaan remisi maupun keadaan kambuh. Untuk mencegahnya diberikan metotreksat (MTX) secara intratekal secara rutin pada setiap pasien baru atau pasien yang menunjukkan gejala tekanan intrakranial meninggi. e. Sitogenik Pada kasus LMK 70-90% menunjukkan kelainan kromosom, yaitu kromosom 21 (kromosom Philadelpia atau Ph 1). 50-70% dari pasien LLA dan LMA mempunyai kelainan berupa: Kelainan jumlah kromosom seperti diploid (2n), hiploid (2n-a), hiperploid (2n+a). Kariotip yang pseudodiploid pada kasus dengan jumlah kromosom yang diploid. Bertambah atau hilangnya bagian kromosom (partial depletion). Terdapatnya marker chromosome yaitu elemen yang secara morfologis bukan merupakan kromosom normal; dari bentuk yang sengat besar sampai yang sangat kecil. Untuk menentukan pengobatannya harus diketahui jenis kelainan yang ditemukan. Pada leukemia biasanya didapatkan dari hasil darah tepi berupa limfositosis lebih dari 80% atau terdapat sel blas. Juga diperlukan pemeriksaan dari sumsum tulang dengan menggunakan mikroskop elektron akan terlihat adanya sel patologis. 2.11 PENATALAKSANAAN MEDIS DAN PENUNJANG a. Penetalaksanaan Medis Transfusi darah, biasanya diberikan jika kadar Hb kurang dari 6g%. Pada trombositopenia yang berat dan perdarahan masif, dapat diberikan transfusi trombosit dan bila terdapat tanda-tanda DIC dapat diberikan heparin Kortikosteroid (prednison, kortison, deksametason dan sebagainya). Setelah dicapai remisi dosis dikurangi sedikit demi sedikit dan akhirnya dihentikan. Sitostatika. Selain sitostatika yang lama (6-merkaptopurin atau 6-mp, metotreksat atau MTX) pada waktu ini dipakai pula yang baru dan lebih poten seperti vinkristin (Oncovin), rubidomisin (daunorubycine) dan berbagai nama obat lainnya. Umumnya sitostatika diberikan dalam kombinasi bersama-sama dengan prednison. Pada pemberian obat-obatan ini sering terdapat efek samping berupa alopesia (botak), stomatitis, leukopenia, infeksi sekunder atau kandidiasis. Bila jumlah leukosit kurang dari 2000/mm3 pemberiannya harus hati-hati. Infeksi sekunder dihindarkan (lebih baik pasien dirawat di kamar yang suci hama/ steril). Imunoterapi, merupakan cara pengobatan terbaru. Setelah tercapai remisi dan jumlah sel leukemia cukup rendah (105-106), imunoterapi mulai diberikan (mengenai cara pengobatan yang terbaru masih dalam pengembangan). Cara pengobatan berbeda-beda pada setiap klinik bergantung dari pengalaman, tetapi prnsipnya sama, yaitu dengan pola dasar : 1. Induksi. Dimaksud untuk mencapai remisi dengan bebagai obat tersebut sampai sel blas dalam sumsum tulang kurang dari 5%. 2. Konsolidasi. Bertujuan agar sel yang tersisa tidak cepat memperbanyak diri lagi. 3. Rumat. Untuk mempertahankan masa remisi agar lebih lama. Biasanya dengan memberikan sitostatika setengah dosis biasa. 4. Reinduksi. Dimaksukan untuk mencegah relaps. Biasanya dilakukan setiap 3-6 bulan dengan pemberian obat-obat seperti pada induksi selama 10-14 hari. 5. Mencegah terjadinya leukemia pada susunan syaraf pusat. Diberikan MTX secara intratekal dan radiasi kranial. 6. Pengobatan imunologik. b. Penatalaksanaan Keperawatan Masalah pasien yang perlu diperhatikan umumnya sama dengan pasien lain yang menderita penyakit darah. Tetapi karena prognosis pasien pada umumnya kurang menggembirakan (sama seperti pasien kanker lainnya) maka pendekatan psikososial harus diutamakan. Yang perlu diusahakan ialah ruangan yang aseptik dan cara bekerja yang aseptik pula. Sikap perawat yang ramah dan lembut diharapkan tidak hanya untuk pasien saja tetapi juga pada keluarga yang dalam hal ini sangat peka perasaannya jika mengetahui penyakit anaknya atau keluarganya. Beberapa cara yang bisa kita anjurkan adalah hindari menyikat gigi terlalu keras, karena bulu sikat gigi dapat mencederai gusi. Menyarankan klien supaya berhati-hati ketika berjalan di lantai yang licin seperti kamar mandi agar tidak jatuh. Memberikan klien dan keluarganya pendidikan kesehatan bagaimana cara mengatasi perdarahan hidung, misalnya dibendung dengan kapas atau perban, posisi kepala menengadah. Untuk menangani infeksi klien harus menjaga kebersihan diri, seperti mencuci tangan, mandi 3x sehari. Menganjurkan keluarga klien untuk menjaga keersihan diri mereka, membatasi jumlah pengunjung karena dikhawatirkan dapat menularkan penyaki-penyakit seperti flu dan batuk. Menciptakan lingkungan yang bersih dan jika perlu pertahankan tehnik isolasi. 2.12 PROSES KEPERAWATAN Pasien Leukemia a. Pengkajian 1. Riwayat pemajanan pada faktor-faktor pencetus, seperti pemajanan pada dosis besar radiasi, riwayat infeksi virus, genetik dan penyakit herediter. 2. Pemeriksaan fisik dapat menunjukkan manifestasi : Pembesaran sumsum tulang dengan sel-sel leukemia yang selanjutnya menekan fungsi sumsum tulang, sehingga menyebabkan beberapa gejala di bawah ini: Sakit kepala Infeksi Pemeriksaan darah menunjukkan perubahan sel darah putih Anemia penurunan berat badan, kelemahan dan kelelahan, pucat, malaise, muntah dan anoreksia. Trombositopenia (jumlah trombosit rendah) Petekia, Ekimosis, mudah memar, Kencenderungan perdarahan (pada gusi) Netropenia Demam, berkeringat pada malam hari. 3. Infiltrasi organ lain dengan sel-sel leukemia yang menyebabkan beberapa gejala seperti : Hepatomegali Splenomegali Limfadenopati Nyri tulang dan sendi Hipertrofi gusi. b. Diagnosa dan Intervensi Keperawatan Diagnosa keperawatan yang mungkin muncul antara lain : 1. Nyeri berhubungan dengan infiltrasi leukosit ke jaringan sistemik Tujuan : setelah dilakukan tindakan keperawatan pada klien nyeri akan berkurang. Kriteria Hasil : Menyatakan nyeri berkurang dengan indikator 1-3 (tidak ada, ringan, sedang ) Ekspresi wajah tenang. Tidak ada petunjuk non verbal tentang nyeri HR 60-100x/mnt, RR 16-24x/mnt, TD 120/80mmHg. Menerima medikasi nyeri sesuai yang diresepkan Mengambil peran aktif dalam pemberian analgetik. Skala nyeri 1-3 (tidak ada, ringan, sedang ) Intervensi Keperawatan : 1. Kaji karakteristik nyeri : lokasi, kualitas, frekuensi, dan durasi. Rasional : Memberikan dasar untuk mengkaji perubahan pada tingkat nyeri dan mengevaluasi intervensi. 2. Berikan terapi analgetik sesuai dengan instruksi dokter. Lakukan penilaian respon pasien terhadap pemberian analgetik Rasional : analgetik merupakan agen farmakologi yang berfungsi mengurangi rasa nyeri, analgetik cenderung lebih efektif ketika diberikan secara dini pada siklus nyeri, respon pasien memberikan informasi tambahan tentang nyeri klien. 3. Berikan dukungan emosional dan menentramkan kekuatiaran pasien. Rasional : mengurangi ketakutan dan ansietas akibat penyakit yang di derita. Ketakutan dan ansietas akan meningkatkan persepsi nyeri. 4. Gunakan metode distraksi seperti relaksasi, teknik pernapsan dalam, mendengarkan musik, dan imajinasi. Raional : teknik pengalihan perhatian atau distraksi dapat membuat mengurangi nyeri yang dirasakan pasien karena pasien tidak fokus terhadap nyeri yang dialaminya. 2. Resiko infeksi berhubungan dengan menurunnya daya tahan tubuh yang berkaitan dengan neutropenia/ menurunnya sistem imun. Tujuan : setelah dilakukan tindakan keperawatan pada klien, klien akan terbebas dari gejala infeksi. Kriteria Hasil: Faktor resiko akan hilang ditunjukkan dengan status imun pasien Pasien menunjukkan pengendalian resiko, dibuktikan dengan indikator berikut ini (antara 1-3: tidak pernah, jarang, kadang-kadang,). Mengindikasi status gastrointestinal, pernapasan, genitourinaria, dan imum dalam batas normal. Menunjukkan higiene pribadi yang adekuat. Leukosit 4000 - 11.000/L, Neutrofil : 150-300/L 36-37oC Intervensi Keperawatan : 1. Rasional : memberikan dasar untuk mengkaji perubahan jika terjadi kemungkinan infeksi 2. Kaji faktor yang meningkatkan serangan infeksi (misalnya: usia lanjut, tanggap imun rendah, malnutrisi). Rasional : untuk menentukan intervensi selanjutnya 3. Instruksikan untuk menjaga higiene pribadi untuk melindungi tubuh terhadap infeksi baik pada pasien maupun keluarga. Rasional : higiene pribadi dapat melindungi tubuh untuk meminimalkan pajanan pada organisme infektif. 4. Berikan terapi antibiotik bila diperlukan sesuai dengan instruksi dokter. Rasional : diberikan sebagai profilaktik atau mengobati infeksi khusus 5. Pertahankan teknik isolasi, bila diperlukan. Rasional : ruangan yang terisolasi dapat meminimalkan terpaparnya pasien dari sumber infeksi. 6. Lindungi pasien dari kontaminasi silang dengan tidak menugaskan perawat yang sama untuk setiap pasien infeksi dan memisahkan pasien infeksi dalam kamar yang berbeda. Rasional : kontaminasi silang dapat memperbesar resiko infeksi pada klien. 3. Intoleransi aktivitas : kelemahan secara menyeluruh akibat anemia. Tujuan : setelah dilakukan tindakan keperawatan pada klien, terjadi peningkatan toleransi aktifitas. Kriteria Hasil: Mentolenrasi aktivitas yang biasa dilakukan dan ditunjukan dengan daya tahan, penghematan energi, dan perawatan diri : Aktivitas Kehidupan Sehari-hari (AKSI). Menunjukkan penghematan energi, ditandai dengan indikator 1-5 (tidak sama sekali, ringan, sedang, berat, atau sangat berat), menyadari keterbatasan energi, menyeimbangkan aktivitas dan istirahat. Mengungkapkan secara verbal pemahaman tentang kebutuhan oksigen, pengobatan, dan/atau peralatan yang dapat meningkatkan toleransi terhadap aktivitas. Kaji Tanda-tanda Vital serta pantau respons kardiorespirasi terhadap aktivitas (misalnya, takikardia, disaritmia lain, dispnea, diaforesis, pucat, tekanan, hemodinamik, dan frekuensi respirasi) pasien dan kadar Hb dalam darah. Rasional : memberikan dasar untuk menentukan intervensi serta tingkat kemampuan klien 2. Evaluasi laporan kelemahan, perhatikan kemampuan untuk berpartisipasi dalam aktifitas sehari-hari. Rasional : menentukan derajat dan efek ketidakmampuan. 3. Berikan lingkungan tenang dan perlu istirahat tanpa gangguan. Rasional : menghemat energi untuk aktifitas dan regenerasi seluler atau penyambungan jaringan. 4. Pantau asupan nutrisi untuk memastikan keadekuatan sumber-sumber energi serta berikan masukan protein dan kalori yang adekuat. Rasional : nutrisi kalori dan proten yang cukup dapat membantu mengembalikan energi yang hilang dan meningkatkan toleransi aktivitas. 5. Ajarkan pengaturan aktivitas dan teknik menajemen waktu untuk mencegah kelelahan. Rasional : pengaturan aktivitas dan menejemen waktu dapat mengatur penggunaan energi sehingga dapat mencegah kelelahan. 4. Resiko cedera : perdarahan berhubungan dengan trombositopenia Tujuan : setelah dilakukan tindakan keperawatan pada klien, menunjukkan resiko cedera menurun. Kriteria Hasil: Menunjukkan pengendalian resiko dibuktikan dengan indikator ini 1-3 (tidak pernah, jarang, kadang-kadang). Menghidari cedera fisik. Mempersiapkan lingkungan yang aman (misalnya, meniadakan ketidakteraturan dan tumpahan, penempatan pegangan tangan, penggunaan tikar karet, serta pegangan tangan di kamar mandi). Tanda-tanda pendarahan berkurang. Ekimosis tidak ada/berkurang, peteki tidak ada, epistaksis tidak ada atau jarang. Trombosit : 150.000-450.000/L Intervensi Keperawatan : 1. Gunakan semua tindakan untuk mencegah perdarahan khususnya pada daerah ekimosis Rasional : karena perdarahan memperberat kondisi pasien dengan adanya anemia. 2. Laporkan setiap tanda-tanda perdarahan serta pantau kadar trombosit dalamdarah (tekanan darah menurun, denyut nadi cepat, dan pucat) Rasional : untuk memberikan intervensi dini dalam mengatasi perdarahan. 3. Gunakan jarum yang kecil pada saat melakukan injeksi Rasional : untuk mencegah perdarahan. 4. Ajarkan keluarga dan pasien yang untuk mengontrol perdarahan hidung. Rasional : untuk mencegah perdarahan. 5. Menggunakan sikat gigi yang lunak dan lembut Rasional : untuk mencegah perdarahan pada gusi. 6. Hindari obat-obat yang mengandung aspirin.
