Penggunaan aliran listrik untuk terapi penyakit.Termasuk ini adalah prosedur:

(a) biofeedback: Suatu proses sistem visual atau auditory yang memaparkan siknyal listrik aktivitas fisiologis pasien.Myofeedback: satu tipe biofeedback yang memaparkan proses fisiologis activitas otot.

READING 1: HEMIPLEGIA

Hemiplegia is one of the most common muscular disorders of central origin. When the paralysis occurs suddenly, it is referred to as stroke or apoplexy.

The involvement affects the corticobulbar and cortico-spinal fibers.

Motor neurons, with cell bodies located in the cortex and axons extending into the cord, are known as uppermotor neurons in contradistinction to those in the anterior horn of the cord which extend toward the muscles and are called lower motor neurons.

A similar division exists for the nerves in regions where their nerve nuclei perform a function corresponding to that of the spinal cord.

A cerebral thrombosis tends to devitalize an upper motor neuron resulting in its degeneration all the way from the thrombotic lesion to the cord. The lower motor neuron remains uninvolved.

Following the initial period of shock, the muscles respond to reflex arcs mediated by the cord. Since these reflexes tend to be exaggerated, the muscles of the hemiplegic patient become spastic.

The spasticity may interfere seriously with the exercise of the voluntary muscle power that the patient has regained.

Drugs and therapies have been tried to overcome the unfortunate handicaps and, in extreme cases, surgical intervention has been imperative. It consist of a trans-section of the motor nerve to the extremity, resulting in flaccid musculature which is preferred to the incapacitating spasticity.

Hemiplegia which occurs suddenly is usually due to the involvement of the internal capsule resulting from a rupture of a small branch of the middle cerebral artery. An occlusion of the main trunk affects a large portion of the cerebral hemisphere and causes contralateral hemiplegia.

Small infarcts may produce transient and permanent paralysis.

Since the advent of angiography, MRI, there has been sufficient evidence that the vascular occlusion may involve the cerebral segment of the internal carotid artery.

It is well known fact that hemiplegia occurs on the side opposite to the brain injury.

The reason for this is that about 90% of upper motor cross over the contralateral side at the level of the medulla oblongata.

With adequate rehabilitation the majority of patients survive a stroke an literally put back on their feet and return to normal living with little or no residual damage depending on the extent of their cerebrovascular accident.

* (Scottish Surgeon: Sir Charles Bell)Paralysis of the facial muscles, usually one-sided and temporary, due to inflammation of the facial nerve. Facial palsy is common and usually comes on suddenly. The cause is unknown, although, rarely, it may be associated with herpes zoster (shingles).

SYMPTOMS:The eyelid and corner of the mouth droop on one side of the face and there may be pain in the ear on the side. It may be impossible to wrinkle the brow or to close the eye, and smiling is distorted.

Depending on which branches of the nerve are affected taste may be impaired or sounds may seem unnaturally loud.

TREATMENT:Corticosteroid drugs or ACTH is sometimes given to reduce inflammation on the nerve, along with analgesics if the ear is painful.

Exercising the facial muscle may facilitate recovery.

Electrostimulation of the nerve is of unproved value. It may be necessary to tape the eyelid shut at bedtime to avoid corneal abrasion.

In most cases, the condition clears up with or without treatment.

A general term for non-progressive disorders of movement and posture resulting from damage to the brain in the later months of pregnancy, during birth, in the newborn period, or in early childhood.

A child with cerebral palsy may suffer:- spastic paralysis, - athetosis (involuntary writhing movement) - ataxia (loss of combination and balance).

The degree of disability is highly variable, ranging from slight clumsiness of hand movement and gait to complete immobility.

Other nervous system disorders, such as hearing defects or epileptic seizures, may present.

Many affected children are also mentally retarded, although a proportion are of normal or high intelligence.

TYPE OF CEREBRAL PALSY:There are 3 (three) different categories of disability in the spastic group:

- Diplegia: all four limbs are affected, the legs more severely than the arms

- Hemiplegia: The limbs on only one side of the body are affected, and the arm is usually worse than the leg.

- Quadriplegia: All four limbs are severely affected, not necessarily symmetrically

TREATMENT:Although cerebral palsy is incurable, much can be done to help children affected by it.

Abilities need to be recognized and developed to the full, as much stimulation as possible should be offered, and loving patience must always be shown.

Physical therapy is required to teach the child how to develop muscular control and maintain balance. This therapy is often given initially at a special school or clinic and then continued at home, possibly with the use of special equipment.

Inadequate speech can be helped greatly by speech therapy.

For children who cannot speak at all, sophisticated techniques and devices have been developed to teach them how to communicate nonverbally.

Every attempt is made to place children with mild cerebral palsy in normal schools, but those who are severely affected need special help available at schools for the physically and/or mentally handicapped.
